Description
技术领域
本发明属于技术领域,尤其涉及一种建立急性冠脉综合征在介入治疗术后的出血风险预测模型的方法。
背景技术
急性冠脉综合征是指冠状动脉内不稳定的动脉粥样斑块破裂或糜烂引起血栓形成所导致的心脏急性缺血综合征,包括不稳定型心绞痛、非ST段抬高型心肌梗死和ST段抬高型心肌梗死。GRACE注册研究发现,急性冠脉综合征发生后4年的死亡率约为50%。目前治疗急性冠脉综合征的主要手段包括经皮冠状动脉介入治疗和抗栓药物治疗。双联抗血小板治疗(DAPT)指联合使用阿司匹林与P2Y12受体拮抗剂(氯吡格雷、替格瑞洛或普拉格雷),是抗栓药物治疗和经皮冠状动脉介入治疗术后治疗的重要方法。PLATO研究发现,相比氯吡格雷,替格瑞洛可显著降低急性冠脉综合征患者的心血管死亡率、心肌梗死和卒中的发生率,但同时会显著增加致死性和非致死性颅内出血等出血事件的发生率。
目前,常用于预测急性冠脉综合征或经皮冠状动脉介入治疗患者的出血风险评分(如CRUSADE、ACTION和ACUITY-HORIZONS等)应用在中国经皮冠状动脉介入治疗术后的急性冠脉综合征患者中时,均存在高估出血的风险的情况。本申请的发明人旨在利用源于中国的真实世界数据,通过机器学习的方法建立预测中国经皮冠状动脉介入治疗术后的急性冠脉综合征患者的近远期出血风险的预测模型,对于包括抗血小板治疗用药选择在内的临床治疗决策、降低出血发生率或有一定的指导意义。
发明内容
本发明的目的在于提供一种建立急性冠脉综合征在介入治疗术后的出血风险预测模型的方法,用源于中国的真实世界数据,通过机器学习的方法建立预测模型,对于包括抗血小板治疗用药选择在内的临床治疗决策、降低出血发生率或有一定的指导意义。

在上述步骤中的收集数据,首先,通过根据患者纳入排除标准,在医院电子病历系统中回顾性收集接受经皮冠状动脉介入治疗的急性冠脉综合征患者的信息,纳入标准包括:(1)年龄≥18岁;(2)确诊急性冠脉综合征的患者;(3)接受经皮冠状动脉介入治疗治疗。排除标准包括:(1)接受搭桥治疗;(2)孕产妇及哺乳期妇女。
本发明将对所有入选患者基线/筛选期回顾性收集包括人口学特征、体征、行为、实验室检查结果、心脏彩超检查结果、合并症、病史、用药、在院期间出血相关事件各项特征在内的所有可获得的脱敏后的患者信息;
上述特征涵盖了除种族和DAPT持续时长(30个月vs.12个月,长于本研究设定的最长观察时长)以外,既有11个急性冠脉综合征患者经皮冠状动脉介入治疗术后出血风险预测评分使用的患者特征,以及其他潜在的急性冠脉综合征患者经皮冠状动脉介入治疗术后出血和心血管事件的影响因素。对于观察期,即患者经皮冠状动脉介入治疗术后一年内,本发明将通过患者诊断、病程、影像学诊断和实验室检查结果等回顾性收集患者TIMI major和minor出血事件相关信息。当患者诊断、病程和/或影像学诊断中确认发生发生‘出血|咯血|血尿|尿血|黑便|柏油样便|便血|潜血阳性|渗血|血肿|假性动脉瘤’时,即认为有出血事件发生。TIMI major出血包括颅内出血(除外仅梯度回波MRI可见<10mm的微出血)、临床显性出血且血红蛋白下降≥5g/dL和致死性出血(出血直接导致7天內死亡);TIMI minor出血包括临床显性出血(包括影像学检查),造成血红蛋白下降3到<5g/dL。

具体的,所述步骤S3中应用的机器学习模型包括SVM、RF、NB、LR等;对提取出的患者数据,应用包括SVM、RF、NB、LR等在内的机器学习方法,将分别建立以所有患者特征和表1中罗列的患者特征预测经皮冠脉介入治疗后出血1个月内(近期)和1年内(远期)发生TIMImajor/minor出血风险的模型。
具体的,所述步骤S4中选取最优的预测模型通过比对最终产生的各风险预测模型的预测价值,包括AUC、灵敏度、特异度、阳性预测值和阴性预测值等,分别选取最优的近、远期出血风险预测模型;通过比对最终产生的各风险预测模型的预测价值,包括ROC曲线下面积、灵敏度、特异度、阳性预测值和阴性预测值等,分别选取最优的近、远期出血风险预测模型。
本发明的工作原理是:收集包括患者的人口学特征、体征、行为、实验室检查结果、心脏彩超检查结果、合并症、病史、用药、在院期间出血相关事件特征在内的所有可获得的入选患者特征和术后一年内出血事件信息,收集信息完成后,对提取出的患者数据,应用包括SVM、RF、NB、LR等在内的机器学习方法,将分别建立以所有患者特征和上述特征的患者特征预测经皮冠脉介入治疗后出血1个月内(近期)和1年内(远期)发生TIMI major/minor出血风险的模型,最后通过比对最终产生的各风险预测模型的预测价值,包括AUC、灵敏度、特异度、阳性预测值和阴性预测值等,分别选取最优的近、远期出血风险预测模型。
