Abstract

一种高位胆肠磁性吻合装置,包括:用于置入成型胆管内的弧形板一,为顺磁性材质,其板面上开有通孔一,该孔可为台阶状,支架引流管端帽或杆状的侧翼卡在其内;用于置入肠道内的弧形板二,为磁性材质,其板面上开有通孔二,弧形板二的凹面与弧形板一凸面磁极相吸且能够紧密贴合,贴合后通孔一与通孔二相对应;支架引流管依次穿过通孔一和通孔二,实现成型胆管与肠道的贯通,本发明的目的在于提供一种高位胆肠磁性吻合装置,使用两种材质的元件,可克服其他方法的缺点,实现无缝线胆肠吻合。

Description

一种高位胆肠磁性吻合装置
技术领域
本发明属于医疗器械技术领域,特别涉及一种高位胆肠磁性吻合装置。
背景技术
目前肝门部肿瘤切除后行高位胆肠吻合仍采用传统的手工针线缝合,因其解剖关系操作难度大,技术要求高,胆漏和吻合口狭窄是其常见的并发症,其对于术后患者的生存质量至关重要。同时,也是非常棘手的不易处理的并发症。现尚未有产品化的胆肠吻合器在临床应用。曾有“一种胆肠吻合支架”、“一种带支架的肝胆管空肠吻合和肠肠端侧吻合磁性装置”、“一种肝胆管空肠吻合和肠肠端侧吻合磁性装置”等专利报道,主要应用单根肝胆管与肠管的吻合,涉及多支胆管或肝管时,需用多个该装置,多次重复同样的操作,除需有相应的屏蔽措施外,还尚有因磁力干扰造成装置从肠道排出障碍的可能。本发明的装置,是在将临近的肝胆管利用外科“成型”的技术,把两个或两个以上的断端变成一个大口径的肝胆管,只用一套该装置进行吻合,同时在缓慢压榨吻合过程中起到支撑作用,克服手工缝线吻合的缺陷,避免现有专利装置多个应用于多支肝胆管肠管吻合可能的排出障碍。
发明内容
为了克服上述现有技术的缺点,本发明的目的在于提供一种高位胆肠磁性吻合装置,使用两种材质的元件,可克服其他方法的缺点,实现无缝线胆肠吻合。
为了实现上述目的,本发明采用的技术方案是:
一种高位胆肠磁性吻合装置,包括:
用于置入成型胆管内的弧形板一1,为顺磁性材质,其板面上开有通孔一2;
用于置入肠道内的弧形板二4,为磁性材质,其板面上开有通孔二5,弧形板二4的凹面与弧形板一1凸面磁性相吸且能够紧密贴合,贴合后通孔一2与通孔二5相对应;
支架引流管,其依次穿过通孔一2和通孔二5,实现成型胆管与肠道的贯通。
所述通孔一2为台阶孔,所述支架引流管包括管身6,在管身6尾端连接有通透的环形的端帽7或者伸出侧壁的杆状的侧翼8,管身6从台阶孔穿出,端帽7或者侧翼8卡在台阶孔内。
所述弧形板一1的凸面设置有凹槽3。
所述弧形板一1和弧形板二4的表面镀有耐受胆汁和肠液腐蚀的镀层,例如镀氮化钛或高分材料,所述弧形板一1可采用顺磁性的医用钢;支架引流管6为非顺磁性材料,例如非顺磁性医用钢、钛合金及高分子材料。
与现有技术相比,本发明的有益效果是:
1.更适合于肿瘤病变切除后肝脏断面暴露的临近多支肝胆管成型后与肠管吻合。
2.弧形板一1的凸面设置有凹槽3,能容纳成型后的肝胆管残端组织,使弧形板二4的凹面与弧形板一1凸面磁极相吸且能够紧密贴合,利于压榨过程完成。
3.在的凹面开有台阶孔2,使支架引流管6其端帽7或者伸出侧壁的杆状的侧翼8卡在台阶孔内,保证弧形板一1的孔2与弧形板二4孔5对位良好,同时有利于胆汁引流排出肠道。
4.置入肠道内的弧形板二4较弧形板一1稍大,在压榨吻合脱落后因重力作用易脱落至肠道内排出,同时保证了吻合口足够大。

具体实施方式
下面结合附图和实施例详细说明本发明的实施方式。
一种高位胆肠磁性吻合装置,包括:
弧形板一1,或可称其为瓦型结构,采用顺磁性材质,使用时置入成型的胆管内,如图1所示,其板面上开有通孔一2,通孔一2可为台阶孔。
弧形板二4,或可称其为瓦型结构,如图1所示,其板面上开有通孔二5,弧形板二4也采用磁性材质,其凹面与弧形板一1凸面磁性相吸且能够紧密贴合,贴合后通孔一2与通孔二5相对应,弧形板二4使用时置入肠道内。
支架引流管,其依次穿过通孔一2和通孔二5,实现成型胆管与肠道的贯通。支架引流管的形式可采用直管,也可加入约束。
加入约束的情形一,如图2所示,通孔一2须为台阶孔,支架引流管包括管身6,在管身6尾端外侧壁连接有通透的环形的端帽7,端帽7卡在台阶孔内。
加入约束的情形二,如图3所示,通孔一2须为台阶孔,支架引流管包括管身6,在管身6尾端外侧壁连接有伸出侧壁的杆状的侧翼8,管身6从台阶孔穿出,侧翼8卡在台阶孔内。
加入约束后,可防止支架引流管从成型的胆管内脱落。
本发明还可在弧形板一1的凸面设置有凹槽3,通孔一2在凹槽3内,凹槽3沿弧形板一1长度走向也即肠道走向设置,可用于容纳胆管残端荷包与部分肠壁
根据上述结构,本发明的工作原理如下:
参考图5,肝门部肿瘤切除后暴露于肝脏断面的肝内胆管9需与小肠10进行吻合,于吻合前将邻近的多支胆管9进行“成型”,将支架引流管从弧形板一1上面的通孔一2穿入,然后将其整体凹面朝上植入“成型”的胆管内,荷包关闭残端。在肠道壁合适的位置上刺孔,支架引流管末端通过该孔,并将支架引流管末端通过肠道断端送入的弧形板二4上的通孔二5引出,牵拉支架引流管末端,使弧形板一1凸面与弧形板二4凹面相吸对合,胆管残端荷包与部分肠壁可容纳于弧形板一1凸面的凹槽3内,胆肠吻合术毕。
当加入约束时,支架引流管的管身从通孔一2穿入,端帽7或侧翼8卡入台阶孔2内,可防止支架引流管从成型的胆管内脱落。

Claims (2)

1.一种高位胆肠磁性吻合装置,其特征在于,包括:
用于置入成型胆管内的弧形板一(1),为顺磁性材质,其板面上开有通孔一(2),所述弧形板一(1)的凸面设置有凹槽(3);
用于置入肠道内的弧形板二(4),为磁性材质,其板面上开有通孔二(5),弧形板二(4)的凹面与弧形板一(1)凸面磁性相吸且能够紧密贴合,贴合后通孔一(2)与通孔二(5)相对应;
支架引流管,其依次穿过通孔一(2)和通孔二(5),实现成型胆管与肠道的吻合贯通;
所述通孔一(2)为台阶孔,所述支架引流管包括管身(6),在管身(6)尾端连接有通透的环形的端帽(7)或者伸出侧壁的杆状的侧翼(8),管身(6)从台阶孔穿出,端帽(7)或者侧翼(8)卡在台阶孔内;
其中,肝门部肿瘤切除后暴露于肝脏断面的肝内胆管(9)需与小肠(10)进行吻合,于吻合前将邻近的多支胆管(9)进行成型,将支架引流管从弧形板一(1)上面的通孔一(2)穿入,然后将其整体凹面朝上植入成型的胆管内,荷包关闭残端;在肠道壁合适的位置上刺孔,支架引流管末端通过该孔,并将支架引流管末端通过肠道断端送入的弧形板二(4)上的通孔二(5)引出,牵拉支架引流管末端,使弧形板一(1)凸面与弧形板二(4)凹面相吸对合,胆管残端荷包与部分肠壁可容纳于弧形板一(1)凸面的凹槽(3)内,胆肠吻合术毕。
2.根据权利要求1所述高位胆肠磁性吻合装置,其特征在于,所述弧形板一(1)和弧形板二(4)的表面镀有耐受胆汁和肠液腐蚀的镀层。
