Abstract
本发明涉及遗传性卵黄样营养不良新致病突变及其应用。本发明基于临床资源,借助基因捕获芯片及深度测序技术,通过遗传学、结合临床表现及人群验证,成功证实所检测到的BEST1基因的14个突变位点为遗传性卵黄样营养不良新致病位点,为该疾病的诊断提供了新的分子生物学基础,基于所述的14个突变位点可以开发遗传性卵黄样营养不良的诊断试剂盒,提高临床诊断准确性。
Description
技术领域
本发明属于疾病诊断和基因检测技术领域,具体地说,涉及十四个遗传性卵黄样营养不良新致病突变及其应用。
背景技术
遗传性卵黄样营养不良是一组由BEST1基因突变导致的眼遗传病,包括:Best卵黄样黄斑营养不良(Best vitelliform macular dystrophy,BVMD),成人卵黄样黄斑变性(adult-onset foveomacular vitelliform dystrophy,AFVD)和隐性遗传性卵黄样营养不良(autosomal recessive bestrophinopathy,ARB)。BVMD是一种常染色体显性遗传性疾病,其典型的临床特征是在青少年时期双眼黄斑部视网膜出现卵黄状的脂质样沉积物,而视力很少受影响。随着病情发展,卵黄样物质逐渐吸收,进入假性积脓期和卵黄破裂期,开始出现视力波动,最终发展为后极部视网膜色素上皮萎缩,纤维瘢痕形成,以及继发性脉络膜新生血管形成和视网膜下出血,导致视力严重的不可逆性损害。ARB临床特征包括中心视力丧失、视网膜病变、EOG明适应反应缺如及ERG振幅降低。ARB误诊为BVMD均不罕见。ARB和BVMD相似之处为后极部卵黄样物质沉积和EOG异常。然而,ARB患者卵黄病变较少见于黄斑中心凹,多发病灶广泛分布于后极部及中周眼底,且ARB患者视网膜层间积液较常见,ARB为隐性遗传,常无明显家族史。
遗传性卵黄样营养不良患者眼底表现的多样性为临床诊断带来了困难,这不仅存在于那些RPE改变相对孤立的轻症患者,也同样存在于伴有瘢痕和(或)萎缩灶的重症患者,如BVMD的晚期损害易误诊为AMD,ARB易误诊为BVMD等。基因检测结果不仅为疾病的临床诊断、预后和发病风险的评估提供支持,还为定制各种治疗方法和确定哪些患者将受益于基于基因的新疗法提供指导依据,对患者及其家庭都会产生非常积极的影响。然而,正如所有的医疗干预一样,基因检测也有一些特定的风险。比如目前在检测过程中经常遇到的一个问题就是多个突变位点致病性判断,即便目前有严格的致病性判断标准,如美国医学遗传学会ACMG标准,受限于多种因素(如:家系信息不全,未报道位点,基因异质性等),在临床工作中仍有大部分位点的致病性无法明确。例如,基因检测的假阳性结果可能影响病人的生育计划、使病人产生焦虑或内疚的情绪、甚至可能扰乱病人与其他家庭成员的关系。基因检测的假阴性结果耽误患者的诊疗,增加患病者的出生率等。因此,扩大对BEST1突变位点致病性的了解,以最大限度地提高基因检测的效益,减少与之伴随的风险,显得尤为迫切。
BEST1基因,分子量约68kDa,编码位于视网膜色素上皮层(retinal pigmentepithelium,RPE)的bestrophin-1蛋白,它是由585个氨基酸构成的跨通道转运蛋白。它具有氯离子通道以及调节胞内Ca离子通道等功能。目前已发现有301个BEST1致病突变,如专利文献CN109112134A,公开了一种视网膜变性疾病的BEST1新突变致病基因,所述新突变致病基因的核酸样本与BEST1基因如SEQ ID NO:1所示的核苷酸序列相比,核苷酸变化:c.1242G>A。然而,仍有相当部分遗传性卵黄样营养不良不能被解释。
发明内容
本发明的目的是针对现有技术中的不足,提供涉及BEST1基因的14个遗传性卵黄样营养不良新致病位点。
第一方面,本发明提供了一种特异性检测受试者基因组DNA中是否存在以下任一种、两种或几种突变的试剂:
作为一个优选例,所述试剂选自:引物、引物对、探针、抗体和核酸芯片中的一种、两种或几种。
第二方面,本发明提供了一种诊断遗传性卵黄样营养不良的试剂盒,所述试剂盒含有如上所述的试剂。
第三方面,本发明提供了如上所述的试剂或试剂盒的用途,所述用途选自:
1)在制备诊断或辅助诊断遗传性卵黄样营养不良产品中的应用;
2)在制备筛查或辅助筛查遗传性卵黄样营养不良患者产品中的应用;
3)在制备预测遗传性卵黄样营养不良患病风险产品中的应用;

具体实施方式
下面结合附图对本发明提供的具体实施方式作详细说明。
实施例1
对一个三代的患有隐性遗传性卵黄样营养不良(autosomal recessivebestrophinopathy,ARB)的家系进行基因突变检测。
一、实验方法
1该家系临床资源的采集及遗传资源库的建立
收集该家系中各成员的临床资料及血液样本,家系图见图1。临床资料主要包括个人病史、家族史、最佳矫正视力(best corrected visual acuities;BCVAs)、裂隙灯检查、眼底照、色觉检查、视野检查(Humphrey视野计)、视觉诱发电位检测(visual evokedpotentials;VEP)、全视野电生理检查(electroretinography;ERG)、EOG、眼底荧光血管造影检查(fundus fluorescein angiography;FFA)及光学相干断层成像检查(opticalcoherence tomography;OCT)等。提取患者及家系成员的外周血,并用血液基因组DNA提取试剂盒(Qiagen,Hilden,Germany)提取DNA。
2借助于高通量二代测序技术检测该家系的致病突变
2.1设计并订制捕获芯片
2.1.1 BEST1基因及转录本序列信息
该基因捕获芯片涵盖了762个遗传性眼病相关基因,其中包括我们BEST1基因。此芯片由华大基因设计。所参照的BEST1基因的转录本编号为:NM_004183.3,选择此转录本的原则是:首先考虑拥有CCDS编码蛋白的转录本,若一个基因有多个转录本均编码蛋白,则首选含氨基酸数最多的蛋白相对应的转录本,若多个转录本氨基酸含量相同,则进一步选择含碱基数最多的转录本。
2.1.2杂交探针的设计、目标区域捕获及深度测序
通过查阅OMIM数据库、眼病基因数据库(Retnet)、文献查询、临床眼病发病情况,筛选出762个基因作为眼病检测探针。在参考人类基因组Hg38下,选择上述762个基因的外显子序列,将外显子区向前和后延伸至30bp,降低重组的影响。探针序列的获取:从Hg38中获得762个基因的外显子序列及其侧翼±30bp。每个参考序列从参考序列的一端开始,并选择预定长度的参考序列来获得探针序列,以便最后探针覆盖参考序列至少一次,探针总长度是907nt,总覆盖目标区域为2.3M。此探针总覆盖深度可达到99.5%,探针设计过程中还增加了后续的样品质量控制环节。
1)DNA提取:采用琼脂糖凝胶电泳法检测样品的完整性;
2)文库准备:参照BGISEQ-500《BGI基因库建设指导手册》;
3)通过超声高性能样品处理系统(协变)随机打断合格的DNA样品,选择片段后得到150bp-250bp的片段;
4)修复DNA片段末端,3’末端加“A”;
5)与Illumina PE接头—寡核昔酸混合物相连接,连接产物经PCR富集,获DNA文库。片段扩增后,用于捕获、富集、洗涤和脱去不富集的杂交文库和眼科基因;
6)扩增产物用于单链分离和循环处理,循环文库生成。采用Qbit检测进行质量控制。
测序:质控合格后采用BGISEQ-500PE100+10进行测序。

2.2对测序数据进行生物信息学分析,筛选出候选致病基因
2.2.1生物信息学分析
1)从测序仪获得原始数据(FASTQ数据);
2)过滤:原始FASTQ数据质量控制,除去低质量数据;
3)Mapping:使用Soap和BWA软件。以Hg38作为参考序列;
4)去除重复序列:基于Picard去重复算法,去除重复序列;
5)变异检测:数据的变异检测基于GATK;
6)变异注释:dbSNP、1K基因组数据库、ESP6500数据库、EXAC数据库、华大基因内部频率注释数据库。HGVS用于变量的标准命名。OIMIM、HGMD等疾病数据库用于突变的病因注释。
2.2.2将患者的测序结果所对应的基因序列进行比较和分析,优先分析插入/缺失突变、无义突变和错义突变,结果可分为三类,包括已知突变、已知基因的新突变和新基因的突变。
2.3经Sanger测序验证,鉴定致病突变
PCR法分别针对筛选出的突变位点及邻近DNA序列在相应家系中进行扩增,所用引物序列采用Primer 3(http://frodo.wi.mit.edu/)引物设计软件设计。所用PCR的反应体系(20μL体系)为:5*buffer 4μL,25mM MgCl2 2μL,DNA 1μL,正向引物F 1μL,反向引物R 1μL,10mM dNTP 0.4μL,Taq酶0.1μL,ddH2O 10.5μL。PCR反应程序:98℃5min,35个循环(98℃10s,60℃15s,72℃1min),72℃7min,4℃5min。3%琼脂糖凝胶电泳检测,于紫外线切胶仪下切取PCR产物凝胶并纯化。对所有的PCR产物分别以正、反向引物测序,并对测序结果进行进一步分析,使用NCBI在线对比工具BLAST(http://blast.ncbi.nlm.nih.gov/),排除假阳性结果,并筛选出在家族中共分离的突变位点。
二、实验结果
1家系临床资料
眼科临床专家对该家系中的2名患者进行详细全面的临床检查后,作出“卵黄样黄斑营养不良”的临床诊断,下面以先证者为例,对其详细临床资料进行说明:
1)BCVAs:右眼0.4,左眼0.6;
2)裂隙灯检查:双眼前房偏浅,晶状体轻混,余未见明显异常;
3)眼底照:黄斑区出现卵圆形、圆形病变,呈桔黄色囊样隆起,病变约1~2视盘直径大小、边界清晰,其周围可见透明液体存留(图2中A、图2中D)。
4)眼底自发荧光:黄斑区见边界清晰、类圆形的弱荧光区,周边高荧光(图2中B、图2中E)。
5)OCT:右眼为RPE和光感受器层之间有一中等密度反射区域,大小与眼底检查所见淡黄色隆起病灶相仿,光感受器层可见沉积物厚;左眼RPE和光感受器层之间有一中等密度反射区域,其下可以高反射隆起(图2中C、图2中F)。
3该家系遗传检测结果
通过对家族中的两名患者及两名正常人进行了二代测序及生物信息学分析后,发现两位患者均携带有可疑的复合杂合突变BETS1c.843C>A p.Phe281Leu和c.767A>Gp.Gln256Arg,未发现其他可疑的致病基因突变位点。经Sanger测序验证证实该突变位点在该家族中表现为共分离,测序结果见图3。突变BETS1c.843C>Ap.Phe281Leu位于11号染色体,物理位置为chr11:61725746的碱基由C突变为A;蛋白水平:BETS1基因编码蛋白第281位氨基酸由Phe变为Leu,该基因相关的突变从未在遗传性卵黄样营养不良患者中发现。突变BETS1c.767A>G p.Gln256Arg位于11号染色体,物理位置为chr11:61725670的碱基由A突变为G;蛋白水平:BETS1基因编码蛋白第256位氨基酸由Gln变为Arg,该基因相关的突变从未在遗传性卵黄样营养不良患者中发现。
根据我们所设计的筛选流程,借助我们所设计的基因芯片及深度测序技术,我们成功证实所检测到的该BETS1基因的两个突变位点c.767A>G p.Gln256Arg和c.843C>Ap.Phe281Leu为遗传性卵黄样营养不良疑似致病位点。
实施例2
针对实施例1中所检测出的两个致病位点在遗传性卵黄样营养不良人群中验证。
一、实验方法
1遗传性卵黄样营养不良患者收集:对疑似遗传性卵黄样营养不良患者进行全面的眼科检查,按照实施例1中的方法对患者进行临床资源的采集及遗传资源库的建立。
2按照实施例1中的方法对患者进行高通量二代测序,检测遗传性卵黄样营养不良患者致病突变。
3从116例确诊的遗传性卵黄样营养不良患者中检测出另外2个由BETS1c.843C>Ap.Phe281Leu及1个由BETS1c.767A>G p.Gln256Arg致病的患者,未发现其他可疑的致病基因突变位点。以下分别选一例进行说明。
4患者A中检测出BETS1c.843C>Ap.Phe281Leu及一个已报道致病突变BEST1c.584C>T p.Ala195Val,家系图见图4。

①BCVAs:右眼0.8,左眼0.04;
②裂隙灯检查:双眼前房偏浅,晶状体轻混,余未见明显异常;
③眼底照:右眼黄斑区色素紊乱,中心凹光反射消失,左眼黄斑区出现卵圆形、圆形病变,呈桔黄色囊样隆起,病变约1~2视盘直径大小、边界清晰;
④眼底自发荧光:右眼黄斑区见边界清晰、类圆形的弱荧光区,周边高荧光,左眼病变区域荧光形态不规则,呈现斑片状强荧光,病灶周围透见荧光;
⑤OCT:右眼为RPE和光感受器层之间有一中等密度反射区域,大小与眼底检查所见淡黄色隆起病灶相仿,光感受器层可见沉积物厚,其下可以高反射隆起;左眼RPE和光感受器层之间有一中等密度反射区域,其下可以高反射隆起(图5)。
2)该家系遗传检测结果
经Sanger测序验证证实该突变位点在该家族中表现为共分离。
5患者B中检测出BETS1c.767A>G p.Gln256Arg纯合,家系图见图6。

眼科临床专家对该患者进行详细全面的临床检查后,作出“卵黄样黄斑营养不良”的临床诊断,下面对其详细临床资料进行说明:
①BCVAs:右眼0.04,左眼0.5;
②裂隙灯检查:双眼前房偏浅,晶状体轻混,余未见明显异常;
③眼底照:右眼黄斑病变区类圆形RPE萎缩灶,病灶内色素脱失与色素沉着并存,左眼黄斑区出现卵圆形、圆形病变,呈桔黄色囊样隆起,病变约1~2视盘直径大小、边界清晰;
④眼底自发荧光:右眼边界清晰的黄斑区低荧。左眼病变区域荧光形态不规则,呈现斑片状强荧光,病灶周围透见荧光;
⑤OCT:右眼中心凹形态消失,神经上皮层变薄,RPE层光反射不连续;左眼RPE和光感受器层之间有一中等密度反射区域,其下可以高反射隆起。
2)该家系遗传检测结果
经Sanger测序验证证实该突变位点在该家族中表现为共分离。
6综上,根据我们所设计的筛选流程,借助我们所设计的基因芯片及深度测序技术,通过遗传学、结合临床表现及人群验证,我们成功证实所检测到的该突变位点BETS1c.843C>A p.Phe281Leu和BETS1c.767A>G p.Gln256Arg为卵黄样黄斑营养不良新致病位点。
本发明的其余新突变位点也已通过遗传学、结合临床表现及人群验证,成功证实确为新致病位点。因本发明相关内容的论文随时可能公开,为最大限度地保护本发明创造,特及时申请专利,计划于本专利申请日起十二个月内,就相同主题的发明创造再次提出专利申请并要求优先权。
以上所述仅是本发明的优选实施方式,应当指出,对于本技术领域的普通技术人员,在不脱离本发明方法的前提下,还可以做出若干改进和补充,这些改进和补充也应视为本发明的保护范围。

1.一种特异性检测受试者基因组DNA中是否存在以下任一种、两种或几种突变的试剂:
2.根据权利要求1所述的试剂,其特征在于,所述试剂选自:引物、引物对、探针、抗体和核酸芯片中的一种或几种。
3.一种诊断遗传性卵黄样营养不良的试剂盒,其特征在于,所述试剂盒含有权利要求1所述试剂。
4.权利要求1所述的试剂或权利要求3所述的试剂盒的用途,其特征在于,所述用途选自:
1)在制备诊断或辅助诊断遗传性卵黄样营养不良产品中的应用;
2)在制备筛查或辅助筛查遗传性卵黄样营养不良患者产品中的应用;
3)在制备预测遗传性卵黄样营养不良患病风险产品中的应用;
4)在制备检测与遗传性卵黄样营养不良相关的基因是否突变产品中的应用;
5)在制备检测与遗传性卵黄样营养不良相关的蛋白质是否突变产品中的应用。
5.一种用于检测遗传性卵黄样营养不良的突变位点组合,其特征在于,所述突变位点组合至少包含一种权利要求1中所述突变。
6.权利要求5所述的突变位点组合在制备遗传性卵黄样营养不良诊断试剂盒中的应用。
7.一种探针组合物,其包含特异性结合至目标基因的探针,其特征在于,所述探针至少能检测一种权利要求1中所述突变。
8.根据权利要求7所述的探针组合物,其特征在于,所述探针组合物以混合物形式提供,或者以独立存在的形式提供。
9.一种基因捕获芯片,其包括固相载体和固定在固相载体上的探针的点阵,其特征在于,所述探针至少能检测一种权利要求1中所述突变。
