Description
技术领域
本发明涉及与内窥镜联动来对体内靶细胞进行电破坏的治疗装置。
背景技术
一般情况下,电穿孔(electroporation)是指为了向细胞内注入脱氧核糖核酸(DNA)等靶物质而使用的治疗技术。具体地,电穿孔的目的在于,通过向细胞膜施加电压来在细胞膜上形成穿孔,并通过该孔注入靶物质。
细胞膜由脂质(lipid)形成,因而当施加高电压时,脂质会向一侧倾斜,从而可能在细胞膜形成穿孔。上述穿孔可根据被施加的电压的强度而重新被填充,通过电压生成的穿孔随时间而重新被填充的情况称为可逆性电穿孔(reversible electroporation)。
相反,通过电压生成的穿孔未随时间而重新被填充的情况称为不可逆性电穿孔(irreversible electroporation)。不可逆性电穿孔的特征在于,通过在靶细胞形成不可恢复的穿孔来使细胞坏死。
细胞中存在调节细胞内代谢所必要的钾离子(K﹢),若细胞膜上存在穿孔,则钾离子会通过穿孔流出细胞外。因钾离子流出于外部而导致细胞内的钾浓度处于异常状态的细胞通过从细胞膜受体接收细胞凋亡(apoptosis)信号而坏死。可对癌细胞或恶性肿瘤等异常细胞实施通过这种不可逆性电穿孔而引起的细胞凋亡。
以往的电穿孔通常以用于向靶细胞传递所需物质的可逆性电穿孔形式形成,而不是以用于使靶细胞坏死的不可逆性电穿孔形式形成。并且,不可逆性电穿孔也几乎不在体内进行。其原因在于,由于不可逆性电穿孔用于使靶细胞坏死,因而准确设定靶细胞很重要,但是很难在体内准确地确定靶细胞。
发明内容
本发明要解决的技术问题
本发明的目的在于,提供可在体内执行不可逆性电穿孔的治疗装置。
技术方案
根据用于解决上述问题的本发明的一实施例,本申请的利用内窥镜联动式电极的治疗装置的特征在于,包括:内窥镜通道;以及电极,可通过上述内窥镜通道插入体内,上述电极通过施加电压来破坏体内的靶细胞,上述电极包括用于通过测量阻抗来确认对靶细胞的破坏程度的第一光纤及第二光纤。

有益效果
根据本发明,可执行不可逆性电穿孔的电极通过与内窥镜联动来可从体内准确地确定靶细胞。
并且,本发明的不可逆性电穿孔仅使靶细胞坏死而不会对组织产生影响,因而可减少因治疗而引起的副作用。
并且,本发明的治疗装置在电极内包括一对光纤,因而可准确地确认靶细胞的坏死程度。
由于本发明的治疗装置在装置内具有可确认靶细胞的坏死程度的结构,因而无需用于确认靶细胞的坏死程度的额外的检查装置。这在进行治疗的过程中减少为了确认治疗的进展情况而需要移除治疗装置并插入额外的检查装置的麻烦,并且提高治疗效率。

内窥镜通道100
内窥镜通道100插入体内来帮助手术人员通过肉眼观察患者的体内。内窥镜通道100包括:主体,可使使用人员用手握持;影像获取部(未图示),用于拍摄患者的体内;外壳(未图示),可使第一电极200插入;以及影像输出部,用于以可视的方式向使用人员提供拍摄到的患者的体内。
内窥镜通道100的主体呈易于使用人员用手握持并操作的形状,在上述主体的内部设置有空间。在主体内部的空间设置有可使第一电极200插入的外壳以及用于连接影像获取部和影像输出部的电线。
当内窥镜通道100插入患者的体内时,主体起到从患者的体液保护电线、第一电极200等的作用。因此,主体需要耐于胃酸等酸性体液。并且,由于主体为与患者的体内直接接触的结构要素,因此不得向人体排放有害的化学物质。为了满足上述两种条件,主体可由碳氟化合物(fluorocarbons)、聚酰胺(polyamide)、聚酯(polyester)、聚酯弹性体(polyester elastomer)等医用高分子材料制造。
影像获取部在主体中位于插入体内的一侧的末端。作为影像获取部,可利用透镜或光缆的末端等可通过接收光来取得影像的多种光学单元。并且,在影像获取部还可设置有可向主体的前侧照射光的发光单元。
外壳为用于收容第一电极200的空间。外壳具有可使第一电极200经过的程度的直径,优选地,可具有100μm至1000μm的直径。
在外壳具有大于1000μm的直径的情况下,用于收容外壳的主体变得过大,因而可能难以通过食道向患者体内插入内窥镜通道100。并且,在外壳直径小于100μm的情况下,可能没有足够的强度来保护电极。外壳也与主体一样需要耐于胃酸等酸性体液,并且不得排放对人体有害的化学物质。因此,外壳也可由碳氟化合物(fluorocarbons)、聚酰胺(polyamide)、聚酯(polyester)、聚酯弹性体(polyester elastomer)等医用高分子材料制造。
影像输出部为以可视的方式向使用人员输出由影像获取部(未图示)取得的患者内部的状态的单元。影像输出部包括用于以可视的方式输出的显示部,并且可以呈可由使用人员携带的便携式装置的形态或计算机等固定装置的形态。
第一电极200
图3为与设置在本发明的另一实施例的治疗装置的电极有关的简图。
参照图3,第一电极200为用于通过向靶细胞施加电压来破坏靶细胞的装置。具体地,第一电极200通过向靶细胞施加高压来在由脂质(lipid)形成的细胞膜形成穿孔。
当向靶细胞施加足够的高压时,上述细胞膜上的穿孔不会随时间的推移而被填充。在穿孔永久存在于细胞膜的情况下,细胞内的钾离子K﹢继续流出到血液中。由于钾离子控制细胞的代谢,因而缺乏钾离子的细胞无法执行细胞复制等正常功能。这种异常细胞通过从细胞膜受体接收致使细胞凋亡(apoptosis)的信号而坏死。
从向靶细胞施加电压开始的上述一系列过程称为不可逆性电穿孔(irreversibleelectroporation),这种通过不可逆性电穿孔进行的细胞凋亡可针对癌细胞或恶性肿瘤等异常细胞实施。
在物理切除癌细胞等异常细胞的情况下,可能影响至周边的正常细胞或血管。但是,在利用如上所述的不可逆性电穿孔的情况下,由于仅使靶细胞坏死,因而可使可能对周边组织产生的不利影响最小化。
优选地,第一实施例的第一电极200可以呈盘状。盘状的第一电极200紧贴于胃壁等靶细胞900。由于盘状的第一电极200不插入靶细胞900,因而不引起因插入而造成的细胞损伤。例如,在胃壁使用治疗装置的情况下,若第一电极200插入胃壁,则在治疗之后会在胃壁留下第一电极200插入过的痕迹,即穿孔。这种穿孔可能被胃酸扩张,并且可能对患者造成胃溃疡等其他的副作用。
第一电极200需要向胃壁施加电压,因而需要由高导电性的物质制造。并且,第一电极200也与外壳或主体一样需要耐于胃酸等酸性体液,并且不得向人体排放有害的化学物质。因此,优选地,第一电极200可由316不锈钢(stainless steel)、Co﹣Cr合金、Ti合金等金属材料制造。
第一光纤210及第二光纤220
图4为示出设置于本发明一实施例的电极内的光纤的图。
参照图4,第一电极200包括用于通过测量阻抗来确认对靶细胞的破坏程度的第一光纤210及第二光纤220。即,在第一电极200的至少一面可分别设置有第一光纤210及第二光纤220。
第一光纤210及第二光纤220通过近红外光谱系统来测量因施加电压而引起的靶细胞的被破坏程度。近红外线从第一光纤210朝向靶细胞发射。靶细胞吸收被发射的近红外线,但根据细胞的结构,近红外线的吸收程度可能发生改变。经过靶细胞的近红外线被第二光纤220测出。因此,使用人员可通过对从第一光纤210发射的近红外线的强度和经过靶细胞后被第二光纤220测出的近红外线的强度进行比较来确认细胞的被破坏程度。在此情况下,阻抗为用于评价近红外线的吸收程度的尺度。
由于红外线不易别水分子、血液分子吸收,因而可渗透到活组织。因此,近红外线在确认细胞的破坏方面有利于具有其他波长范围的波动。优选地,可用作近红外线的波动的波长范围为0.75μm至3μm。在波动的波长小于0.75μm的情况下,因波动而可能引起细胞的突变,在波动的波长大于3μm的情况下,波动几乎未被细胞吸收,因而根据细胞结构,可能难以确认波动被细胞吸收的情况。

第一电极200为了进行不可逆性电穿孔而向靶细胞施加的电压可以为1KV以上。在向靶细胞施加小于1KV的电压的情况下,由于发生可逆性电穿孔(reversibleelectroporation),因而靶细胞膜上的穿孔可能重新被封闭。根据可逆性电穿孔,无法执行通过在靶细胞的细胞膜形成永久性的穿孔来使细胞坏死的本发明的治疗方法。
第一电极200具有与内窥镜通道100相连接的固定轴230,从而能够以固定轴230为中心进行旋转。例如,当第一电极200位于内窥镜通道100的外壳内时,第一电极200能够以与外壳平行的方式取向。这具有缩小可使第一电极200经过的外壳的必要直径的效果。并且,第一电极200在到达靶细胞的情况下可与靶细胞的外部的弯曲相应地旋转。人体内部的靶细胞多数呈曲线形态,因而与靶细胞的外部弯曲相应地进行旋转的电极可以进一步紧贴于曲线形态的靶细胞,这提高电压的施加效率。
根据图4,第一光纤210及第二光纤220可以具有光纤孔,以提高阻抗测量精度。光纤孔可使从第一光纤210发射的近红外线直接向第二光纤220移动而不迂回,从而减少因近红外线的迂回而可能发生的靶细胞的破坏程度的测量误差。
第二实施例的治疗装置
图5为具体示出图2所示的电极200的图。
参照图2及图5,通过第二实施例公开如下的治疗装置,上述治疗装置的特征在于,包括:内窥镜通道;以及导管,可通过内窥镜通道插入体内,上述导管包括:穿孔部,位于插入体内的上述导管的末端;以及多个电极,可通过上述穿孔部从上述导管的末端突出来渗透到靶细胞。
本发明第二实施例的装置的内窥镜通道100结构实质上与第一实施例的内窥镜通道100相同。因此,以下对第二实施例的导管300及不同于第一电极的第二电极320进行详细观察。
导管300
本发明第二实施例的治疗装置包括导管300。导管300包括:穿孔部310,位于插入体内的上述导管300的末端;以及多个第二电极320,可通过上述穿孔部310从导管300的末端突出来渗透到靶细胞。
导管300需要耐于胃酸等酸性体液,且不得向人体排放有害的化学物质。因此,导管300可由碳氟化合物、聚酰胺、聚酯、聚酯弹性体等医用高分子材料制造。
在导管300的一侧末端设置有穿孔部310。穿孔部310执行使第二电极320从导管300突出的通道的功能。穿孔部310的数量不受限制,因而普通技术人员可根据需要调节其数量。
本发明第二实施例的第二电极320通过插入靶细胞来向靶细胞施加电压。因此,优选地,第二电极320可制造成探针形态,以便容易插入。第二电极320需要向胃壁施加电压,因而需要由高导电性的物质制造。并且,第二电极320也与导管300同样需要耐于胃酸等酸性体液,且不得排放对人体有害的化学物质。因此,优选地,第二电极320可由316不锈钢、Co﹣Cr合金、Ti合金等金属材料制造。
本发明第二实施例的第二电极320为了进行不可逆性电穿孔而向靶细胞施加的电压可以为1KV以上。在向靶细胞施加小于1KV的电压的情况下,由于发生可逆性电穿孔(reversible electroporation),因而靶细胞膜上的穿孔可能重新被封闭。根据可逆性电穿孔,无法执行通过在靶细胞的细胞膜形成永久性的穿孔来使细胞坏死的本发明的治疗方法。
根据本发明的第二实施例,第二电极320也可设置有多个。优选地,第二电极320的数量可与穿孔部310的数量相同。
多个第二电极320分别在第二电极320内包括阳极及阴极。若电流在第二电极320插入靶细胞的状态下在第二电极320中流动,则在第二电极320内的阳极与阴极之间发生电场。通过上述电场向靶细胞施加电压。由于本发明的第二电极320分别具有阳极及阴极这两者,因而即使若干个第二电极320发生故障,也可通过其他的第二电极320来继续进行治疗。
本发明第二实施例的第二电极320也可包括用于通过测量阻抗来确认对靶细胞的破坏程度的第一光纤及第二光纤。与第一光纤及第二光纤有关的内容与第一实施例的第一光纤210及第二光纤220有关的内容相同。
参照图5,在第二电极320能够以表面弯曲的形态形成至少一个突出部。至少一个突出部的表面呈圆或椭圆形态。并且,在第二电极设置有朝向至少一个方向贯通第二电极的至少一个孔。至少一个孔能够以与至少一个突出部的位置相对应的方式形成。至少一个孔可呈圆或椭圆形态。
当内窥镜通道100插入患者的体内时,尤其,当第二电极320插入组织或靶细胞、胃肠等时,组织或靶细胞、胃肠等会持续运动。由此,第二电极320无法插入组织或靶细胞、胃肠等或即使在插入之后也因移动而重新脱离的情况发生较多。
但是,在第二电极320的表面设置有至少一个突出部或在第二电极320设置有至少一个孔,因而,若增加第二电极320的表面接触面,则可提高第二电极320的表面粘结力。若以第二电极320的表面接触面增加的状态插入组织或靶细胞、胃肠等,则因插入结合力增加而可提高疗效率。
图6为具体示出图2所示的电极的另一图。
与图2一同参照图6,本发明的第三电极330设置有多个,优选地,第三电极330的数量可与穿孔部310的数量相同。
这种多个第三电极330分别以至少一个正(﹢)电极及负(﹣)电极分开构成。即,各个第三电极330可呈阳极电极与阴极电极相平行的形态。在多个正(﹢)电极及负(﹣)电极以相互平行的方式配置的情况下,可提高电压施加效率。
多个正(﹢)电极及负(﹣)电极以相互平行的方式配置的第三电极330插入组织或靶细胞、胃肠等之后,若电流在第三电极330中流动,则在多个正(﹢)电极及负(﹣)电极之间发生电场。通过上述电场来向靶细胞施加电压。像这样,若多个正(﹢)电极及负(﹣)电极以相互平行配置的状态插入组织或靶细胞、胃肠等,则因插入结合面积增加而可提高治疗效率。
另一方面,在以相互平行的形态分开构成的正(﹢)电极及负(﹣)电极能够以表面弯曲的形态设置有至少一个突出部。
至少一个突出部的表面呈圆或椭圆形态。并且,在正(﹢)电极及负(﹣)电极设置有朝向至少一个方向贯通的至少一个孔。至少一个孔能够以与至少一个突出部的位置相对应的方式形成。至少一个孔可呈圆或椭圆形态。
在正(﹢)电极及负(﹣)电极的表面形成至少一个突出部或至少一个孔,因而,若正(﹢)电极及负(﹣)电极的表面接触面增加,则可提高正(﹢)电极及负(﹣)电极的表面粘结力。若以正(﹢)电极及负(﹣)电极的表面接触面增加的状态插入组织或靶细胞、胃肠等,则因插入结合力增加而可提高治疗效率。
以上,参照附图及实施例来进行了说明,但本发明所属技术领域的普通技术人员可以理解可在不脱离以下发明要求保护范围中所记载的实施例的技术思想的范围内以多种方式修改及变形实施例。
Claims (4)
1.一种利用内窥镜联动式电极的治疗装置,其特征在于,
包括:
内窥镜通道;以及
导管,能够通过内窥镜通道插入体内,
上述导管包括:
穿孔部,位于插入体内的上述导管的末端;以及
多个电极,能够通过上述穿孔部从上述导管的末端突出来渗透到靶细胞,
上述多个电极均设置为探针形状,且探针形状的上述电极上设置有表面以圆或椭圆形态弯曲的至少一个突出部,
至少一个贯通孔被配置成圆形或椭圆形以对应于上述至少一个突出部的位置,
上述电极通过向上述靶细胞施加1KV以上的电压来破坏上述靶细胞。
2.根据权利要求1所述的利用内窥镜联动式电极的治疗装置,其特征在于,多个上述电极分别包括阳极及阴极。
3.根据权利要求1所述的利用内窥镜联动式电极的治疗装置,其特征在于,上述电极包括用于通过测量阻抗来确认对靶细胞的破坏程度的第一光纤及第二光纤。
4.根据权利要求1所述的利用内窥镜联动式电极的治疗装置,其特征在于,多个上述电极以阴极电极和阳极电极分开构成,上述阴极电极和上述阳极电极以平行的方式配置。
