Abstract
The invention discloses a device and a method for widening an attached gingiva for oral implantation repair, which comprises an arc-shaped substrate, screw caps penetrating through the upper surface and the lower surface of the arc-shaped substrate, an arc-shaped cavity arranged in the arc-shaped substrate, a plurality of micropores arranged on the lower surface of the arc-shaped substrate, a liquid anti-inflammatory drug arranged in the arc-shaped cavity, M through holes arranged on the upper surface of the arc-shaped substrate and an arc-shaped cover plate, wherein the arc-shaped substrate is provided with a plurality of through holes; the arc-shaped cover plate is provided with M bulges, each bulge is respectively matched and connected with each through hole, and M is larger than 5. The invention has the characteristics of ensuring that the implant is fixed in the affected area, eliminating hematoma, achieving higher survival rate of free gingival flap and improving the success rate of dental implant surgery.
Description
Technical Field
The invention relates to the technical field of equipment related to dental implants, in particular to a device and a method for widening an attached gingiva for oral implant restoration.
Background
The gingiva attached around the implanted periodontal can resist the invasion of bacterial root, improve the health condition of the implanted periodontal, and prevent bone resorption. Some patients have the condition that the implanted teeth are attached to the gum and the gum loss can reduce the width of the effective keratinized mucous membrane, aggravate the inflammatory reaction of the mucous membrane around the implanted teeth and increase the accumulation of bacterial plaque.
The missing or defective attached gingiva also results in alveolar bone resorption at the neck of the implant and loss of attachment capability, resulting in failure of the implant surgery. Therefore, it is necessary to take measures to increase the width of the attached gingiva around the implanted tooth. Currently, repair of defects is mainly addressed by free gum transplantation. The traditional widening surgery for implanting periodontal attached gingiva often fails due to the lack of good blood supply and the poor combination of skin flaps of the affected area.
Disclosure of Invention
The invention aims to overcome the defect that the surgery for widening the implanted periodontal attached gingiva in the prior art is easy to fail, and provides a device and a method for widening the implanted gingiva by oral implantation repair.

The arc-shaped substrate is used for covering the gum after the gum soft tissue is subjected to incremental surgery, so that the transplanted palatal free gum flap and the original gum can grow together as soon as possible, the function of fixing the position of the free gum flap is achieved, the graft is more easily ensured to be fixed in a receiving area, hematoma is eliminated, the higher survival rate of the free gum flap is achieved, and the success rate of the dental implant surgery is improved.
Preferably, a cylinder is arranged on the lower surface of each through hole, and a gap is formed between the lower end of each cylinder and the lower surface of the cavity; a floating ball is arranged in the cylinder, and the diameter of the floating ball is larger than the clearance.
The arrangement of each floating ball is convenient for amplifying the vibration of the liquid anti-inflammatory drug in the cavity of the arc-shaped substrate, so that the liquid anti-inflammatory drug permeates out to diminish the inflammation of the implant.
Preferably, the liquid anti-inflammatory agent comprises liquid penicillin and glycerol. Glycerol can provide lubrication, make the surgical site softer, and reduce the pain experienced by the patient.
A method based on a device for oral implant repair of attached gum widening, comprising the steps of:
(4-1) windowing and taking a model at the position where the patient needs to implant the tooth, manufacturing an intra-oral model of the patient by using the superhard gypsum, and drawing an edge line of the arc-shaped substrate on the model;
(4-2) installing a hollow screw at the dental implant of the patient, manufacturing an arc-shaped base plate and an arc-shaped cover plate by using a material for manufacturing the temporary tooth according to the edge line, and polishing the arc-shaped base plate and the arc-shaped cover plate;
(4-3) after the increment operation is carried out on the soft gum tissues around the hollow screw, inserting the lower part of the nut into the hollow screw, extending the long screw into the nut and the hollow screw, and screwing the long screw, the nut and the hollow screw by using a tool; the arc-shaped substrate and the hollow screw are fixedly installed;
(4-4) covering the arc-shaped cover plate on the arc-shaped base plate, and enabling the protrusions to be respectively matched and connected with the through holes;
when a patient speaks or eats, the arc-shaped substrate vibrates, and the liquid anti-inflammatory medicine leaks out from the micropores to diminish inflammation of the gum after incremental surgery.
Therefore, the invention has the following beneficial effects: the position of the free gingival flap is fixed, the implant is ensured to be fixed in a receptive area, hematoma is eliminated, the higher survival rate of the free gingival flap is achieved, and the success rate of the dental implant surgery is improved.

The embodiment shown in fig. 1 and 2 is a device for widening an attached gingiva for oral implant restoration, which comprises an arc-shaped substrate 1, a screw cap 8 penetrating through the upper surface and the lower surface of the arc-shaped substrate, an arc-shaped cavity 2 arranged in the arc-shaped substrate, a plurality of micropores 3 arranged on the lower surface of the arc-shaped substrate, a liquid anti-inflammatory drug arranged in the arc-shaped cavity, 12 through holes 4 arranged on the upper surface of the arc-shaped substrate, and an arc-shaped cover plate 5; the arc-shaped cover plate is provided with 12 bulges 51, and each bulge is respectively matched and connected with each through hole.
As shown in fig. 1, a cylinder 6 is arranged on the lower surface of each through hole, and a gap is arranged between the lower end of the cylinder and the lower surface of the cavity; a floating ball 7 is arranged in the cylinder, and the diameter of the floating ball is larger than the clearance. as shown in fig. 3, (4-2) installing a hollow screw 9 at the implant site of the patient, making an arc-shaped base plate and an arc-shaped cover plate according to the edge line by using the material for making the temporary tooth, and polishing the arc-shaped base plate and the arc-shaped cover plate; the material of the temporary tooth is a Lopa Saite temporary crown material.
(4-3) after the increment operation is carried out on the soft gum tissues around the hollow screw, inserting the lower part of the nut into the hollow screw, extending the long screw 10 into the nut and the hollow screw, and screwing the long screw, the nut and the hollow screw by using a tool; the arc-shaped substrate and the hollow screw are fixedly installed; taking a free gingival flap of the palate of a patient for implanting periodontal soft tissue increment; the force applied when screwing the long screw is 20N;
(4-4) covering the arc-shaped cover plate on the arc-shaped base plate, and enabling the protrusions to be respectively matched and connected with the through holes;
when a patient speaks or eats, the arc-shaped substrate vibrates, and the liquid anti-inflammatory medicine leaks out from the micropores to diminish inflammation of the gum after incremental surgery.
And (3) removing the arc-shaped cover plate and the arc-shaped base plate 10 days after the incremental surgery, ensuring that the soft tissue of the gum is well healed, removing the suture line of the surgical area, windowing and taking a model of the conventional dental implant, and manufacturing the permanent upper prosthesis.
In addition, the arc-shaped substrate can be manufactured by a 3D printing method, when the implant bone is well combined, CBCT of the oral cavity of a patient is shot, three-dimensional data such as the position of the implant and the thickness of soft tissue are obtained, a three-dimensional scanner is used for scanning to obtain STL data of the three-dimensional surfaces of dentition and gum of the patient, digital software is used for comparing intraoral scanning data before an operation with an ideal postoperative effect, the space required by the soft tissue amount required by the soft tissue increment operation is calculated, the surface area and the thickness of the arc-shaped substrate are obtained, and 3D printing of the arc-shaped substrate is carried out.

the manufacturing method of the device comprises the following steps:
(1-1) windowing and taking a model at a position where a patient needs to implant a tooth, manufacturing an intra-oral model of the patient by using superhard gypsum, and drawing an edge line of an arc-shaped substrate on the model;
(1-2) installing a hollow screw (9) at a dental implant of a patient, manufacturing an arc-shaped base plate and an arc-shaped cover plate by using a material for manufacturing a temporary tooth according to an edge line, and polishing the arc-shaped base plate and the arc-shaped cover plate;
(1-3) inserting the lower part of the nut into the hollow screw, extending the long screw (10) into the nut and the hollow screw, and screwing the long screw, the nut and the hollow screw by using a tool; the arc-shaped substrate and the hollow screw are fixedly installed;
(1-4) covering the arc-shaped cover plate on the arc-shaped base plate to enable the protrusions to be respectively matched and connected with the through holes;
when a patient speaks or eats, the arc-shaped substrate vibrates, and the liquid anti-inflammatory medicine leaks out from the micropores to diminish inflammation of the gum after incremental surgery.
2. The device for widening attached gingiva for oral implant restoration according to claim 1, wherein a cylinder (6) is provided on the lower surface of each through hole, and a gap is provided between the lower end of the cylinder and the lower surface of the cavity; a floating ball (7) is arranged in the cylinder, and the diameter of the floating ball is larger than the clearance.
3. The device for oral implant repair attached gum widening according to claim 1, wherein the liquid anti-inflammatory agent comprises liquid penicillin and glycerol.
