Abstract

本发明属于分子诊断技术领域,公开了Wnt5a作为种植体周围炎诊断标志物的应用。本发明提出,通过检测种植体周围组织或种植体周围液中的Wnt5a的表达水平,可以判断受试者患有种植体周围炎的风险或者确诊种植体周围炎的状态。相比传统的种植体周围炎的诊断方法,使用Wnt5a作为蛋白标志物来诊断种植体周围炎,具有及时性、特异性和灵敏性等显著优点,患者可在种植体周围炎发生的早期就知晓疾病风险,针对风险高低采取相应的预防和治疗措施。

Wnt5a作为种植体周围炎诊断标志物的应用
技术领域
本发明属于分子诊断技术领域,具体涉及Wnt5a的一种新应用。
背景技术
口腔种植技术不断发展,种植修复逐渐在各类牙列缺损、牙列缺失的修复中占据越来越重要的位置。种植义齿以其舒适、美观、稳固、咀嚼功能强、不损伤邻牙等独特优势备受青睐。但是,种植失败的现象也时有发生,其最主要的原因可分为生物并发症和机械并发症两类。其中,最常见的生物并发症即种植体周围炎(Peri-implantitis)(Graziani F,Figuero E,Herrera D.Systematic review of quality of reporting,outcomemeasurements and methods to study efficacy of preventive and therapeuticapproaches to peri-implant diseases.J Clin Periodontol.2012;39Suppl 12:224-244.)。
种植体周围炎是发生在种植体周围软硬组织的炎症性疾病。其早期为局限于种植体周围软组织的可逆的种植体周围黏膜炎,炎症继续发展引起种植体周围骨组织的吸收,严重者最终导致骨结合丧失、种植失败(Valente NA,Andreana S.Peri-implant disease:what we know and what we need to know.J Periodontal Implant Sci.2016;46:136-151.)。种植体周围炎是种植体脱落的主要原因,在中国发病率约为22%。欧洲第六届牙周病学研讨会关于种植体周围炎的共识报告指出,种植患者中28%-56%的个体、12%-43%的种植体发生种植体周围炎(Lindhe J,Meyle J;Group D of European Workshop onPeriodontology.Peri-implant diseases:Consensus Report of the Sixth EuropeanWorkshop on Periodontology.J Clin Periodontol.2008;35:282-285.)。
目前种植体周围炎的诊断方法主要包括探诊检查和影像学检查。其中探诊深度、探诊出血、溢脓可作为诊断种植体周围炎的常规指标,影像学检查为评估种植体周围支持骨水平的必要指标。但目前难以通过这一方法进行早期诊断,往往使患者错失了早期治疗的机会。所以目前亟需方便有效的生物标志物能帮助种植体周围炎早期诊断,并提高诊断的正确性。
发明内容
本发明的目的在于提供Wnt5a作为种植体周围炎诊断标志物的应用以及检测Wnt5a表达的试剂在制备诊断种植体周围炎的试剂盒中的应用,上述应用可有助于实现临床上对种植体周围炎的早期、精确诊断。
为解决上述技术问题,本发明的实施方式提供了Wnt5a作为种植体周围炎诊断标志物的应用。
本发明首次提出了Wnt5a表达与种植体周围炎相关,通过检测受试者中Wnt5a的表达,可以判断受试者是否患有种植体周围炎、或者判断受试者是否存在患有种植体周围炎的风险,从而指导临床医师给受试者提供预防方案或者治疗方案。相比传统的种植体周围炎的诊断方法,使用Wnt5a作为蛋白标志物来诊断种植体周围炎,具有及时性、特异性和灵敏性等显著优点,从而使患者在疾病早期就能知晓疾病风险,针对风险高低采取相应的预防和治疗措施。
进一步地,基于Wnt5a表达水平与种植体周围炎之间的相关性,本发明的实施方式还提供了检测Wnt5a表达的试剂在制备诊断种植体周围炎的试剂盒中的应用。其中,所述诊断种植体周围炎的试剂盒用于检测种植体周围组织或种植体周围液中的Wnt5a表达水平;如种植体周围组织或种植体周围液中的Wnt5a表达水平升高,则提示种植体周围炎的发生。
更进一步地,本发明的实施方式所提供的诊断种植体周围炎的试剂盒中,检测Wnt5a表达的试剂包括:基于RT-PCR、实时定量PCR、免疫检测、原位杂交、芯片或高通量测序检测Wnt5a表达水平的试剂。具体地,所述基于RT-PCR检测Wnt5a表达水平的试剂至少包括一对特异扩增Wnt5a的引物;所述基于实时定量PCR检测Wnt5a表达水平的试剂至少包括一对特异扩增Wnt5a的引物;所述基于免疫检测技术检测Wnt5a表达水平的试剂至少包括与Wnt5a特异性结合的抗体;所述基于原位杂交技术检测Wnt5a表达水平的试剂至少包括与Wnt5a的核酸序列杂交的探针;所述基于芯片技术检测Wnt5a表达水平的试剂至少包括:蛋白芯片和基因芯片;所述蛋白芯片包括与Wnt5a特异性结合的抗体,所述基因芯片包括与Wnt5a的核酸序列杂交的探针;所述基于高通量测序平台检测Wnt5a表达水平的试剂至少包括:用于检测Wnt5a转录水平的引物和/或探针。
在本发明的实施方式中,所提及的“Wnt5a”包括Wnt5a以及Wnt5a的任何功能等同物的多核苷酸。Wnt5a(Chromosome 3,NC_000003.12(55465715..55505261))序列可在国际公共核酸序列数据库GeneBank中查询到,是Wnt蛋白家族中的成员之一。在本发明的上下文中,Wnt5a表达产物包括Wnt5a以及Wnt5a的部分肽。所述Wnt5a的部分肽含有与种植体周围炎相关的功能域。“Wnt5a”包括Wnt5a以及Wnt5a的任何功能等同物。所述功能等同物包括Wnt5a保守性变异蛋白质、或其活性片段,或其活性衍生物,等位变异体、天然突变体、诱导突变体、在高或低的严紧条件下能与Wnt5a的DNA杂交的DNA所编码的蛋白质。通常,已知的是,一个蛋白质中一个或多个氨基酸的修饰不会影响蛋白质的功能。本领域技术人员会认可改变单个氨基酸或小百分比的氨基酸或对氨基酸序列的个别添加、缺失、插入、替换是保守修饰,其中蛋白质的改变产生具有相似功能的蛋白质。提供功能相似的氨基酸的保守替换也是本领域公知的。
通过添加一个氨基酸或多个氨基酸残基修饰的蛋白质的例子是Wnt5a的融合蛋白。对于与Wnt5a融合的肽或者蛋白质没有限制,只要所得的融合蛋白保留Wnt5a的生物学活性即可。在本发明的上下文中,“诊断种植体周围炎”既包括判断受试者是否已经患有种植体周围炎、也包括判断受试者是否存在患有种植体周围炎的风险。

具体实施方式
为使本发明的目的、技术方案和优点更加清楚,下面将对本发明的各实施方式进行详细的阐述。然而,本领域的普通技术人员可以理解,在本发明各实施方式中,为了使读者更好地理解本申请而提出了许多技术细节。但是,即使没有这些技术细节和基于以下各实施方式的种种变化和修改,也可以实现本申请各权利要求所要求保护的技术方案。
实施例1Western-blot检测种植体周围组织中的Wnt5a在种植体周围炎患者和正常种植体人群中的表达差异
1、研究对象:收集种植体周围炎患者8例,男4例,女4例,年龄38-61岁。入组标准:诊断标准均符合种植体周围炎临床诊断标准(参考“Schwarz F,Sculean A,Rothamel D,Schwenzer K,Georg T,Becker J.Clinical evaluation of an Er:YAG laser fornonsurgical treatment of peri-implantitis:a pilot study.Clin Oral ImplantsRes 2005;16:44-52.”)。排除标准:(1)在过去六个月内接受过口服或全身去污(冲洗或抗生素)治疗的患者;(2)未控制的牙周病患者;(3)全身性疾病患者;(4)吸烟者;(5)孕妇和哺乳母亲。此项研究己通过医院伦理委员会批准且所有患者签署知情同意书。对照组:选取年龄37-59岁的健康种植体志愿者8例,男4例,女4例,。两组之间年龄、性别差异无统计学意义(P>0.05),具有可比性。剪取部分种植体周围组织用于进一步分析。
3、Western-blot检测Wnt5a的表达:使用二辛可宁酸测定法定量种植体周围组织裂解液中的总蛋白质含量。然后,将20μg总蛋白加载到10%聚丙烯酰胺凝胶孔中,通过电泳分离,并转移到硝酸纤维素膜。将膜与抗Wnt5a一抗一起孵育,在4℃冰箱过夜。然后将它们与相应的HRP标记的二抗一起孵育,并使用Fusion Solo系统用ECL底物显色印迹。
4、统计学处理:化学发光定量并通过直方图呈现。采用t检验确定Western-blot结果的统计学显着性,所有数据均表示为平均值±SD,P<0.05差异有显著性意义。
5、结果:检测结果如附图1、2所示。附图1是本实施例中利用Western-blot检测种植体周围组织中的Wnt5a在种植体周围炎患者和正常种植体人群中表达差异的结果图;附图2是本实施例中利用Western-blot检测种植体周围组织中的Wnt5a在种植体周围炎患者和正常种植体人群中表达差异的相对表达量柱状图。与正常种植体人群相比,种植体周围炎患者的种植体周围组织中Wnt5a水平显著升高,差异具有统计学意义(P<0.05)。
本领域的普通技术人员可以理解,上述各实施方式是实现本发明的具体实施例,而在实际应用中,可以在形式上和细节上对其作各种改变,而不偏离本发明的精神和范围。

2.检测Wnt5a表达的试剂在制备诊断种植体周围炎的试剂盒中的应用。
3.根据权利要求2所述的应用,其特征在于,所述诊断种植体周围炎的试剂盒用于检测种植体周围组织或种植体周围液中Wnt5a的表达水平。
4.根据权利要求3所述的应用,其特征在于,所述检测Wnt5a表达的试剂包括:基于RT-PCR、实时定量PCR、免疫检测、原位杂交、芯片或高通量测序检测Wnt5a表达水平的试剂。
5.根据权利要求2所述的应用,其特征在于,
所述基于RT-PCR检测Wnt5a表达水平的试剂至少包括一对特异扩增Wnt5a的引物;
所述基于实时定量PCR检测Wnt5a表达水平的试剂至少包括一对特异扩增Wnt5a的引物;
所述基于免疫检测技术检测Wnt5a表达水平的试剂至少包括与Wnt5a特异性结合的抗体;
所述基于原位杂交检测Wnt5a表达水平的试剂至少包括与Wnt5a的核酸序列杂交的探针;
