Description

Nasobiliary external drainage tube for preventing papillary hemorrhage and application thereof
Technical Field
The invention relates to a nasobiliary drainage tube and application thereof, in particular to a nasobiliary external drainage tube for preventing papillary hemorrhage and application thereof.
Background
With the continuous development of endoscopic technology and the continuous improvement of accessories thereof, the duodenal endoscopic technology (ERCP) is an important measure for treating diseases of liver, gallbladder and pancreas, including choledocholithiasis, bile duct injury, bile duct stenosis, biliary tract complications after liver transplantation and the like. Compared with the traditional laparotomy, the technique has the advantages of small wound, high safety factor, quick recovery and the like. The patient can observe under direct vision, and can detect diseases of liver, gallbladder and pancreas which can not be detected by conventional examination of liver, gallbladder, spleen and pancreas, ultrasound, magnetic resonance and the like, and treat the diseases simultaneously, such as duodenal papillary inflammation, papillary stenosis, diverticulum beside the papilla, overlong duodenal papillary, biliary-pancreatic confluence abnormality and the like.
Endoscopic lower papillary sphincter incision and endoscopic nasobiliary drainage are conventional technologies in ERCP operation, and can be directly applied to treatment of obstructive jaundice, acute suppurative cholangitis, biliary leakage and other diseases; the postoperation indwelling nasobiliary duct can fully drain bile, reduce the pressure of biliary tracts, reduce the probability of postoperation pancreatitis and cholangitis, and simultaneously observe the property and outflow condition of the bile; for patients with acute suppurative cholangitis, the infection of biliary system can be controlled by injecting sensitive antibiotics through the nasal biliary drainage tube; the nasal bile duct instillation of hemostatic drugs such as adrenalin and thrombin has the function of preventing the delayed biliary tract hemorrhage.
The head end of the nasal biliary drainage tube for clinical application can automatically roll and squat in the bile duct, and the drainage tube has the following defects: 1. the head end of the drainage tube is naturally bent, and the anti-slip function is realized by the tension of the head end of the drainage tube on the wall of the biliary tube, but the supporting force is small, and the drainage tube is easy to slip out due to the nipple of duodenum; 2. irritation of the head end of the drainage tube to the bile duct mucosa can result in local mucosal reactive hyperplasia; 3. in the process of injecting the hemostatic drug through the ENBD tube, the drainage effect on infectious bile is lost, and the bile pressure can be increased to cause bile reflux and aggravate cholangitis; 4. when the biliary tract radiography determines that residual stones with small residues exist, secondary ERCP calculus removal treatment is needed; 5. in the process of tube drawing, the head end of the drainage tube can cause the injury of bile ducts and intestinal mucosa, and can even induce complications such as acute cholangitis, acute pancreatitis, upper gastrointestinal hemorrhage and the like; 6. stimulation of the curved tip of the drainage tube to the throat often results in severe nausea and even vomiting symptoms in the patient.
