Abstract

Description
本发明是申请日为2013年7月15日,申请号为201380047722.6,名称为“确定运动障碍的设备”的专利申请的分案申请。
技术领域
本发明涉及一种确定患者表现出的运动障碍的存在和类型的设备。该设备可以由患者穿戴,并不妨碍他们的日常生活。
背景技术
左旋多巴引起的运动障碍(LID)的临床治疗是帕金森病(PD)的治疗中最具挑战的问题之一。主要难题之一是运动障碍发作时患者和临床医生常常无法具体描述,因此尝试改变可能不熟悉的药物,导致几乎没有进展,或者甚至使问题恶化。
由于一些原因,可能发生对运动障碍的不准确记录。首先,存在集中运动障碍的子类型,如在峰值剂量时发生的舞蹈病样运动,低谷剂量时发生的肌张力障碍以及在血浆药物的剂量增加和降低时均发生的双相障碍。这使得对于医师、当然对于患者来说难以描述特征以及每个症状的具体时间。例如,峰值剂量的运动障碍可能看起来与PD颤抖相似-都是不自觉的运动-但是前者是由于高药物水平(on),而后者是亚治疗水平(off)。如果药物的剂量朝错误的方向调整的话,对不自觉的运动的不准确标记可能因此导致问题无意地恶化。另外晚期PD的患者可能快速从处于off状态的颤抖状态移动至处于on状态的运动障碍状态,进一步使情况复杂化。
第二,尽管患者和/或临床医生正确地识别了运动障碍的不同类型,但是他们难以精确描述这些时期的延长时间,而不对患者的日常活动造成明显妨碍。如果不知道症状的时间,那么治疗医师在决定哪个特定的剂量需要改变上具有难度。一些患者能够在几天内细心地描述他们自己的症状来帮助医师,但这显然是非常麻烦,一种能够记录该信息的电子装置对于患者来说会容易得多,并且对于医师来说更精确。如果运动障碍极其麻烦,且当门诊患者无法成功掌控这些运动障碍的话,则患者可能需要被允许到病房进行详细监视。这需要专科护士或初级医生来每隔一小时左右进行评估并在表格上为顾问医生记录这些信息,然后用于引导治疗决策。这显然是一种昂贵且耗时的方法,但可能是应对具有复杂症状和药物状况的唯一方式。
第三,当运动障碍变得难以解决时PD,药物作用通常很复杂,强调这点是很重要的。也就是说,通常患者会服用两类或更多类的药物,一般是左旋多巴为基础的药物(诸如信尼麦或Stalveo),和多巴胺兴奋剂药物(诸如普拉帕索或罗平尼咯),左旋多巴为基础的药物可能每天服用3-6次(或者对于严重的疾病来说甚至为10次),多巴胺兴奋剂药物通常每天服用1-4次,因此,对于医师可能仅仅根据运动障碍来改变药物的说法是过于简单化的。在处于on状态(高药物水平)时,通常患者会具有运动障碍的时期,与处于off的状态(亚治疗水平)交替。在峰值剂量的运动障碍时期之前所给的药物需要降低或剂量的频率需要降低,并且在off状态时期前所给的药物需要提高。当24小时期间高达10中药物剂量被使用时这显然是一项非常精细的任务,并且精确记录运动障碍确切地是什么时候发生的是极为重要的。
第四,记录突发症状的技术方法被运用于神经学(如癫痫症患者的视频遥测)和内科学(心脏病的阵发性心肌炎的霍尔特(Holter)监视记录)的其他领域中,但目前没有应用于帕金森疾病的运动障碍的同样的方法。这是不可思议的,因为PD是一种非常普通的情况,影响着大约1%的60岁以上的人们,并且运动障碍在大约40%的服用左旋多巴4-6年的PD患者以及几乎90%的治本发明疗了10年的PD患者中有所发展。
发明内容
根据本发明,提供了一种确定运动障碍在治疗对象内的程度的设备,该设备包括与治疗对象的肢体、躯干或头部可拆卸地连接的传感器,所述传感器包括检测3-D运动的第一检测器,
保持检测器所产生的数据的数据保持装置;
将所述数据传递到数据保持装置的传递装置;
处理装置,处理数据保持装置内的数据并分类处理后的数据;
输出装置,将诊断状况显示给用户;
其中所述处理装置在分类所述数据并与已知的运动障碍情况相对比时使用了进化算法。
该设备允许与治疗对象进行接触式监视,不需要临床医生出席,并且对治疗对象的状况快速进行精确分析。这代表节省了临床医生的时间,并且由于传感器的非侵入式的特性,也会减小治疗对象的不适。
方便地,第一运动检测器包括一个或多个加速计,以便提供3-D运动数据。
优选地,该设备包括第二运动检测器,以便检测设备的倾斜、滚动以及偏航。
方便地,该设备包括另一个运动检测器,用于测量位置数据,另外方便地,参考笛卡儿坐标来确定位置数据,该位置数据可以通过处理装置来处理,产生速度和/或加速数据。
优选地,传递装置是面向远程数据保持装置的无线传送装置,这允许用户佩戴较小的设备,所述设备不是必须包括处理元件,也允许治疗对象在家里时被评估,同时数据集中在医院或诊所被处理。

具体实施方式
尽管帕金森疾病可以通过口服左旋多巴药物来治疗,但是正确地服用的剂量是非常重要的。不足的和过量的左旋多巴可能导致患者经受不必要的影响。由于多种因素,情况是复杂的。首先,通常关注的是很可能处于意识的昏迷状态的患者是否在正确地服用规定的药物。第二,错误剂量的影响可能非常常见,尤其对于未经训练的眼睛来说彼此是无差别的。例如,对于左旋多巴水平太高的人,被误诊为相反的问题并给予附加剂量的药物,以及对于水平极低的患者,停止服用药物,这些无疑是未知的。
通常,精确评估患者的状态的唯一方式是在医院里在高医术的医生的细心监督下进行封闭监视。
本发明提供了一种对患者的运动进行监视的设备,该设备包括用于有效确定患者所展示的症状并且分类所述症状以提供对患者的评估的装置。那么这使得正确的治疗得以实施,以解决上述的第二个问题,并且在一些情况下避免了第一个问题。
在最广泛的方面,本发明构想了一种用于确定运动障碍的设备,该运动障碍包括患者所表现出的运动障碍的类型。运动障碍的类型与患者体内的左旋多巴的水平有关。设备、或多种设备记录了接受调查的治疗对象的运动。然后针对给定的生物标志对该运动进行分析,这些生物标志显示了特定运动障碍的类型。
在本发明的一个实施例中,治疗对象患者佩戴者一些传感器,通常是八个,在其肢体周围,并且也连接到他们的头部和躯干。这些传感器记录了肢体的方向、速度以及加速度。这样可以在连续、自动的基础上收集数据,且治疗对象没有不适,治疗对象可以在他们的正常环境中,也不需要执业医师出席来观察治疗对象。
如图1所示,传感器是无线速度计陀螺仪,其足够小以系在治疗对象上并由治疗对象佩戴。与患者的连接可以是传统的方式,如VelcroTM。传感器包括六个测量装置在三个相互垂直的方向x、y、z上以及针对倾斜、滚动和偏航(如关于那三个方向中的每个进行旋转)来测量运动。根据传感器内的计时机构以大约100Hz的速率对数据进行采样。可以预期诸如10、20、60Hz,以及120、150、200Hz等的其他数据采集速率。技术人员可根据所期望的精确度和可利用的数据的记忆容量来决定所使用的速率。提高数据位数的数量通常要求更大的数据存储容量。
任何需要传送的数据可以通过,例如蓝牙,被传送到智能手机。当然可以使用可替换的装置。如果分析装置没有被包括在每个传感器内,那么可以远程实施数据处理。
处理后的数据的例子如下。第一,单个传感器的运动的速度数据可以从加速计所获得的数据获得,或者其他位置数据可以从几何学的应用中获得。因此,例如在两个不同的数据采样时间(t-1)和t期间移动的距离d,是d=sqr[(xt-xt-1)2+(yt-yt-1)2+(zt-zt-1)2],其中x、y和z表示在笛卡儿坐标中。这转化成速度d/Δt,其中Δt是t和t-1之间的时间。对于100Hz的采样速率,Δt是0.01秒。
另外,角位移可以利用下述值来确定:abs(rollt-rollt-1)+abs(pitcht-pitcht-1)+abs(yawt-yawt-1),其中abs表示操作“取绝对值”,roll表示滚动,pitch表示倾斜,yaw表示偏航。
一经计算,速度数据呈现给分析装置,以确定主题所表现出的运动障碍的类型。
分析装置采用在GB1100794.5中公开的进化分析方法学以帮助作出处理决定。作出处理决定的初始培训阶段被使用并具体如下,其中来自设备的数据被传递并通过方法学处理以预测之前在方法学中登记的五种情况之一。然后当另外的情况被识别时,这些情况可以被包括在方法学处理中。
构成进化分析的算法被执行很多次。每次执行产生一个或多个分类。然后通过选择在进化算法的所有执行的过程中所发现的最大区别分类的子分类来产生整体分类。可以通过下述方式完成最大区别分类的选择:(i)对数据的不同子分类实施不同的进化算法的运行,或—(ii)事后分析,其中每个分类的行为被明确地区分,且具有最小行为交迭的那些行为被选作整体分类。在意义上,行为可以是分类对数据的不同子分类的不同响应,或者分类对数据中的特定模式的识别能力。
另外,角位移数据通过第二进化分析方法学进行类似的处理,然后再次将该结果与之前登记的情况进行对比,来提供对治疗对象的评估。
来自速度和角位移分类的数据可以结合在一种整体分类分析方法学中,在获得结论上产生更高的精确度。另外,可以使用人工生物网络(ABN)来结合进化算法和整体分类。
受分析的数据的结果可以与产生分类结果相匹配,特别地,该分类可以用于分析方法学的培训阶段,产生该分类的其中一个方式涉及利用一个或多个资深的临床医生来实施评估。例如,临床医生可以对不同的运动障碍的每种类型赋值,例如1-4,可以考虑使用更大范围的值,尽管这提高了评估赋予哪个更有限的值的难度。例如,可以使用的四类是1)最小的,2)轻微的,3)适中的,4)严重的/不适合的。可以以这样的方式进行分类的明显的运动障碍的例子是a)舞蹈病样运动障碍,b)张力障碍运动障碍,c)其他运动障碍,d)颤抖,e)运动徐缓。另外,可以考虑其他运动障碍也可以被描述为对技术人员来说明显的特征。以及运动障碍(涉及药物左旋多巴的副作用)与帕金森疾病的症状之间的区别。
一旦利用进化算法来尝试识别运动障碍,如运动徐缓(帕金森疾病的主要症状,缓慢的运动)以及帕金森静止性震颤,对设计表达进行检查来鉴别所有治疗患者的运动混乱的那些共性方面,该运动混乱大多归因于设计表达。可以针对那些特定的方面上对第二进化算法进行培训,来设计更有差异的表达。
当然,可以理解的是本发明不限于在此仅仅通过实例给出的所描述的特定细节,在本发明的精神范围内还存在多种修改和变化。
Claims (10)
1.一种确定治疗对象的运动障碍的程度的设备,所述设备包括:
可拆卸地连接至治疗对象的肢体、躯干或头部的传感器,所述传感器包括检测3-D运动的第一检测器,
数据保持装置,所述数据保持装置用于保持检测器所产生的数据;
传递装置,所述传递装置将所述数据传递到所述数据保持装置;
处理装置,所述处理装置用于处理所述数据保持装置内的数据并对处理后的数据进行分类;
输出装置,所述输出装置将诊断状况显示给用户;
其中所述处理装置在分类所述数据并与已知的运动障碍情况相对比时使用了进化算法。
2.根据权利要求1所述的确定治疗对象的运动障碍的程度的设备,其中所述第一运动检测器包括用于提供3-D运动数据的一个或多个加速计。
3.根据权利要求1或2所述的确定治疗对象的运动障碍的程度的设备,其中所述装置包括用于检测所述设备的倾斜、滚动和偏航的第二运动检测器。
4.根据前述任何一项权利要求所述的确定治疗对象的运动障碍的程度的设备,其中所述设备包括用于测量位置数据的另一个运动检测器。
5.根据权利要求4所述的确定治疗对象的运动障碍的程度的设备,其中所述位置数据是参考笛卡尔坐标来确定的。
6.根据权利要求4或5所述的确定治疗对象的运动障碍的程度的设备,其中所述处理装置将位置数据转换为速度和/或加速度数据。
7.根据前述任何一项权利要求所述的确定治疗对象的运动障碍的程度的设备,其中所述传递装置是面向远程数据保持装置的无线传送装置。
8.根据前述任何一项权利要求所述的确定治疗对象的运动障碍的程度的设备,其中所述设备包括多个传感器。
9.根据前述任何一项权利要求所述的确定治疗对象的运动障碍的程度的设备,其中所述传感器或每个传感器包含10-200Hz速率的数据读数。
10.根据权利要求8所述的确定治疗对象的运动障碍的程度的设备,其中所述数据读数在大约100Hz的速率下获得。
