Description
技术领域
本发明涉及医学成像领域的方法和设备。本发明尤其适用于计算机断层摄影(CT)血管造影,特别是肝脏成像。
背景技术
肝癌是一种主要的疾病,每年约有75万人死亡。多阶段肝脏CT成像是一种用于诊断、分期和处置监测的主要工具。对多阶段造影剂动态进行成像并用于识别不同的病变类型。肝脏有两种供血:肝动脉和门静脉。这样,在给定时间处注入的造影剂将在肝脏的主动脉供血和门静脉供血中在两个不同时间处到达。由于某些病变类型在造影剂摄取和冲洗动态方面的表现不同,因此对主动脉子网和门静脉子网两者进行成像是有意义的。例如,肝实质主要从门静脉系统接收血液,而肿瘤通常由肝动脉供血。

为了实施根据本发明的方法,必须向患者预先注入至少两种造影剂。在优选实施例中,在第一时间ti1处向患者注入第一造影剂CA1,然后在第二时间ti2处向患者注入第二造影剂。在优选实施例中,该方法用于对肝脏及其门静脉和动脉供血网进行成像。图2示出了在动脉供血系统(线210)和门静脉供血系统(线211)中第一造影剂CA1随时间的量以及在动脉供血网(线212)和门静脉供血网(线213)中第二造影剂CA2随时间的量213。选择ti2与ti1之间的间隔ti2-ti1以便大致对应于造影剂CA1到达门静脉供血网(tp1)所需的时间,偏离了它到达动脉供血网的时间(ta1)。这样,假设造影剂CA1和CA2都以相同的速度在全身血液循环中导航,造影剂CA1将大致在造影剂CA2首次到达动脉供血网的同时(ta2)开始到达门静脉供血网(tp1)。因此,当存在仅包含第一造影剂的感兴趣区域(即,门静脉供血网)和包含第一造影剂和第二造影剂两者的另一感兴趣区域(即,动脉供血网)时,将存在一时间段P*。当造影剂CA2首次到达动脉供血网时(ta2≈tp1)并且持续直至其到达到门静脉供血网时(tp2),所述时间段P*开始。
在根据本发明的方法的优选实施例中,在CA1已经处于门静脉阶段而CA2仍处于动脉阶段的阶段中,也就是说在时间段P*期间,执行对来自动脉供血网和门静脉供血网的图像数据的同时采集。这对应于图4中图示的图的步骤a。由于这些图像是同时采集的,因此可以使用减影技术来生成额外的功能信息而不受运动伪影的影响。通过使用K边缘检测和/或碘描绘,然后可以得到造影剂CA1和CA2两者的浓度图(MCA1和MCA2),如图4的步骤b中示意性图示的。然后可以如图4的步骤c中那样,识别与门静脉供血网大致对应的仅包含造影剂CA1的区以及与动脉供血网大致对应的包含造影剂CA1和CA2两者的另一区。然后,分割动脉和门静脉允许测量有关的供血输入功能:ACA1(与造影剂CA1有关的动脉供血)、ACA2(与造影剂CA2有关的动脉供血)、PCA1(与造影剂CA1有关的门静脉供血),以及PCA2(与造影剂CA2有关的动脉供血,因为在P*期间在门静脉供血网中没有造影剂CA2,因此其应该等于零)。
在大多数情况下,可以合理地假设两个CA的浓度在动脉和门静脉中都是恒定的。通过进一步假设肝实质内CA的浓度梯度可以忽略不计并且血管和肝组织中CA摄取之间存在固定关系,提出以下模型:
MCA2=XA*ACA2
MCA1=XA*ACA1+XP*PCA1
其中,XA和XP是针对动脉血和门静脉血的部分血容量图。在图4中示意性地图示的根据本发明的方法的步骤d主要在于从第一公式导出XA。然后可以估计XA*ACA1=ACA1*MCA2/ACA2并因此导出XP,其对应于步骤e。
图3图示了动脉供血网XA和门静脉供血网XP的实际部分血容量图的范例。
根据本发明的方法的一个非常感兴趣的优点在于其临床价值,因为它允许在没有来自动脉造影剂的贡献的情况下导出门静脉血液图XP。
根据本发明的方法还允许帮助医师建立诊断。事实上,肝脏病变的鉴别诊断主要基于观察到对比增强。首先,检查病灶内是否有造影剂摄取很重要,例如坏死性病灶的囊性病变不会增强造影剂。因此,在动脉供血网和门静脉供血网两者中,有必要比较注入造影剂之前(原生)的图像与注入造影剂之后的图像。另外,能够在高血管化的病灶中对肝脏病灶进行细分,这种病灶较早且较强烈地增强,而低血管化的肿瘤较晚且较不强烈地增强。对比增强的不同形态模式(例如,边缘增强、周边结节增强、“填充”、“洗出”)是放射学家区分良性病变(如血管瘤或局灶性结节性增生)与恶性肿瘤(例如,转移灶或肝细胞癌)的最重要信息。在MRI中,利用原生扫描和若干其他扫描(常规诊断最多5次)在不同时间点处的图像采集是用于评价局灶性肝脏病变的标准流程。然而,在CT中,对辐射剂量的考虑是主要的限制,因此用于评价肝脏病变的专用CT方案的最小版本将引起三次扫描采集:原生扫描以及在动脉供血网中施予造影剂后的扫描以及在门静脉供血网中施予造影剂后的扫描。
根据本发明的方法首次允许从单次CT扫描采集中提取原生、动脉阶段与门静脉期对比增强图像。结果,对于CT中肝脏的诊断成像,只需进行一次CT扫描而不是三次扫描。这样,用于表征肝脏病变的提供有用信息的值显著增大。
另外,该方法允许计算肝脏的排他性门静脉血液图作为关于肝实质的门静脉供血的补充信息。在标准CT中,这些信息被动脉供血导致的增强所掩盖。例如在用门静脉血栓形成、门静脉高压或门静脉分支的医源性阻塞作为肝脏节段性切除术前的预治疗流程的患者中,这种额外的信息为评价肝实质提供了一个全新的特征。
尽管已经在附图和前面的描述中详细图示和描述了本发明,但是这样的图示和描述应当被认为是图示性或示范性的,而非限制性的;本发明不限于所公开的实施例。
本领域技术人员通过研究附图、公开内容以及权利要求,在实践请求保护的发明时能够理解并实现对所公开的实施例的其他变型。在权利要求中,“包括”一词不排除其他元件或步骤,并且词语“一”或“一个”不排除多个。单个处理器或其他单元可以实现在权利要求中记载的若干项的功能。尽管某些措施被记载在互不相同的从属权利要求中,但是这并不指示不能有利地使用这些措施的组合。权利要求中的任何附图标记都不应被解释为对范围的限制。
Claims (14)
1.一种计算机断层摄影成像方法,包括:
a.执行对根据至少两种造影剂进入血管网的图像数据的单个采集,所述至少两种造影剂中的第一造影剂比所述至少两种造影剂中的第二造影剂在所述血管网中存在了更长的时间,
b.使用K边缘检测和/或碘描绘来处理所述图像数据以分离与每种造影剂相关联的数据,以便获得每种造影剂的浓度图,
c.根据所述图像数据来确定所述血管网的包括所述第一造影剂和所述第二造影剂两者的第一部分,以及所述血管网的仅包括所述第一造影剂的第二部分,
d.基于第二造影剂的总量和所述第二造影剂的所述浓度图来计算所述血管网的所述第一部分的部分血容量图,
e.基于第一造影剂的总量、所述第一造影剂的所述浓度图以及所述血管网的所述第一部分的所述部分血容量图来计算所述血管网的所述第二部分的部分血容量图。
2.根据权利要求1所述的方法,所述血管网是肝血管网,包括动脉子网和门静脉子网。
3.根据权利要求2所述的方法,所述血管网的所述第一部分包括所述动脉子网。
4.根据权利要求2和3中的任一项所述的方法,所述血管网的所述第二部分包括所述门静脉子网。
5.根据前述权利要求中的任一项所述的方法,步骤b使用K边缘检测和碘描绘两者来分离所述造影剂。
6.根据权利要求1至4中的任一项所述的方法,步骤b仅使用K边缘检测来分离所述造影剂。
7.根据前述权利要求中的任一项所述的方法,所述至少两种造影剂的数量恰好是两个。
8.根据前述权利要求中的任一项所述的方法,包括定位高血管化区。
9.根据前述权利要求中的任一项所述的方法,包括定位低血管化区。
10.根据前述权利要求中的任一项所述的方法,其被用作在对肝脏的节段性切除之前的准备步骤。
11.根据前述权利要求中的任一项所述的方法,包括将所述血管网的所述第一部分的所述部分血容量图和所述血管网的所述第二部分的所述部分血容量图存储到存储设备中。
12.根据前一权利要求所述的方法,包括从被存储在所述存储设备中的所述部分血容量图中生成数据库。
13.一种被配置为实施根据前述权利要求中的任一项所述的方法的设备,所述设备包括:
a.医学X射线扫描器,其配备有光子计数探测器技术,提供足够的能量分辨率以使得能够对至少一种K边缘材料进行K边缘成像,
b.用于将至少两种造影剂材料连续注入患者的器件,
c.重建单元,其被配置为基于K边缘或碘描绘来生成两幅分离的造影剂图/图像,以及
d.用于对动脉部分血容量图和/或门静脉部分血容量图进行计算和可视化的器件。
14.一种用计算机可读指令编码的计算机可读存储介质,所述计算机可读指令在由处理器运行时使所述处理器执行根据权利要求1-12中的任一项所述的方法。
