Description
技术领域

本发明可用于治疗学领域,特别是诊断领域。

背景技术
系统性红斑狼疮(SLE)和慢性淋巴细胞白血病(CLL)到现在还是不治之症,具有非常多样的进展,且其诊断有时非常困难。

在SLE和CLL的病变中,描述了SLE和CLL中的LB的钙信号的破坏是静止的并随后刺激LB抗原受体(BCR)(Liossis SN,Kovacs B,Dennis G,Kammer GM,Tsokos GC.B cellsfrom patients with systemic lupus erythematosus display abnormal antigenreceptor-mediated early signal transduction events.J Clin Invest.1996;98:2549-57,([6]);Dühren-von Minden M,R,Schneider D,Wossning T,Bach MP,Buchner M,et al.Chronic lymphocytic leukaemia is driven by antigen-independent cell-autonomous signalling.Nature.2012;489:309-12,([7]))。
因此CLL和SLE的诊断是基于非理想的临床和生物标准的汇集,因此需要进展新的比已知的那些更有效的用于诊断以及预后的标准。
此外,用于CLL的动物模型缺乏或验证不完全,存在若干用于SLE的动物模型以及研究最多的一个是狼疮性小鼠模型MRL/Lpr,其再现人SLE的大部分特征(Shlomchik,M.J.,Madio,M.P.,Ni,D.,Trounstine,M.&Huszar,D.J.Exp.Med.180,1295–1306(1994)[(10)])。
因此实际需要提供用于诊断SLE和CLL的新方法,其克服现有技术的缺陷、缺点和障碍。
发明内容
本发明通过将位于质膜处的STIM1蛋白质部分用作系统性红斑狼疮(SLE)和/或慢性淋巴细胞白血病(CLL)的诊断方法中的生物标记物使得可以满足这些需要,STIM1是钙通道激活和调节中涉及的蛋白质。
在发展本发明之前,在这时没有响应针对CLL和SLE的治疗的预示标记物。因此在显著的和创新的调查研究的背景下,申请人成功发展了用于CLL和/或SLE的诊断和/或预后的新方法。有利地,可以将该方法应用于MRL/Lpr狼疮性小鼠模型。
本发明使得可以在这些病理学条件下以完全创新的方式将膜STIM1表达上的变化用作用于CLL和/或SLE的诊断和/或预后的生物标记物。
在狼疮患者的LB和LT(T淋巴细胞)和来自MRL/Lpr狼疮性小鼠模型的LB中,在本发明的背景下出乎意料地观察到位于质膜处的STIM1蛋白质的总体表达和STIM1部分的诱导升高,其在人和鼠对照LB和对照LT中保持低或甚至是零。因此,本发明建议将SLE中的STIM1的表达的测量值用作:
1)诊断生物标记物。LB和LT中的STIM1表达水平是SLE的区分标记物。申请人已经证明了该标记物用于区分狼疮患者与其他自身免疫病理学病症(Goujerot-Sjogren综合症和类风湿性关节炎)的优点;
2)预后生物标记物。申请人在本发明的背景下已经证明了在人中的膜STIM1的量和狼疮疾病的活性(SLEDAI得分)之间;以及鼠狼疮性小鼠MRL/Lpr中的膜STIM1的量和蛋白尿之间存在相反的联系。
3)治疗学响应生物标记物。
在CLL LB中,申请人出乎意料地证明了根据膜(组I)中STIM1蛋白质的存在或其不存在(组II)可以区分两个患者组。本发明的上下文中还示出了总STIM1表达增加以及用于组I的CLL LB的STIM1启动子的甲基化状态缺乏。
因此,本发明建议将STIM1表达的测量值和/或CLL中的STIM1启动子的甲基化状态用作:
1)诊断生物标记物。将STIM1表达水平用作诊断标记物,特别是用于最初的形式和Matutes得分为3(5%的CLL)的CLL。
2)预后生物标记物。LB中的STIM1表达水平与疾病的临床进展相关(Binet得分);
3)治疗响应生物标记物。在本发明的上下文中,将STIM1的膜部分的表达水平和/或STIM1启动子的甲基化状态用作响应于治疗以及利妥昔单抗(抗-CD20)的标记物。
本发明在若干方面是有利的,特别是由于位于质膜处的STIM1蛋白质仅存在于受影响的细胞上而不存在于健康的细胞上的事实,从而使能够增进特异性和选择性。此外,STIM1蛋白质在质膜上通过免疫细胞的表达促进其可接近性,用于更快和更容易地诊断。
本发明第一目的因此涉及一种用于诊断可能患有其的受试者的系统性红斑狼疮(SLE)和/或慢性淋巴细胞白血病(CLL)的方法,包括体外检测来自受试者的生物样品中的位于细胞质膜处的STIM1蛋白质的部分的表达。
本发明的另一个目的涉及一种用于预测可能患有其的受试者的CLL和/或SLE的进展和/或监测其进展的方法,包括体外检测来自受试者的生物样品中的位于细胞质膜处的STIM1蛋白质的部分的表达。
出于本发明的目的,术语“生物样品”旨在是指来自受试者的任何组织或细胞和它们的衍生物。其例如可以来自取自受试者的样品。其可以是例如血液样品、骨髓样品或来自淋巴结活检的样品。

检测的表达可以是蛋白质的存在或量或通过使用靶向位于质膜处的STIM1蛋白质的部分的化合物调节的任何生物影响,例如对钙流、细胞存活率、自我吞噬、细胞因子生产、增殖和细胞迁移的影响。使用的检测手段可以是本领域技术人员已知的任何手段,例如通过定量信使RNA,例如通过定量RT-PCR和/或通过杂交技术,例如使用标记探针或通过使用DNA芯片。在其他实施方式中,根据本发明的方法包括定量生物样品中位于细胞质膜处的STIM1蛋白质的部分,例如通过使用抗-STIM1抗体、通过流式细胞术、通过免疫印迹法、通过显微术、通过质谱分析、通过免疫测定、特别是通过ELISA。还包括测试STIM1启动子的甲基化状态的方法,例如染色质免疫沉淀技术、甲基化敏感的PCR、CpG芯片的使用以及用亚硫酸氢钠处理之后的任何研究。
在一个具体的实施方式中,检测位于质膜处的STIM1蛋白质的部分还可以包括检测STIM1蛋白质的多种细胞部分。STIM1的多种细胞部分可以是位于质膜和/或内质网处的蛋白质的部分或总的STIM1部分。
在一个具体的实施方式中,检测位于质膜处的STIM1蛋白质的部分的表达是测量STIM1启动子的甲基化状态。可以通过本领域技术人员已知的任何技术进行STIM1启动子的甲基化状态的测量。其可以是例如使用对DNA甲基化敏感的或不敏感的限制性内切酶,例如MspI或HpaII,或用抗-5-甲胞嘧啶抗体温育或不温育的“甲基化DNA免疫沉淀”技术。有利地,膜STIM1启动子的甲基化状态在患CLL或SLE的受试者和没有患其的健康受试者之间不同。例如,不同于没有患有其的受试者的对照分布的分布允许疾病的阳性诊断、或对疾病治疗响应微弱的减除、或疾病进展的减除。

出于本发明的目的,术语“用于诊断的过程”旨在是指任何过程,其使得可以识别患SLE和/或CLL的受试者和/或没有患SLE和/或CLL的受试者。本发明的过程是体外过程,即期望在受试者体外进行诊断。其是例如在来自待诊断的受试者的生物样品上进行的测试。
在本发明的诊断方法的上下文中,当在本发明的方法的上下文中检测的表达是位于细胞质膜处的STIM1蛋白质的部分与来自没有患SLE或CLL的受试者的对照细胞中的所述部分的表达水平比较的过表达时,这允许进行SLE或CLL的阳性诊断。
出于本发明的目的,术语“过表达”旨在是指数量上大于在没有患SLE或CLL的受试者中测量的那些的表达。例如,过表达可以比在没有患所述病理学病症的受试者中测量的表达高三倍。
本发明的诊断方法可以例如包括以下步骤:

(b)比较在步骤(a)中得到的位于细胞质膜处的STIM1蛋白质的部分的表达或活性与在来自一个或多个对照受试者(例如健康受试者)的一个或多个生物样品中检测或定量的位于细胞质膜处的STIM1蛋白质的部分的表达或活性。
有利地,将受试者或患者诊断为患有所述疾病或容易患有所述疾病,如果它们的生物样品表现出大于来自健康患者的样品中的那些的位于细胞质膜处的STIM1蛋白质的部分的表达或活性。在另一方面,如果在步骤(a)中确定的位于细胞质膜处的STIM1蛋白质的部分的活性或表达小于或等于来自(健康的)对照患者的样品中的那些,那么可以认为诊断是阴性的。如果来自患者的样品表现出是来自健康患者的样品的那些的至少10%以上、优选地至少20%以上的位于细胞质膜处的STIM1蛋白质的部分的表达或活性,则可以认为来自患者的样品是阳性的。
出于本发明的目的,术语“预测进展”旨在是指使得可以给出患者的病症和/或疾病在未来的预后的任何过程。其可以是给定时间段内疾病的进展和/或疾病的可能结果的评估的概率。其可以是短期预测,例如在三个月到一年。预测使得可以评估在预定时间段内疾病缓解、稳定或恶化的概率。
在一个具体的实施方式中,可以通过位于细胞质膜处的STIM1蛋白质的部分的表达水平以及通过计算针对SLE的SLEDAI(系统性红斑狼疮疾病活性指数)得分和针对CLL的Binet得分的疾病的临床进展之间的相关性进行进展的预测。
可以如文献Bombardier et al.([2])中提到的计算SLEDAI得分。
可以如文献Binet et al.([3])中提到的计算Binet得分。
例如,当位于细胞质膜处的STIM1蛋白质的部分的表达水平比在没有患病理学病症的患者中计算的值低十倍时,通过SLEDAI得分计算的疾病的临床进展是有利的(负的预测值)。
例如,当位于细胞质膜处的STIM1蛋白质的部分的表达水平比在没有患病理学病症的患者中计算的值低三倍时,通过Binet得分计算疾病的临床进展是有利的(负的预测值)。
出于本发明的目的,术语“监测进展”旨在是指用于确定疾病是缓解、稳定还是恶化的任何过程。可以通过比较在给定时刻检测的位于细胞质膜处的STIM1蛋白质的部分的表达与之前检测的该部分的表达进行进展的监测。之前的检测可以是例如在阳性诊断疾病的当天的检测。通过可能每隔一定间隔进行的至少两次检测、以及有利地大于两次检测进行进展的监测。当进行多于两次检测时,它们可以是例如每隔三个月到一年。
监测患者中疾病的进展可以包括由以下各项组成的步骤:
(a)检测或定量在优选地在时间t1从患者采集的血液的第一生物样品中的位于细胞质膜处的STIM1蛋白质的部分的活性或表达,
(b)检测或定量优选地从患者采集的血液的第二生物样品中的位于细胞质膜处的STIM1蛋白质的部分的活性或表达,第二样品是在时间t1之后的时间t2采集的,
(c)比较步骤(b)中位于细胞质膜处的STIM1蛋白质的部分的表达或活性与步骤(a)中确定的那些。
步骤(c)中位于细胞质膜处的STIM1蛋白质的部分的表达或活性的比较是标准,其使得可以确定疾病的进展或阶段。因此可以确定疾病是缓解、稳定还是恶化。
如果步骤(b)中的样品表现出小于步骤(a)中确定的表达或活性的位于细胞质膜处的STIM1蛋白质的部分的表达水平,则可以得出结论疾病缓解。相反地,如果步骤(b)中的样品表现出大于在第一步骤中测量的那些的位于细胞质膜处的STIM1蛋白质的部分的表达或活性,则疾病正在进展。
有利地,可以响应于人和小鼠中的治疗来监测进展。在这方面,可以通过在多个治疗时刻体外检测位于细胞质膜处的STIM1蛋白质的部分的表达,和通过比较通过检测得到的数据与例如在开始治疗之前可能在相同受试者中或没有患病理学病症的受试者中得到的对照值来进行监测。例如,可以从开始治疗的那天起约每月、或约每两个月、或约每三个月、或约每六个月进行监测。有利地,通过监测得到的结果的比较使得可以确定响应于治疗的病理学病症的进展的减少、和/或疾病的进展减速或阻断和/或疾病的一种或多种症状已经缓解、减轻或减缓的事实,或所述疾病已经痊愈的事实。
在该实施方式中,体外评估疾病的治疗的疗效可以包括由以下各项组成的步骤:
(a)检测或定量受试者的生物样品(例如来自治疗之前的患者)中的位于细胞质膜处的STIM1蛋白质的部分的表达或活性,

通过比较样品(a)和(b)中位于细胞质膜处的STIM1蛋白质的部分的表达可以确定治疗的疗效。如果在步骤(b)中的样品中的位于细胞质膜处的STIM1蛋白质的部分的表达或活性低于步骤(a)的样品中的那些,则意味着治疗是有效的。如果存在位于细胞质膜处的STIM1蛋白质的部分的表达增加,则根据观察的变化大小可以认为治疗是不积极的或不起作用的。在某些实施方式中,如果涉及位于细胞质膜处的STIM1蛋白质的部分的表达或活性减少至少10%、优选地至少20%,则认为治疗对疾病具有治疗学影响。治疗之前和治疗之后采集的生物样品优选地是血液样品。
治疗性治疗还可以包括与所述部分相互作用的物质。该物质可以是与位于细胞质膜处的STIM1蛋白质的部分互相作用的任何分子。相互作用可以是以下类型如将候选分子结合至位于细胞质膜处的STIM1蛋白质。可替换地,相互作用可以是调节该蛋白质的活性或表达。位于质膜处的STIM1蛋白质的部分的活性的调节可以是由于将STIM1嵌入到质膜中的修饰、或其和与其相关的蛋白质相互作用的修饰。调节蛋白质的活性可以导致钙流变更、如细胞内钙的基本进入的变更或在刺激受体期间激活的钙进入的变更如钙池调控钙进入(SOCE)。调节表达可以是位于质膜处的STIM1蛋白质的表达与应用候选分子之前对相同细胞或类似细胞测量的水平比较的升高或降低。调节STIM1蛋白质的表达可以例如联系到转录修饰、表观遗传修饰或调节对于STIM1蛋白质的膜通行所必要的糖基化过程。
物质可以是天然或合成来源。其可以是化学或通过任何生物工程过程例如纯化生产的蛋白质。物质可以是例如序列SEQ ID NO.3的针对位于质膜处的STIM1蛋白质的细胞外片段的抗体。该序列可以对应于STIM1的位置23-213之间的氨基酸。其可以是抗-GOK/STIM-1抗体(克隆:44,BD生物科学参考910954)。

本发明的另一个主题涉及与位于细胞质膜处的STIM1蛋白质的部分互相作用的物质用于体外检测用于治疗CLL和/或SLE的药剂的疗效、预测其进展、监测其进展和/或评估其的用途。
有利地,与位于细胞质膜处的STIM1蛋白质的部分互相作用的物质因为其与集中到质膜的STIM1蛋白质的部分的特异性相互作用没有穿透进入细胞中并停留在质膜处。换句话说,物质可以是非穿透分子,即分子没有跨越质膜。有利地,由于物质没有跨越质膜,所以其没有与集中到内质网的STIM1蛋白质相互作用。

实施例1:用于证明膜STIM1的过程
在除去T淋巴细胞(使用用神经氨糖酸苷酶预处理的绵羊红血细胞的花结技术)和除去单核细胞(负消耗技术(negative depletion technique),没有CD43的B细胞试剂盒,干细胞技术)之后由在葡聚糖(ficoll)梯度上得到的外周血单核细胞(PBMC)纯化B淋巴细胞。通过流式细胞计验证CD19-阳性LB的纯度,示出纯度大于95%。
A/B-通过免疫印迹法在SDS-PAGE上的LB的蛋白质分析使得可以区分除STIM1的网状部分(84±2kDa)之外的用于SLE LB(A)和用于某些CLL疾病(B,mSTIM1+组)的膜和糖基化形式。该蛋白质分析首先使用了抗-STIM1抗体克隆Gok/44(BD生物科学),然后是过氧化物酶连接的抗-小鼠IgG抗体(GE医疗保健),以及最终通过化学发光(chimioluminescence)(ECL先进试剂盒,GE医疗保健)显示。

实施例2:通过流式细胞术研究膜STIM1表达
示踪包括使用抗-STIM1单克隆抗体(mAb)(克隆GOK/44,BD生物科学,参考610954)。可以将该mAb与例如藻红蛋白(PE)连接,这使得可以使其与其他示踪的mAb结合,例如人中的连接至Krome Orange的抗-CD19 mAb(克隆J3-119,Beckman Coulter,参考A96418)、以及连接至APC-AF700的抗-CD5 mAb(克隆Bl1a,Beckman Coulter,参考A78835);和小鼠中连接至APC-AF700的抗-B220 mAb(克隆RA3-6B2,Biolegend,参考10323(1/2))、连接至Cy5.5的抗-CD5 mAb(克隆53-7.3,Biolegend,参考10062(3/4))以及与荧光素连接的抗-IgM mAb(克隆RMN-1,Biolegend,参考40650(5/6))。在不存在连接的情况下,可以用荧光素连接的抗-IgG mAb(抗-IgG F(ab')2,Dako,参考F0479)显示抗-STIM1 mAb。
示踪10-15分钟之后(表1),洗涤细胞,然后通过流式细胞术分析(Navios 10颜色体系和FC500 5颜色体系,Beckman Coulter)。然后可以在淋巴细胞上进行膜STIM1示踪的分析,通过流式细胞术根据细胞的尺寸和结构或根据选择的人中的其他标记物例如B淋巴细胞(CD19+)和T淋巴细胞(CD19-CD5+)、和小鼠中的不成熟的/幼B细胞(B220低IgM+)和成熟B细胞(B220高IgM+)将其区分。

实施例3:总STIM1表达的测量值
在渗透化细胞之后测量总STIM1表达以测量包含在质膜处以及内质网膜处的多种STIM1部分。实际上,在之前纯化和固定(多聚甲醛在磷酸盐缓冲盐水(PBS)中的4%溶液,在4℃下10min)然后渗透化(皂草苷在PBS-BSA(牛血清白蛋白)1%中的0.1%的溶液,在4℃下10min)的500 000个LB上进行分析。示踪包括使用抗-STIM1mAb(克隆GOK,BD生物科学),使用荧光素连接的抗-小鼠IgG抗体(抗-IgG F(ab')2,Dako,参照F0479)显示。

表4示出了由通过健康对照得到的值计算的用于总STIM1的阳性阈值的测量值(阴性对照的平均值+/-3标准偏差)。该阈值(实施例中5.0)使得可以相对于阴性患者区分患有系统性红斑狼疮(SLE)和慢性淋巴细胞白血病(CLL)的阳性患者。

实施例4:确定STIM1启动子的甲基化状态
从之前纯化的B淋巴细胞(Biosprint 15DNA血液试剂盒,Qiagen)中提取基因组DNA,然后定量(Nanodrop 2000c,热科学(Thermo Scientific))。将Daudi(ATCC CCL-213)和Ramos(ATCC CRL-1596)人系列用作B淋巴细胞对照。将两种技术(3a和3b)用于确定STIM1启动子的DNA的甲基化状态。
技术3a:该第一种技术是基于使用限制性内切酶如对CCGG基序内的CpG基序的甲基化状态敏感的HpaII和对CpG基序的甲基化状态不敏感的MspI。实际上,在37℃下在单独的缓冲液、HpaII酶(10单位,新英格兰生物实验室(New England Biolabs),参照r0171)或MspI酶(10单位,新英格兰生物实验室,参照R0106)的存在的情况下温育200ng的DNA持续12h。
技术3b:该第二种技术要求DNA断裂的第一步骤(dsDNA Sherase,在42℃下20min,然后在65℃下5min以灭活酶,Zymo Research,参见E2018)。然后在抗-5-甲胞嘧啶抗体和A蛋白(甲基化的DNA免疫沉淀(IP)试剂盒,Zymo Research,参照D5101)存在的情况下温育DNA片段。最后,分离DNA/抗体/A蛋白络合物,然后灭活抗体(75℃,5min)。
技术3c(该步骤与技术3a和3b通用):通过基因组PCR(聚合酶链式反应)扩增靶标区域。保持扩增的PCR循环是以下:首先预变性步骤(95℃,5min),然后包括变形阶段(95℃,30s)、杂交阶段(60℃,30s)以及伸长阶段(72℃,1min)的35个循环,以及最后最终的伸长步骤(72℃,5min)。
读取技术3a+3c:将HpaII管的扩增与没有酶的管(100%甲基化)以及MspI管(0%甲基化)比较。
读取技术3b+3c:甲基化-DNA IP管的扩增提供了关于由抗-5-甲胞嘧啶抗体捕获的区的甲基化状态的信息。将甲基化-DNA IP管的扩增与没有IP的管(100%甲基化)比较。
图3示出了结果。

Claims (12)
1.用于诊断可能患有系统性红斑狼疮(SLE)和/或慢性淋巴细胞白血病(CLL)的受试者的所述系统性红斑狼疮(SLE)和/或慢性淋巴细胞白血病(CLL)的方法,包括体外检测来自所述受试者的生物样品中的位于细胞质膜处的STIM1蛋白质的部分的表达。

3.用于预测CLL和/或SLE的进展和/或监测其进展的方法,包括体外检测位于细胞质膜处的STIM1蛋白质的部分的表达。
4.根据前述权利要求中任一项所述的方法,其中所述细胞选自B淋巴细胞和T淋巴细胞。
5.根据前述权利要求中任一项所述的方法,其中位于所述质膜处的所述STIM1蛋白质的部分的所述检测还包括检测所述STIM1蛋白质的各种细胞部分。
6.根据前述权利要求中任一项所述的方法,其中位于所述质膜处的所述STIM1蛋白质的部分的表达的所述检测是测量STIM1启动子的甲基化状态。
7.根据权利要求3至6中任一项所述的方法,其中通过位于所述细胞质膜处的所述STIM1蛋白质的部分的表达水平与通过计算针对SLE的SLEDAI(系统性红斑狼疮疾病活性指数)得分和针对CLL的Binet得分的疾病的临床进展之间的相关性进行所述进展的预测。
8.根据前述权利要求中任一项所述的方法,其中,所述细胞是分离的整个细胞。
9.根据权利要求3至8中任一项所述的方法,其中所述进展响应于治疗性治疗。
10.根据权利要求9所述的方法,其中所述治疗性治疗还包含与所述部分相互作用的物质。
11.根据权利要求10所述的方法,其中所述物质是针对序列SEQ ID NO.3的所述STIM1蛋白质的片段的抗体。
