After the two ends of the blood vessel are closed and tied, sometimes the clamp applier needs to be moved out of the abdominal cavity, and the assembly is reinstalled, so that the operation time and efficiency are seriously affected.
In the prior art, a clip applier with a patent number of CN105476690 and a name of one-time double-clip cutting clip applier is generally adopted for clip applier operation, the distance between the double clips of the clip applier is constant, and the distance cannot be adjusted. However, in the actual clinical use process, according to the size, thickness, position and artery and vein of the blood vessel, the distance between the two clamps for cutting the blood vessel ligation is also quite different, so that the accurate adjustment of the distance between the two clamps is the key for ensuring the operation safety and preventing the bleeding, and the clamp applier of the patent cannot adjust the distance at any time, so that the requirements cannot be met, and the risks of operation failure and bleeding are caused.
In addition, in the conventional operation, after the blood vessel is ligated, the blood vessel can be cut after the blood vessel is firmly ligated after the blood vessel is observed under direct vision, which is also the key for ensuring the safety of the operation, and the patent directly cuts the blood vessel after the blood vessel is ligated, so that the operation concept of clinical safety is not met.

Wherein, by rotating the rotary handle 8 relative to the setting sleeve 16, the pair of first clip applying rods 21, the pair of second clip applying rods 5 and the distance adjusting rod 4 are driven to rotate relative to the setting sleeve 16, the angle of application required during the ligation of blood vessels can be adjusted.
The rotary handle 8 may be inserted into the setting sleeve 16 with a gap between the rotary handle and the setting sleeve 16, or the rotary handle 8 may be screwed with the setting sleeve. The front end of the return spring of the opening and closing device can be propped against the tail end of the rotating handle, and a limit stop used for propping against the front end of the return spring can also be arranged on the inner wall of the placement sleeve.

Similarly, in order to enable the pair of second clip applying rods 5 to rotate along with the rotating handle 8, as shown in fig. 10-12, at least one clip applying ring groove 19 is formed on the inner wall of the clip applying connecting sleeve 12 in a ring manner, and a clip applying connecting rod 15 slidably connected with the Shi Gahuan groove 19 is arranged at the tail end of the second clip applying rod 5. Preferably, clip applier bar 15 is perpendicular to second clip applier bar 5, and the distal end of clip applier bar 15 is provided with a protruding enlarged portion for snap-fit engagement within clip applier ring groove 19. When the rotary handle 8 rotates relative to the mounting sleeve 16 through external force, the second clip applying rod 5 is driven to rotate, and the tail end of the clip applying connecting rod 15 connected with the second clip applying rod 5 is always inserted into the clip applying ring groove 19 and moves circumferentially along the clip applying ring groove 19, so that the second clip applying rod 5 can generate rotary motion relative to the clip applying connecting sleeve 12, namely, the rotary motion of the second clip applying rod 5 relative to the mounting sleeve 16 is realized.
During design, the internal cavity of the placement sleeve and the positions of the components are reasonably arranged according to the actual arrangement positions of the two sliding grooves, so that the components of the opening and closing device and the components of the distance adjusting device cannot interfere.
When the adjustable double-clamp clip applier is adopted for clip applier operation, the clip applier is firstly in a natural state in an open state, the distance between a pair of first chucks and the distance between a pair of second chucks are both minimum, and the ligature clamp is installed in the state and is fixedly installed in a double-clamp box provided with a plurality of double clamps. During installation, the adjustable-distance double-clamp clip applier is opened, the upper and lower clamping grooves of the two pairs of clamping heads are respectively aligned with the double clamps in the double-clamping-seat box, the double pairs of clamping heads are slightly pushed, the two pairs of clamping heads are installed in the upper and lower clamping grooves of the two pairs of clamping heads of the clip applier, and a new pair of ligature clamps are installed after one time of use, so that the ligature clip applier is repeatedly used.
Next, the working principles of the adjustable-distance double-clamp clip applier according to the present embodiment for distance adjustment, clip applier operation, and azimuth adjustment will be briefly described.
And (3) distance adjustment: the distance adjusting control handle 11 is moved forwards and backwards to push the distance adjusting connecting sleeve 10 to send force to the distance adjusting rod 4, so that the angle between the pair of connecting rods 3 is adjusted, and the aim of adjusting the distance between the two groups of chucks is fulfilled.
Clamping operation: the clamping control handle 13 is moved back and forth to push the clamping connecting sleeve 12 to send force to the second clamping rod 5 to close the first clamping head 1 and the second clamping head 2, so that the clamping effect is achieved. Wherein, when the hand is released, the double clamp heads are in an open natural state due to the reset action of the return spring 9.
Azimuth adjustment: by using the rotary handle 8, the first clip applying rod, the second clip applying rod and the distance adjusting rod can rotate relative to the arranging sleeve 16 under the action of the parallel maintaining sleeve 20, so that the second Shi Gagan rotates around the respective centers in the circular distance adjusting connecting sleeve 11 and the distance adjusting rod in the clip applying connecting sleeve 12, thereby achieving rotation of any angle such as 360 degrees, and being suitable for vascular ligation under different conditions.
The distance-adjusting connecting sleeve 10 and the clamping connecting sleeve 12 of the embodiment are both limited on a sliding groove in the setting sleeve 16 to move linearly along the front-back direction. In addition, annular ring grooves 18 and 19 are formed in the inner walls of the distance-adjusting connecting sleeve 10 and the clamping connecting sleeve 12, and the distance-adjusting connecting rod 14 connected with the distance-adjusting rod and the head-side expansion end of the clamping connecting rod 15 connected with the second clamping rod are limited to be in the ring grooves respectively. Therefore, the distance adjusting connecting sleeve 10 and the clamping connecting sleeve 12 can drive the distance adjusting connecting rod 14 and the clamping connecting rod 15 to move back and forth. When the rotary handle 8 rotates, the expansion ends of the head sides of the distance-adjusting connecting rod 14 and the clamping connecting rod 15 can only be in the corresponding annular grooves, so that the rotary handle 8 can be adjusted to realize the movement of any angle of 360 degrees.
In summary, the adoption of the adjustable-distance double-clamp clip applier of the embodiment can bring the following beneficial technical effects:
the adjustable-distance double-clamp clip applier is simple in structure, convenient to operate and high in practicality, not only can clamp the adjustment of the distance and ligate the blood vessels according to different conditions of blood vessels with different sizes, thicknesses and positions and arteries and veins, but also brings more convenience to vascular treatment in endoscopic surgery, brings sufficient safety to surgery, meets the operation requirement, and ensures the safety of the surgery.
