Abstract
The invention discloses an esophageal stomach anastomosis fistula plugging drainage device, which comprises a flexible air bag, wherein the inner side wall and the outer side wall are sealed into a whole through sealing surfaces, the flexible air bag is in a jacket structure, the upper part and the lower part of the flexible air bag are respectively expanded to form an upper expansion section and a lower expansion section, a drainage tube body is arranged in the flexible air bag in a penetrating manner from the top of the upper expansion section of the flexible air bag, the lower part of the drainage tube body outwards penetrates out from the middle part of the outer side wall of the flexible air bag, a first flexible sealing pad is arranged at the top of the upper expansion section corresponding to the drainage tube body, an air inlet and outlet pipe with an air inlet and outlet valve is arranged on the first flexible sealing pad in an upward extending manner, a second flexible sealing pad is arranged on the outer side wall of the flexible air bag corresponding to the drainage tube body, and a metal sheet is arranged on the second flexible sealing pad in a spacing manner. The invention has the advantages that the flexible air bag is used for plugging the anastomotic fistula, so that the plugging is tight and the comfort level is high; the cleaning treatment is carried out on the intrathoracic pyogenic cavity or the mediastinal endopyogenic cavity of the patient through the flushing pipe and the drainage pipe, so that the treatment effect is effectively ensured.

Technical Field
The invention relates to a medical auxiliary instrument, in particular to an esophageal and gastric anastomotic stoma fistula plugging and drainage device.
Background
Esophageal gastrostomy fistula is a serious complication after thoraco-esophageal radical surgery, and seriously affects the life quality and the life cycle of patients. Investigation shows that the incidence rate of the neck anastomosis fistula is about 9%, the incidence rate of the intrathoracic anastomosis fistula is about 5%, and the death rate of the anastomosis fistula is as high as 50-70%. After the patient has an esophageal-gastric anastomotic stoma fistula, saliva and gastric contents continuously overflow from the stoma to the thoracic cavity or the mediastinum, so that infection of the thoracic cavity or the mediastinum is not cured for a long time, and organism infection and consumption seriously threaten the life of the patient.
At present, three methods of conservative treatment, surgical treatment and interventional treatment are mainly adopted as the treatment method of the esophageal and gastric anastomosis fistula. The conservative treatment often has insufficient drainage, malnutrition and poor curative effect; the secondary surgery is large in wound and high in risk, and many patients are difficult to tolerate and accept; the interventional therapy is micro-traumatic, low in cost, good in curative effect and easy to accept and endure by patients. However, no medical apparatus is specially used for plugging fistulae and draining pus at present, and during treatment, the fistulae are plugged by a bracket by embedding an esophageal stent in an esophagus, and then drainage and flushing treatment are carried out on the chest pus or the pus in the mediastinum. Because the esophageal stent is arranged in the esophagus, the foreign body sensation is strong; the stent is easy to shift, and the fistula is not tightly plugged, so that the treatment effect is affected; and the stent must be removed in a short time (two months) or else restenosis of the esophagus is liable to develop, further increasing the risk of the patient.
Disclosure of Invention
The invention aims to provide an esophageal and gastric anastomotic stoma fistula plugging drainage device which is high in safety, high in comfort level and tight in plugging.
In order to achieve the above purpose, the invention adopts the following technical scheme:
the invention discloses a plugging and drainage device for an esophageal gastric anastomotic stoma fistula, which comprises a flexible air bag, wherein the inner side wall and the outer side wall are sealed into a whole through sealing surfaces, the flexible air bag is in a jacket structure, the upper part and the lower part of the flexible air bag are respectively expanded to form an upper expansion section and a lower expansion section, a drainage tube body is penetrated in the flexible air bag from the top of the upper expansion section of the flexible air bag, the lower part of the drainage tube body penetrates outwards from the middle part of the outer side wall of the flexible air bag, a first flexible sealing gasket is arranged at the top of the upper expansion section corresponding to the drainage tube body, an air inlet and outlet pipe communicated with the inner cavity of the flexible air bag and provided with an air inlet and outlet valve is upwards arranged on the first flexible sealing gasket, a second flexible sealing gasket is arranged on the outer side wall of the flexible air bag corresponding to the drainage tube body, and a metal sheet is embedded on the second flexible sealing gasket at intervals;
the drainage tube body comprises a flushing tube and a drainage tube which are fixedly connected into a whole in parallel, the lower parts of the flushing tube and the drainage tube are bent and penetrate out of the outer side wall of the flexible air bag, the lower end part of the drainage tube extends out of the lower end of the flushing tube and is uniformly provided with drainage side holes in a staggered manner, and the lower end part of the flushing tube is uniformly provided with flushing side holes; flow control valves are arranged at the tops of the flushing pipe and the liquid discharge pipe.
An antibiotic layer is arranged on the outer side wall of the flexible air bag at the position corresponding to the second flexible sealing pad.
The included angle alpha between the lower bending section and the upper section of the flushing pipe is 90-150 degrees.
The section of the flushing pipe and the section of the liquid discharge pipe are of annular structures formed by connecting arc-shaped parts and straight line parts, and the flushing pipe and the liquid discharge pipe are fixedly connected into a whole through the straight line parts.
The outer side wall of the air inlet and outlet pipe is fixedly connected with the outer side wall of the flushing pipe and the outer side wall of the liquid discharge pipe.
The invention has the advantages that the flexible air bag is used for plugging the anastomotic fistula, so that the plugging is tight and the comfort level is high; meanwhile, the washing pipe and the liquid discharge pipe are used for washing and treating the inner thoracic cavity or the inner longitudinal diaphragmatic cavity of the patient, so that the treatment effect is effectively ensured. The flexible air bag adopts a hollow structure, so that food and saliva can pass through conveniently, the lower expansion section is used for supporting the gastric cavity, and the upper expansion section ensures tighter plugging of the fistula; the flushing pipe and the liquid discharge pipe penetrate out of the flexible air bag and are connected in a sealing way through the first flexible sealing gasket and the second flexible sealing gasket, so that the air tightness of the flexible air bag is effectively ensured; the lower parts of the flushing pipe and the liquid discharge pipe are bent, so that the flushing pipe and the liquid discharge pipe can smoothly penetrate out of the flexible air bag and are inserted into the thoracic cavity or the mediastinum pus cavity of a patient, and the treatment effect is ensured; the metal sheet has a developing function, so that the plugging is ensured to be in place.

Detailed Description
As shown in fig. 1 and 2, the esophageal stomach anastomosis fistula plugging drainage device comprises a flexible air bag 1 which is formed by sealing an inner side wall and an outer side wall into a whole through sealing surfaces and is in a jacket structure, wherein the upper part and the lower part of the flexible air bag 1 are respectively expanded to form an upper expansion section 2 and a lower expansion section 3, the upper expansion section 2 is in an arc-shaped structure, a fistula is plugged more tightly, and the lower expansion section 3 is in an arc-shaped structure and is used for supporting a gastric cavity; a drainage tube body penetrates through the top of the upper expansion section 2 of the flexible air bag 1, the lower part of the drainage tube body penetrates out of the middle part of the outer side wall of the flexible air bag 1, a first flexible sealing pad 4 is arranged at the top of the upper expansion section 2 corresponding to the drainage tube body, an air inlet and outlet pipe 5 which is communicated with the inner cavity of the flexible air bag 1 and is provided with an air inlet and outlet valve is arranged on the first flexible sealing pad 4 in an upward extending manner, a second flexible sealing pad 6 is arranged on the outer side wall of the flexible air bag 1 corresponding to the drainage tube body, and metal sheets 7 are embedded on the second flexible sealing pad 6 at intervals, so that the sealing is in place due to the development effect;
as shown in fig. 1-3, the drainage tube body comprises a flushing tube 8 and a drainage tube 9 which are fixedly connected into a whole in parallel, the sections of the flushing tube 8 and the drainage tube 9 are all of annular structures formed by connecting arc-shaped parts and straight line parts, and the straight line parts of the flushing tube 8 and the drainage tube 9 are fixedly connected into a whole; the lower parts of the flushing pipe 8 and the liquid discharge pipe 9 are bent and penetrate out of the outer side wall of the flexible air bag 1, the included angle between the bent section of the lower part of the flushing pipe 8 and the upper section of the flushing pipe is 90-150 degrees, the bending angle of the liquid discharge pipe 9 is consistent with that of the flushing pipe 8, and the flushing pipe 8 and the liquid discharge pipe 9 are ensured to smoothly penetrate out of the flexible air bag 1 and be inserted into a thoracic cavity or a mediastinum cavity of a patient; the lower end part of the liquid discharge pipe 9 extends out of the lower end of the flushing pipe 8 (namely, the length of the lower bending section of the liquid discharge pipe 9 is longer than that of the lower bending section of the flushing pipe) and is uniformly provided with liquid discharge side holes 10 in a staggered manner, so that smooth liquid discharge is ensured, the lower end part of the flushing pipe 8 is uniformly provided with flushing side holes 11, the flushing area is increased, and the flushing effect is ensured; the top of the flushing pipe 8 is connected with a flushing pump or other flushing devices, the top of the liquid discharge pipe 9 is connected with a water suction pump or a liquid discharge bag and the like, and flow control valves 12 are arranged at the tops of the flushing pipe 8 and the liquid discharge pipe 9, so that the flushing rate and the liquid discharge rate can be conveniently adjusted.
In order to reduce the infection probability of patients, the outer side wall of the flexible air bag 1 corresponding to the second flexible sealing pad 6 is provided with an antibiotic layer, so that the fistula can be diminishing inflammation and the safety is improved.
As shown in fig. 1 and 3, the outer side wall of the air inlet and outlet pipe 5 is fixedly connected with the outer side walls of the flushing pipe 8 and the liquid discharge pipe 9, so that the pipe is convenient to place, and the difficulty of placing the pipe is reduced.
In actual manufacturing, the flushing pipe 8 and the liquid discharge pipe 9 are made of materials with certain elasticity and toughness, can bear certain negative pressure, is convenient for negative pressure suction, and can be bent when being placed in the pipe so as to meet the plugging requirements of anastomotic fistula at different positions; the size of the flexible air bag 1 can be made into a plurality of different specifications so as to meet the requirements of different anastomotic fistulae and patients.
In actual operation, the flexible air bag 1 is ensured to be in an unfilled state, then a conventional catheter and a guide wire enter a pus cavity of a mediastinum or a thoracic cavity through a nasal cavity, an upper esophageal segment and an anastomotic fistula under perspective, the lower part of a drainage tube body is placed into the pus cavity of the mediastinum or the thoracic cavity through the guide wire, the flexible air bag 1 is ensured to be positioned at the outer side of the anastomotic fistula, the second flexible sealing gasket 6 is positioned at the outer side of the anastomotic fistula, then air is introduced into the flexible air bag 1 through the air inlet and outlet pipe 5, the second flexible sealing gasket 6 is ensured to be tightly attached to the fistula, the fistula is tightly plugged through the extrusion force of the flexible air bag 1 to the fistula, physiological saline and other therapeutic drugs are injected into the pus cavity of a patient through the flushing pipe 8, pus or flushing waste liquid in the pus cavity of the patient is discharged to the outside through the liquid discharge pipe 9, the pollution probability of the anastomotic fistula of the patient is reduced, and the infection probability of the patient is further reduced.
