Abstract

本发明公开颅底‑颞下颌关节联合假体,包括颅底部,颧弓部,关节窝部,髁突头部和关节柄,所述颅底部修补颅底缺损,为钛合金材质,其边缘有若干固定孔,用于固定在颞骨上,所述颧弓部修补颧弓缺损,为钛合金材质,前端有若干固定孔,用于固定在颧弓上,所述关节窝部模拟人体下颌运动的关节面,为超高分子量聚乙烯材质,所述颅底部,颧弓部和关节窝部为一体连接,所述髁突头部呈圆柱形中空,为钴铬钼合金材质,所述关节柄修补部分升支,为钛合金材质,关节柄上设置固定孔,所述髁突头部和关节柄为一体连接。不同部位不同材质实现最好效果,个性化的颅底部,颧弓部和关节窝部,以及髁突头部和关节柄分别为异种材料一体连接,使得假体更加稳定牢固,固定效果更好。

Description

颅底-颞下颌关节联合假体
技术领域
本发明涉及医疗器械领域,更具体地讲,涉及一种颅底-颞下颌关节联合假体。
背景技术
颅底占位性病变由于位置比较隐匿,当发现时往往已引起颅底骨破坏,甚至会波及外部的髁突头,影响颞下颌关节(temporomandibular joint,TMJ)结构和功能,如颞骨巨细胞瘤、神经鞘瘤等;而颞下颌关节窝紧邻颅底,最薄处仅相隔3.2mm骨质,原发于颞下颌关节的肿瘤可能突破颅底进颅,如骨软骨瘤、侵袭性滑膜软骨瘤病、腱鞘巨细胞瘤及其他恶性肿瘤等;二者可谓“唇齿相依”。因此,在行颅底区域肿瘤性疾病的手术治疗时,目前多采用瘤体+颅底病变骨+髁突关节头一并切除。
颅底骨是颌面大血管入颅、脑神经出颅的“交通要道”,是分隔颅内神经组织、保证其正常工作环境的必要结构;同时,颞下颌关节是人体中惟一可以同时进行滑行和铰链运动的联动关节,其结构精细,功能复杂,是人体最为活跃的关节,每天行使功能超过20000次,包括张闭口、咀嚼、语言、吞咽等,对颌面功能运动至关重要。但如何同时对颅底骨和关节头缺损区域进行完整地、功能性地修复成为一个很棘手的问题。
目前,对关节头的修复技术相对成熟,如肋骨软骨、胸锁关节、下颌支、人工关节等;而对颅底骨的修复,可采用如自体髂骨、自体顶骨外板及钛网、Medpore等人工材料。人工颞下颌关节假体避免了第二术区的创伤,又可进行个体化塑形,是关节修复的热点和趋势。人工关节窝、关节头的全关节假体已获得成熟的应用,但颅底、关节窝、关节头、下颌骨作为一个整体的人工假体体系未见报道。
为此,本研究计划根据国人关节区解剖生理学和运动学特点,设计符合生物力学要求的颅底-颞下颌关节重建的联合假体。本研究拟在前期研究的基础上,对临床各类关节疾病个性化需求的关节假体进行分类,找出其共性和个性元素;再借鉴大关节外科诸如在人工膝关节、髋关节等研发上的外科新技术、治疗理念、材料工艺、加工能力以及国内外成功经验的基础上,研发适合中国人TMJ特性的个性化颅底-颞下颌关节联合假体,开启大范围颅颌面人工关节假体研发的新篇章。
发明内容
本发明所要解决的技术问题是,提供一种能同期修复波及颅底、颧弓、颞下颌关节等大范围的缺损,更符合正常解剖形态和相对位置的颅底-颞下颌关节联合假体。
为了解决上述问题,本发明提供颅底-颞下颌关节联合假体,其特征在于,包括颅底部,颧弓部,关节窝部,髁突头部和关节柄,所述颅底部修补颅底缺损,为钛合金材质,其边缘有若干固定孔,用于固定在颞骨上,所述颧弓部修补颧弓缺损,为钛合金材质,前端有若干固定孔,用于固定在颧弓上,所述关节窝部模拟人体下颌运动的关节面,为超高分子量聚乙烯材质,所述颅底部,颧弓部和关节窝部为一体连接,所述髁突头部呈圆柱形中空,为钴铬钼合金材质,所述关节柄修补部分升支,为钛合金材质,关节柄上设置固定孔,所述髁突头部和关节柄为一体连接。
作为一个优选方案,所述颧弓部下部设置有密集微孔,所述关节窝部上部镶嵌在所述颧弓部下部的密集微孔中形成异种材料一体连接。
作为一个优选方案,所述密集微孔大小为20*20mm,孔隙率70-80%,孔径400-800um。
作为一个优选方案,所述关节柄为头端呈圆柱形平台的钛合金结构,与所述髁突头部通过锥度连接形成异种材料一体连接。
本发明的优点在于,个性化的颅底-颞下颌关节联合假体各个组成部分的形状和相对位置与正常解剖结构类同,能同期修复波及颅底、颧弓、颞下颌关节等大范围的缺损,更符合正常解剖形态和相对位置。不同部位不同材质实现最好效果,颅底部,颧弓部和关节窝部,以及髁突头部和关节柄分别为异种材料一体连接,使得假体更加稳定牢固,固定效果更好。

具体实施方式
下面结合附图对本发明做详细说明,实施例的用途仅在于解释本发明而非限定技术方案。
实施例1
请参见图1,颅底-颞下颌关节联合假体包括颅底部1,颧弓部2,关节窝部3,髁突头部4和关节柄5,所述颅底部1修补颅底缺损,为钛合金材质,其边缘有若干固定孔11,用于固定在颞骨上,所述颧弓部2修补颧弓缺损,为钛合金材质,前端有若干固定孔21,用于固定在颧弓上,所述关节窝部3模拟人体下颌运动的关节面,为超高分子量聚乙烯材质,所述颅底部1,颧弓部2和关节窝部3为一体连接,所述髁突头部4呈圆柱形中空,为钴铬钼合金材质,所述关节柄5修补部分升支,为钛合金材质,关节柄5上设置固定孔51,所述髁突头部4和关节柄5为一体连接。
所述颅底部-颧弓部为个性化设计颅底和颧弓假体的范围和大小的钛合金材质,向内修复关节窝和中颅窝的骨质缺损,向上可修复颞骨或蝶骨缺损,向后可修复颞骨岩部缺损,向前可修复颧弓和眼眶的骨质缺损。在其上部,前部,后部和内侧存在数个翼板,其中间有孔,用于钛合金螺钉固定。
请参见图2和图3,颧弓部2下部设置有密集微孔22(密集微孔大小为20*20mm,孔隙率70-80%,孔径400-800um),关节窝部3假体(超高分子量聚乙烯材质)上部通过搅拌摩擦焊接原理,镶嵌在所述颅底部-颧弓部下部设置的密集微孔中,形成牢固的异种材料一体连接(金属和非金属材料连接)。
请参见图,4和图5,髁突头部4是内部为圆柱形中空的钴铬钼合金,关节柄5为头端呈圆柱形平台的钛合金结构(高为8mm,锥度为8-10°),两者通过锥度连接形成异种材料一体连接。
以上所述仅是本发明的优选实施方式,应当指出,对于本技术领域的普通技术人员,在不脱离本发明原理的前提下,还可以做出若干改进和润饰,这些改进和润饰也应视为本发明的保护范围。

1.颅底-颞下颌关节联合假体,其特征在于,包括颅底部,颧弓部,关节窝部,髁突头部和关节柄,所述颅底部修补颅底缺损,为钛合金材质,其边缘有若干固定孔,用于固定在颞骨上,所述颧弓部修补颧弓缺损,为钛合金材质,前端有若干固定孔,用于固定在颧弓上,所述关节窝部模拟人体下颌运动的关节面,为超高分子量聚乙烯材质,所述颅底部,颧弓部和关节窝部为一体连接,所述髁突头部呈圆柱形中空,为钴铬钼合金材质,所述关节柄修补部分升支,为钛合金材质,关节柄上设置固定孔,所述髁突头部和关节柄为一体连接。
2.根据权利要求1所述的颅底-颞下颌关节联合假体,其特征在于,所述颧弓部下部设置有密集微孔,所述关节窝部上部镶嵌在所述颧弓部下部的密集微孔中形成异种材料一体连接。
3.根据权利要求1所述的颅底-颞下颌关节联合假体,其特征在于,所述密集微孔大小为20*20mm,孔隙率70-80%,孔径400-800um。
4.根据权利要求1所述的颅底-颞下颌关节联合假体,其特征在于,所述关节柄为头端呈圆柱形平台的钛合金结构,与所述髁突头部通过锥度连接形成异种材料一体连接。
