Abstract
本发明提供了一种肱骨小头锁定接骨板。其包括,由近到远依次连接的接骨板近侧部组件、接骨板远侧部组件、以及接骨板远侧部组件垂直延伸出的接骨板侧翼部组件。本发明便于术中操作,并增加骨折固定的可靠性,减少术后并发症,值得在肱骨小头骨折术中推广使用。
Description
技术领域
本发明涉及医疗器械的骨折固定接骨板系统,具体涉及一种肱骨小头锁定接骨板。
背景技术
肱骨小头骨折是临床常见损伤,成人及儿童均可发生。目前尚无专门针对肱骨小头骨折设计的接骨板系统,尤其是肱骨小头的冠状面骨折及粉碎性骨折临床处理极为棘手,常出现骨不愈合、骨折块再移位、甚至肱骨小头骨坏死等并发症,导致术后肘关节功能障碍。
在创伤所致的肱骨小头骨折中,冠状面骨折较为多见,其次是高能量损伤所致的粉碎性骨折。对肱骨小头的冠状面骨折,临床常采用普通的空心螺钉进行骨折固定,但骨折端在肘关节活动时受剪切应力作用,很难达到坚强固定。若骨折块较小,单纯空心螺钉无法有效固定,且有螺钉损伤关节软骨或穿入关节腔导致关节功能障碍的风险。对于高能量损伤所致的肱骨小头粉碎性骨折,则临床处理更为棘手,尚无有效的固定方式。应用传统的肱骨远端钢板,在肱骨小头处可用于固定骨折块的螺钉数量有限,且较大直径的螺钉可能会导致粉碎骨块的移位,导致关节面的骨折移位更加严重,后期发生创伤性骨关节炎及肱骨小头骨坏死的可能性增加。另外,传统的肱骨远端钢板无法实现肱骨小头的多角度、多平面骨折固定,因而难以维持关节运动过程中骨折块的稳定性。
发明内容
为了克服上述现有技术的不足,本发明提供了一种能够用于肱骨小头多角度多平面骨折固定的肱骨小头锁定接骨板。
本发明提供的肱骨小头锁定接骨板,包括,由近到远依次连接的接骨板近侧部组件、接骨板远侧部组件、以及接骨板远侧部组件垂直延伸出的接骨板侧翼部组件,其中,
所述接骨板近侧部组件包括接骨板近侧底板、设于所述的接骨板近侧底板上由近到远依次排列的多个近侧部锁定螺纹孔和近侧部滑动螺纹孔、与所述多个近侧部锁定螺纹孔相匹配的多个近侧部锁定螺钉和与所述近侧部滑动螺纹孔相匹配的近侧部加压螺钉;
所述接骨板远侧部组件包括接骨板远侧底板、设于所述的接骨板远侧底板上由近到远依次排列的多个远侧部近排锁定螺纹孔、远侧部远排锁定螺纹孔和多个远侧部中心锁定螺纹孔、克氏针孔、与所述多个远侧部近排锁定螺纹孔相匹配的多个远侧部近排锁定螺钉、与所述远侧部远排锁定螺纹孔相匹配的远侧部远排锁定螺钉及与所述多个远侧部中心锁定螺纹孔相匹配的远侧部中心锁定螺钉;克氏针固定孔为1.2mm直径的普通克氏针固定孔。
所述的接骨板侧翼部组件包括接骨板侧翼板、设于所述的接骨板侧翼板上的多个侧翼锁定螺纹孔及与所述的多个侧翼锁定螺纹孔相匹配的侧翼部锁定螺钉。
较佳地,所述的接骨板远侧底板的轴线与所述的接骨板近侧底板的轴线之间的夹角在25°~35°之间。
较佳地,所述的接骨板近侧底板与所述接骨板远侧底板连接处起至所述接骨板远侧底板端部厚度逐渐变薄,所述的接骨板远侧底板端部厚度与所述的接骨板侧翼板厚度相同。
较佳地,所述的远侧部中心锁定螺纹孔的轴线与所述的接骨板远侧底板的轴线之间的夹角在50°~60°之间;
所述的远侧部远排锁定螺纹孔的轴线与所述的接骨板远侧底板的轴线之间的夹角在60°~70°之间;
所述的远侧部近排锁定螺纹孔的轴线与所述远侧部中心锁定螺纹孔的轴线平行。
较佳地,所述的远侧部中心锁定螺钉、所述的远侧部远排锁定螺钉和所述的远侧部近排锁定螺钉的直径均在2.2~2.7mm之间。
较佳地,所述的侧翼锁定螺纹孔的轴线与所述的接骨板侧翼板垂直。
较佳地,所述的侧翼锁定螺钉的直径在2.0~2.7mm之间。
本发明的肱骨小头锁定接骨板,接骨板贴近肱骨小头特殊解剖形态,利于手术中骨折的复位与固定。远侧部及侧翼超薄结构,减少关节周围软组织干扰,降低术后软组织并发症。远侧部肱骨小头中心锁定螺钉、肱骨小头近排锁定螺钉、肱骨小头远排锁定螺钉,并结合侧翼的侧方锁定螺钉对肱骨小头进行三维立体固定,达到骨折块的稳定固定,有利于的骨折复位、固定及术后稳定的维持,减少术后骨不愈合,骨折块移位和肱骨小头骨坏死等并发症。
因此,本发明便于术中操作,并增加骨折固定的可靠性,减少术后并发症,值得在肱骨小头骨折术中推广使用。

具体实施方式
下面结合具体的实施例对本发明作进一步地说明,以更好地理解本发明。
如图1和图2所示,本发明的肱骨小头锁定接骨板包括,由近到远依次连接的接骨板近侧部组件1、接骨板远侧部组件2、以及接骨板远侧部组件2垂直延伸出的接骨板侧翼部组件3,其中,接骨板近侧部组件1包括接骨板近侧底板17、设于接骨板近侧底板17上由近到远依次排列的两个近侧部锁定螺纹孔5和近侧部滑动螺纹孔4、与两个近侧部锁定螺纹孔5相匹配的两个近侧部锁定螺钉12和与近侧部滑动螺纹孔4相匹配的近侧部加压螺钉11;分别匹配近侧部加压螺钉11和近侧部锁定螺钉12用于接骨板近端固定。
接骨板远侧部组件2包括接骨板远侧底板18、设于接骨板远侧底板18上由近到远依次排列的两个远侧部近排锁定螺纹孔8、远侧部远排锁定螺纹孔7和两个远侧部中心锁定螺纹孔6、两个克氏针孔9、与远侧部近排锁定螺纹孔8相匹配的远侧部近排锁定螺钉15、与远侧部远排锁定螺纹孔7相匹配的远侧部远排锁定螺钉13及与远侧部中心锁定螺纹孔6相匹配的远侧部中心锁定螺钉14;
接骨板侧翼部组件3包括接骨板侧翼板19、设于接骨板侧翼板19上的两个侧翼锁定螺纹孔10及与两个侧翼锁定螺纹孔10相匹配的侧翼部锁定螺钉16。侧翼部组件3的末端延伸为2页。
接骨板远侧底板18的轴线与接骨板近侧底板17的轴线之间的夹角在25°~35°之间。符合肱骨髁部部前屈的解剖特点,利于术中复位与固定,避免术中进行接骨板的反复塑形导致接骨板固定强度的降低。
接骨板近侧底板17与接骨板远侧底板18连接处起至接骨板远侧底板18端部厚度逐渐变薄,接骨板远侧底板18端部厚度与接骨板侧翼板19厚度相同。超薄的远侧部及侧翼部结构减少了接骨板对肘关节周围软组织的干扰,降低术后软组织并发症。
远侧部中心锁定螺纹孔6的轴线与接骨板远侧底板18的轴线之间的夹角在50°~60°之间,本实施例为55°,匹配远侧部中心锁定螺钉14,用于肱骨小头中心点的失状面固定。远侧部远排锁定螺纹孔7的轴线与接骨板远侧底板18的轴线之间的夹角在60°~70°之间;本实施例为65°,匹配远侧部远排锁定螺钉13,用于肱骨小头下极的失状面固定。远侧部近排锁定螺纹孔8与远侧部中心锁定螺纹孔平行,匹配远侧部近排锁定螺钉15,用于肱骨小头上极的失状面固定。本实施例的接骨板远侧部通过三排锁定螺钉分别实施肱骨小头中心、上极和下极的失状面固定。
远侧部近排锁定螺纹孔8的轴线与远侧部中心锁定螺纹孔6的轴线平行。远侧部中心锁定螺钉14、远侧部远排锁定螺钉13和远侧部近排锁定螺钉15的直径均在2.2~2.7mm之间。侧翼锁定螺纹孔10的轴线与接骨板侧翼板19垂直。上述垂直导致螺纹方向与侧翼部接骨板垂直,术中需要时可通过锁定套统调整侧翼末端两页的方向来调整锁定螺钉的锁定方向,完成肱骨小头冠状面的稳定固定。侧翼锁定螺钉16的直径在2.0~2.7mm之间。
本发明的肱骨小头锁定接骨板的形态符合人体肱骨远端的解剖特点,有利于手术操作。远侧部和侧翼部的超薄结构,减少对肘关节周围的软组织干扰,降低软组织并发症的发生。远侧部的三排锁定螺钉固定分别对肱骨小头中心点、上极和下极进行失状面的锁定固定,侧翼部的两枚锁定螺钉完成对肱骨小头的冠状面固定,通过三维立体固定增加了骨折块固定的可靠性和稳定性,从而大大减少骨不愈合、骨折移位及肱骨小头坏死的可能性。值得在肱骨小头骨折的病例中推广和使用。
因此,本发明便于术中操作,且其为一体式结构,并增加骨折固定的可靠性,减少术后并发症,值得在肱骨小头骨折术中推广使用。

1.一种肱骨小头锁定接骨板,其特征在于,包括,由近到远依次连接的接骨板近侧部组件、接骨板远侧部组件、以及接骨板远侧部组件垂直延伸出的接骨板侧翼部组件,其中,
所述接骨板近侧部组件包括接骨板近侧底板、设于所述的接骨板近侧底板上由近到远依次排列的多个近侧部锁定螺纹孔和近侧部滑动螺纹孔、与所述多个近侧部锁定螺纹孔相匹配的多个近侧部锁定螺钉和与所述近侧部滑动螺纹孔相匹配的近侧部加压螺钉;
所述接骨板远侧部组件包括接骨板远侧底板、设于所述的接骨板远侧底板上由近到远依次排列的多个远侧部近排锁定螺纹孔、远侧部远排锁定螺纹孔和多个远侧部中心锁定螺纹孔、克氏针孔、与所述多个远侧部近排锁定螺纹孔相匹配的多个远侧部近排锁定螺钉、与所述远侧部远排锁定螺纹孔相匹配的远侧部远排锁定螺钉及与所述多个远侧部中心锁定螺纹孔相匹配的远侧部中心锁定螺钉;
所述的接骨板侧翼部组件包括接骨板侧翼板、设于所述的接骨板侧翼板上的多个侧翼锁定螺纹孔及与所述的多个侧翼锁定螺纹孔相匹配的侧翼部锁定螺钉。
2.根据权利要求1所述的肱骨小头锁定接骨板,其特征在于,所述的接骨板远侧底板的轴线与所述的接骨板近侧底板的轴线之间的夹角在25°~35°之间。
3.根据权利要求1所述的肱骨小头锁定接骨板,其特征在于,所述的接骨板近侧底板与所述接骨板远侧底板连接处起至所述接骨板远侧底板端部厚度逐渐变薄,所述的接骨板远侧底板端部厚度与所述的接骨板侧翼板厚度相同。
4.根据权利要求1所述的肱骨小头锁定接骨板,其特征在于,所述的远侧部中心锁定螺纹孔的轴线与所述的接骨板远侧底板的轴线之间的夹角在50°~60°之间;
所述的远侧部远排锁定螺纹孔的轴线与所述的接骨板远侧底板的轴线之间的夹角在60°~70°之间;
所述的远侧部近排锁定螺纹孔的轴线与所述远侧部中心锁定螺纹孔的轴线平行。
5.根据权利要求1所述的肱骨小头锁定接骨板,其特征在于,所述的远侧部中心锁定螺钉、所述的远侧部远排锁定螺钉和所述的远侧部近排锁定螺钉的直径均在2.2~2.7mm之间。
