Abstract

本发明公开了一种组合式骨盆漂移微创内固定装置,该装置实现的是骨盆前环和后环的全锁定漂移微创内固定技术,其创新装置提供的是万向组配螺钉(空心或实心)、球头连接器、连接棒(光滑或滚花)以及移动固定块(或钩)和与其连接的万向螺钉(全螺纹或半螺纹)。本装置利用器械本身和配套工具可针对骨盆创伤实施加压、撑开、按压、提拉、撬拨、旋转等技巧进行骨折整复,再通过万向组配螺钉、球头连接器、连接棒和移动固定块之间的锁定连接而形成稳定的内固定装置,该技术采用小切口微创操作,术中不显露腹部沟血管神经,连接装置置于皮下,手术操作简便快捷,出血量小,创伤小,有效避免术中及术后并发症,并以可调节全锁定恢复骨盆整体结构。

Description

组合式骨盆漂移微创内固定装置
技术领域
本发明涉及一种骨科医疗器械技术领域,具体涉及一种适合于治疗骨盆骨折或矫形的漂移式皮下内固定装置。
背景技术
骨盆骨折因复位要求不高,而传统的治疗方式多为保守牵引卧床或外固定支架固定,但保守治疗存在卧床时间过长、功能恢复不佳,而外固定治疗又存在固定不稳,护理困难,甚至发生钉道感染和骨髓炎的严重问题。
近些年来,随着内固定技术的发展,临床采用切开复位内固定治疗,这种手术治疗的问题是手术创伤大,出血多,手术时间长,骨折显露困难,内固定置入易造成血管神经损伤等问题。
本发明装置力图通过器械结构及功能的优势,实施一种创新的内固定技术,该技术实现的技术优势是微创置钉、皮下置棒和全锁定漂移式固定,实现的操作优势是利用器械本身的复位功能在锁定前进行骨折整复,整个手术过程均采用MIPO技术,内固定置入均在皮下完成,不显露骨折部位和血管神经,手术操作简便快捷,同时这一技术从器械性能及手术操作也超越现有的内固定技术和外固定技术。
发明内容
本发明装置实现的是骨盆前环和后环的全锁定漂移微创内固定技术,其创新装置提供的是由万向组配螺钉、球头连接器、连接棒以及移动连接块和万向钉组成。其中万向组配螺钉由球头螺钉、双球面垫圈、顶丝和连接体组成;球头连接器一端为球头,其直径与球头螺钉的球头直径一致,另一端为偏心为盲孔端,棒孔段至少设置设有一个螺栓孔和螺栓,连接棒为圆棒,其表面为光滑或不同形式的滚花处理,移动连接块与主平面平行设有一个棒孔,与垂直面设有至少一螺钉孔和锁定孔,其螺钉孔与球头螺钉和顶丝配合。锁定孔与锁定螺帽配合,固定钩与主平面平行设有一个棒槽,与垂直面设有至少一个螺钉孔和一个螺栓孔,其螺钉孔与球头螺钉和顶丝配合,螺栓孔与螺栓配合。本发明装置利用器械本身和配套工具实施骨盆骨折的加压、撑开、按压、提拉、撬拨、旋转等操作进行骨折整复。

锁定机制
1.万向组配螺钉与球头连接器间锁定:连接体内先置入球头螺钉,在置入双球面垫圈,当球头连接器的球头经“U”形槽置入连接体内的球面垫圈上面,同时顶丝沿连接体内螺纹拧入,通过顶丝底面依次挤压连接器球头、双球面垫圈、螺钉球头及连接体底面,以之构成万向组配螺钉与球头连接器的锁定连接。
2.球头连接器与连接棒间锁定:当连接棒插入连接器的棒孔,通过螺栓孔拧入螺栓,其螺栓头部挤压棒表面而构成球头连接器与连接棒的锁定连接。
3.移动固定块与连接棒间的锁定:当连接棒穿入固定块的棒孔,锁帽通过锁定孔拧入,其帽头下缘挤压锁定孔上缘使棒孔回缩环抱连接棒,同时帽头外缘通过交叉孔挤压棒表面,二种挤压协同作用将棒与固定块构成锁定连接。
4.移动固定钩与连接棒间锁定:当固定钩的棒槽挂靠连接棒后,通过螺栓孔拧入螺栓,其螺栓前部挤压连接棒表面而构成棒钩的锁定连接。
5.万向钉与连接块间锁定:球头螺钉沿连接块的螺钉孔置入,顶丝通过螺钉孔拧入并挤压螺钉球头,使钉连接块构成锁定连接。
工作原理
1.将球头连接器的置于万向组配螺钉的连接体内,通过顶丝将钉、体和球锁紧。万向组配螺钉实现的是万向置钉,多置于双侧髂后上下脊之间或髂前上下脊,其功能是作为漂移固定的“桥基”作用,并利用其实施骨折整复。
2.连接棒插入连接器的棒孔内通过螺栓将其锁紧。连接棒实现的功能是将双侧万向组配螺钉和连接棒连为一体,并在实施撑开和加压时调整棒的长短。
3.移动连接块穿入连接棒,可在棒上任意滑动和旋转,确定理想位置后利用锁帽将其锁定,再将万向钉通过螺钉孔以任意方向置钉,并通过顶丝将其锁紧。固定钩可选择任意理想位置挂钩并锁定和置入万向钉。移动连接块和万向钉将连接棒置于皮下的漂浮状态,其实现的功能是万向置钉,并通过连接块将钉棒构成锁定连接。万向钉置于髂骨或耻骨上肢作为漂移固定的“桥墩”,并以之实施骨折整复。

Claims (14)

1.一种组合式骨盆漂移微创内固定装置,包括万向组配螺钉、球头连接器、连接棒、移动连接块和万向钉组成,其特征是:所述的固定装置主平面上设置至少一个万向组配螺钉、球头连接器、连接棒、移动连接块和万向钉。所述的球头连接器的球端与万向组配螺钉配合,球头连接器的棒孔端与连接棒配合,连接棒与移动连接块配合,移动连接块上的螺钉孔与万向钉配合,锁定孔与锁帽配合。
2.如权利要求1所述的组合式骨盆漂移微创内固定装置,其特征是:所述的万向组配螺钉由连接体、球头螺钉、垫圈和顶丝组成。
3.如权利要求2所述的组合式骨盆漂移微创内固定装置,其特征是:所述的连接体为中空柱状体,中空内壁上段设置有内螺纹,与顶丝的外螺纹配合;其底部为球面孔,球面表面与球头螺钉的球面配合形成“万向”结构;上半部一侧为U形开口,开口对侧设置有一个棒孔,供连接棒穿入。
4.如权利要求2所述的组合式骨盆漂移微创内固定装置,其特征是:所述的球头螺钉分为空心螺钉和实心螺钉,钉体为松质骨螺纹;螺钉通过连接体底部孔置入骨质,其球头外径与连接体内径一致,球头下表面与连接体底孔球面状配合,球头顶面与垫圈的底面相配合;球头顶部设置有供梅花改锥头插入的梅花改锥孔。
5.如权利要求2所述的组合式骨盆漂移微创内固定装置,其特征是:所述的垫圈为双球面圆形,为上下空心双球面壁,其球面与球头螺钉的球面和球头连接器的球面相配合。
6.如权利要求1所述的组合式骨盆漂移微创内固定装置,其特征是:所述的球头连接器一端为球状,其球头外径与连接体内径一致,其球头面分别与双球面垫圈的上球状面和顶丝的下球状面相配合;另一端为偏心盲孔椭圆柱状体,椭圆柱状体设置有一个偏心棒孔,椭圆体偏心一侧垂直于棒孔至少设置一个螺栓孔,螺栓孔与棒孔相通,螺栓孔设置有内螺纹并与螺栓的外螺纹相配合。
7.如权利要求1所述的组合式骨盆漂移微创内固定装置,其特征是:所述的连接棒为圆形棒,可塑形,其表面设置为光滑面或粗糙面或不同形态滚花处理。
8.如权利要求1所述的组合式骨盆漂移微创内固定装置,其特征是:所述的移动连接块由固定块与锁帽组成或固定钩与螺栓组成,其中固定块或固定钩与万向钉锁定连接形成棒钉连接体。
9.如权利要求8所述的组合式骨盆漂移微创内固定装置,其特征是:所述的移动连接块的固定块与主平面平行的一侧设有棒孔,并与连接棒相配合,另一侧与主平面垂直设有至少一个螺钉孔和一个锁定孔;锁定孔与棒孔交叉为交叉孔,锁定孔的上部与锁帽的帽头相配合,下段设置有内螺纹与锁帽的锁定螺纹相配合;螺钉孔为万向钉孔,其孔底设有球面,并与球头螺钉的球面相配合,螺钉孔上段设置有内螺纹,并与顶丝的外螺纹配合。
10.如权利要求8所述的组合式骨盆漂移微创内固定装置,其特征是:所述的移动连接块的固定钩与主平面平行的一侧设有棒槽,棒槽弧面与连接棒表面相配合,另一侧与主平面垂直设有至少一个锁定孔和螺栓孔,螺栓孔与主平面构成一定角度,底孔经向棒槽,其内设有内螺纹,并与螺栓的外螺纹配合;螺钉孔为万向孔,其孔底设有球面,并与球头螺钉的球头底部球面相配合;螺钉孔的上段设有内螺纹,并与顶丝的外螺纹相配合。
11.如权利要求9所述的组合式骨盆漂移微创内固定装置,其特征是:所述的锁帽的锁定螺纹与锁定孔的内螺纹配合,帽头顶部设有梅花改锥孔,底部弧面与锁定孔的上部弧面配合,帽头外缘经交叉孔突出于棒孔内。
12.如权利要求2和3所述的组合式骨盆漂移微创内固定装置,其特征是:所述的顶丝为外螺纹柱状形,其外螺纹与连接体内壁的内螺纹配合;顶面设置有供梅花改锥头插入梅花改锥孔;底部为球面状与球头连接器的球头表面相配合。
13.如权利要求8和10所述的组合式骨盆漂移微创内固定装置,其特征是:所述的螺栓为外螺纹柱状形,外螺纹与连接器的螺栓孔的内螺纹相配合;顶面设置有梅花改锥孔,底部有一定的锥度,当螺栓拧入螺栓孔时,底部紧压于棒表面。
