Description

Visual accurate positioning prostate expanding and cracking catheter
Technical Field
The invention relates to a prostate expanding catheter, in particular to a prostate expanding catheter with a visual accurate positioning function.
Background
The transurethral water sac expansion operation is also called as prostatic urethra and membranous urethra expansion operation, is an advanced prostatic hyperplasia treatment technology at present, is an operation which only keeps the original viscera in various operations of the prostate, has extremely small wound, short operation time and high safety coefficient, becomes an effective method for solving the urination difficulty of benign prostatic hyperplasia patients, and is an ideal choice for patients who are not willing to resect prostatic tissue or cannot tolerate the resection operation due to the old and the weak.
The most important surgical instrument in the prostate expanding operation is a prostate expanding catheter, and the existing expanding catheter has the following three defects:
1. accurate positioning problem: the existing tube placement of the expanded split catheter is dependent on the personal technology of medical staff, and whether the catheter is placed in place or not is judged by subjective intuition and experience, so that the tube placement operation is complex, the skill requirement on doctors is high, the tube placement deviation rate is extremely high, the inaccuracy of tube placement can lead to that the partially-proliferated prostate is not expanded, the effect of operation is reduced, and urine control injury and urethra injury can be caused by excessive expansion of non-prostatic urethra.
For example, the invention patent of China patent No. 201010578449.8 entitled "composite columnar Water bag prostate dilation catheter" discloses a dilation catheter which is used for dilating a urethra after water injection of a water bag at the front part is dilated, and a part of the urethra is not dilated or damaged if the position of the bag deviates from the position of the bag in the dilation catheter which is used for dilating the urethra by using one bag body to expand.
2. Water bag expansion deviation problem: once the water sac of the prostate expanding and splitting catheter is deviated, a small section of water sac head stretches into the bladder, and the pressure of the two water sacs is different due to the fact that the space of the bladder is large and the space in the urethra is small, so that liquid is squeezed into the water sac positioned on one side of the bladder during water injection, the expansion of the whole water sac is uneven, and the expansion of the water sac positioned on the urethra is small, so that the effect of expanding and splitting the urethra is seriously affected. From the above, it can be seen that the above problems occur in the prior art of such a dilation catheter that dilates the urethra after inflation with a water balloon or balloon, and it is critical that the problem of balloon positioning is not solved, which does not ensure that the balloon can be positioned exactly at the prostatic urethra and that the length of the balloon exactly matches the length of the prostatic urethra of each patient.
3. Replacement of urinary catheter problems: the existing expanding catheter has no fixed air bag after the expanding operation is finished, so that the catheter needs to be pulled out and reinserted, the catheter needs to be inserted and pulled out in the urethra of the patient for multiple times in one operation, and secondary injury of the urethra of the patient is easily caused.
Disclosure of Invention
The invention aims at: the prostate expanding catheter with the visual accurate positioning function can ensure that the expanding bag is just positioned at the prostatic urethra part, the expanded length of the expanding bag is kept to be accurately matched with the prostatic urethra part of a patient, and the expanding bag can be used as a catheter after operation without secondary intubation.
The invention is realized by the following technical scheme: the utility model provides a prostate expands and splits pipe with visual accurate positioning function which characterized in that:
the main body of the device is a crack-expanding catheter with a urethral catheterization opening at the front end part, and the urethral catheterization opening is communicated with an inner pipeline of the crack-expanding catheter;
the periphery of the expanding catheter is sequentially sleeved with a fixing bag for fixing the expanding catheter in the urethra and an expanding bag for expanding the urethra from front to back; the periphery of the expanding and cracking catheter is also provided with a positioning sleeve which is movably sleeved on the periphery of the expanding and cracking bag and can axially move relative to the expanding and cracking catheter;
the front end of the expansion catheter is respectively communicated with the fixed bag and the expansion bag, the fixed bag communicating cavity and the expansion bag communicating cavity are arranged in the expansion catheter, and the rear ends of the fixed bag communicating cavity and the expansion bag communicating cavity are communicated with the outside; the urethral catheterization opening is positioned in front of the fixed bag.
Here, in order to ensure that the dilation balloon can be adapted to different sizes of prostate, the actual length of the dilation balloon is thus larger than the usual urinary tract prostate.
The working principle and the working process are as follows:
the application steps are as follows:
step one: as shown in fig. 3, the positioning sleeve is placed into the patient's urethra from the meatus on the prostate and the front end of the positioning sleeve is secured at least to the point where the urethra is coupled to the bladder.
Step two: as shown in fig. 4, the dilation catheter is extended from within the positioning sleeve until the front of the dilation catheter is in the bladder;
step three: as shown in fig. 5, at this time, normal saline is injected into the communicating cavity of the fixed bag to expand the fixed bag, and after the fixed bag is fully expanded, the expanding and cracking catheter is always pulled backwards, and the fixed bag is just clamped at the outlet of the bladder, so that the pre-fixing of the expanding and cracking catheter in the urethra and the positioning of the front end part of the expanding and cracking catheter are completed; (since the front end of the dilatation balloon borders the fixing balloon, when the fixing balloon is just clamped at the bladder orifice, it means that the front end of the dilatation balloon is just positioned at the beginning of the prostatic part of the urethra, i.e. the position where the urethra is connected with the bladder)
Step four: as shown in fig. 6, the positioning sleeve is slowly withdrawn out of the urethra until the front end surface of the positioning sleeve just exits out of the external urethral sphincter, thereby completing the positioning of the rear end of the laceration bladder; (the prostate length of the patient can be confirmed by using the imaging examination result before operation, the front end of the positioning sleeve can be guaranteed to just withdraw from the prostatic urethra after the positioning sleeve is withdrawn backwards by a distance corresponding to the length during operation, and the part of the expansion bag outside the prostatic urethra is tightly wrapped by the positioning sleeve, so that even if water is injected into the expansion bag, the expansion bag at the inner part of the positioning sleeve cannot expand, namely, the rear end part of the expansion bag is just positioned at the distal end position of the prostatic urethra when the expansion bag is actually expanded, thereby realizing accurate positioning and not damaging other urethra.)
Then, liquid is injected into the communicating cavity of the expanding bag to enable the expanding bag to slowly expand so as to gradually expand the whole urethra prostate part until the prostate expanding operation is completed;
step five: as shown in fig. 7, the liquid in the expanding and cracking bag is discharged, the positioning sleeve is pulled out, the invention is continuously remained in the patient, and the expanding and cracking catheter is directly used as a catheter.
For better implementation of the present embodiment, the following optimization scheme is also provided:
furthermore, a flushing cavity for flushing the urethra is further arranged in the expanding and cracking catheter, one end of the flushing cavity is positioned at the front end part of the expanding and cracking catheter and communicated with the outside, and the other end of the flushing cavity is positioned at the rear end part of the expanding and cracking catheter and communicated with the outside.
In the operation, medical personnel can pour into the flushing fluid into the flushing cavity so as to conveniently flush the bladder of a patient.
Further, the expansion catheter consists of a flexible inner tube positioned at the front part and used for extending into the urethra and a rigid outer tube positioned at the rear part and used for being exposed out of the urethra, and the rigid outer tube can facilitate the tube placement and provide accurate positioning.
Here, the expanding catheter is divided into a flexible inner tube and a rigid outer tube, the flexible inner tube is adopted in order not to hurt the urethra and can be used as a common urinary catheter to be left after operation, the rigid outer tube is adopted in order to be matched with a metal core to be used as a urinary expander, and the rigid outer tube provides accurate positioning and regulates the length of the expanding bag.
The flexible inner tube can be made of materials such as silica gel, rubber, soft plastic and the like, and the rigid outer tube can be made of materials such as hard plastic or metal materials and the like.
Furthermore, the positioning sleeve is made of rigid materials, and a locking mechanism for locking or unlocking the relative moving positions of the positioning sleeve and the expanding catheter is arranged on the positioning sleeve.
The locking mechanism is used for preventing the positioning sleeve from sliding after the positioning sleeve is retreated to a required position to cause errors in positioning, and simultaneously, one hand of a medical staff is released to ensure that the medical staff is not always held by the hand for fixing the position between the positioning sleeve and the positioning sleeve.
There are a variety of locking mechanisms, such as clips, fastening screws, threaded connections, latch fixtures, and the like.
Preferably, the locking mechanism is a fastening screw arranged on the positioning sleeve.
Furthermore, in order to achieve the purpose of increasing the relative movement friction force with the crack expanding bag and preventing the positioning sleeve from sliding so as to enable the positioning to be more accurate, a plurality of sieve holes are distributed on the periphery of the pipe wall at the front part of the positioning sleeve.
Further, a cannula introduction device for placing the positioning cannula into the urethra;
the sleeve guiding device comprises a guide core and a handheld handle, wherein the guide core is used for being sleeved in the positioning sleeve body and conveying the positioning sleeve into the urethra, the handheld handle is integrally connected with the rear end part of the guide core, and a limiting boss used for limiting the positioning sleeve to slide backwards is arranged at the joint between the guide core and the handheld handle.
Here, as shown in fig. 10-11, the function of the cannula introduction device is to facilitate placement of the positioning cannula into the urethra; the use method is that the positioning sleeve is sleeved on the guide core, then the guide core is inserted into the urethra from the urethral orifice to guide the positioning sleeve into the urethra, when the front end of the positioning sleeve reaches the proximal end of the urethra, the guide core can be pulled out, and at the moment, the front part of the positioning sleeve can be reserved in the prostatic part of the urethra.
The invention is another invention point that the positioning sleeve is firstly put in and then put in the expanding catheter, the positioning sleeve has the positioning function and also has the function of preventing the urethral and urinary mucous membrane from being damaged;
the existing expanding and cracking catheters are generally made of silica gel or rubber, and because the expanding and cracking bag part is more outstanding, the surface is rough, so when the existing expanding and cracking catheters are directly inserted into the urethra, the friction force between the surface of the expanding and cracking catheter and the urethral mucosa is large, the urethral mucosa is easily damaged, and part of patients are easy to generate false channels, thereby causing operation failure.
The positioning sleeve is usually made of a metal material, the surface is smooth, and the front angle is controllable when the positioning sleeve is placed in the urethra due to the rigid material, so that the urethral mucosa is not easily damaged when the positioning sleeve is placed in the urethra, the success rate of placing the positioning sleeve is high, and after the positioning sleeve is placed in place, the crack-expanding catheter passes through the positioning catheter, so that the urethral mucosa is prevented from being damaged due to a large amount of friction between the rough crack-expanding catheter and the urethra.
Preferably, in order to facilitate the better insertion of the guide core into the urethra while avoiding causing damage to the urethral mucosa, the front portion and the rear portion of the guide core are movably connected, and the front portion of the guide core can tilt up relative to the rear portion of the guide core.
Here, the positioning sleeve is first placed over the guide core, and then the front portion of the guide core is tilted and placed into the urethra.
Preferably, in order to facilitate the grasping of medical staff, a handle is fixedly arranged outside the positioning sleeve.
Preferably, in order to enable medical staff to calculate the withdrawal amount of the positioning sleeve more accurately, the outer surface of the expanding and cracking catheter is marked with scales extending along the length direction of the expanding and cracking catheter.
Compared with the prior art, the invention has the beneficial effects that:
1. the invention adopts the cooperation of the fixed bag and the positioning sleeve to ensure that the expanding bag is just positioned at the prostatic part of the urethra and the length of the expanding bag is just matched with the length of the prostatic part of the urethra of the patient, thereby greatly reducing the difficulty of operation and realizing visual accurate positioning.
2. After the prostate expansion operation is finished, the invention can be directly remained in a patient to be used as a catheter because the fixed bag is arranged, thereby avoiding the injury to the urethra of the patient caused by the secondary insertion and extraction;
3. the invention indirectly inserts the expanding catheter into the urethra through the positioning sleeve, thereby avoiding the damage to the urethra when the expanding catheter is directly inserted into the urethra and improving the success rate of the operation.
