Abstract
The invention discloses an expandable retractor for breast tumor surgery, which is characterized in that a tail end claw is expanded by rotating a handle to support tissues around a breast surgery incision and provide a visual field and an operation space for the operation of a doctor; the retractor is mainly divided into a movement unit part and a retractor main body structure, wherein the movement unit part comprises a vertical movement unit (1), a horizontal movement unit (2) and a self-locking device (3); the retracting structure consists of a fixed frame (4), an adjusting handle (5), a lead screw (6), a nut (7), a guide rail (8), an intermediate connecting plate (9), an adjusting connecting rod group (10), a mechanism main shaft (11), an intermediate transmission shaft (12) and a tail end clamping jaw (13). According to the invention, the opening and closing movement of the end claw (13) is controlled by rotating the adjusting handle (5), the opening task of tissues around a wound is completed, the vertical (1) and horizontal movement units (2) are adjusted, and the retractor is inserted into the incision to expand and support the tissues, so that a visual field and an operation space are provided for a doctor.
Description
Technical Field
The present invention relates to an expandable retractor for breast tumor surgery; in particular to a retracting device which provides a supporting function for the tissues in the front of the breast tumor operation incision and provides an operation visual field and an operation space for a main doctor.
Background
Breast tumors have become a major public health problem in the current society, and surgery is the primary means for treating breast tumors. With the rapid development of medical science and technology, breast surgery is also advancing. The traditional surgical excision treatment method cannot meet the requirement of a patient on breast beautifying because of large trauma and permanent scar left on the breast skin. At present, excision of segmental or partial breast tissue and auxiliary axillary lymph node cleaning are the most common surgical modes, and the main advantages of the excision are that the wound is small, and the shape of the breast can be kept; however, the small incision surgery has a small surgical field, and a corresponding retractor device is needed to enlarge the surgical field and keep the wound open.
The retractor is used for retracting tissues, exposing the operation visual field, being convenient for exploration and operation and can be divided into two types of hand-held drag hooks and automatic drag hooks. At present, for small incision operations such as breast tumor and the like, due to the characteristics of individual difference of patients, different incision positions and the like, a handheld draw hook is often adopted; in the operation process, the retracting device needs to be operated by an assistant to be always in a lifting state, which is very inconvenient, and the retracting method needs more assistants; the existing retractor specially used for thoracic cavity operation can realize the functions of fixation and retraction, but the tail end of the retractor is heavy in shape and is not suitable for breast tumor operation with small incision. In addition, because the tail end of the retractor needs to be adjusted in real time according to the operation process in the operation process, the tail end adjusting mode of the existing retractor is labor-consuming, and the operation by an assistant is inconvenient. To the best of our knowledge, no such retractor has been reported which is suitable for small incisions and which can be adjusted to the size of the patient's breast. In order to facilitate the retraction operation of the small incision operation of the breast tumor for doctors, an expandable automatic retractor for the breast tumor operation is needed to be designed.
Disclosure of Invention
The invention aims to provide an expandable retractor for breast tumor surgery. The retractor mainly comprises a motion unit part and a retractor body part; the moving unit part comprises a vertical moving unit (1), a horizontal moving unit (2) and a self-locking device (3); the retractor main body consists of a fixed frame (4), an adjusting handle (5), a lead screw (6), a nut (7), a guide rail (8), an intermediate connecting plate (9), an adjusting connecting rod group (10), a mechanism main shaft (11), an intermediate transmission shaft (12) and a tail end clamping jaw (13). The opening and closing movement of the tail end claw (13) is controlled by rotating the adjusting handle (5), so that the tissue around the wound is supported; by adjusting the vertical and horizontal linear guide rails, the retractor is inserted into the incision and the tissues are expanded, so that the view field is provided for a doctor.
In order to achieve the purpose, the invention provides the following technical scheme:
the motion unit part of the retractor device consists of a vertical motion unit (1), a horizontal motion unit (2) and a self-locking device (3). The vertical movement unit (1) and the horizontal movement unit (2) are vertically arranged; the position of the end claw (13) along the insertion direction of the incision is adjusted by rotating the handle of the horizontal movement unit (2); the vertical position of the retractor is adjusted by rotating a handle of the vertical movement unit (1); the movement unit is locked by a self-locking device (3). Meanwhile, the rear part of the vertical motion unit (1) can be connected with different fixed supporting structures to fix the retractor on an operating table.
Furthermore, in the main structure of the retractor, a fixed frame (4) is connected with the moving unit part, and the other end of the fixed frame is fixedly connected with a mechanism main shaft (11); the screw rod (6) is connected with a nut (7) fixedly connected on the fixed frame (4), one end of the screw rod is fixedly connected with the adjusting handle (5) through a set screw, and the other end of the screw rod is connected with the middle connecting plate (9) and can rotate relative to the middle connecting plate. The middle connecting plate (9) is connected with a guide rail (8) fixedly connected below the fixed frame (4) through a sliding block and moves along the guide rail, a middle shaft is arranged on the other side of the middle connecting plate (9), and the middle shaft penetrates through one end of the adjusting connecting rod set (10).

Preferably, the opening and closing of the end jaw (13) is realized by the rotation of the adjusting handle (5). Preferably, the rotation angle of the claw (13) is determined by the length of the connecting rod in the adjusting linkage (10). The distance between the connecting holes connected with one end of the middle connecting plate (9) and the connecting holes connected with the main shaft (11) is equal, and the adjusting handle (5) is rotated due to different lengths of the connecting rods forming the connecting rod group (10), and the rotating angles and directions of the connecting rods of different groups in the connecting rod group (10) are different, so that the tail-end clamping jaws (13) are driven to rotate by different angles to form a rake-shaped structure.
Furthermore, the end claw (13) is made of medical stainless steel; the adjusting handle (5) of the retractor and the handle of the motion unit are positioned in front of the retractor and can be operated by an operator.
The invention has the beneficial effects that:
the retractor can realize the opening and closing of the tail end claw (13) by manually rotating the adjusting handle (5), and the driving mode is simple and reliable; and the adjusting handle (5) is operated by an assistant positioned opposite to the doctor, so that the operation of the main doctor positioned at the rear side of the retractor is not hindered. The retractor can be fixed on the edge clamping groove of the operating table through a certain fixed supporting structure, the fixing mode is simple, the retractor is not required to be held by hands after being fixed, and the workload of medical staff is reduced.
When the claw is closed, the size is small, the claw can be smoothly inserted into the incision and is opened in the incision, so that the size of the surgical incision is reduced; the end claw is bent, so that the damage to the tissue can be reduced.
The position of the tail end clamping jaw (13) is adjusted by adjusting the vertical movement unit (1) and the horizontal movement unit (2), and the handle in the guide rail does not need to be held all the time after adjustment. The end claw (13) of the retractor is opened in the operation process, and after the position of the end claw is adjusted, the adjusting handle (5) can be loosened, so that the operation is convenient. The operation can be performed by one medical staff, so that the number of the medical staff in the operation process is reduced.

The invention will now be described in detail with reference to the accompanying drawings, which are illustrative of the presently preferred embodiments of the retractor of the invention and are not intended to represent the only forms in which the invention may be constructed or utilized.
An expandable retractor for breast tumor operation, fig. 1 is a structural diagram of the retractor of the invention, the invention is mainly divided into a movement unit part and a retractor main body structure; the motion unit part comprises a vertical motion unit (1), a horizontal motion unit (2) and a self-locking device (3); the retractor main body structure comprises a fixing frame (4), an adjusting handle (5), a lead screw (6), a nut (7), a guide rail (8), an intermediate connecting plate (9), an adjusting connecting rod group (10), a mechanism main shaft (11), an intermediate transmission shaft (12) and a tail end clamping jaw (13).
Referring to fig. 1, the motion unit part of the retractor consists of a vertical motion unit (1) and a horizontal motion unit (2), the two motion units can realize two-degree-of-freedom motion and control the position of a tail end claw (13) in a cross section vertical to an operating table. Adjusting the position of the end jaw (13) in the incision insertion direction by rotating the horizontal movement unit end handle; the vertical position of the retractor is adjusted by rotating a handle at the tail end of the vertical movement unit, and the movement unit is locked by a self-locking device (3) after being adjusted.
The rear part of the vertical motion unit (1) is provided with a mounting hole which is connected with different fixed supporting structures to fix the retractor on an operating table.
Referring to fig. 1 and 2, the retractor comprises a fixing frame (4), an adjusting handle (5), a lead screw (6), a nut (7), a guide rail (8), an intermediate connecting plate (9), an adjusting linkage (10), a mechanism spindle (11), an intermediate transmission shaft (12) and a terminal jaw (13). Wherein the fixed frame (4) is connected with the moving unit part, and the other end is fixedly connected with a mechanism main shaft (11); the lead screw (6) adopts a trapezoidal lead screw and can be self-locked, the middle part of the lead screw is connected with a nut fixedly connected on the fixed frame (4) through trapezoidal threads, one end of the lead screw is fixedly connected with the adjusting handle (5) through a set screw, and the other end of the lead screw is connected with the middle connecting plate (9) through a bearing and can rotate relative to the fixed connecting plate (9); the end part of the screw rod (6) is connected with a fixed nut, so that the screw rod can be prevented from being separated from the bearing and the intermediate connecting plate (9).
The middle connecting plate (9) is connected with a guide rail (8) fixedly connected below the fixed frame (4) through a sliding block and moves along the guide rail (8), and a middle shaft is arranged on the other side of the middle connecting plate (9) and penetrates through one end of the four groups of adjusting connecting rod groups (10) to be connected with the middle shaft.
The adjusting connecting rod groups (10) are four groups in total, each group of adjusting connecting rods (10) consists of two connecting rods with different lengths, and the two connecting rods are connected with each other and can rotate mutually. The end part of one connecting rod is penetrated by a middle shaft in the middle connecting plate (9), and the connecting rod can rotate along the middle shaft; the middle part of the other connecting rod is provided with a positioning hole which is penetrated by the mechanism main shaft (11), the connecting rod can rotate along the mechanism main shaft (11), and the end part of the connecting rod is fixedly connected with the middle transmission shaft (12).
The front and back movement of a middle connecting plate (9) in the retractor can drive four groups of adjusting connecting rod groups (10) to rotate along different directions relative to a main shaft (11), the other ends of the adjusting connecting rod groups (10) are fixedly connected with tail end clamping jaws (13) through a middle transmission shaft (12), and the clamping jaws (13) penetrate through the main shaft (11) of the mechanism and can rotate relative to the main shaft (11) of the mechanism. Wherein, a tail end claw is fixedly connected with the tail end of the main shaft of the mechanism all the time and is not connected with the middle transmission shaft (12). Meanwhile, the rotation angle of the tail end claw (13) is consistent with the rotation angle of the adjusting connecting rod group (10) driving the tail end claw to rotate.
The opening and closing of the tail end claw (13) are realized by the rotation of the adjusting handle (5). The adjusting handle (5) is rotated to enable the screw rod (6) to move back and forth along the nut (7) while rotating, the middle connecting plate (9) is driven to move back and forth along the guide rail (8), the middle connecting plate (8) simultaneously drives the four adjusting connecting rod sets (10) to rotate along different directions relative to the mechanism main shaft (11), the adjusting connecting rod sets (10) move to the tail end clamping jaws (13) through the middle transmission shaft (12), and the different tail end clamping jaws (13) are driven to rotate to different angles relative to the mechanism main shaft (11).
The rotation angle of the claw (13) in the retractor is determined by the length of the connecting rod in the adjusting linkage (10). As shown in fig. 4, which is a schematic diagram of the movement of the adjusting linkage of the present invention, four groups of adjusting linkages (10) are composed of two linkages with different lengths, namely a linkage assembly 1 and a linkage assembly 2, the linkage assemblies 1 and 2 of each two groups of linkages are equal in length but symmetrically installed, and the distance between the connecting hole connecting one end of the middle connecting plate (9) and the connecting hole connecting the main shaft (11) is equal, thereby ensuring the smooth installation and movement of the four groups of linkages. Because the connecting rods in different connecting rod groups are different in length, the two connecting rods in the connecting rod groups rotate relatively along with the movement of the middle connecting plate (9) by the same distance, and the rotating angle and the direction of the connecting rod assembly 2 of each connecting rod group are different, as shown in fig. 4, when the connecting rod groups rotate to a certain position, the angle 1 and the angle 2 of different connecting rod groups are different, and the rotating angle and the direction of the connecting rods are different, so that the tail end clamping jaws (13) are driven to rotate by different angles and directions to form a rake-shaped structure.
The using method of the retractor comprises the steps of fixing the retractor by using a mounting hole at the rear part of a vertical movement unit (1) before an operation is started, adjusting the vertical movement unit (1) and a horizontal movement unit (2) and enabling a tail end claw (13) to move to a proper position; the adjustment handle (5) is turned to bring the end jaws (13) into a closed position, i.e. the closed position of the end jaws of the invention shown in fig. 3.
When the operation is started, the handle of the horizontal movement unit (2) is adjusted to enable the tail end claw (13) to be inserted into the operation incision and continuously move forwards, meanwhile, the adjustment handle (5) is rotated to gradually open the tail end claw (13), and finally, the tail end claw is inserted into the proper position of the wound and opened to the proper angle, namely, the tail end claw opening state shown in fig. 1.
Then the handle of the vertical movement unit (1) is rotated to lift the height of the end claw (13), thereby providing the doctor with an operation visual field and an operation space. After the adjustment is finished, the vertical movement unit (1) and the horizontal movement unit (2) are locked, and the opening state of the retractor is kept in the operation.
After the operation, according to the opposite process, the vertical movement unit (1) is unlocked, and the height of the tail end claw (13) is reduced; rotating the adjusting handle (5) to gradually close the tail end claw (13) to the state shown in figure 2; unlocking the horizontal movement unit (2), adjusting the handle of the horizontal movement unit to slowly draw the end claw (13) out of the operation incision, and then adjusting the vertical movement unit (1) to lift the position of the retractor main body to keep the retractor main body away from the patient.

in the main structure of the retractor, a fixed frame (4) is connected with a movement unit part, and the other end of the fixed frame is fixedly connected with a mechanism main shaft (11); the screw rod (6) is connected with a nut (7) fixedly connected on the fixed frame (4), one end of the screw rod is fixedly connected with the adjusting handle (5) through a set screw, the other end of the screw rod is connected with a middle connecting plate (9) and can rotate relative to the middle connecting plate, the middle connecting plate (9) is connected with a guide rail (8) fixedly connected below the fixed frame (4) through a sliding block and moves along the guide rail, an intermediate shaft is arranged on the other side of the middle connecting plate (9), and the intermediate shaft penetrates through one end of the adjusting connecting rod set (10);
