Abstract
本发明公开了一种多功能气管插管,包括弧形管体、内充气囊、外充气囊、内充气管、外充气管、吸痰管、注药管和呼吸机接头;弧形管体的前端为前端部向内弯曲斜通气口;内充气囊和外充气囊环设在弧形管体的前部;内充气管、外充气管、吸痰管和注药管的前段设置在弧形管体的管壁内,后段均设置在弧形管体的外部;内充气囊和外充气囊充气鼓起后的外周面为圆柱面;内充气囊和外充气囊充气鼓起后的后端面与弧形管体的外表面之间形成环形凹槽;吸痰管的前端端口设置在弧形管体的外表面上,并位于环形凹槽内;呼吸机接头与弧形管体的后端部连接。本发明的弧形管体的斜通气口的前端部向内弯曲,能够避免斜通气口划伤患者的气管,有效提高安全性。
Description
技术领域
本发明涉及一种多功能气管插管。
背景技术
在我国医院感染中,下呼吸道感染居首位,在感染部位构成比中占据了医院感染的39.7%。全国医院感染监控网对全国医院感染横断面的调查结果显示,下呼吸道感染现患率呈上升趋势,是今后医院感染控制工作中的重点。气管插管术是机械通气中建立人工气道应用最广泛、最快捷的手段。气管插管术是心肺复苏及伴有呼吸功能障碍的急危重症患者抢救过程中的重要措施。
患者行气管插管术后,声门开启,吞咽和咳嗽反射被抑制,口咽部分泌物和胃肠道反流物便滞留、沉积于声门下、气囊上区域。当气囊压过低或定时行气囊放气时,滞留沉积物就会经气囊与气管壁间的间隙吸入下呼吸道,引起下呼吸道感染。美国科研人员运用
支气管检查镜在猪身上所做的试验表明,气管插管插入气管后短短5分钟时间,即可见到有分泌物沿着球囊壁向下渗漏。胃肠道反流物与口咽分泌物误吸是细菌侵入下呼吸道,引发下呼吸道感染的主要途径。2005年美国胸科协会和美国感染病协会共同颁布的医院内获得性肺炎新指南指出,对声门下气囊上方分泌物的持续吸引是降低呼吸机相关性肺炎发生的有效途径。
随着医疗技术的发展,对气管插管的功能要求越来越高。临床希望有安全性高,能够注药和吸痰的气管插管。
多功能气管插管的优点在于:内外双球囊的气管插管具有内外两个球囊,防止外球囊意外情况下破裂,另外内外双球双球囊可以交替充气,防止气管壁长期受到压迫;带吸痰管的气管插管可以预防机械通气相关的感染发生;注药的气管插管可以方便插管过程中给病人注药治疗,拔管过程中,注入局部麻醉药,减轻拔管相关的呛咳等并发症。
现有的多功能气管插管的缺点在于:1、现有的多功能气管插管的前端通常设置斜通气口,但是由于管体的材料具有一定的硬度,在插入患者气管的过程中易造成病人呼吸通道组织结构的损伤,引发病人的二重医源性伤害,加重原有疾病的恶化,甚至会出现更加严重的插管并发症发生,不但给病人的救治带来危险,还埋下了医疗事故和医患矛盾的隐患;2、气囊充气后呈球形,与患者气管的接触为线接触,不仅有可能会对患者的气管造成伤害,而且患者咽部的分泌物极易向肺部渗透。
发明内容
本发明的目的是提供一种安全性高的多功能气管插管,降低气管插管插入患者气管过程中的风险。
实现本发明目的的技术方案是:一种多功能气管插管,包括弧形管体、内充气囊、外充气囊、内充气管、外充气管、吸痰管、注药管和呼吸机接头;所述弧形管体的前端为斜通气口;所述斜通气口的前端部向内弯曲;所述内充气囊和外充气囊环设在弧形管体的前部,并且内充气囊位于外充气囊内;所述内充气管、外充气管、吸痰管和注药管的前段均沿长度方向设置在弧形管体的管壁内,后段均设置在弧形管体的外部;所述内充气管和外充气管的前端分别与内充气囊和外充气囊连通;所述内充气囊和外充气囊充气鼓起后的外周面为圆柱面;所述内充气囊和外充气囊充气鼓起后的后端面与弧形管体的外表面之间形成环形凹槽;所述吸痰管的前端端口设置在弧形管体的外表面上,并位于环形凹槽内;所述注药管前端端口设置在弧形管体的斜通气口内;所述呼吸机接头与弧形管体的后端部连接。
所述弧形管体的内壁上设有不锈钢弹簧。
所述弧形管体的外壁上设有刻度。
所述内充气管、外充气管、吸痰管和注药管的前段沿弧形管体的周向均匀分布。
所述内充气管、外充气管、吸痰管和注药管的后端部均连有接头。
所述内充气管、外充气管、吸痰管和注药管的后端部的接头内均设有单向阀。
采用了上述技术方案,本发明具有以下的有益效果:(1)本发明的弧形管体的斜通气口的前端部向内弯曲,能够避免斜通气口划伤患者的气管,有效提高安全性。
(2)本发明的内充气囊和外充气囊充气鼓起后的外周面为圆柱面,能减少气囊对患者气管的伤害,进一步提高安全性。
(3)本发明的内充气囊和外充气囊充气鼓起后的后端面与弧形管体的外表面之间形成环形凹槽,不仅有利于收集口咽分泌物,便于冲洗和抽吸,同时也可对分泌物向肺部渗透起到一定的阻挡作用。
(4)本发明的弧形管体的内壁上设有不锈钢弹簧,防止气管插管在使用过程中打折压憋,防止病人因此窒息。
(5)本发明的弧形管体的外壁上设有刻度,能让医生确切的知道弧形管体的插入深度。
(6)本发明的内充气管、外充气管、吸痰管和注药管的后端部的接头内均设有单向阀,有效防止回流现象的产生。

具体实施方式
(实施例1)
见图1,本实施例的多功能气管插管,包括弧形管体1、内充气囊2、外充气囊3、内充气管4、外充气管5、吸痰管6、注药管7和呼吸机接头8。
弧形管体1的前端为斜通气口11。斜通气口11的前端部向内弯曲,避免斜通气口划伤患者的气管。弧形管体1的内壁上设有不锈钢弹簧12,能防止气管插管在使用过程中打折压憋,防止病人因此窒息。弧形管体1的外壁上设有刻度,能让医生确切的知道弧形管体1的插入深度。
内充气囊2和外充气囊3环设在弧形管体1的前部,并且内充气囊2位于外充气囊3内。外充气囊3意外情况下破裂后使用内充气囊2,内充气囊2和外充气囊3可以交替充气,防止气管壁长期受到压迫。
内充气管4、外充气管5、吸痰管6和注药管7的前段均沿长度方向设置在弧形管体1的管壁内,后段均设置在弧形管体1的外部。内充气管4、外充气管5、吸痰管6和注药管7的前段沿弧形管体1的周向均匀分布。内充气管4、外充气管5、吸痰管6和注药管7的后端部均连有接头。接头内均设有单向阀。内充气管4和外充气管5的前端分别与内充气囊2和外充气囊3连通。内充气囊2和外充气囊3充气鼓起后的外周面为圆柱面,能减少气囊对患者气管的伤害。
内充气囊2和外充气囊3充气鼓起后的后端面与弧形管体1的外表面之间形成环形凹槽。吸痰管6可以吸引声门下和球囊上方的分泌物,防止机械通气相关的感染发生。吸痰管6的前端端口设置在弧形管体1的外表面上,并位于环形凹槽内,不仅有利于收集口咽分泌物,便于冲洗和抽吸,同时也可对分泌物向肺部渗透起到一定的阻挡作用。
注药管7前端端口设置在弧形管体1的斜通气口11内。注药管7可以方便插管过程中给病人注药治疗,使药物迅速抵达气管内中下端,拔管过程中,注入局部麻醉药,减轻拔管相关的呛咳等并发症。呼吸机接头8与弧形管体1的后端部连接。

1.一种多功能气管插管,其特征在于:包括弧形管体(1)、内充气囊(2)、外充气囊(3)、内充气管(4)、外充气管(5)、吸痰管(6)、注药管(7)和呼吸机接头(8);所述弧形管体(1)的前端为斜通气口(11);所述斜通气口(11)的前端部向内弯曲;所述内充气囊(2)和外充气囊(3)环设在弧形管体(1)的前部,并且内充气囊(2)位于外充气囊(3)内;所述内充气管(4)、外充气管(5)、吸痰管(6)和注药管(7)的前段均沿长度方向设置在弧形管体(1)的管壁内,后段均设置在弧形管体(1)的外部;所述内充气管(4)和外充气管(5)的前端分别与内充气囊(2)和外充气囊(3)连通;所述内充气囊(2)和外充气囊(3)充气鼓起后的外周面为圆柱面;所述内充气囊(2)和外充气囊(3)充气鼓起后的后端面与弧形管体(1)的外表面之间形成环形凹槽;所述吸痰管(6)的前端端口设置在弧形管体(1)的外表面上,并位于环形凹槽内;所述注药管(7)前端端口设置在弧形管体(1)的斜通气口(11)内;所述呼吸机接头(8)与弧形管体(1)的后端部连接。
