背景技术
精神障碍(mental disorders)是一种重要的认知、情感、行为等心理障碍的综合症,可引起个体的紊乱、痛苦、甚至致残。常见的精神障碍主要表现为痴呆、抑郁、焦虑、躁狂、妄想、幻觉、情感障碍、行为怪异、人格分裂等。据估计,约有四分之一的人一年内会经历至少一种精神障碍,全球约有4.5亿人遭受精神健康方面的影响。据2009年的统计,全球在精神障碍方面每年的花费超过2.5万亿美元,到2030年将超过6万亿美元。已经成为个人、家庭和社会的重要的经济和社会负担。
精神障碍的病因比较复杂,仍缺乏清楚的病因学基础。大多数研究认为精神障碍是由生物,心理和环境因素共同导致的。随着研究的深入,近年来越来越多的证据表明肠道微生物通过脑-肠轴或菌-肠-脑轴(brain–gut axis或microbiota–gut–brain axis)能够影响中枢神经系统,在精神障碍的发病过程中发挥着重要作用。
已有研究表明肠道菌群可能能够影响压力反应(stress reactivity),焦虑(anxiety),抑郁(depression),自闭症(autism),精神分裂(schizophrenia),帕金森氏病(Parkinson's disease)等精神疾病。然而,大多研究结果都是针对啮齿动物模型的,针对人类的较少。目前,对人体肠道微生物与精神障碍的相关研究还很少,肠道微生物的组成和功能与精神障碍之间的具体关系仍不清楚。有研究认为,肠道菌群某些菌的过度增殖可能引起宿主肠道菌群失衡,以及一些病源菌的代谢产物影响了正常的脑部活动。另有研究认为,肠道中一些菌的相对比例的变化引起了肠道菌群的紊乱,导致有益菌减少,有害菌增多,过多的有害菌引起宿主的代谢异常,其产生的多种毒害物质可破坏肠粘膜的完整性,导致通透性改变,引起肠漏,大分子物质及毒素变得容易透过肠壁进入血液系统进而改变血脑屏障的通过性,可直接进入中枢神经系统,进而引起大脑出现免疫或炎症反应,阻碍大脑的正常工作,最终使人出现精神错乱。现代社会人们生活压力大,生存环境日益变差,一些心理异常,如女性的产前焦虑、产后抑郁,儿童自闭症、多动症,成年人的焦虑、抑郁、躁狂症、精神分裂症、老年人的帕金森氏症、阿尔兹海默症等开始困扰人类健康,通过检测肠道微生物有望用于对心理疾患的筛查,监测和干预。然而,到目前为止,具体的影响机制并不是很清楚,肠道菌群的构成和系统发育机制仍需要进一步的研究。
发明内容
本发明的目的是提供特异性的针对精神障碍的肠道微生物标志物,为早期精神障碍状态的检测和评估提供一种非侵入式的,无创的的方法。通过对肠道微生物标志物进行干预能够间接评价精神障碍状态,一方面,通过直接补充或通过促进有益菌的生长,抑制有害菌或潜在致病菌的增殖可以减少毒害物质的产生,保护肠粘膜的完整性,进而保护血脑屏障的正常通透性;另一方面,通过将获得的肠道微生物标志物中的某些保护性微生物进行分离、纯化、培养和加工制成益生菌,可以用于改善并恢复肠,道微生物平衡,对于降低有害微生物,减少毒性物质的产生,减轻精神障碍的身体和心理症状具有重要意义。

比较待测粪便样本和健康对照组粪便样本中Prevotella属与Bacteroides属中的比例;和/或
比较待测粪便样本和健康对照组粪便样本中Prevotella属与Bacteroides属的相对数量。
本发明的第四个方面提供了一种肠道微生物标志物筛选方法,其特征在于,所述的微生物标志物筛选方法采用随机森林算法构建判别模型,所述的微生物标志物筛选方法中特征变量选自Prevotella、Bacteroides、Ruminococcus、Dorea、Paraprevotella、Faecalibacterium、Blautia、Lachnospira、Roseburia、Sutterella、Bifidobacterium、Enterubacterium、Oscillospira、Bilophila、Anaerostipes、Parabacteriodes、Streptococcus、Fusobacterium和Enterococcus中的一种或多种组合。
本发明的第五个方面提供了一种干预精神障碍的食物,所述食物能够抑制或减少选自精神障碍组的代表微生物Bacteroides属中的一种或几种,和/或促进或者增加选自健康对照组的代表微生物Prevotella属中的一种或几种。
本发明的第六个方面提供了干预精神障碍的益生菌,所述益生菌能够抑制或减少选自精神障碍组的代表微生物Bacteroides属中的一种或几种,和/或促进或者增加选自健康对照组的代表微生物Prevotella属中的一种或几种。
本发明的第七个方面提供了干预精神障碍的药物,所述药物能够抑制或减少选自精神障碍组的代表微生物Bacteroides属中的一种或几种,和/或促进或者增加选自健康对照组的代表微生物Prevotella属中的一种或几种。
本发明的第八个方面提供了筛选食物、益生菌或药物的方法,包括比较给予该食物、益生菌或药物治疗或干预之前和之后的粪便样本中的Prevotella属与Bacteroides属中的比例;和/或粪便样本中Prevotella属与Bacteroides属的相对数量。
所述的食物/益生菌/药物在治疗或干预前后,分别检测个体肠道微生物中所述的肠道微生物标志物的含量。
所述方法更具体地包括:
a,采集所述的食物、益生菌或药物治疗或干预前后的个体粪便样本并妥善保存;
b,从个体粪便中提取DNA;
c,以粪便DNA为模板,对16s rDNA进行PCR扩增和建库;
d,对16srDNA进行高通量测序,获得测序结果;
e,对测序结果进行生物信息学分析,确定所述个体的粪便中所述肠道微生物标志物的量。
f,将检测的肠道微生物标志物的量输入判别模型进行判别。
优选地,所述的检测流程a中采集和保存过程具体的包括常温和低温保存两种方式。常温保存时需要添加DNA保护液。低温保存需要10摄氏度以下。
优选地,所述的检测流程b中粪便提取过程具体的包括添加玻璃,金属或陶瓷小球。
本发明的第九个方面提供了一种检测精神障碍的试剂盒,其包括用于从粪便样本中提取总DNA的试剂、用于扩增所提取的DNA的PCR扩增试剂和测序试剂。
本发明的有益效果:提供了诊断或治疗精神障碍类疾病的试剂盒,可通过检测肠道微生物的标志物用于对精神障碍类易感或高发人群进行筛查,有利于早期发现,为心理疾患的早期诊断提供了重要的生物学检测指标;可以监测精神障碍类疾病的干预效果,能够通过检测肠道微生物标志物对病情进行判断,掌握疾病的恢复情况;将发现的肠道微生物标志物用作开发或筛选针对精神障碍类疾病的食物/益生菌/药物的评价指标,并制作成检测试剂盒;提供了利用发现的肠道微生物标志物中的有益微生物用作益生菌或功能食品。

具体实施方式
下面将结合附图和实施例对本发明的实施方案进行详细描述。除另外定义,本文所用的所有科技术语一般具有本发明所属领域普通技术人员所通常理解的含义。一般地,本文所用的命名和实验方法是公知的,并且在本领域中常规使用。使用标准技术进行的所有操作通常是根据仪器耗材生产商的产品说明书和常规技术要求以及本文中提供的参考资料进行的。需要注意的是,本领域技术人员将理解,下列附图和实施例仅用于说明本发明,而不是对本发明的范围的限定。根据附图和优选实施方案的下列详细描述,本发明的目的和由此带来的有利方面对于本领域技术人员来说是显而易见的。
实施例中,我们共招募到精神障碍组31人以及健康对照组32人。通过测序和生物信息学分析,表明两组之间在检测到的基因数量,肠道微生物物种多样性和不同分类水平上存在不同种类的代表肠道微生物。两组之间在属水平的主要差别在Prevotella和Bacteroides等几种微生物标志物。我们将62个精神障碍个体以及健康对照作为模型的训练样本集,采集精神障碍组的10人以及健康对照组的10人作为验证样本集。通过对模型学习样本集中的粪便样品人肠道微生物的群落及功能组成特征进行对比分析找到两组之间的显著差异的潜在肠道微生物标志物。随后,通过验证样本集对上述潜在的肠道微生物标志物进行验证,从而筛选出最终的肠道微生物标志物。参照图1的实验流程,筛选精神障碍的相关肠道微生物标志物,本领域技术人员所熟知的实验步骤或者操作细节未标注,主要的实验步骤如以下几个实施例所述。实施例1:样本收集检测和DNA提取
实验共采集了63例粪便样品,其中包括31个精神障碍人群及32个年龄和地域匹配健康志愿者的粪便和血液样本。经卡方检验,两组人群在背景信息方面没有显著差异(表1)。所有个体的纳入标准为:50岁以内;身体健康;过去六个月内没有使用过抗生素、益生菌、益生元、合生元等。每个个体的新鲜粪便样品分成5份,每份约1g,样本放入冷藏保温箱中立即运输到实验室中-80℃冰箱冷冻保存。分别从粪便样品中提取总DNA。采用TIANampStool DNA kit(天根生化科技(北京)有限公司)试剂盒提取DNA,方法按照试剂盒说明实施。DNA的质量和数量检测分别采用NanoDrop ND-1000分光光度计(美国热电科技)测定在260nm和280nm的吸光率以及1%琼脂糖凝胶电泳进行检测。血液样本为5ml加入肝素钠和乙二胺四乙酸(EDTA)抗凝剂的静脉血,采集完成后立即送入实验室,3000rpm常温离心15分钟后分离血浆到灭菌的离心管中-20℃保存。采用酶联免疫检测试剂盒分别检测血液样本中两种主要神经递质五羟色胺和多巴胺的含量。

实施例2:以粪便DNA为模板,对16s rRNA进行PCR扩增和测序
按照IlluminaMiSeq测序平台(Illumina,San Diego,CA)的操作指南对细菌高可变区(V1-V3)通用的引物(正向引物序列:5′AGAGTTTGATCCTGGCTCAG3′,反向引物序列:5′TTACCGCGGCTGCTGGCAC3′)对粪便DNA样品16s rRNA基因进行Barcoded PCR扩增,将扩增产物电泳并回收纯化主要DNA片段后依据测序公司的实验要求构建文库。将上述实施例中构建好的文库按照IlluminaMiseq测序平台的操作指南对文库进行PE2×150bp测序。
实施例3:测序数据的基本处理
每个样本平均产生74620条原始序列,平均长度253bp。使用QIIME(QuantitativeInsights Into Microbial Ecology,http://qiime.org)软件(Caporaso,J.G.,J.Kuczynski,J.Stombaugh,K.Bittinger,F.D.Bushman,E.K.Costello,et al.(2010):QIIME allows analysis of high-throughput community sequencing data.NatureMethods.7(5):335-336.)对测序数据进行处理,经过质量控制,去除低质量序列和嵌合体后共获得2514535条高质量序列(BJ,H.,G.D,E.AM,F.M,W.DV,G.G,et al.(2011):Chimeric16S rRNA sequence formation and detection in Sanger and 454-pyrosequenced PCRamplicons.Genome Res.21(3):494-504.)。
实施例4:OTU聚类和物种注释
经过质量控制的测序数据通过与已知数据库进行比对和注释。可在界、门、纲、目、科、属、种水平,将每个注释上的物种或OTU在不同样品中的序列数进行整理。使用Uclust程序包按照97%的相似性算法进行OTU(Operational Taxonomy Units)的聚类(Edgar,R.C.(2010):Search and clustering orders of magnitude faster thanBLAST.Bioinformatics.26(19):2460-2461.)。根据Greengenes database,采用RDPclassifier软件进行序列比对和系统分类(McDonald,D.,M.N.Price,J.Goodrich,E.P.Nawrocki,T.Z.DeSantis,A.Probst,et al.(2012):An improved Greengenestaxonomy with explicit ranks for ecological and evolutionary analyses ofbacteria and archaea.The ISME Journal.6:610-618.)。最后,采用SINA(Version1.2.11,www.arb-silva.de)对序列进行注释,参考数据库为SILVA archaeal database(Pruesse,E.,J.Peplies,and F.O.(2012):SINA:Accurate high-throughputmultiple sequence alignment of ribosomal RNA genes.Bioinformatics.28(14):1823-1829.)。经检测,实验组共检测到37164个OTU,而对照组共检测到41266个OTU。
实施例5:肠道微生物标志物的筛选
5.1肠道微生物之间的差异检验
经过独立样本t检验分析,精神障碍人群和健康对照组之间在焦虑(STAI,State-Trait Anxiety Inventory-State)和抑郁(SDS,Self-rating depression scale)两项指标间存在显著差异(p<0.01)。此外,通过检测血液中的五羟色胺和多巴胺的含量,发现实验组血液中五羟色胺的含量(2.126±0.211ng/mL)要显著高于对照组(1.812±0.397ng/mL)的含量(p<0.01),并且实验组血液中多巴胺的含量(0.116±0.023ng/mL)要显著高于对照组(0.087±0.025ng/mL)的含量(p<0.01),表明实验组人群两种神经递质与对照组相比明显异常。由于各被试样本中肠道微生物的数据存在较多的空白,因此需借助非参数检验的方式进行统计分析。通过Mann-Whitney U检验两组之间分别在门水平和属水平注释的肠道微生物的差异,共检测到20个门的微生物,发现两组在Bacteroidetes,Firmicutes和Proteobacteria等门水平存在显著差异(表2),此外,还检测到218个属,发现两组在Bacteroides,Prevotella,Ruminococcus,Parabacteroides等属水平存在显著差异(表3列出了存在显著差异的前十种菌)。其中,含量超过1%的细菌分别占实验组的76.61%和对照组的94.69%。

多样性指数是反映生态系统内的物种丰富度和均匀度的综合指标。alpha多样性指数结果表明,在Shannon,Simpson和InverseSimpson指数方面,实验组的多样性指数低于对照组的多样性指数,只是Simpson指数没达到显著。而在菌群丰富度估计指数方面,chao1和PD_whole_tree指数都是对照组明显高于实验组,表明实验组无论在菌群多样性还是丰富度方面都低于对照组(表4)。

除此之外,多样性指数的相异检测也表明,通过检验表明,两组之间肠道微生物多样性在ANOSIM(0.13,p<0.01),adonis(0.47,p<0.01)和MRPP(0.08,p<0.01)方面都达到显著。
主成分分析(PCA,Principal component analysis)结果表明,两组之间在属水平的肠道微生物之间可以明显的区分开(图2),表明实验组和对照组之间存在显著差异。
层次聚类结果也表明,实验组和对照组能够明显的区分开,并且发现Bacteroides,Prevotella,Ruminococcus和Parabacteroides属细菌在两组之间差异较为明显(图3)。其中Prevotella在实验组中明显低于对照组,而Bacteroides则在实验组中明显高于对照组。此外,具有抗炎作用的Faecalibacterium属在实验组中明显减少,而促炎作用的Streptococcus、Fusobacterium、Enterococcus属则增多,显示实验组可能存在较高的炎症水平。通过Prevotella/Bacteroides的比例能够明显的区分实验组和对照组(图4,*p<0.05)。
由于同一属的不同种细菌所起的作用可能不同,因此,我们进一步分析了Prevotella属和Bacteroides属内种水平的细菌组成。由于我们采用了基于16S rRNA的元基因组分析方法,测得的序列较短,比对到种水平容易造成较高的误差,因此,我们对获得的属水平的数据进行了再处理。首先,用Uclust算法对Prevotella属和Bacteroides属水平的序列依据95%的序列相似性重新分散为不同的OTU。分别获得25和32个OTU,然后,我们将上述25个OTU序列与RDP数据库中46种已知典型的可培养的Prevotella以及44种已知典型的可培养的Bacteroides内种水平的16S rRNA序列一起进行序列多重比对分析,分析结果表明Prevotella属25个OTU中能够聚为三类,分别能够与Prevotellaoulorum(S000414455-ATCC43324/L16472)Prevotellaoralis(S000546838-ATCC 33269/AY323522)和Prevotellaruminicola(S004056737-ATCC 19189/L16482)同源性较高。而Bacteroides属的32个OTU则与Bacteroides stercoris(S000010358-ATCC 43183/X83953)同源性较高。通过以上分析,可能精神障碍者肠道中缺少Prevotella属Prevotellaoulorum、Prevotellaoralis和Prevotellaruminicola,而富含Bacteroides stercoris。健康人群中则会富集Prevotella属Prevotellaoulorum、Prevotellaoralis和Prevotellaruminicola,而缺乏Bacteroides stercoris。
5.2肠道微生物标志物的筛选
为了筛选肠道微生物的标志物,我们将两组之间有显著差异的肠道微生物数据用于构建模型,我们再从所有被试中随机抽取。我们再采集实验组和对照组各10位个体的数据用于验证模型。
随机森林(Random Forest)是一种基于分类树(classification tree)的算法,需要模拟和迭代,被归类为机器学习的一种算法(Breiman L.2001.Random forests.MachineLearning,45,5-32)。在机器学习的诸多算法中,随机森林因高效而准确被誉为当前最好的算法之一(Iverson LR,Prasad AM,Matthews SN,Peters M.2008.Estimatingpotentialhabitat for 134eastern US tree species under six climate scenarios.ForestEcology and Management,254,390-406;Genuer R,Poggi JM,Tuleau-MalotC.2010.Variable selection usingrandom forests.Pattern Recognition Letters,31,2225-2236)。随机森林算法包含多个决策树的分类器,可以产生高准确度的分类器,作为有监督的学习方法不容易出现过拟合,能够平衡数据带来的平衡误差,并能够处理大量的输入变量,能够在决定类别时评估变量的重要性。随机森林算法已经成为重要的数据分析工具,被广泛应用于众多科学研究,如微阵列数据处理,图像处理,人脸识别,运动识别等。我们利用随机森林的特点来发现影响精神障碍的特征肠道微生物。我们采用R语言中的randomForest软件包进行随机森林运算(Liaw A.2012.Package"randomForest")。通过特征重要性排序分析(MeanDecreaseAccuracy),我们发现Prevotella属和Bacteroides属是最重要的两个特征变量(表5)。

基于上述十五个属水平的细菌重要特征作为输入特征,构建判别随机森林模型的训练数据集。将训练数据集输入随机森林进行训练,获得判别随机森林模型。模型包括不同的分类器个数和各分离点输入特征的随机抽取变量个数,通过选择不同的分类器个数和各分离点随机抽取变量个数可构建不同参数组合的判别随机森林模型。在采用默认设置的情况下进行模型训练,再依据运算结果进行调整,依据不同判别随机森林模型的ROC曲线,将ROC曲线的AUC面积值作为优选指标,将AUC面积最大的模型作为训练模型,最终选择的分类器个数为ntree=260,变量个数为4。随机森林算法采用R语言的randomForest软件包实现,ROC曲线采用R语言的qROC软件包实现。上述训练模型可用于测试集的测试,进而将精神障碍者与正常对照加以区分。
实施例6:肠道微生物志物的筛选模型的验证与预测
为了证实实施例5中的发现,进一步比较验证模型中的10个健康人及10个精神障碍人群的特征菌群丰度。将测试数据集输入上述步骤5获得的随机森林分类模型中,即可得到最终识别结果。将肠道微生物数据按照上述步骤4和5进行处理,得到待检测特征向量,再将上述特征向量输入上述步骤5获得的随机森林分类模型中,即可得到最终识别结果(表6)。

利用上述物种标记物以及随机森林分类模型来筛选、诊断或治疗精神障碍者,监测治疗进程,或者生产筛选药物、功能性食品、益生菌,生产检测上述物种标记物的试剂盒以及装置等为本领域技术人员所知悉,皆在本发明的保护范围之内。
物种标志物可以选自精神障碍者富集的物种标记物或者健康人群中富集的物种标记物中的一种或者多种。优选的,对于精神障碍者或易感人群,应该检测到Prevotella属的物种标志物被富集,同时Bacteroides属中物种标记物不被富集。
治疗方案中,优选的,使Bacteroides属中的物种标志物生长得到抑制或者清除,使Prevotella属中物种标记物被富集。
Claims (3)
1.选自普雷沃菌属(Prevotella)和拟杆菌属(Bacteroides)肠道微生物中的一种或多种组合在制备诊断焦虑或抑郁的试剂中的用途;
其中,所述肠道微生物为Prevotella属的龈炎普氏菌(Prevotella oulorum)、口腔普雷沃菌(Prevotella oralis)和 栖瘤胃普雷沃氏菌(Prevotella ruminicola);Bacteroides属的粪便拟杆菌(Bacteroides stercoris)。
2. 筛选干预焦虑或抑郁的食物、益生菌或药物的方法,包括比较该食物、益生菌或药物治疗或干预之前和之后的粪便样本中的Prevotella属与Bacteroides属的比例;和/或粪便样本中Prevotella属与Bacteroides属的相对数量;其中Prevotella属为龈炎普氏菌(Prevotella oulorum)、口腔普雷沃菌(Prevotella oralis)和 栖瘤胃普雷沃氏菌(Prevotella ruminicola);所述Bacteroides属为粪便拟杆菌(Bacteroides stercoris)。
3.根据权利要求2所述的方法,其包括:
a,采集所述的食物、益生菌或药物治疗或干预前后的个体粪便样本并妥善保存;
b,从个体粪便中提取DNA;
c,以粪便DNA为模板,对16s rDNA进行PCR扩增和建库;
d,对16srDNA进行高通量测序,获得测序结果;
e,对测序结果进行生物信息学分析,确定所述个体的粪便中Prevotella属与Bacteroides属的量;
f,将检测的Prevotella属与Bacteroides属的量输入判别模型进行判别,其中,判别模型采用随机森林算法来构建。
