Description
技术领域
以下总体涉及成像热测量,并且利用与介入过程相结合的对计算机断层摄影(CT)的具体应用来进行描述,在所述介入过程中,对正在被处置的组织的温度监测是感兴趣的。以下也接受其他成像,例如X射线、荧光透视和/或与这样的介入过程结合使用的其他成像。
背景技术
一些像消融的介入过程的限制是归因于消融不足或不充足的安全裕量的高复发率。此外,一些并发症由可能损伤邻近的健康组织的过度消融导致。实时温度监测可以有助于减轻这样的问题,并且允许用于防止消融不足、过度处置和/对邻近组织的潜在损伤的重复的介入过程计划调节。
磁共振(MR)成像能够用于在消融和/或其他过程期间监测组织中的温度变化。利用MR成像,温度中的变化引起磁场之内的质子的共振频率的线性成比例的变化,并且因此是使用被称为“质子共振频率移位(PRFS)”的相位敏感脉冲序列扫描可探测的温度变化。利用PRFS,在两个不同的时间点处获得两幅图像,并且两幅图像之间的相位差用于计算在每个像素处的温度差。PRFS成像引起与图像像素的密度相等的体积的温度采样密度。该数据用于规划和递送以及组织损伤的处置后评价。
然而,消融通常利用超声或CT引导来执行;由于有限的MR兼容器材和/或MR扫描器使用,因此在消融期间很少使用MR引导。遗憾的是,实时温度监测目前不可能利用CT,并且由制造商提供的预定的消融区未考虑组织属性、肿瘤组织的异质性、热量同步或患者运动。许多因素影响消融区,其包括患者特性、肿瘤组织结构和定位以及用于消融的能量的类型。此外,计算模型能够是耗时的并且依赖于繁重的数学计算。

具体实施方式
以下描述了一种方法,所述方法用于基于基线和间歇式CT数据(例如,锥形射束CT数据和/或常规CT数据)和/或X射线数据或荧光透视数据,生成并呈现针对基于针的介入过程(例如,RF、激光、微波、高强度聚焦超声(HIFU)等消融和/或药物递送)的物体或对象中的感兴趣体积或区域的热图的方法。总体而言,热图被实时(即,当正在执行消融或药物递送时)生成并呈现,这允许介入医师减轻消融不足(以及因此的后续消融至完成消融)、过度消融,并且确保充足的安全裕量。进一步地,它减轻与测量感兴趣体积或区域中的温度的热电偶的插入相关的感染和/或出血的风险。此外,该方法补偿患者运动(例如,由于呼吸等)和低分辨率的图像数据。

介入设备支持器116包括用于支持介入设备的设备支撑物118、用于安置设备支撑物118并且因此安置由其支撑的介入设备的臂120、以及可移除地附接到对象支撑物114并且支撑臂120的基座122。臂120可以是通过手动和/或电子器件可移动的。诸如消融设备的介入设备124被示出为由设备支撑物118支撑。介入设备控制器126控制介入设备124,例如,打开和关闭设备。在图示的实施例中,介入设备控制器126包括附接到对象支撑物114并且控制介入设备124的手激活控制件,例如操纵杆等。在另一实例中,诸如脚踏板等的脚激活控制件能够用于控制介入设备124。

处置规划器146允许介入列表以基于基线、间歇、热图和/或其他信息来规划和/或修正介入过程计划。通过非限制性范例的方式,基线图像数据和温度图能够由介入列表使用以确保适当的针位置、完全消融以及对健康组织损伤的减轻。例如,从该数据中,介入列表能够基于正在处置的组织和/或周围组织的温度等来决定针位置合适还是需要移动、继续进行消融还是停止消融。处置导航器148在视觉上呈现图像数据,所述图像数据通过相对于要被处置的组织的插入和放置介入针来引导介入列表。
在图示的实施例中,计算装置128包括任选的推荐器150。在一个实例中,推荐器150经由显示器145、音频和/或其他设备提供回到用户的关于所生成的热图的引导。在一个非限制性实例中,引导展示哪里需要额外的热量,并且任选地提供一种路径,所述路径将确保热剂量被令人满意地传送。在另一非限制性实例中,如果热过程计划是令人满意的,则该引导提供能够用于“实时”更新计划、从而跟踪处置的进展的数据。

在范例中,介入设备识别器130识别基线图像数据和间歇式图像数据中的介入设备针尖端。在一个实例中,介入设备识别器130使用自动的感兴趣物体识别算法来识别尖端。在另一实例中,介入设备识别器130基于用户输入识别尖端,例如,使用用户识别的尖端视野计的尖端的识别、用户安置和尺寸调节的预定几何形状等。自动的、半自动的和手动的方法的结果被确认并且能够由经授权的用户改变。
感兴趣体积或区域识别器132识别关于在基线图像数据和间歇式图像数据中识别的消融针尖端(或消融区)的感兴趣体积或区域。在一个实例中,感兴趣体积或区域识别器132使用自动的感兴趣区域算法,例如基于所识别的尖端区域以外的预定的裕量来识别感兴趣体积或区域。在另一实例中,感兴趣区域识别器132基于用户输入来识别区域,例如,所识别的尖端区域之外的用户识别区域。预定的制造商区也能够用于促进识别感兴趣体积或区域。自动、半自动和手动方法的结果被确认并且能够由经授权的用户改变。
图像配准部件134基于配准算法136中的一项或多项来配准基线图像数据和间歇式图像数据,所述配准算法136包括刚性的和/或弹性的(非刚性的)配准算法。合适的配准算法的范例是补偿基线与间歇式采集之间的诸如呼吸和/或其他运动的运动的可变形的图像配准。以下讨论了这样的算法的非限制性范例。

热测量图像生成器138基于热测量算法140中的一项或多项根据经配准的基线图像数据和间歇式图像数据生成热测量图像数据。合适的热测量算法140的范例是朗斯基行列式变化探测器,所述朗斯基行列式变化探测器探测基线图像数据与间歇式图像数据之间的差异,其中,温度差异是图像数据之间的整体差异的分量。在Durucan等人的“Change detection andbackground extraction by linear algebra,(Proceedings of the IEEE.89,第1368-1381页,2001年)中讨论了探测低信噪比图像数据中的变化的朗斯基行列式变化探测器。
总体上,朗斯基行列式变化探测器是基于:1)图像的向量模型,以及2)向量之间的线性相关/无关的概念。通过将图像建模为向量的总和,能够利用在它们的长度或方向上的变化(相对于初始的未改变的状态)。在数学上,这分别对应于线性相关和线性无关的概念。关于一个向量是否是线性相关于另一个的决定提供决定是否已经发生变化或没有发生变化的可能性。用于确定向量的线性相关或无关的一种测试是朗斯基行列式判定。

热图生成器142基于热测量图像和对于温度查找表144的值的变化来生成针对感兴趣区域或体积的逐个像素或逐个体素的热图或温度图(例如,集中在感兴趣体积上的2D图像的堆栈),所述温度查找表144提供热测量图像数据与温度之间的映射。为了生成对于温度查找表144的值的变化,在参考温度处(例如室温或37℃)以及一个或多个其他预定的或已知的更高的温度处(例如42℃到82℃)扫描诸如琼脂凝胶、水、解剖和/或其他体模的均匀的体模。
最小二乘法、卡方和/或其他方法能够用于将线性模型、立方模型等模型拟合到热测量图像的平均值和温度,使得其中,m=2或4。能够使结果最小化以用于最佳拟合,其中,n是数据点的数目,并且pi是模型的参数。针对线性模型或立方模型的参数的数量m分别是2和4。校准的倒数被存储为查找表144。

12.根据权利要求11所述的计算设备,其中,所述热图包括将灰度级别映射到对应的温度值的检索表。
13.根据权利要求1至10中的任一项所述的计算设备,其中,使用预定的颜色图显示所述热图,所述颜色图指示所述感兴趣体积或区域的不同区域之间的相对温度差。
14.根据权利要求13所述的计算设备,其中,所述热图包括将颜色映射到对应的温度值的检索表。
15.一种方法,包括:
基于热测量图像数据和对于温度查找表的体素值或像素值的预定变化来生成针对表示对象的感兴趣体积或区域的图像数据体素或像素的热图,所述热测量图像数据包括指示所述感兴趣体积或区域中的温度变化的体素或像素。
16.根据权利要求15所述的方法,还包括:
在视觉上呈现与所述感兴趣体积或区域的图像数据相结合的所述热图。
17.根据权利要求16所述的方法,其中,使用预定的灰阶或颜色映射显示所述热图,所述预定的灰阶或颜色映射指示所述感兴趣体积或区域的不同区域之间的相对温度差。
18.根据权利要求17所述的方法,还包括:
在视觉上呈现与所述热图一起的检索表,其中,所述检索表将灰度级别或颜色映射到对应的温度值。
19.根据权利要求15至17中的任一项所述的方法,还包括:
采用所述热图来调节用于处置所述感兴趣体积或区域的组织的介入过程计划。
20.根据权利要求19所述的方法,其中,所述调节包括持续所述介入过程或终止所述介入过程中的至少一个。
21.根据权利要求15至18中的任一项所述的方法,还包括:
基于在热量应用于所述感兴趣体积或区域之前采集的基线图像数据和在热量应用于所述感兴趣体积或区域之后采集的间歇式图像数据来生成所述热测量图像数据。
22.根据权利要求21所述的方法,其中,所述热测量图像生成器采用朗斯基行列式变化探测器算法来生成所述热测量图像数据。
23.根据权利要求21至22中的任一项所述的方法,还包括:
配准所述基线图像数据与所述间歇式图像数据,其中,所述热测量图像生成器基于经配准的图像数据生成所述热测量图像数据。
24.根据权利要求23所述的方法,其中,所述图像配准部件采用补偿所述感兴趣体积或区域的运动的可变形的配准算法来配准所述基线图像数据与所述间歇式图像数据。
25.一种被编码具有计算机可读指令的计算机可读存储介质,当处理器运行所述计算机可读指令时,所述计算机可读指令使所述处理器:基于热测量图像数据和对于温度查找表的体素值或像素值的预定变化来在介入过程期间生成并显示针对表示对象的感兴趣体积或区域的图像数据体素或像素的热图,所述热测量图像数据包括指示所述感兴趣体积或区域中的温度变化的体素或像素。
