Abstract

技术领域
本发明涉及用于局部施用的组合物,及涉及其作为化妆产品或药物产品的用途,所述组合物用于皮肤病症的治疗,尤其用于痤疮的治疗。
背景技术
痤疮是一种常见的多因素病理(multi-factor pathology),其侵袭富含皮脂腺的皮肤(面部、肩部区域、手臂和擦烂区域(intertriginal areas))。这是最常见的皮肤病发生形式。以下五种致病因素在痤疮的形成中发挥了决定性的作用:
1.遗传素因;
2.皮脂过度产生(皮脂溢);
3.雄激素;
4.毛囊角化病症(粉刺产生(comedogenesis)),和
5.细菌定殖和炎性因素。
存在着数种形式的痤疮,所有形式的共同因素是侵袭毛囊皮脂腺滤泡。可特别提到聚合性痤疮、颈项部瘢痕瘤性痤疮、药物性痤疮(acne medicamentosa)、复发性粟粒状痤疮、坏死性痤疮、新生儿痤疮、经前痤疮、职业性痤疮、玫瑰痤疮、老年性痤疮、日光性痤疮和寻常痤疮。
寻常痤疮(也称为多形态的青少年痤疮)是最常见的。其包含四个阶段:
-阶段1对应于粉刺性痤疮(comedonic acne),特征在于大量开放的和/或闭合的粉刺和大量微囊;
-阶段2,或丘疹脓疱性痤疮(papulopustular acne),具有轻度至中度的严重性。其特征在于存在开放的和/或闭合的粉刺,存在微囊,而且还存在红色丘疹和脓疱。其主要影响面部并留下少许疤痕;
-阶段3,或丘疹粉刺性痤疮(papulocomedonic acne),更加严重并扩展至背部、胸部和肩部。其伴随大量疤痕;
-阶段4,或结节囊性痤疮(nodulocystic acne),伴随众多疤痕。其展现出结节及还有疼痛的大体积的(voluminous)深红色脓疱。
可用诸如抗皮脂溢药剂和抗感染药之类的活性药剂(例如过氧化苯甲酰(特别由Pierre Fabre公司销售的产品))、用维甲类(例如维甲酸(特别由Galderma销售的产品)或异维甲酸(由Roche Laboratoires销售的产品))、或用萘甲酸衍生物来治疗上述各种形式的痤疮。萘甲酸衍生物(例如6-[3-(1-金刚烷基)-4-甲氧基苯基]-2-萘甲酸,通常将其称为阿达帕林(由Galderma销售的产品))被广泛描述并公认为是与维甲酸同样有效的用于痤疮治疗的活性成分(Ioannides D.,Rigopoulos D.和Katsambas A.,2002.Topical adapalene gel 0.1%vs.isotretinoingel0.05%in the treatment of acne vulgaris:a randomized open-label clinicaltrial[Br J Dermatol.Sep;147(3):523-7]。
使用Rx(有处方)产品(主要是局部/口服的维甲类)出现的一些不良事件(AE)产生重要的相关AE和频繁的皮肤副作用,例如Ziana:27%对象(subject)具有相关施用位点AE,且最重要的是皮肤干燥。
皮肤科医生推荐的用于痤疮治疗的皮肤护理方案包括以下步骤:
-步骤1:清洗
-步骤2:用药治疗(Rx治疗)
-步骤3:水合和保护
拥有改善痤疮征兆/症状的护肤产品是有用的。
已经充分确定Rx治疗是有效的。但是对新的良好耐受的局部药物组合物存在需求,所述组合物兼具治疗、改善皮肤质量和清洗皮肤(优选痤疮患者的皮肤)的性能。从而步骤2的用药治疗根据本发明是任选的或可被认为通过在组合物中使用过氧化苯甲酰(BPO)来自动实现。
发明内容
本发明提供局部皮肤病学用(dermatological)/药物组合物并特别提供稳定和良好耐受的BPO清洗组合物(wash composition)。
使BPO与皮肤接触时,其效力与其分解相关联。在该分解期间产生的自由基的氧化性能产生了期望的效果。因此,为了维持过氧化苯甲酰的最佳效力,防止其在使用前(即,储存期间)的分解是重要的。
BPO是一种不稳定的并与一大批原材料(特别是表面活性剂和油)反应的化合物。这种固有的不稳定性使得BPO难以在最终产品(特别是含有表面活性剂(出于它们的清洁和发泡性能的益处)的清洗组合物)中配制。
发明人还已观察到,众所周知的表面活性剂(例如两性表面活性剂)与BPO不相容且导致较不稳定的组合物。还已知具有清洁性能的传统的表面活性剂对皮肤有刺激作用。
BPO清洗产品已存在于市场上;但是,很多不是很稳定。数种产品使用两性表面活性剂,其已经显示使BPO不稳定。从而存在着这些组合物可能较不化学稳定的风险。这些产品中的一些在包装上表明在使用之前需要有力摇晃该产品。这表明组合物和/或BPO悬浮液经历沉淀并因此展现物理不稳定的形式。另外,这些产品中的一些使用的BPO和/或一些表面活性剂的浓度如此之高以致加剧了与痤疮治疗相关的刺激。一些患者的意见突出了各种产品的刺激作用。这些产品中的数种不具有患者所优选的最佳发泡性能。这些限制影响患者顺从性并最终影响痤疮治疗的效力。
从而,仍然对非刺激性的BPO发泡清洗产品存在需求。在具体实施方案中,本发明提供BPO清洗组合物,优选为清洗凝胶形式,具有期望的耐受性、稳定性和发泡性能。
本专利权人已发现,该需要可在相同组合物中使用至少一种具体的表面活性剂、葡糖酸锌和甘草酸或甘草次酸的盐或衍生物得到满足。
具体实施方式
因此本发明的一个方面是组合物,其为局部清洗组合物,包含:
a.过氧化苯甲酰(BPO)
b.至少一种与BPO相容的温和的表面活性剂,其选自阴离子和/或非离子表面活性剂类别
c.葡糖酸锌
d.d)甘草酸或甘草次酸的盐或衍生物
所述组合物包括过氧化苯甲酰(BPO)。
可将BPO溶解或分散在组合物中。在本发明的具体实施方案中,过氧化苯甲酰在组合物中呈分散形式。通过根据本发明的分散形式,BPO被认为在组合物中维持在稳定的悬浮液中。或者,过氧化苯甲酰被封装(如US专利7,758,888中所述的封装技术除外)或被吸附或吸收/涂布到支撑物上或作为自由形式使用。
例如,可将BPO封装在由多孔微球构成的聚合物系统中,例如由Cardinal Health以过氧化苯甲酰MICROSPONGE P009A的名称销售的微海绵或甲基丙烯酸烯丙酯交联聚合物例如由Amcol HBS以名称Poly-Pore 438BP/过氧化苯甲酰销售的多孔材料(Poly-pore)。
优选地,组合物包含介于0.5%和10%w/w之间的BPO,优选介于1%w/w和5%w/w之间及优选介于2.5%w/w和3.5%w/w之间。重量对重量百分比(%w/w)由相对于组合物的总重量的活性成分的重量表示。
本专利权人已注意到,使用含有相对低浓度的BPO(例如<5%w/w BPO)的制剂比使用较高浓度的(制剂)更难以实现BPO的化学稳定性。但是,由于低刺激性和高耐受性的需求,本发明的组合物应当含有优选不高于3.5%的浓度的BPO。该低浓度导致确保随着时间推移在组合物中可接受的BPO稳定性的额外难度。
本发明人已观察到,大多数众所周知的表面活性剂与BPO不相容且这导致较不稳定的组合物。还发现,在利用特定种类的表面活性剂时大大改善了包含BPO的组合物的化学稳定性。的确,考虑到一大批(large panel of)的表面活性剂,例如两性表面活性剂、非离子阴离子表面活性剂或阳离子表面活性剂,在实施例中已显示只有某些阴离子表面活性剂和非离子表面活性剂能够提供用于本发明目的的具有BPO的稳定的组合物。因此,在一种实施方案中,本发明提供组合物,该组合物具有新一代的非常温和的表面活性剂阴离子和/或非离子表面活性剂,其具有清洁和/或发泡性能,适用于痤疮和敏感皮肤且与BPO相容。
当其施用导致皮肤的最小程度的肿胀、紧绷和刺激时,则认为表面活性剂是温和的。月桂基硫酸钠经常被选作刺激性的表面活性剂的参考实例。温和的表面活性剂不仅比月桂基硫酸钠(SLS)的刺激性小,而且比月桂基醚硫酸钠(sodium lauryl ethersulphate)的刺激性小。通常,月桂基醚硫酸钠被认为是比SLS刺激性小的阴离子表面活性剂。
第一类表面活性剂是温和的表面活性剂,其具有去污,清洁和/或发泡性能,其选自阴离子表面活性剂和/或非离子表面活性剂,更具体地选自以下列表。它们可单独使用或组合使用。

相应地,按相对于组合物的总重量的活性材料的重量(%w/w AM)表示,阴离子表面活性剂和/或非离子表面活性剂具有的总浓度介于0.2%w/w和20%w/w之间,优选介于0.25%w/w和10%w/w之间,甚至更优选介于0.5%w/w和5%w/w之间。

根据本发明,组合物还包含葡糖酸锌。葡糖酸锌(也称zincum gluconium)是葡糖酸的锌盐。它是由2mol葡糖酸根对1mol锌构成的离子化合物。葡糖酸锌是作为膳食补充剂用于递送锌的广受欢迎的形式。
葡糖酸是天然存在的,且通过葡糖的发酵经工业化制备,典型地通过黑曲霉(Aspergillus niger),也通过其它真菌,例如青霉属(Penicillium),或通过细菌,例如醋酸杆菌属(Acetobacter)、假单胞菌属(Pseudomonas)和葡糖杆菌(Gluconobacter)进行。在其纯的形式中,它是白色至灰白色粉末。其也可通过电解氧化来制备,尽管这是一种较昂贵的方法。优点是较低的微生物学分布(profile),和较完全的反应,得到具有较长货架期的产品。
在优选的实施方案中,按相对于组合物的总重量的重量表示,葡糖酸锌的浓度介于0.1和1%之间,优选介于0.15和0.3之间,更优选0.2%。
根据本发明,组合物还含有甘草酸或甘草次酸的盐或衍生物。
甘草酸来源于植物光果甘草(Glycyrrhiza glabra)或甘草根,其被认为提供抗刺激和抗炎症性能。甘草提取物的舒缓和镇定性能使得它们成为包括在用于敏感皮肤病症(例如湿疹、红斑(erhythema)、脂溢性皮炎(seborric dermatitis)和瘙痒)的治疗中的令人感兴趣的候选物。
甘草次酸是从甘草酸(别名:甘草甜素或甘草酸(glycyrrhizinic acid))的水解获得的β-香树素类型的五环三萜化合物衍生物,所述甘草酸从草药甘草获得。其用于调味且其掩蔽药物(如芦荟和奎宁)的苦味。其在消化性溃疡的治疗中是有效的,且还具有化痰(镇咳)性能。其具有一些额外的药理学性能,包括抗病毒、抗真菌、抗原生动物和抗细菌活性。

本发明的组合物所解决的一个问题是含有BPO的发泡清洗组合物的稳定性。稳定性包括化学稳定性和物理稳定性。

根据本发明,当BPO含量在目标活性药物物质浓度的90至110%之内时,认为组合物是化学稳定的。

对于实施例3至21,制备过程如下:
步骤1:在主烧杯中,称取所需量的纯化水并在分散硅酸铝镁之前将其加热至75℃。
步骤2:冷却至60℃并在混合的同时加入黄原胶。
步骤3:维持在60℃并继续混合直至胶凝剂被水合,随后加入EDTA和发泡表面活性剂。混合直至均匀。
步骤4:冷却至50℃并在混合的同时并入PEG-75(适当时)和甘草酸二钾。混合直至均匀。
步骤5:冷却至40℃并引入葡糖酸锌,并用柠檬酸调节pH。混合直至均匀。
步骤6:在第二个烧杯中,高剪切并维持在低温下(用冰浴)下,使用泊洛沙姆和丙二醇制备BPO的预分散体。
步骤7:混合的同时在主烧杯中加入BPO的预分散体。继续冷却。
步骤8:混合的同时向制剂中加入其它所需的添加剂(如香料)。冷却至30℃并停止混合。
以下描述的所有实施例在RT(室温)、30℃和40℃下3个月之后是物理稳定的。

Claims (17)
1.局部清洗组合物,特征在于其包含:
a.按相对于所述组合物的总重量的活性成分的重量表示,介于1%w/w和3.5%w/w之间的过氧化苯甲酰,
b.按相对于所述组合物的总重量的活性材料的重量表示,介于0.5%w/w和5%w/w之间的至少一种阴离子和/或非离子表面活性剂,其选自椰油醇聚醚硫酸酯锌、椰油酰羟乙磺酸钠、月桂酰甲基羟乙磺酸钠和癸基葡糖苷,
c.介于0.1%w/w和1%w/w之间的葡糖酸锌
d.介于0.1%w/w和1%w/w之间的甘草酸二钾
e.介于0.15%w/w和7%w/w之间的至少一种胶凝剂,其选自单独的或作为混合物的聚丙烯酰胺家族、“对电解质不敏感的”卡波姆、多糖、纤维素衍生物、膨润土、硅酸铝镁和中和的聚合磺酸。
2.根据权利要求1所述的组合物,其中其是发泡组合物。
3.根据权利要求1所述的组合物,其中其呈凝胶形式。
4.根据权利要求1-3中的任一项所述的组合物,其中所述胶凝剂选自单独的或作为混合物的多糖和硅酸铝镁。
5.根据权利要求4所述的组合物,其中所述胶凝剂选自单独的或作为混合物的黄原胶和硅酸铝镁。
6.根据权利要求1-3和5中的任一项所述的组合物,其中所述组合物进一步包含润湿剂。
7.根据权利要求6所述的组合物,其中所述润湿剂选自泊洛沙姆和/或二醇家族。
8.根据权利要求6所述的组合物,其中所述润湿剂选自单独使用的或混合使用的丙二醇或synperonic PE/L44。
9.根据权利要求6所述的组合物,其中以相对于总的组合物重量的重量计,所述润湿剂的浓度介于1%至7%之间。
10.根据权利要求1-3、5和7-9中的任一项所述的组合物,其中过氧化苯甲酰呈分散形式。
11.根据权利要求1-3、5和7-9中的任一项所述的组合物,其中按相对于所述组合物的总重量的活性成分的重量表示,所述组合物包含介于2.5%和3.5%之间的过氧化苯甲酰。
12.根据权利要求1-3、5和7-9中的任一项所述的组合物,其中按相对于所述组合物的总重量的重量表示,葡糖酸锌的浓度介于0.15%和0.3%之间。
13.根据权利要求1-3、5和7-9中的任一项所述的组合物,其中按相对于所述组合物的总重量的重量表示,甘草酸二钾的存在浓度介于0.15%和0.3%之间。
14.根据权利要求1-13中的任一项所述的组合物在制备用于预防、抑制或治疗寻常痤疮的药物中的用途。
15.根据权利要求1-13中的任一项所述的组合物在制备用于改善、预防或抑制与痤疮治疗相关联的皮肤病症的药物中的用途。
16.根据权利要求1至13中任一项所述的组合物在制备用于治疗或改善痤疮个体皮肤的药物中的用途。
17.根据权利要求1-3、5和7-9中任一项所定义的组合物,其用于预防、抑制或治疗寻常痤疮的用途。
