长度可调节植入物
技术领域
本发明涉及具有可变轴向长度并且适合于在脊柱或创伤手术中应用的植入物。植入物包括第一构件和第二构件,第二构件相对于第一构件被引导以在轴向方向上向第一构件可移动以便调节植入物的总长度。此外,植入物包括接合构件。在第一配置中,接合构件配置成接合第一构件和第二构件以将第一构件相对于第二构件锁定。在第二配置中,接合构件配置成接合第一构件,但是不接合第二构件,使得第二构件相对于第一构件可移动。禁止接合构件从第一构件脱离。
背景技术
呈用于插入两个椎骨之间的长度可调节间隔器的形式的植入物从US 6,200,348B1获知。间隔器包括第一套筒状构件和第二套筒状构件,第二构件在第一构件内在轴向方向上相对于第一构件可滑动地被引导以便调节总长度。第二构件在其面对第一构件的外表面上包括具有棘爪槽口的轴向延伸部分。第一构件包括接合构件,所述接合构件与棘爪槽口协作以便移位到期望总长度。接合构件由工具形成,所述工具包括在其前面上的弹性偏压球,所述弹性偏压球配置成接合棘爪槽口。在合适的轴向长度的调节之后,使用锁定螺钉以使第一和第二构件相对于彼此锁定并且移除工具。
用于插入两个椎骨之间的另一间隔器从US 7,803,191B2获知。它包括第一管状构件和第二管状构件。通过使用工具将第二管状构件伸缩到第一管状构件之外调节间隔器的长度并且一旦获得期望长度,插入一个或多个锁定螺钉以将第二构件相对于第一构件锁定。
另一长度可调节椎间间隔器从US 8,241,363B2获知。该间隔器包括通过接合构件锁定在一起的一对同轴环带,所述接合构件包括固定螺钉和压力板,所述压力板具有接触固定螺钉的外面和具有齿的内面,所述齿适合于与第二构件的棘爪槽口配合。
呈长度可调节骨板的形式的另一植入物从US 5,964,762获知。以与上述间隔器类似的方式,使用工具使第一板部分相对于第二板部分可滑动地布置并且用锁定螺钉固定期望长度。
发明内容
本发明的目的是提供一种具有可变轴向长度的、用于在脊柱或创伤手术中应用的植入物,其关于使用的安全性和在设计的简单性方面得到改善。
该目的由根据本申请的植入物实现。本申请还给出了另外的改进。
根据本发明的植入物包括第一构件和第二构件以及接合构件,第二构件相对于第一构件被引导以在轴向方向上向第一构件可移动以便调节植入物的总轴向长度。在第一配置中,接合构件配置成接合第一构件和第二构件使得第一构件和第二构件相对于彼此锁定。在第二配置中,接合构件配置成接合第一构件使得第二构件相对于第一构件可移动。通过抵接件禁止接合构件从第一构件脱离。抵接件防止外科医生不慎完全拧出接合构件。所以,在手术期间,接合构件不能从第一构件脱离并且不能在患者的体内弄丢。这增强植入物的使用的安全性。
接合构件是特别呈固定螺钉的简单形式的整体部件,其包括在螺纹部分和接合部分之间的套环。套环形成抵接件以防止接合构件从第一构件脱离。因此,接合构件具有简单设计并且容易制造。
在禁止接合构件脱离到背离第二构件的第一构件的外部的同时,也禁止它朝着与第一构件相对的第二构件的一侧脱离,这是因为接合构件的至少一部分被包围在第一构件和第二构件之间。这也有助于使用的安全性。

具体实施方式
从图1至10可以看到,根据第一实施例的植入物是用于插入两个椎骨之间的间隔器1。间隔器1包括第一管状构件2和第二管状构件3。第一管状构件2和第二管状构件3是大致圆柱形构件。然而应当注意圆柱形状不是必要的,例如具有卵形、肾形、矩形或方形的横截面的任何套筒状构件可以被预料。第二管状构件3的外径选择成允许第二管状构件3在第一管状构件2中的滑动引导。一个或多个接合构件4被提供用于将第一构件2相对于第二构件3接合和锁定。
第一管状构件2具有第一端部2a、相对的第二端部2b、中心纵轴线C和由管的壁围绕的中空内部。在管状构件的壁中,设有用于允许血管和组织的向内生长的多个开口。例如,多个圆形开口21a可以设在更靠近第一端部2a的第一构件2的上部分中。圆形开口21a可以以圆周排布置,其中一排可以从相邻排偏移。类似地,多个菱形或方形开口21b可以沿着更靠近第二端部2b的区域中的壁也成排地提供,其中一排从另一排偏移。邻近第二端部2b的菱形开口的最后排可以沿着圆周线被切割,使得形成叉状件22,该叉状件22配置成接合椎体的端板。叉状件22可以被锐化以增强接合。在第二端部2a处,第一构件2具有自由端表面。第一构件在叉状件22的区域中的内径相比于剩余部分的直径略大。
在其外表面的一侧,第一构件2包括加厚部分23,所述加厚部分可以从第一端部2a例如沿着第一构件的轴向长度的一半以上延伸。在圆周方向上,加厚部分23例如可以一直延伸到第一构件2的圆周的三分之一。加厚部分23在径向方向上的厚度使得一个或多个接合构件4可以容纳在加厚部分23的壁中。然而,加厚部分23的尺寸不限于实施例中所示的尺寸。它可以例如取决于接合构件的尺寸和数量而更大或更小。
另外,在轴向方向上从离第一端部2a一定距离处延伸到离加厚部分23的底端一定距离处的两个长形开口24设在加厚部分23中。长形开口24具有轴向长度,所述轴向长度允许它们与具有剪状臂的工具(未显示)接合,当第二构件3可滑动地接合在第一构件2中时所述剪状臂用于伸缩植入物1。将预料到可以存在仅仅一个长形开口24或者没有长形开口并且以另外方式实现第二构件3相对于第一构件2的伸缩。
在两长形开口24之间设有一个或多个螺纹孔25,所述螺纹孔在轴向方向上彼此间隔开并且所述螺纹孔的中心定位在平行于中心轴线C的线上。螺纹孔25用于容纳接合构件4或容纳对应于适合于接收接合构件4的螺纹孔25的数量的多个接合构件4。
轴向凹陷26(例如圆柱形凹陷)大致在圆周方向上的加厚部分的中心处设在加厚部分23的内壁中,如图5中最佳地所示。凹陷26朝着第一构件2的顶端2a敞开并且朝着加厚部分23的下端闭合。它提供空间以便接合构件4的接合部分在其中延伸,并且在第二构件3插入第一构件2中之前允许将接合构件4安装到第一构件2中。此外,凹陷27存在于螺纹孔25的每一个之后,从而相比于螺纹孔25提供更大的内径。借助于此,形成以环形方式围绕螺纹孔25延伸的抵接件26a。抵接件26a具有与接合构件4的一部分协作的功能,如下所述。而且,通过凹陷27在抵接件26a和第二管状构件3的外壁之间形成空间。凹陷27和轴向凹陷26在径向方向上距离中心轴线C的尺寸使得接合构件4可以从螺纹孔25朝着第二构件3移动。
参考图15a,加厚部分23的凹陷26、27与第二构件3的外壁一起形成隔室,所述隔室包围接合构件4的一部分,使得接合构件4也不能脱离到间隔器1的内部。应当注意,小脊28可以存在于加厚部分23的内壁中以分隔隔室以便用于不同的接合构件4。
在所示的实施例中,在轴向方向上提供两个螺纹孔25。应当注意,也可以存在仅仅单一或两个以上螺纹孔。可选地,可以有另一螺纹孔29,所述另一螺纹孔在所示的实施例中最靠近第一端部2a并且可以用于常规锁定螺钉或用于具有更小尺寸的另一接合构件(未显示)。
再次参考图15a和例如图6,在与加厚部分23相对的一侧,可以设有用于接收销61的小横向通孔60,所述销具有一定长度使得它的一个端部延伸到第一构件2的内部中空部分中和第二构件3的长形凹陷中。销61的功能可以是双重的。首先,它保持第一构件2和第二构件3在圆周方向上对准。其次,它可以限制总体的长度调节,使得第一构件2和第二构件3不会脱离。
现在参考图7至10,第二管状构件3具有第一端部3a和相对的第二端部3b、在已组装状态下与第一构件2的中心轴线C重合的中心轴线C、中空内部,以及由管限定的壁。在已组装状态下,第二端部3b面朝着第一管状构件2的第二端部2b。第二构件3也包括多个开口,其中第一组开口31a可以为圆形并且以彼此偏移的排布置在更靠近第二端部3b的区域中,并且其中第二组开口31b为大致菱形或方形并且布置在更靠近第一端部3a的区域中。类似于第一管状构件2,第二管状构件3包括在第一端部处的叉状件32,所述叉状件可以通过在中心切割一排菱形开口31b使得保留多个叉状件32而生成。叉状件32可以被锐化并且用于与脊椎端板接合。在离第二端部3b一定距离处设有两个圆周间隔开的长形开口34,所述长形开口的轴向长度小于第一构件2的长形开口24的轴向长度。长形开口34用于与牵开(distract)第二构件3和第一构件2的工具接合。
在两长形开口34之间,轴向的凹陷35的线设在第二构件3的外壁中。凹陷35为球段形并且布置在平行于间隔器1的中心纵轴线C的线中。而且,凹陷35并排形成并且具有的深度小于其半径。在已组装状态下,如图2和15a中所示,凹陷35的线至少在与螺纹孔25的线相同的长度上延伸。凹陷35的线形成第二构件3上的棘爪状接合部分,所述棘爪状接合部分与接合构件4的接合部分接合。
如图10中所示,在凹陷35的线的相对侧,不完全延伸通过壁的长形凹陷36形成于第二构件3的外壁中。长形凹陷36用于容纳安装到第一构件2的销孔60中的销61的前端。
第二构件3在一直到离第一端部3a一定距离处的叉状件32的区域中的内径可以略小于第二构件在凹陷35的线的区域中的内径。
现在更详细地参考图11至14,接合构件4是包括第一端部4a和相对的第二端部4b的整体部件。它基本上是固定螺钉(set screw),具有邻近第一端部4a、带有外螺纹的第一螺纹部分41,所述外螺纹与第一构件2的螺纹孔25的内螺纹协作。在第一端部4a处,对于固定螺钉而言常见地,设有用于驱动器的接合凹陷42。在第一部分41和第二端部4b之间,圆柱形套环43向外延伸并且在径向方向上超出螺纹部分41。套环43具有的外径大于螺纹部分41的外径。
套环43面朝着第一端部4a的表面部分43a用作与第一构件2的加厚部分23的内壁中的抵接件26a协作的抵接件。在套环43和接合构件的第二端部4b之间,设有球段形部分或球形突起44,其形成接合部分,用于与由第一构件中的轴向的凹陷35的线提供的接合部分接合。接合突起44配合到球段形凹陷35中,如图15b中所示。
在接合构件4的螺钉轴线的方向上的接合构件4的总长度使得如图15b和15c中所示,在第一配置中接合突起44配合到第二构件3中的凹陷的线的凹陷35中并且第一端部4a与加厚部分23的外表面大致齐平。然而,接合构件也可以稍稍突出到加厚部分23的外表面之上。在第二配置中,如图15c中所示,接合构件4用表面43a抵靠设在加厚部分23的内壁中的抵接件26a,并且接合突起44与凹陷35脱离接触。螺纹部分41可以突出到螺纹孔25之外。应当注意套环43完全围绕螺钉轴线延伸可能是有利的,但是如果套环仅仅由在圆周方向上具有有限范围、但是仍然满足抵靠抵接件26a的要求的部分组成,也可能就足够了。
植入物的所有部件由生物相容金属(如钛或不锈钢)或生物相容金属合金(如镍钛诺(Nitinol))制造。它们也可以由生物相容塑料材料(如聚醚醚酮(PEEK))制造。部件可以全部由相同或不同材料制造。
如下组装间隔器1。首先,从第一管状构件2的内部用接合构件4的第一、螺纹部分41将接合构件4或多个接合构件4拧接到螺纹孔25中,使得接合突起44容纳在凹陷27中,优选地,使得套环43用其表面43a抵靠加厚部分23的内壁中的抵接件26a。其后,将第二构件3插入第一构件中使得凹陷35的线在圆周方向上与第一构件2的螺纹孔25的中心对准。然后,通过将销61插入销孔60中直到它接合第二构件3的长形凹陷36而固定该位置。借助于此,第一构件2和第二构件3相对于彼此旋转地固定。在已组装状态下,第二管状构件3的长形孔34在圆周方向上与第一管状构件2的长形孔24对准。
在使用中,在将间隔器插入两个椎骨之间之前可以用骨移植物填充间隔器。例如,在移除椎骨之后间隔器用于桥接空间。在间隔器具有其最小长度的配置中将间隔器插入。在该配置中,第一构件2可以相对于第二构件3锁定。然后,调节间隔器1的长度。首先,稍稍向外拧动接合构件4以将接合突起44移动到凹陷35之外,使得它们不再阻止第二构件3相对于第一构件2的运动。如图15c中所示,接合构件4可以仅仅拧出到这样的程度,使得套环43抵靠抵接件26a。所以,防止整个接合构件4的不慎拧出并且接合构件不会在手术期间在患者的体内弄丢。因此,外科医生不必注意小心地拧出接合构件。而且,由于在加厚部分23的内壁和第二构件3的外壁之间形成的隔室,接合构件4不会脱离到间隔器1的内部。
使用分别地接合第一构件2和第二构件3的长形开口24和34的工具进行长度调节。工具可以是具有剪状臂的工具,所述剪状臂可以在长形开口中展开并且由此牵开间隔器1。
当获得期望长度时,再次朝着第一构件1拧动接合构件4直到获得第一配置,其中接合突起44接合凹陷35以阻止第二构件3相对于第一构件2的进一步运动。
将参考图16和17解释呈长度可调节骨板的形式的第二实施例。与第一实施例相同或相似的第二实施例的部件和部分用相同的附图标记进行标示并且将不重复它们的描述。
骨板10包括第一构件200、第二构件300,所述第二构件相对于第一构件200被引导使得第一构件200相对于第二构件300在轴向方向上可移动。轴向方向由形成引导结构的中心轴线和第一构件200或第二构件300的主轴线的纵轴线L限定。此外,提供与第一实施例的接合构件相同的接合构件4。
第一构件200包括顶侧2a和底侧2b。在使用中,底侧2b朝着骨表面定向并且顶侧2a背离骨表面。底侧2b可以具有凹陷表面部分210,所述凹陷表面部分可以容纳第二构件300的一部分使得第二构件300可以在大致平行于骨表面的轴向方向上相对于第一构件200移位。在顶侧2a可以设有加厚部分230,所述加厚部分包括螺纹孔250以便在其中接收接合构件4。底侧2b可以在凹陷部分210的区域中具有另一凹陷270,所述另一凹陷远离底表面210延伸以提供用于在其中容纳接合构件4的套环43和接合突起44的空间。围绕螺纹孔250的区域形成用于接合构件的套环43的抵接件260a。加厚部分230的外部轮廓可以为大致长方体形。在相对两侧可以设有突起240,所述突起可以接合形成于第二构件300上的引导结构。
第一构件200还可以具有一个或多个孔290,所述孔用于在其中容纳骨螺钉(未显示)的头部以便将骨板固定到骨。
第二构件300包括顶侧3a和底侧3b,其中底侧的至少一部分具有与第一构件200的底侧2b的最下部分大致齐平的表面。在其顶侧3a,设有凹槽310,所述凹槽用于在其中在轴向方向上引导第一构件200以允许调节骨板的总长度。凹槽310具有在每一侧的向内延伸的小突起311,所述向内延伸的小突起稍微在突起240之上延伸使得安全地引导第一构件。
在顶侧3a上,设有轴向的球段形凹陷350的线。当组装第一构件200和第二构件300使得侧向突起240在凹槽310中被引导并且接合构件4插入螺纹孔250中时,凹陷350的线在横向方向上与螺纹孔250的中心对准,使得接合构件4的接合部分44可以接合凹陷350。
第二构件300也可以具有一个或多个孔360,所述孔从顶侧3a延伸到底侧3b以便在其中容纳骨螺钉。应当注意各种不同形状可以被预期用于第一和第二构件。形状在任何情况下不限于图16和17中所示的形状。
在使用中,在组装骨板之前插入到第一构件的螺纹孔250中的接合构件可以呈两个配置。在图17中所示的第一配置中,接合构件朝着第二构件300拧动直到它的接合突起44接合凹陷350的凹陷线中的一个凹陷。在该配置中第一构件200相对于第二构件300锁定。在第二配置中,接合构件4向后拧动直到套环43抵靠设在加厚部分230的内壁的内部处的抵接表面260。突起44与凹陷350脱离接触并且第一构件200可以在轴向方向上相对于第二构件300移位。套环43防止接合构件4从第一构件200不慎脱离。这增强植入物的使用的安全性。
应当注意,可以使用若干接合构件4。此外,与第一实施例中一样,套环43可以为环形或者可以仅仅设在圆周的有限部分中。
上述实施例的修改是可想到的。例如,棘爪状(ratchet-like)凹陷可以具有不同形状以及接合构件的接合突起。凹陷不需要具有圆形,而是可以为槽状或其它形状。接合构件的接合突起可以具有适合于与凹陷协作的任何形状。
也可以预料接合构件的接合突起关于接合构件的剩余部分弹簧偏压使得当调节植入物的长度时它可以不必使接合突起从接合凹陷完全脱离。
第一构件和第二构件也可以互换,使得接合构件可以设在第二构件中并且第一构件可以具有接合凹陷。

Claims (13)

1.一种用于在创伤手术中应用、具有可变轴向长度的植入物,所述植入物(1;10)包括:
第一构件(2;200)和第二构件(3;300),所述第二构件相对于所述第一构件被引导以在轴向方向上相对于所述第一构件可移动以便调节所述植入物的总长度;以及
至少一个接合构件(4),
在第一配置中,所述接合构件配置成接合所述第一构件(2;200)和所述第二构件(3;300)使得所述第一构件(2;200)和所述第二构件(3;300)相对于彼此锁定,并且
在第二配置中,所述接合构件配置成接合所述第一构件(2;200),使得所述第二构件(3;300)相对于所述第一构件(2;200)可移动;
其中所述接合构件(4)通过抵接件紧固而防止从所述第一构件(2;200)脱离,并且其中所述接合构件(4)是整体部件,其特征在于,所述接合构件(4)包括配置成接合所述第一构件(2;200)的第一部分且包括第二部分,所述第二部分形成配合到所述第二构件(3;300)的接合部分中的接合部分,并且其中限定抵接件的抵接部分设在所述第一部分和所述第二部分之间。
2.根据权利要求1所述的植入物,其中所述接合构件(4)是配置成接合所述第一构件(2;200)中的螺纹孔(25;250)的固定螺钉。
3.根据权利要求2所述的植入物,其中所述接合构件(4)的第一部分是第一螺纹部分(41),接合构件(4)的接合部分是接合突起(44),接合构件(4)的抵接部分是在所述第一螺纹部分(41)和所述接合突起(44)之间的套环(43),所述套环(43)的最大外部宽度大于所述第一螺纹部分(41)的外径。
4.根据权利要求1或2所述的植入物,其中所述第二构件(3;300)包括一系列凹陷(35;350),所述凹陷形成所述第二构件(3;300)的接合部分,所述凹陷在轴向方向上布置且配置成在所述第一配置中由所述接合构件(4)的接合部分接合。
5.根据权利要求4所述的植入物,其中所述凹陷是沿在轴向方向上延伸的线设在所述第二构件的外壁中的棘爪状布置的凹陷(35;350)。
6.根据权利要求1或2所述的植入物,其中所述第一构件(2;200)的内壁部分和所述第二构件(3;300)的外壁部分形成包围所述接合构件(4)的至少一部分(44、43)的隔室。
7.根据权利要求1或2所述的植入物,其中所述第一构件(2)是套筒状部件并且所述第二构件(3)是在所述第二配置中相对于所述第一构件(2)可移动的套筒状部件。
8.根据权利要求7所述的植入物,其中所述植入物(1)是用于插入两个椎骨之间的间隔器。
9.根据权利要求1或2所述的植入物,其中所述第一构件(200)是板状部件并且所述第二构件(300)是板状部件。
10.根据权利要求9所述的植入物,其中所述第二构件(300)包括用于引导所述第一构件(200)的凹槽(310)。
11.根据权利要求9所述的植入物,其中所述植入物(10)是长度可调节骨板。
12.根据权利要求1或2所述的植入物,其中所述接合部分是球状的。
13.根据权利要求1所述的植入物,其中所述创伤手术是脊柱手术。
