一种斜骨面打孔的稳钻器具,其特征是防滑筒(2)焊接在稳固钳(1)的两边钳嘴上,防滑筒是由壁厚1-5毫米、内直径1-8毫米的两个半圆筒组成,两个半圆筒的结合部一边设有阳齿(4),一边设有凹槽(5),防滑筒的长度为1-5厘米,防滑筒的远端还设有防滑牙(6);稳固钳的手柄中间设有扣紧齿(3)。它能够减少钻头反复钻入的次数,有效保护股骨颈粗隆外侧壁骨质的强度。减少医源性损伤,有利于钻头打入预设的位置,提高钻孔的精度。

Description

一种斜骨面打孔的稳钻器具
技术领域
本发明是应用于临床医学骨科、特别是用于一种股骨颈圆锥空心螺钉导向器的辅助工具,在其它斜骨面打孔也很好发挥作用。它涉及医疗器械领域。
背景技术
目前,10-15%股骨颈骨折患者会出现医师几乎无法控制的并发症,如骨折不愈合与股骨头坏死等。其发生原因与原发损伤及骨折移位程度有关。而解剖复位和稳定内固定后,骨折骨不连及股骨头坏死发生率理应降至可以接受的较低水平。骨折固定的方法对股骨颈骨折后缺血坏死和骨不连的发生有一定的影响,一般可采用多钉固定,但对于一些位于基底部的囊内骨折和严重粉碎的股骨颈骨折,多钉固定的稳定性并不理想。空心螺钉内固定通常适用于大多数股骨颈骨折。Rehnberg和Oerud发现大粗隆外侧皮质的强度对螺钉内固定有明显的影响。根据这一研究,他们认为螺钉不能牢靠地固定于大粗隆外侧壁上,将很容易发生并发症,且随着科学技术的进步与医疗技术的提高,微创技术在外科治疗方面得到突飞猛进的发展。传统的微创治疗股骨颈骨折,手术治疗方法为经皮X线机监测下经皮平行空心螺钉固定术,但实际操作过程中空心螺钉准确打入股骨颈的成功率较低。斜骨面打孔极易打滑,免强钻成的孔不是歪就是斜了或是位移,反复多次打孔伤及其它组织,造成医源性损伤,不利于患者恢复健康,本人研制了一种斜骨面打孔的稳钻器具,解决了医学界斜骨面打孔难的问题。
发明内容
本发明的目的是创造一种能防止钻头在斜骨面打滑专用器具,提供一种斜骨面打孔的稳钻器,它能够减少钻头反复钻入的次数,有效保护股骨颈粗隆外侧壁骨质的强度。减少医源性损伤,有利于钻头打入预设的位置,提高钻孔的精度。
为了实现上述目的,本发明采用如下的技术方案:
一种斜骨面打孔的稳钻器具,其特征是防滑筒(2)焊接在稳固钳(1)的两边钳嘴上,防滑筒是由壁厚1-5毫米、内直径1-8毫米的两个半圆筒组成,两个半圆筒的结合部一边设有阳齿(4),一边设有凹槽(5),防滑筒的长度为1-5厘米,防滑筒的远端还设有防滑牙(6);稳固钳的手柄中间设有扣紧齿(3)。
本发明的工作原理为:将患者麻醉后置于牵引床上,骨折的斜骨面经复位后,在C型臂引导下置入定位导针,定位导针置入于股骨颈轴心中,深度至头下5mm。钉尾处切皮3cm,安装导向圆锥及加压套筒,导向圆锥尖部到粗隆外侧骨皮质,固定加压套筒,明确定位导针位骨质内深度,调整圆锥根据股骨颈直径分别选择三点作为入钉点,分别位于股骨距上方、股骨颈后上方及前上方,尽量靠近股骨颈骨皮质。置入导针后,经C型臂透视,导针位于股骨颈内,股骨头软骨下,去除加压套筒,定位导针及导向圆锥,分别根据定位导针位于骨质内长度,计算并钻孔的长度。钻头通过导向孔达斜骨面打滑时,将稳固钳两边钳嘴,防滑筒两边分开,用防滑筒套住钻头,并使防滑筒的阳齿与凹槽相合,然后将扣紧齿扣上,再把防滑筒的防滑牙抵住斜骨面,找好用力支撑点后开钻钻孔。
本发明的优点在于:能够大大降低打钻时出现的风险,同时提高打钻钻孔的精度,使安全性与实用性共存。
