技术领域
本实施例涉及(晶状体)超声乳化套管和针头(针),以及更具体地涉及套管和针头的组合,所述组合具有用于在通过切口的插入过程中和插入之后将套管到针头的附接保持在所需位置的特征。
背景技术
众所周知的是在现有技术中使用双轴超声乳化(phaco)手持件。超声乳化手持件具有附接到手持件远端的超声乳化针头,挠性的冲洗套管围绕大部分超声乳化针头并且也附接到手持件。套管通常将冲洗流体从手持件的出口携带到外科手术部位。冲洗流体在套管的内部与针头的外部之间流动,并通过形成于套管中的端口进入到外科手术部位内。冲洗流体有助于在使用过程中冷却针头,并且还替换通过针头内的管腔从外科手术部位抽吸出来的流体,以防止眼球在外科手术过程中塌陷。因此,针头和套管以双轴方式提供两种功能,即冲洗和抽吸。
冲洗套管通常由诸如硅树脂或其它相对软的、可压缩的材料的顺从性材料形成,以使套管不会损伤切口界面处的组织,此外也为了在切口处形成至少局部密封以使在外科手术过程中从眼球泄漏的流体减到最少。
在眼科外科手术中的当前趋势要求比以往更小直径的针头和更小的切口。据信更小的切口导致角膜曲率的较少变化和更好的术后视觉灵敏度。这些更小的切口导致了与套管和针头的更紧密的界面。变得越来越普遍的问题是当外科医生试图通过切口插入套管和针头时套管在切口处折叠。该问题在图1处示出。针头10已经通过眼睛14中的切口12,但套管16在眼睛14的外部在18处折叠。针头10以已知的方式附接到超声乳化手持件20,通常经由螺纹连接进行附接,如图所示。套管16随同针头10通过切口12并进入到眼睛14内是至关重要的,以便套管16可保护眼组织免受损伤,并且将冲洗流体提供到眼睛14内。

具体实施方式
图2示出根据本公开的超声乳化针头套管20。套管20附接到具有远端24和近端26的超声乳化针头22。近端26通常附接到常规的超声乳化手持件(未示出),其可以包括或可以不包括用于在超声乳化针头套管20与针头22之间输送冲洗流体的端口。套管20包括细长的、弹性的基本上管状的主体部分28,所述主体部分28具有用于围绕超声乳化针头22的轴(杆)的一部分的近端32和远端30。套管近端32包括扩大部段,如图所示,所述扩大部段形成在主体部分28的近端上,以围绕针头22的鼓突部(在下面的图3中示出),以及附接到外科手术手持件(未示出)。套管远端30由针头结构保持(在下面的图3中示出),所述针头结构邻近(紧邻)针头远端24,用于将所述套管远端30保持在针头远端24的附近。套管20通常还包括邻近所述套管远端30形成的至少一个冲洗端口34(参见图2)。
套管20通常由硅树脂或其它适于在外科手术中使用的柔韧的、可压缩的、弹性材料形成。以这种方式,套管20具有绝缘(隔热)和密封两种功能,以便在白内障的外科手术中使用。图3示出图2沿着线3-3所取的截面视图。如可以看到的那样,近端32的扩大部段围绕针头22的超声乳化针头鼓突部36。在外科手术过程中,乳化的白内障组织和冲洗流体通过管腔38被抽吸到超声乳化手持件(未示出)内,并最终进入到眼睛外科手术系统(也未示出)的泵送系统的收集储槽内,其中所述管腔38跨越针头22的从近端26到远端24的长度,所述眼睛外科手术系统例如可从Bausch&Lomb Incorporated购得。冲洗流体可被引入以包围针头22且被包含在套管20内,并且最初将在箭头40的方向上流动。这种冲洗流体的引入与未在针头22和套管20之间引入这种流体的情况相比给套管22提供更为绝缘和密封的效果。针头近端26通常包括用于配合连接到外科手术手持件的螺纹部分(未示出)的螺纹接口42。套管远端30由针头结构44保持在针头远端24的附近。
图3所示的针头结构44呈环形槽的形式,如图所示,用于与套管20的凸缘46配合接合。槽44和凸缘46配合,以防止图1中所示的套管折叠。当套管20和针头22通过切口插入时,通过将套管远端30保持在针头远端24的附近,套管将适当地定位,以便在外科手术过程中给眼睛提供冲洗流体,以及提供其绝缘和密封性能。这增加了外科手术的效率,并最小化外科医生将需要停止外科手术并操作套管、针头和切口以将套管通过切口插入的可能性。这导致对切口处组织的更少创伤。将套管保持在槽44中还提供套管远端30相对于所述针头远端24的一致性位置,这有助于外科医生看到外科手术部位。槽44可具有所示圆形形状之外的横截面形状,诸如正方形、三角形或其它几何形状。
图4和5公开了一个替代性实施例,其中保持结构为环形凸台50,用于与套管54的槽52配合接合。凸台50可由任何已知的方法形成在针头56上,所述方法例如为模制、机械加工或其它方法。凸台50还可以是粘附到或摩擦地附接到针头56的单独的环。凸台50也可具有所示圆形形状之外的横截面形状,诸如正方形、三角形或其它几何形状。槽52应具有与凸台50充分配合的形状,从而使得套管54将被保持在针头远端58的附近。
图6示出另一实施例,其中保持结构是邻近针头远端62的锥形部段60,由此使得锥形部段的外径从远侧点64到近侧点66增大,从而使得在近侧点66处的锥形部段60的外径大于套管远端68的外径。锥形部段60可由任何已知的方法形成在针头70上,所述方法包括在上面相对于形成凸台50所述的那些方法。重要的是,在近侧点66处的外径大于套管远端68的外径,从而当针头和套管的组合通过切口插入时套管远端68将不会在切口上卡住并折叠。锥形部段60还提供斜坡或楔形部,以便以最小的组织创伤展开切口,并允许套管通过所述切口而不在切口上卡住。
如上所示的实施例允许冲洗套管和超声乳化针头的组合易于通过长度小于2毫米的切口而插入。除了在保持结构和套管远端方面的差异之外,所有实施例的针头和套管基本上是相同的。

Claims (5)

1.冲洗套管和超声乳化针头的组合,所述组合包括:
冲洗套管,所述冲洗套管具有近端和远端,所述近端用于附接到外科手术手持件;
超声乳化针头,所述超声乳化针头具有近端、远端和跨越所述针头的从所述近端到所述远端的长度的管腔;
其中,所述针头近端用于连接到外科手术手持件,并且所述针头具有邻近所述针头远端形成锥形部段的结构,由此使得所述锥形部段的外径从远侧点到近侧点增大;以及
其中,所述冲洗套管远端邻近所述锥形部段近侧点地围绕所述针头,从而使得在所述近侧点处的所述锥形部段的外径大于所述套管远端的外径。
2.根据权利要求1所述的套管和针头的组合,其特征在于,所述锥形部段将所述套管远端保持在所述针头远端附近。
3.根据权利要求1或2所述的套管和针头的组合,其特征在于,所述套管具有邻近所述套管远端形成的至少一个端口。
4.根据权利要求1或2所述的套管和针头的组合,其特征在于,所述套管由顺应性材料形成。
5.根据权利要求1或2所述的套管和针头的组合,其特征在于,所述针头近端包括用于配合连接到外科手术手持件的螺纹部分的螺纹接口。
