Abstract
本发明提供了一种调节肿瘤患者体内CD4+T细胞水平的系统和方法,具体地,本发明提供了一种用于提高哺乳动物的CD4+T细胞水平的装置,所述哺乳动物具有癌组织,所述装置包括:冷处理单元和热处理单元,所述冷处理单元用于给所述癌组织降温;所述热处理单元用于给所述癌组织升温。本发明还提供了一种提高肿瘤患者体内CD4+T细胞水平的方法,包括步骤冷处理癌变组织和热处理癌变组织。
Description
技术领域
本发明属于医学工程领域,具体地说,本发明涉及一种调节肿瘤患者体内CD4+T细胞水平的系统和方法。
背景技术
肿瘤是影响人类健康的主要原因之一。从理论上说,肿瘤作为机体的非我,免疫系统能够识别,从而抑制肿瘤的发生发展,但事实上肿瘤宿主通常免疫力低下,无法产生有效的抗肿瘤免疫。近年来,有关肿瘤发生发展与机体免疫系统相互作用的肿瘤免疫编辑假说引起了人们的关注。该假说认为肿瘤细胞通过降低自身的免疫原性、下调细胞表面协同刺激分子的表达、释放大量免疫抑制因子、召集多种免疫抑制细胞,形成稳定的肿瘤免疫耐受格局,保护肿瘤细胞逃脱机体的免疫监视并促进肿瘤的发生发展。也正是这种由肿瘤诱导的免疫耐受导致肿瘤治疗的效果不佳。因此研究肿瘤免疫耐受的产生机制及如何打破肿瘤免疫耐受成为肿瘤学界关注的重点。大多数恶性肿瘤都会引起不同程度的免疫抑制,以利于肿瘤逃避机体免疫监视和攻击,促进肿瘤发展。但这些治疗方法均不能有效地缓解机体免疫抑制微环境,不能有效地刺激机体产生全身性抗肿瘤免疫响应,从而不能完全抑制癌症转移的发展,治疗效果十分有限。临床上,乳腺癌治疗后仍有很大一部分患者面临死亡。癌症治疗新手段的理想目标应全面解除机体免疫抑制微环境,并充分刺激机体产生抗肿瘤免疫响应。
人体的免疫器官可分为中枢免疫器官和外周免疫器官。中枢免疫器官包括骨髓和胸腺,外周免疫器官则包括脾脏、淋巴结及粘膜相关淋巴组织。对肿瘤抗原的外周免疫耐受是抗肿瘤免疫发挥效应的主要障碍。通常,淋巴结被认为是肿瘤抗原呈递给T细胞和引发外周免疫耐受的主要部位。然而,在淋巴结中不成熟髓系细胞的聚集是极为有限的。现有研究发现,脾脏作为外周主要的免疫器官,在肿瘤的发展中,经历了骨髓细胞的增殖,在肿瘤引发的免疫耐受中起到了关键的作用。在对脾脏免疫环境进行分析时,发现CD4+和CD8+T细胞是最重要的抗肿瘤细胞免疫介导者。因此本领域技术人员一直致力于开发一种能够提高肿瘤患者体内CD4+和/或CD8+T细胞水平的方法。
乳腺癌是女性中最常见的恶性肿瘤,而转移是癌症患者死亡的主要原因。目前肿瘤治疗如手术、放疗和化疗,效果还不理想,且副作用很大。特别对于已发生转移的肿瘤,几乎没有有效的治疗方法。由于恶性肿瘤血管异常增生,与正常血管相比,管壁薄弱、分化程度低,缺乏平滑肌及完整的基底膜结构,内皮细胞之间存在较大缝隙,通透性强,更有利于肿瘤细胞穿透血管壁形成远端转移灶。针对目前乳腺癌治疗现状,迫切需要寻求新型的肿瘤治疗方法。
发明内容
本发明的目的在于提供一种提高患癌哺乳动物的抗肿瘤免疫响应的方法和实现该方法的系统。
本发明的第一方面,提供了一种用于提高哺乳动物的CD4+T细胞水平的装置,其中,所述哺乳动物具有癌组织,所述装置包括:冷处理单元和热处理单元,
所述冷处理单元用于给所述癌组织降温;所述热处理单元用于给所述癌组织升温。
在另一优选例中,所述冷处理单元中包括冷源,所述冷源包括液氮、液氧或液态笑气。
在另一优选例中,所述冷处理单元利用液氮、液氧或液态笑气给所述癌组织降温。
在另一优选例中,所述冷处理单元利用节流装置获得的低温氩气或者液氩进行降温。
在另一优选例中,所述热处理单元利用射频、微波、红外或激光的辐照,或者通过电加热等方式给所述癌组织加热。在另一优选例中,所述装置还包括温度监测单元,所述温度监测单元用于监测所述癌组织的温度。
在另一优选例中,所述装置还包括CD4+T细胞水平监测单元,所述CD4+T细胞水平监测单元用于检测所述哺乳动物体内的CD4+T细胞水平。
在另一优选例中,所述装置还包括一接触头,所述的接触头具有一接触面,所述接触面用于与所述癌组织贴合,所述冷源通过所述接触头给所述癌组织降温,和/或所述热源通过所述接触头给所述癌组织升温。
在另一优选例中,所述接触面面积为0.1-10cm2,较佳地,所述接触面面积为0.25-5cm2。
在另一优选例中,所述接触面为圆形平面或者所述接触面为球形或近似球形的曲面。
本发明的第二方面,提供了一种用于提高哺乳动物的CD4+T细胞水平的方法,其中,所述哺乳动物具有癌组织,所述方法包括步骤:
(1)冷处理:对所述哺乳动物的至少一处或多处癌组织进行冷处理,所述冷处理包括使得被处理癌组织的温度降至≤-10℃,并维持2-20分钟;
(2)复温处理:将上述经冷处理的癌组织,升温至5-25℃;和
(3)热处理:对上一步骤的经复温处理的癌组织进行热处理,所述热处理包括使癌组织的温度升高至45-60℃,并维持2-20分钟。
在另一优选例中,所述方法还包括步骤:
(4)重复步骤(1)~(3)一次或多次。
在另一优选例中,所述癌组织的体积小于1000cm3;优选地,所述癌变组织的体积小于500cm3;更优选地,所述癌变组织的体积在0.1cm3~100cm3。
在另一优选例中,所述癌组织包括:肿瘤、被癌细胞侵染的组织、肿瘤转移灶、肿瘤病人血液。
在另一优选例中,所述的癌组织为浅表性癌组织、或位于体内的癌组织。

本发明的第四方面,提供了一种释放(展示)癌组织免疫原的方法,方法包括步骤:
(1)冷处理:对癌组织进行冷处理,所述冷处理包括使得被处理癌组织的温度降至≤-10℃,并维持2-20分钟;
(2)复温处理:将上述经冷处理的癌组织,升温至5-25℃;和
(3)热处理:对上一步骤的经复温处理的癌组织进行热处理,所述热处理包括使癌组织的温度升高至45-60℃,并维持2-20分钟。
在另一优选例中,所述方法还包括步骤:

具体实施方式
本发明人通过广泛而深入的研究,获得一种提高肿瘤患者体内CD4+T细胞水平的方法,实验结果表明,通过冷热交替处理癌变组织能够显著的提高肿瘤患者体内CD4+T细胞水平。本发明还提供了适用该方法的系统。
治疗系统或装置:
在一个优选的实施方式中,本发明的用于提高哺乳动物的CD4+T细胞水平的装置,包括:冷处理单元和热处理单元,所述冷处理单元用于给所述癌组织降温;所述热处理单元用于给所述癌组织升温。所述冷处理单元中包括冷源,所述冷源可以为液氮、液氧或液态笑气,利用液氮、液氧或液态笑气给所述癌组织降温。所述热处理单元利用射频、微波、红外或激光的辐照,或者通过电加热等方式给所述癌组织加热。
在另一优选例中,所述装置还包括温度监测单元,所述温度监测单元用于监测所述癌组织的温度。
在另一优选例中,所述装置还包括CD4+T细胞水平监测单元,所述CD4+T细胞水平监测单元用于检测所述哺乳动物体内的CD4+T细胞水平。

(2)利用节流喷嘴设计和氩气的节流效应,达到本专利所述温度。
在较佳的实施方式中,可以实现本发明中热处理(加热)癌变组织的手段包括:利用射频、微波、红外、激光的辐照,或者直接通过电加热等方式达到本专利所述的加热温度。
在本发明的一个较佳的实施方式中,使用的冷热交替治疗系统为中国专利CN200510027343.8和CN200410017864.0中所记载的装置。
本发明的主要优点在于:
(1)本发明首次揭示了一种有效提高肿瘤患者体内CD4+T细胞水平的方法,实验结果表明采用本发明的方法处理荷瘤小鼠,能够显著提高小鼠体内的CD4+T细胞水平;
(2)采用本发明的方法处理荷瘤小鼠,能够显著提高荷瘤小鼠的生存率。
(3)采用本发明的方法处理荷瘤小鼠,能够显著降低荷瘤小鼠体内肿瘤细胞的转移。
下面结合具体实施例,进一步陈述本发明。应理解,这些实施例仅用于说明本发明而不用于限制本发明的范围。除非另外说明,否则百分比和份数按重量计算。
实施例
1.材料和方法
1.1动物、细胞系及主要试剂
6~8周龄SPF级Balb/c雌性小鼠(上海斯莱克动物中心),饲养在独立换气盒笼中,人工控制12小时昼夜变换光照。小鼠自由摄取60Co辐射灭菌的饲料及高温灭菌水。小鼠4T1乳腺癌细胞(获自上海市第一人民医院,在其它实施方式中,完全可以使用其它的乳腺癌细胞如T47D细胞系),培养在加有10%的新生胎牛血清(杭州四季清有限公司)及双抗(100U/mL青霉素、100g/mL链霉素)(上海生工生物工程技术服务有限公司)的RPMI1640培养基中(美国Hyclone公司)。超细沉淀硫酸钡颗粒购于上海泽文贸易有限公司。用于流式细胞术和免疫荧光的抗体FITC标记的CD4和PE标记的CD8购于Biolegend公司。苏木素和伊红溶液购自上海虹桥乐翔医用试剂有限公司。
1.24T1乳腺癌模型的建立及肿瘤大小的测定
本实验所用的4T1小鼠乳腺癌细胞可通过血源性途径向肺脏、肝脏、骨髓和脑部等转移,是高度转移性的乳腺癌模型。制备1×106U/0.1mL4T1细胞悬液置于冰上待用。按0.5mL/100g小鼠体重腹腔注射0.016g/mL戊巴比妥钠对动物进行麻醉,在小鼠背部皮下注射0.1mL细胞悬液。肿瘤接种21d后,游标卡尺测量肿瘤体积,按如下公式计算:V(cm3)=p×肿瘤长轴(cm)×肿瘤短轴(cm)×肿瘤高度(cm)/6。随机将荷瘤小鼠分为对照组、手术切除治疗组、加热治疗组和冷热结合治疗组。在本发明人建立的4T1小鼠乳腺癌动物模型,运用PET/CT扫描,F18标记活体小鼠发现接种肿瘤细胞后21天在肝脏等处已出现微转移。因此本发明人选择肿瘤接种21天作为治疗的时间窗。
1.3冷热交替治疗
1.3.1实验方案
对照组(荷瘤小鼠,Control,记为C),手术切除治疗组(Resectiontreatment,记为R),加热治疗组(Heat treatment,记为H)和冷热结合治疗组(Alternate cooling and heating treatment,记为A)。研究内容如下:(1)接种21天后,将小鼠随机分为四组并分别作不同的处理:对照组(C),手术切除治疗组(R),加热治疗组(H)和冷热结合治疗组(A)。治疗后,每组取六只小鼠,观察其长期治疗效果;此外,每组取三只小鼠,于治疗后28天取小鼠的肺脏,检测治疗后小鼠乳腺癌的肺转移情况。(2)治疗前0天(即接种后21天)及治疗后7天、14天、21天、28天,小鼠被处死,取各组小鼠脾脏及外周血进行免疫细胞分析。此外,为进一步观察转移灶肺脏中免疫环境的改变,治疗后14天,每组另取小鼠3-5只进行肺脏分析。
1.3.2冷热交替治疗系统
本实施例中用于提高哺乳动物的CD4+T细胞水平的装置,包括:冷处理单元和热处理单元,冷处理单元用于给所述癌组织降温;热处理单元用于给所述癌组织升温。冷处理单元中包括冷源液氮,利用液氮给癌组织降温。热处理单元利用射频辐照给所述癌组织加热。该装置包括一接触头,接触头具有一接触面用于与所述肿瘤组织贴合,冷源通过接触头给肿瘤组织降温,热源通过接触头给肿瘤组织升温。接触面为圆形的平面特别适合与本实验中的体表肿瘤模型。
1.3.3实验过程
肿瘤接种21d时进行治疗,治疗前测量肿瘤体积,平均值小于0.2cm3为治疗对象。治疗前,将荷瘤小鼠随机分为四组:对照组、手术切除治疗组、加热治疗组和冷热结合治疗组。首先对需要治疗的小鼠麻醉,然后用酒精和碘酊对肿瘤部位消毒。
治疗时,探针贴在肿瘤表面,将一根测温用的热电偶插入肿瘤基底部。温热治疗采用射频加热,将肿瘤的温度加热至50℃(热电偶所测的温度),并保持15min。冷热交替治疗分为三个过程:(1)冷冻用液氮致冷的方法将肿瘤温度降至-20℃,保持5min;(2)复温冷冻治疗后,等待肿瘤自然复温至10℃左右;(3)加热复温过程结束,射频加热将肿瘤温度升至50℃,保持10min。
1.3.4冷热治疗的疗效评估
治疗后,综合性观察小鼠的生存状况。这部分内容主要包括:观察对照组原位肿瘤的生长情况、治疗组原位肿瘤的消融和复发情况及各组小鼠的转移情况(每天),记录小鼠治疗前后的体重变化(一周两次),作小鼠长期生存率和生存时间的统计等等。这些指标均能有效地反映小鼠的生存状况,其中生存率和生存时间的统计是治疗效果评估的最重要指标。
1.3.5肺脏H&E染色分析
治疗后28天,小鼠被处死。通过气管向肺脏灌注4%甲醛后取下肺脏,置于福尔马林中固定48小时。石蜡包埋肺脏后,使用石蜡切片机切至8微米的肺脏石蜡薄片,室温保存。用二甲苯脱蜡,经梯度酒精(100%-90%-80%-70%)至水。进行石蜡切片的苏木素伊红(H&E)染色,苏木素溶液孵育约10分钟,经自来水浸洗约20min-30min;伊红溶液孵育约1-2分钟,自来水浸洗20min-30min。经梯度酒精(70%-80%-90%-100%)脱水,二甲苯固定两小时后,使用中性树胶封片,风干。染色结束后,将玻片放至正置显微镜明场下观察肺脏转移情况,对感兴趣区域进行观察、拍片并作记录。
1.3.6流式细胞分析
治疗前0天(即接种后21天)及治疗后7天、14天、21天、28天,小鼠被处死,取各组小鼠脾脏及外周血进行免疫细胞分析。将小鼠的脾脏取下后,置于70μm尼龙细胞过滤器上,用5ml注射器研磨,并用DMEM培养液冲至50ml离心管中。2000rpm离心10分钟后弃上清液,加入2ml红细胞裂解液并吹打均匀,室温下裂解5分钟。加入PBS尽量稀释后离心去上清,得脾脏白细胞。重悬后分管,加入1μl相应荧光抗体(标记细胞表面分子以检测特定细胞群,具体标记见表7),在4℃条件下避光孵育30分钟。经PBS清洗后重悬,移入流式管内上机检测。另外,本实验进一步鉴定了T细胞的功能,利用流式细胞仪分选出标记的CD4+T细胞和CD8+T细胞。
1.3.7数据统计
本实验中所有实验数据的统计与分析都采用Graph pad Prism软件进行,利用Student’s t test作组间显著性差异分析,结果以平均值+标准差显示。肺转移面积数据的统计使用Image Pro Plus软件,流式图像的处理使用FlowJo软件。
2结果
2.1脾脏中CD4+T细胞水平的变化
研究发现治疗后14天内,单热、手术切除组脾脏中抗肿瘤免疫细胞(CD4+/CD8+T cells)均与冷热治疗组类似,均呈增加趋势。14天后,单热、手术切除组脾脏中CD4+/CD8+T细胞开始逐渐降低,至第28天单热、手术切除组的CD4+/CD8+T细胞基本降低到与对照组一致的水平。而冷热治疗组中,在21天内,CD4+/CD8+T细胞的水平一直有显著升高,21天以后,CD8+T细胞的水平开始降低,但是CD4+T细胞的水平仍然有较大幅度提升。实验结果可参见图1,图1显示了不同治疗组引发的脾脏中免疫细胞的变化。
2.2冷热治疗提高小鼠生存率
对治疗后的小鼠治疗进行3个月的观察,每组6只小鼠。冷热交替治疗后,其中4只小鼠生活状态良好,对照组,単热组和手术切除组小鼠几乎全部死亡。因此,冷热交替治疗肿瘤可以极大的提高小鼠生存率,达到良好的治疗效果。图6显示了不同组小鼠总体生存率和生存时间展示(每组6只)。
2.3冷热治疗抑制肿瘤转移
手术切除组、单冷治疗和单热治疗组小鼠治疗治疗后三周,均出现小鼠体表开始出现转移(见图3)。而冷热治疗后,肿瘤开始消退、结痂,及至消融(如图4所示),尤其在荷瘤小鼠经过冷热交替治疗后6周,原位肿瘤完全消融,体表没有转移瘤,小鼠生活状态良好。图3显示了治疗三周后小鼠体表肿瘤转移情况,(A)为手术切除治疗组,(B)为单冷治疗组,(C)为单热治疗组。图4显示了冷热治疗后6周内的小鼠状态。(A)为治疗前;(B)为治疗后;(C)为治疗后1周;(D)为治疗后3周;(E)为治疗后6周。
随后对不同治疗组小鼠的肺脏石蜡切片进行苏木素伊红染色,观察并比较对照组和不同治疗组小鼠肺脏转移的情况(见图5)。研究发现在手术切除治疗组中,观察到异常肺结构和明显的肿瘤转移,肿瘤转移区域与对照组相比没有显著差异;单热治疗组中出现一些小的转移结节,肺组织结构相对正常,肺脏肿瘤转移区域面积有明显降低;而在冷热治疗组中,可观察到一些小的转移结节,肺组织结构已相对正常,其肿瘤转移区域面积减少最为显著。图5显示了治疗后28天的肿瘤肺部转移情况,可以看到冷热交替治疗组的转移面积显著低于其它各组。
3.总结和讨论
本研究以4T1乳腺癌为模型,研究冷热交替局部肿瘤治疗后对外周血和脾脏中CD4+和CD8+T细胞的影响,结果意外的发现,经过冷热交替处理肿瘤组织以后,在21天内小鼠体内的CD4+和CD8+T细胞水平均显著提高,但是21天以后,小鼠体内的CD8+T细胞水平开始下降,而伴随着CD8+T细胞水平的下降CD4+T细胞水平又开始了显著的增长。
有研究结果显示,CD4+T细胞同样具有细胞毒性作用[1],在特定情况下,例如淋巴细胞减少时,CD4+T细胞能够获得直接杀伤细胞的能力[2,3]。在CD4+T细胞中,则主要有Fas途径和另一种TNF相关的凋亡诱导配体途径来介导其细胞毒性作用[1]。本实验中,经观察发现,冷热治疗后,相比于CD8+T细胞,脾脏和血中CD4+T细胞有着更为明显的增加,并且同样能够分泌细胞毒性细胞因子IFN-γ,具有强的细胞毒性杀伤作用。且在外周血中,治疗后14天,相对于其他组中CD4+T细胞明显下降的情况,冷热组中该类细胞则维持着长时间的较高水平。这些实验结果提示,CD4+T细胞起着更为重要的作用,本发明通过冷热治疗诱发机体产生CD4+T细胞,增强机体的抗肿瘤免疫,显著降低了荷瘤小鼠的死亡率,极大地延长了实验小鼠的生存时间。

Claims (10)
1.一种用于提高哺乳动物的CD4+T细胞水平的装置,其中,所述哺乳动物具有癌组织,其特征在于,所述装置包括:冷处理单元和热处理单元,
所述冷处理单元用于给所述癌组织降温;所述热处理单元用于给所述癌组织升温。
2.如权利要求1所述的装置,其中,所述冷处理单元利用液氮、液氧或液态笑气给所述癌组织降温,或者所述冷处理单元利用节流装置获得的低温氩气或者液氩进行降温;和/或
所述热处理单元利用射频、微波、红外或激光的辐照,或者通过电加热等方式给所述癌组织加热。
3.如权利要求1所述的装置,其中,所述装置还包括温度监测单元,所述温度监测单元用于监测所述癌组织的温度;和/或
所述装置还包括CD4+T细胞水平监测单元,所述CD4+T细胞水平监测单元用于检测所述哺乳动物体内的CD4+T细胞水平。
4.如权利要求1所述的装置,其中,所述装置还包括一接触头,所述的接触头具有一接触面,所述接触面用于与所述癌组织贴合,所述冷源通过所述接触头给所述癌组织降温,和/或所述热源通过所述接触头给所述癌组织升温。
5.一种用于提高哺乳动物的CD4+T细胞水平的方法,其中,所述哺乳动物具有癌组织,其特征在于,所述方法包括步骤:
(1)冷处理:对所述哺乳动物的至少一处或多处癌组织进行冷处理,所述冷处理包括使得被处理癌组织的温度降至≤-10℃,并维持2-20分钟;
(2)复温处理:将上述经冷处理的癌组织,升温至5-25℃;和
(3)热处理:对上一步骤的经复温处理的癌组织进行热处理,所述热处理包括使癌组织的温度升高至45-60℃,并维持2-20分钟。
在另一优选例中,所述方法还包括步骤:
(4)重复步骤(1)~(3)一次或多次。
6.如权利要求5所述的方法,其中,所述癌组织包括:肿瘤、被癌细胞侵染的组织、肿瘤转移灶、肿瘤病人血液。
7.如权利要求6所述的方法,其中,在冷处理或热处理步骤中采用无创或微创的方法进行操作。
8.如权利要求6所述的方法,其中,所述方法还包括:在步骤(3)的热处理后,测定CD4+T细胞的水平。
9.一种治疗肿瘤的方法,其特征在于,使用权利要求5-8中任一所述的方法提高肿瘤患者体内CD4+T细胞水平。
10.一种释放癌组织免疫原的方法,其特征在于,所述方法包括步骤:
(1)冷处理:对癌组织进行冷处理,所述冷处理包括使得被处理癌组织的温度降至≤-10℃,并维持2-20分钟;
(2)复温处理:将上述经冷处理的癌组织,升温至5-25℃;和
(3)热处理:对上一步骤的经复温处理的癌组织进行热处理,所述热处理包括使癌组织的温度升高至45-60℃,并维持2-20分钟。
