Abstract

急切需要防止这类XDR/PDR鲍氏不动杆菌感染的新的方法,尤其因为在下一个十年内在抗细菌药规划中无治疗这些感染的新药(Boucher等,Clin Infect Dis48:1-12(2009);Spellberg等,Clin Infect Dis46:155-164(2008)。由于了解了鲍氏不动杆菌感染的风险因素(Beavers等,2009;Caricato等,Intensive Care Med35:1964-1969(2009);D'Agata等,Infect Control Hosp Epidemiol21:588-591(2000);Furniss等,J Burn Care Rehabil26:405-408(2005);Metan等,Eur J Intern Med20:540-544(2009);Zakuan等,Trop Biomed26:123-129(2009),处于极高风险患者的疫苗接种是防止这类感染的有前景的方法,基于抗体的免疫疗法有希望改善感染的结果。
发明概述
本发明提供包含OmpA或其抗原片段的疫苗组合物和针对鲍氏不动杆菌感染的主动免疫的相关方法。本发明还提供与OmpA特异性结合的抗体及其抗原结合片段和针对鲍氏不动杆菌感染的被动免疫的相关方法。本发明的组合物和方法可用于预防或治疗鲍氏不动杆菌感染,包括由碳青霉烯类和除多黏菌素或替吉环素以外的所有其它抗生素的耐药菌株引起的那些感染,亦称为极端耐药(XDR)鲍氏不动杆菌感染,和对每种FDA批准的抗菌素有耐药性的那些感染,亦称为泛耐药(PDR)鲍氏不动杆菌感染。

发明详述
本发明部分基于鲍氏不动杆菌OmpA作为鲍氏不动杆菌靶向疫苗的抗原靶的发现。本发明提供包含OmpA或其抗原片段的疫苗组合物和针对鲍氏不动杆菌感染的主动免疫的相关方法。本发明还提供与OmpA特异性结合的抗体及其抗原结合部分和针对鲍氏不动杆菌感染的被动免疫的相关方法。本发明的组合物和方法可用于预防或治疗鲍氏不动杆菌感染,包括由碳青霉烯类和除多黏菌素或替吉环素以外的所有其它抗生素的耐药菌株引起的那些感染,亦称为极端耐药(XDR)鲍氏不动杆菌感染,和对每种FDA批准的抗菌素有耐药性的那些感染,亦称为泛耐药(PDR)鲍氏不动杆菌感染。
如本文所述,OmpA提供鲍氏不动杆菌靶向疫苗的抗原。如实施例中所述,OmpA被鉴定为基于小鼠感染期间的体液免疫显性的疫苗。OmpA在多个临床分离株间是高度保守的,并与人蛋白质组共有最低同源性。
在过去十年中,在全世界,鲍氏不动杆菌已成为感染的最多抗生素抗性原因之一,具有因其所致的不可接受的高死亡率。在下一个十年内,可能无法获得能够治疗XDR/PDR鲍氏不动杆菌的新疗法,本发明根据鲍氏不动杆菌靶向疫苗的抗原的发现,提供预防和治疗这类感染的新策略。rOmpA被鉴定为基于小鼠感染期间的体液免疫显性的疫苗。OmpA在多个临床分离株间是高度保守的,并与人蛋白质组共有最低同源性。当给予Al(OH)3佐剂时,在无免疫应答的DKA小鼠的极高和快速致死鼠模型中观察到显著功效,并且在吸入性肺炎大鼠模型中也观察到显著功效。在用Al(OH)3的两种截然不同的模型中的功效表明疫苗候选物的可转移性(translatability),因为Al(OH)3是世界上最广泛使用的佐剂之一,并且在超过半个世纪给予数百万患者后,具有已证实的安全性和功效记录(Lindblad,Vaccine22:3658-3668(2004);Lindblad,Immunol Cell Biol82:497-505(2004)。
如本文所例示的,各小鼠抗体效价与存活相关,≥1:102,400或1:204,800的IgG效价截止值在预测小鼠存活时是十分准确的。此外,免疫血清是疫苗介导的保护的效应物,并且在被动免疫期间是有效的。之前已报道了鲍氏不动杆菌对补体介导的杀灭作用有抗性(Kim等,FEMSMicrobiol Lett301:224-231(2009);King等,FEMS Microbiol Lett301:224-231(2009),这与当前的研究结果一致。通过提高生物的调理吞噬性杀灭来介导针对鲍氏不动杆菌的免疫诱导保护。这些结果与以下事实一致:中性白细胞减少小鼠易感鲍氏不动杆菌感染(van Faassen等,Infect Immun75:5597-5608(2007)和超氧化物缺陷型gp91phox-/-小鼠超易感鲍氏不动杆菌鼻内感染(Qiu等,Infect Immun75:5597-5608(2009)。总的来说,这些结果证实,通过经基于抗体的调理吞噬作用提高鲍氏不动杆菌的摄取和杀灭,导致更有效地从组织中清除鲍氏不动杆菌。
发现鲍氏不动杆菌OmpA在模型系统中具有多种有价值的生物学性质。例如,已表明OmpA与真核细胞结合,易位至核中,并诱导细胞死亡(Choi等,Cell Microbiol10:309-319(2008);McConnell和Pachon,Protein Expr Puriff77(1):98-103(2010)。
OmpA是一种可防止XDR/PDR鲍氏不动杆菌感染的新的疫苗。如本文所例示的,用可转移佐剂(translatable adjuvant)的可行剂量证实了功效。
如本文所述,本发明提供预防性或治疗性治疗哺乳动物对象(优选人)的鲍氏不动杆菌感染的方法,所述方法包括给予对象免疫有效量的本发明的鲍氏不动杆菌OmpA疫苗组合物、抗体组合物或抗血清。在一个实施方案中,本发明提供预防性或治疗性治疗对象的鲍氏不动杆菌感染的方法,所述方法包括给予对象免疫有效量的包含鲍氏不动杆菌外膜蛋白A(OmpA)或其抗原片段的疫苗组合物。在一个具体的实施方案中,对象是人。
本文所用术语“OmpA”或“鲍氏不动杆菌OmpA”意指对应于图4所示任何氨基酸序列的鲍氏不动杆菌的外膜蛋白A。该术语还包括本领域已知的在序列、免疫原性和功能上基本相似的OmpA氨基酸序列,包括例如表1所示的一个或多个OmpA序列,所述序列通过引用其NCBI登录版本号和gi序列标识符号结合到本文中。本发明的OmpA序列可以与图4所示序列有例如至少80%、至少85%、至少87%、至少88%、至少89%、至少90%、至少91%、至少92%、至少93%、至少94%、至少95%、至少96%、至少97%、至少98%、至少99%同一性。本发明的OmpA可为例如长度小于360个氨基酸、长度小于359个氨基酸、长度小于358个氨基酸、长度小于357个氨基酸、长度小于356个氨基酸、长度小于355个氨基酸、长度小于354个氨基酸、长度小于353个氨基酸、长度小于352个氨基酸、长度小于350个氨基酸、长度小于349个氨基酸、长度小于348个氨基酸、长度小于347个氨基酸、长度小于346个氨基酸、长度小于345个氨基酸。OmpA蛋白的长度可为346个氨基酸。可用于本发明的组合物和方法的鲍氏不动杆菌OmpA氨基酸序列与表4所示序列基本类似,并且可分离或重组制备(rOmpA)。当与例如氨基酸序列中没有至少80%、至少85%、至少87%、至少88%、至少89%、至少90%、至少91%、至少92%、至少93%、至少94%、至少95%同一性的OmpA相比,本发明的OmpA可具有预料不到的高免疫原性。

本发明还提供编码选自SEQ ID NO:1-6的氨基酸序列的分离核酸分子以及包含这类核酸分子的组合物。本发明另外提供包含本发明的分离核酸分子的载体。本发明还提供包含本发明的核酸组合物的疫苗组合物或含有本发明的核酸分子的载体。

鲍氏不动杆菌OmpA的“抗原片段”、“抗原表位”或“抗原决定簇”是指包括或相当于被淋巴细胞或分泌抗体识别或结合的顺序或构象免疫活性区的鲍氏不动杆菌OmpA部分。抗原片段可为任何部分直到全长的鲍氏不动杆菌OmpA,例如至少介于300-350个氨基酸之间、至少介于250-300个氨基酸之间、至少介于200-250个氨基酸之间、至少介于150-200个氨基酸之间、至少介于100-150个氨基酸之间、至少介于50-100个氨基酸之间、至少介于20-50个氨基酸之间、至少介于10-20个氨基酸之间、至少介于2-10个氨基酸之间、至少介于4-8个氨基酸之间、至少介于5-7个氨基酸之间。

本发明提供保护易感哺乳动物、优选人对象抵抗鲍氏不动杆菌感染的一种或多种表现的疫苗组合物,所述表现例如由鲍氏不动杆菌引起的血流感染、医院和社区获得性肺炎、肾感染、尿道感染、膀胱感染、伤口感染、脑膜炎、心内膜炎、眼内炎(endopthalmitis)和角膜炎。在一些实施方案中,易感人对象患有糖尿病、高血压、肝硬化、肾功能不全、人免疫病毒感染、中性粒细胞减少(绝对嗜中性粒细胞计数大于500个细胞/mm)、恶性肿瘤、褥疮、败血症性休克和缺氧性脑病;经历透析或免疫抑制治疗;移植物接受者或气管造口术患者、使用机械呼吸机。本发明的疫苗组合物可特别适用于防止感染的医院患者和军事人员的主动免疫接种,因为鲍氏不动杆菌是伤口感染最常见的原因。
本发明的疫苗组合物可按生理上可给予的形式提供,并且适于通过皮下或鼻内接种给予。
在其它实施方案中,本发明还提供用于产生抗原组合物的抗原或免疫原的方法。所述方法包括(a)提供编码所述抗原的DNA片段,并将所述片段引入表达载体;(b)将含有所述DNA片段的所述载体导入相容的宿主细胞中;(c)在由所述DNA片段编码的产物表达需要的条件下培养步骤(b)提供的所述宿主细胞;和(d)从培养的宿主细胞中分离表达产物,和任选(e)通过亲和层析法或本领域已知的其它层析方法纯化步骤(d)的分离产物。
在又一个实施方案中,本发明提供用于制备含有作为免疫组分的本发明的抗原或免疫原性组合物的疫苗组合物的方法。所述方法包括将抗原或免疫原性组合物和药学上可接受的载体混合。还提供用于产生抗血清的方法,所述方法包括将本发明的抗原制剂给予哺乳动物宿主以在宿主中产生抗体,并且回收含有在宿主中产生的抗体的抗血清。还提供预防性或治疗性治疗哺乳动物对象(适宜为人)的鲍氏不动杆菌感染的方法,所述方法包括给予对象免疫有效量的本文所述的本发明的疫苗组合物或抗血清。在又一个实施方案中,本发明提供用于保护哺乳动物对象免于鲍氏不动杆菌感染或减轻感染的严重程度的方法,所述方法包括将本发明的疫苗组合物给对象皮下或鼻内接种以在对象中诱导针对鲍氏不动杆菌的免疫应答。
本发明还提供用于被动免疫的抗体制剂,其包含对本发明的鲍氏不动杆菌OmpA蛋白或多肽有特异性的至少一种抗体或其抗原结合片段。可预防性或治疗性地使用针对鲍氏不动杆菌感染的抗体制剂,且抗体制剂当给予易被鲍氏不动杆菌感染的哺乳动物对象时,还可提供被动免疫。被动免疫可以是其它治疗(包括主动免疫)的辅助疗法。
在一个具体的实施方案中,本发明提供包含抗体或其抗原结合片段的组合物,其中抗体或抗原结合片段与由选自SEQ ID NO:1-6的氨基酸序列编码的表位特异性结合。在又一个实施方案中,表位可包含这样的抗原片段,其包含不同于SEQ ID NO:11的氨基酸序列至少一个氨基酸的氨基酸序列,或其中抗原片段包含选自SEQ ID NO:7-10和SEQ IDNO:1-6的氨基酸1-18、19-25、26-32、51-65、91-130、151-153、154-165、166、221-235、265-280和307-331的氨基酸序列。例如,至少一个氨基酸可在氨基酸35F、39N、48M、56T、83I、85V、119A、124A、128V、129F、131G、137V、141M、151E、153E、156P、179I、184A、191G、194H、296A和339N处不同于SEQ ID NO:11序列。
可根据制药领域和兽药领域普通技术人员熟知的标准技术,考虑特定抗原、佐剂(如果存在的话)、特定动物或患者的年龄、性别、体重、种类和状况和给药途径等这类因素,来决定待给予人或动物的本发明疫苗的量和给药方案。在本发明中,提供有效剂量的用于针对脑膜炎奈瑟氏球菌(N.meningitidis)的疫苗接种的多糖-蛋白质载体的量可为介于约0.02μg-约5μg/kg体重之间。在本发明优选的组合物和方法中,剂量介于约0.1μg-3μg/kg体重之间。例如,如果感染后消逝的时间较少,则有效剂量将需要较少的抗体,因为有较少时间供细菌繁殖。同样地,有效剂量将取决于诊断时的细菌负荷。可考虑在一天时间内给予多次注射用于治疗应用。本发明的组合物可作为单剂量或连续(即以“加强剂量”或“多次加强剂量”)给予。在本发明的一个实施方案中,优选的给药途径为肌内或皮下,优选肌内途径。给药可通过注射或通过备选递送装置。
在本发明的优选实施方案中,将疫苗组合物配成含或不含防腐剂的无菌液体、无热原的磷酸盐缓冲生理盐水。合适载体和其它添加剂的选择将取决于确切的给药途径和具体剂型的性质,例如液体剂型(例如不论组合物配成溶液剂、混悬剂、凝胶剂还是另一种液体形式),或固体剂型(例如不论组合物配成丸剂、片剂、胶囊剂、囊片剂、延时形式或充液形式)。
本发明的抗体或其片段与鲍氏不动杆菌OmpA特异性结合,且人免疫系统对其耐受良好。
抗体是指全长(即天然存在的或通过正常免疫球蛋白基因片段重组过程形成的)免疫球蛋白分子(例如IgG抗体)或有免疫活性的免疫球蛋白分子的抗原结合部分,如抗体片段。如下文更详细的描述,抗体片段是抗体的部分,例如F(ab')2、F(ab)2、Fab'、Fab、Fv、scFv等。不论结构如何,抗体片段结合被完整抗体识别的相同抗原。术语抗体片段还包括由可变区组成的分离片段(例如由重链和轻链可变区组成的“Fv”片段)和其中轻和重可变区通过肽接头连接的重组单链多肽分子(“scFv蛋白”)。本文所用术语抗体片段不包括无抗原结合活性的抗体的部分,例如Fc片段或单个氨基酸残基。其它抗体片段,例如单域抗体片段是本领域已知的,可用于所要求保护的构建体。(参见例如Muyldermans等,TIBS26:230-235,2001;Yau等,J Immunol Methods281:161-75(2003);Maass等,J Immunol Methods324:13-25(2007);Harlow和Lane,Antibodies:ALaboratory Manual,Cold Spring Harbor Laboratory,Cold Spring Harbor NY(1988))。
在一个实施方案中,本发明提供与鲍氏不动杆菌OmpA或其抗原片段选择性结合的抗体或其片段,其是人源化的或完全人的。抗体或其片段对鲍氏不动杆菌OmpA或其抗原片段显示高亲和力。因此,本发明同与鲍氏不动杆菌OmpA或其抗原片段特异性结合的单克隆或多克隆抗体及其片段有关。
优选选择本发明的抗体或其片段使得它具有特定的结合动力学(例如高亲和力、几乎无解离、低解离速度(off rate)、强中和活性)用于与鲍氏不动杆菌OmpA或其抗原片段特异性结合。抗体优选为分离抗体。按照又一方面,抗体是中和抗体。本发明的抗体特别包括单克隆抗体和重组抗体。本发明的单克隆抗体来源于杂交瘤(例如由借助杂交瘤技术例如Miller和Milstein的标准杂交瘤方法产生的杂交瘤分泌的抗体)。本发明的抗体可来源于杂交瘤,并对鲍氏不动杆菌OmpA或其抗原片段具有特异性。
本发明的抗体可包含完全来源于单一物种的氨基酸序列,因此可以是例如人抗体或小鼠抗体。按照其它实施方案,抗体可以是嵌合抗体或CDR移植抗体或人源化抗体的另一种形式。
术语“抗体”欲指由4条多肽链(两条重(H)链和两条轻(L)链)组成的免疫球蛋白分子。各链通常通过二硫键连接在一起。每条重链由重链可变区(本文简写为HCVR或VH)和重链恒定区组成。重链恒定区由3个结构域CH1、CH2和CH3形成。每条轻链由轻链可变区(本文简写为LCVR或VL)和轻链恒定区组成。轻链恒定区由CL结构域形成。VH和VL区可进一步分成超变区,其被称为互补决定区(CDR),且散布有被称为构架区(FR)的更保守区域。每个VH和VL区由按以下顺序从N端到C端排列的3个CDR和4个FR形成:FR1、CDR1、FR2、CDR2、FR3、CDR3、FR4。
用来提及抗体的术语“片段”或“抗原结合片段”或“结合片段”是指对鲍氏不动杆菌OmpA具有特异性的抗体的一个或多个片段,所述片段仍具有特异性结合鲍氏不动杆菌OmpA或其抗原片段的能力。已表明,抗体的抗原结合功能可由完全抗体的片段承担。结合片段的实例包括抗体(i)Fab片段,即由VL、VH、CL和CH1结构域组成的单价片段;(ii)F(ab)2片段,即包含在铰链区通过二硫桥连接在一起的两个Fab片段;(iii)Fd片段,其由VH和CH1结构域组成;(iv)Fv片段,其由抗体单臂的VL和VH结构域组成;(v)dAb片段(Ward等,(1989)Nature341:544-546),其由VH结构域或VH、CH1、CH2、DH3或VH、CH2、CH3组成;和(vi)分离的互补决定区(CDR)。虽然Fv片段的两个结构域(即VL和VH)由不同的基因编码,但是它们还可通过采用重组方法经合成接头连接在一起,籍此它们可作为单一蛋白质链产生,其中VL和VH区一起存在以形成单价分子(称为单链Fv(ScFv),参见例如Bird等,Science242:423-426(1988);以及Huston等,Proc.Natl.Acad.Sci.USA85:5879-5883((1988)。术语抗体的“抗原片段”也欲包括这类单链抗体。单链抗体的其它类型例如同样属于该类型的双抗体。双抗体是二价双特异性抗体,其中VH和VL结构域在单一多肽链上表达,但是使用对两个结构域是太短的接头使之存在于相同链上,结构域因此被迫与另一链的互补结构域配对,并形成抗原结合部位(参见例如Holliger,P.,等,Proc.Natl.Acad.Sci.USA90:6444-6448(1993);Poljak,R.J.,等,Structure2:1121-1123(1994)。
又一个实施方案针对作为较大免疫粘附分子的部分的抗体或其抗原结合片段,所述免疫粘附分子通过抗体或抗体部分与一种或多种其它蛋白质或肽共价或非共价缔合形成。这类免疫粘附分子可包括使用链霉抗生物素核心区以产生四聚scFv分子(Kipriyanov,S.M.Human Antibodiesand Hybridomas6:93-101(1995)和使用半胱氨酸残基、标志物肽和C端多聚组氨酸标签以制备二价生物素化scFv分子(Kipriyanov,S.M.,MolImmunol31:1047-1058(1994)。
抗体部分,例如Fab和F(ab')2片段,可通过采用常规技术(例如用木瓜蛋白酶或胃蛋白酶消化)由完整抗体产生。另外可通过采用标准重组DNA技术来得到抗体、抗体部分和免疫粘附分子。
可通过采用重组技术,产生、表达、形成或分离对鲍氏不动杆菌OmpA或其抗原结合片段有特异性的抗体,例如通过采用转染至宿主细胞的重组表达载体表达的抗体;自重组组合抗体文库分离的抗体;自因人免疫球蛋白基因所致是转基因的动物(例如小鼠)分离的抗体(参见例如Taylor,L.D.等,Nucl Acids Res.20:6287-6295(1992);或者以其中特定的免疫球蛋白基因序列(例如人免疫球蛋白基因序列)与其它DNA序列组合的任何其它方式产生、表达、形成或分离的抗体。重组抗体包括例如嵌合抗体、CDR移植抗体和人源化抗体。
对鲍氏不动杆菌OmpA具有特异性的人抗体具有对应于如例如由Kabat等人(参见Kabat等(1991)Sequences of Proteins of ImmunologicalInterest,第5版,U.S.Department of Health and Human Services,NIH公布号91-3242)描述的人种系的免疫球蛋白序列的可变区和恒定区或来源于其中。然而,本发明的人抗体可包含非由人种系免疫球蛋白序列编码的氨基酸残基(例如通过体外随机或位点专一诱变或通过体内体细胞突变引入的突变),例如在CDR中,尤其在CDR3中。本发明的重组人抗体具有可变区,并还可包含来源于人种系的免疫球蛋白序列的恒定区(参见Kabat,E. A.等(1991) Sequences of Proteins of Immunological Interest,第5版,U.S. Department of Health and Human Services, NIH公布号91-3242)。然而,按照具体的实施方案,对这类重组人抗体进行体外诱变(或如果由于使用人Ig序列所致是转基因的动物,则进行体细胞体内诱变),使得重组抗体VH和VL区的氨基酸序列是这样的序列,即虽然它们与人种系的VH和VL序列或来源于其中的VH和VL序列有关,但并不是天然存在于体内人抗体种系库内。按照具体的实施方案,这类重组抗体是选择诱变或回复突变或两者的结果。
在又一个实施方案中,本发明提供诊断鲍氏不动杆菌感染的方法,所述方法包括从疑似鲍氏不动杆菌感染的对象中获得组织样品,使疑似包含鲍氏不动杆菌的组织样品与OmpA片段、引物、抗体或其抗原结合片段接触,通过本领域已知方法检测样品中鲍氏不动杆菌OmpA的存在情况。
将参照下列本发明的构思的若干优选实施方案详细给出的说明性非限制性实例,对本发明做进一步描述。在不偏离本发明精神的情况下,本发明的其它实例对本领域技术人员而言将是清楚明了的。

实施例I
在小鼠感染期间产生的特异性抗鲍氏不动杆菌抗体
使用鲍氏不动杆菌的6个临床分离株(表2)。从各种不同的机体感染部位收获这些分离株。5个菌株除多黏菌素以外对所有抗生素都有抗性(表5)。菌株分型通过之前描述的多基因座序列分型进行(Bartual等,JClin Microbiol 43:4382-4390 (2005);Tian等,Antimicrob Agents Chemother55:429-432 (2011)。Balb/c小鼠用于所有实验。对于一些实验,使用退役种小鼠(> 6月龄),而其它实验使用年幼(6-10周龄) Balb/c小鼠。感染前10天,通过腹膜内注射含200 mg/kg链脲佐菌素的0.2 ml柠檬酸盐缓冲液诱发糖尿病。如前所述,在链脲佐菌素处理后7天,在所有小鼠中证实了糖尿和酮尿(Ibrahim等,J Antimicrob Chemother 58:1070-1073(2006);Ibrahim等,J Clin Invest 117:2649-2657 (2007);Spellberg等,Antimicrob Agents Chemother49:830-832(2005)。所用细菌菌株见表2。
通过修改标准的公开方法,来产生鲍氏不动杆菌细胞膜制备物(Molloy等,Eur J Biochem267:2871-2881(2000);Soares等,Proteome Sci7:372009)。简单地说,在振荡的同时,使鲍氏不动杆菌菌株在胰蛋白酶大豆肉汤(TSB)中于37℃下生长过夜。在振荡的同时,使细菌在37℃下传代直到对数生长中期。通过在4℃下以3,500g离心15分钟来收获细胞,用10mL0.9%(w/v)NaCl洗涤两次。将所得沉淀重新悬浮于分解缓冲液(7.8g/L NaH2PO4、7.1g/L Na2HPO4、0.247g/L MgSO47.H2O+蛋白酶抑制剂混合物(GE Healthcare,USA)+核酸酶混合物(GE Healthcare,USA))中,在冰上超声处理5分钟的3个周期。通过以1,500g离心分离未破碎的细胞。将上清液在4℃下以4,500rpm离心30分钟,并通过0.45μM过滤器(Milipore,USA)以除去细胞碎片。将等体积的冰冷的0.1M碳酸钠(pH11)加入所得上清液中,在冰上慢慢搅拌混合物过夜。通过在4℃下以100,000g超速离心45分钟,收集碳酸盐处理的膜蛋白质,将膜重新悬浮于500μl H2O中。最后,蛋白质提取物用2-DE Cleanup Kit(Bio-Rad,USA)处理。
如Pitarch等(Pitarch等,Mol Cell Proteomics5:79-96(2006);Pitarch等,Electrophoresis20:1001-1010(1999)所述,通过大小和等电聚焦(IEF),使用鲍氏不动杆菌细胞膜制备物的二维SDS/10%-PAGE凝胶分离蛋白质。对于等电聚焦(IEF),使用具有4-7pH梯度条带(ReadyStrip IPG条带,Bio-Rad,USA)的Bio-Rad-PROTEIN IEF系统(Bio-Rad,USA)。将蛋白质在8M脲、2%(w/v)CHAPS、40mM DTT和0.5%(v/v)相应的再水化缓冲液(Bio-Rad,USA)中增溶。使条带再水化过夜后,进行电泳,在250V下20分钟,4000V下2小时,4,000V10,000V-h,均在室温下。在第二维(SDS-PAGE)前,聚集的IPG条带用缓冲液I和II平衡10分钟(ReadyPrep2-D Starter Kit,Bio-Rad,USA)。将蛋白质在8-16%Criterion Pre-cast Gel(Bio-Rad,USA)中分离,并转移至immune-BlotPVDF膜(Bio-Rad,USA)上。膜用Western Blocking Reagent(Roche)处理过夜,用免疫前或免疫鲍氏不动杆菌感染的小鼠血清探测。将膜洗涤,与第二HRP缀合的山羊抗小鼠IgG(Santa Cruz Biotech,USA)一起温育。在与SuperSignal West Dura Extended Duration Substrate(Pierce,USA)一起温育后,使用CCD照相机检测信号。
切离目标蛋白质斑点,送往UCLA W.M.Keck Proteomic Center以在配备Eksigent(Dublin,CA)NanoLiquid色谱法-1D plus系统和Eksigent自动进样器的Thermo LTQ-Orbitrap XL质谱仪(San Jose,CA)中鉴定。按照Shevchenko等(Shevchenko等,Proc Natl Acad Sci USA93:14440-14445(1996);Shevchenko等,Anal Chem68:850-858(1996)所述,对斑点内的蛋白质进行凝胶内胰蛋白酶消化。将洗脱的肽加载至CVC Microtech(Fontana,CA)35mm长的100μm ID C18pre-Trap柱上,以5μl/分钟的流速用100%缓冲液A(含有0.1%甲酸的2%乙腈)洗涤10分钟。使用300nl/分钟的流速,在15cm New Objective ProteoPepIntegraFrit柱(Woburn,MA)上分离肽。使用下列洗脱梯度:0-15分钟0-30%缓冲液B(含有0.1%甲酸的98%乙腈),15-20分钟30-80%缓冲液B和20-22分钟80%缓冲液B。然后将柱用缓冲液A平衡13分钟。使用2300V的电喷雾电离电压、45V的毛细管电压、130V的镜筒透镜和200℃的毛细管温度,以正模式将洗脱的分析物喷雾至LTQ-Orbitrap MS。进行信息依赖性获取,其中使用60K分辨率FTMS扫描,选择300-1600的m/z范围内的6种最强离子,并使用35的归一化碰撞能的宽带碰撞诱导的解离其进行MS-MS和LTQ检测。排除进一步MS-MS达60秒钟时间的峰。
利用Matrix Science MASCOT Daemon搜索引擎(Boston,MA),以鲍氏不动杆菌菌株ATCC17978数据库(gib.genes.nig.ac.jp/single/blast2/main.php?spid=Abau_ATCC17978)为标准,对所得MS/MS光谱进行搜索。使用下列搜索参数:肽公差:±10ppm,MS/MS公差±0.3Da,最大未酶切位点(maximum missed cleavage):2,固定修饰:羧甲基(C)和可变修饰:脱酰胺(ND)和氧化(M)。在某一项内鉴定的蛋白质包括具有被排序为第1号和具有p<0.05的离子分值的最小两个独特肽的蛋白质。
如前所述(Luo等,J Infect Dis201:1718-1728(2010);Spellberg等,Infect Immun76:4574-4580(2008),在大肠杆菌(Escherichia coli)pQE-32表达系统(Qiagen)中产生带His标签的rOmpA(氨基酸2-347)。简言之,用以下引物从鲍氏不动杆菌17978基因组DNA扩增ompA:OmpA-F CATCACCATGGGATCCTTGTTGCTGCTCCATTAGCT和OmpA-R CTAATTAAGCTTGGCTGCAGTTATTGAGCTGCTGCAGGA并按照生产商的说明书(Clontech Laboratories),使用In-Fusion2.0Dry-Down PCR Cloning Kit,克隆至QE-32的BamHII和Pst I位点。按照生产商的说明书(Qiagen),使带6X-His标签的蛋白质经Ni-琼脂糖亲和柱纯化。使用Detoxin Gel Endotoxin Removing Columns(NorgenBiotek,Canada),从rOmpA中除去内毒素,并按照生产商的说明书,用Limulus Amebocyte Lysate endochrome(Charles River)测定内毒素水平。利用该程序,减少内毒素于<1EU/剂用于免疫接种。通过皮下注射磷酸缓冲盐水(PBS)中的含rOmpA的0.1%Al(OH)3(Alhydrogel,BrenntagBiosector,Frederikssund,Denmark)使小鼠免疫。在相同方案中,对照小鼠只接受佐剂。在感染前5周和在感染前2周再次使小鼠免疫。加强免疫后4天(感染前10天),如上所述使小鼠得糖尿病。
在振荡的同时,使鲍氏不动杆菌菌株在TSB肉汤培养基中于37℃生长过夜。在振荡的同时,使细菌在37℃下传代至对数生长中期。细胞用PBS洗涤两次,并以适当浓度重新悬浮用于感染。通过接种物的定量培养来证实最终浓度。用PBS中的亚致死(106)或致死(所达目标2x107)接种物通过尾静脉iv感染小鼠。所有动物实验皆经洛杉矶生物医学研究所(Los Angeles Biomedical Research Institute)的公共机构动物管理与使用委员会(Institutional Committee on the Use and Care of Animals)批准。
感染后2天(预期对照小鼠开始死亡当天),收获器官,在含1%triton与蛋白酶抑制剂混合物(Sigma-Aldrich Corp.St.Louis,MO,USA)的无菌PBS中匀浆。定量培养来自各个标记小鼠的匀浆器官以确定组织细菌负荷。
之前发表的ELISA测定法(Ibrahim等,Infect Immun74:3039-3041(2006);Ibrahim等,Infect Immun73:999-1005(2005);Spellberg等,JInfect Dis194:256-260(2006);Spellberg等,Infect Immun73:6191-6193(2005)经修改以检测针对鲍氏不动杆菌细胞膜制备物和rOmpA的抗体。简单地说,将ELISA板用100μl/孔5μg/ml的rOmpA或细胞膜制备物包被。包被的孔用牛血清白蛋白封闭,与小鼠血清一起温育,洗涤,并用与辣根过氧化物酶缀合的山羊抗小鼠第二抗体染色。再次洗涤孔,与邻苯二胺底物与H2O2一起温育。允许显色20分钟,之后通过加入等体积的3N HCl终止反应,在微量滴定板读数器中在490nm处测定光密度(OD)。阴性对照孔接收无关同种型对照单克隆抗体而非小鼠血清。ELISA效价以最后的血清稀释度的倒数记录,其OD读数≥(阴性对照样品的平均OD+(标准差*2))。
使鲍氏不动杆菌HUMC1在胰蛋白酶大豆肉汤(TSB)中于37℃培养过夜,传代至对数生长中期,漂洗,等分至96孔微量滴定板中。对于补体研究,将非免疫或免疫血清加入孔中1小时。在基线和再次在1小时时定量培养孔内容物。调理吞噬杀灭测定法基于对之前采用方法的修改[25-26]。测定了鼠RAW264.7巨噬细胞细胞(均来自美国典型培养物保藏中心(American Type Culture Collection),Rockville,MD),因为已知它们在分化后能够杀灭微生物[15-17]。将细胞在37℃、5%CO2下在含10%胎牛血清(FBS)、1%青霉素、链霉素和谷氨酰胺(Gemini BioProducts)和50μMβ-巯基乙醇(Sigma-Aldrich,St.Louis,MO)的RPMI1640(IrvineScientific,Santa Ana,CA)中培养。RAW274.7细胞通过暴露于100nMPMA(Sigma-Aldrich)中3天被激活。在用BD Falcon细胞刮棒(FischerScientific)刮擦后,收获活化RAW264.7巨噬细胞,以巨噬细胞与细菌20:1的比率加入微量滴定孔中。在轻轻振荡的同时温育1小时后,将各孔的等分样品定量接种于胰蛋白酶大豆琼脂(TSA)中。将共培养管的菌落形成单位(CFU)与只含有微生物而无巨噬细胞的生长对照管的CFU进行比较。%杀灭计算为1–(共培养孔的CFU/无巨噬细胞的生长对照孔的CFU)。
存活通过非参数Log Rank检验比较。适当时,对抗体效价、细菌负荷、MPO水平和细胞因子水平进行比较,对非配对比较用Wilcoxon RankSum检验,或对于配对比较用Wilcoxon Signed Rank检验。通过SpearmanRank检验确定相关性。采用Kyplot执行全部统计。如果p值<0.05,则差异视为显著。
作为用于疫苗开发的先导抗原候选物的基础,在自然感染后,测定对来自鲍氏不动杆菌的表面蛋白质的体液免疫应答。因为糖尿病是鲍氏不动杆菌感染采集和鲍氏不动杆菌感染的更坏结局的风险因子(Alsultan等,J Chemother21:290-295(2009);Furniss等,J Burn Care Rehabil26:405-408(2005);Metan等,Eur J Intern Med20:540-544(2009),因此糖尿病酮症酸中毒(DKA)毛霉菌病小鼠模型(Ibrahim等,J AntimicrobChemother58:1070-1073(2006);Ibrahim等,J Clin Invest117:2649-2657(2007);Spellberg等,Antimicrob Agents Chemother49:830-832(2005)经修改用于鲍氏不动杆菌感染的体内研究。通过尾静脉切开给各个标记的DKA小鼠采血,以测定基线免疫前抗鲍氏不动杆菌细胞膜蛋白质抗体效价。然后通过尾静脉用6个鲍氏不动杆菌临床分离株的可存活接种物使小鼠感染(表2和表5)。感染后2周,自小鼠中获得配对免疫血清。配对免疫前血清相对于免疫血清的ELISA证实,感染后2周时,感染所有菌株的小鼠在抗鲍氏不动杆菌细胞膜IgG-抗体效价方面形成显著增加(10-100培)(图1)。
在证实了对生物的特异性体液免疫应答后,寻找该应答的免疫显性抗原靶。来自用于感染小鼠的全部6个菌株的鲍氏不动杆菌细胞膜蛋白质制备物通过二维凝胶电泳分离,得自上述感染小鼠的配对免疫前血清和免疫血清通过蛋白质印迹染色。二维凝胶证实来自所有6个临床分离株的膜蛋白质通过大小和等电聚焦(IEF)有效分离(图2A)。在所有情况下,免疫后血清鉴定出不被免疫前血清识别的有限数目的独特斑点(图2B)。
通过代表不同菌株类型的3个不同鲍氏不动杆菌分离株的印迹间的MALDI-TOF分析,选择用于鉴定的相同的3个斑点(图2B)(表2)。存在于所有斑点中的蛋白质被鉴定为OmpA,已知其是鲍氏不动杆菌外细胞膜的主要成分(Choi等,Cell Microbiol10:309-319(2008)。在来自感染鲍氏不动杆菌的小鼠的配对免疫前血清相对于免疫血清中测定抗OmpA抗体效价。就总的抗鲍氏不动杆菌抗体而言,在感染鲍氏不动杆菌的所有小鼠中,抗rOmpA IgG效价提高(图3),证实了OmpA是感染后适应性体液免疫的靶标。
实施例II
作为疫苗抗原的OmpA
用于疫苗开发的理想抗原在临床分离株间应是保守的,且不应与人蛋白质组同源。对用于感染的6个临床分离株的OmpA基因进行了测序。在所有临床分离株间蛋白质序列具有99%同一性(图4),分离株相隔58年(1951-2009)自不同的临床来源(脑脊液、肺、血流、伤口)收获,包括卡巴培南耐药菌株和卡巴培南敏感菌株两者(表2和表5)。针对PubMed中的鲍氏不动杆菌的14个其它序列的比对显示所有序列间有89%同一性(表4)。使用ATCC17978OmpA序列对人蛋白质组进行PubMedBLAST搜索显示仅7个具有最小同源性的序列(E值范围为0.53-6.2)。因此OmpA在宽泛系列的鲍氏不动杆菌临床分离株间是保守的,但与人蛋白质共有最小同源性。
使rOmpA在大肠杆菌中表达,通过与His标签结合的镍-琼脂糖纯化。内毒素水平降低至小于1EU/疫苗剂量。在最初的实验中,用含rOmpA的0.1%氢氧化铝(Al(OH)3)给退役种(>6月龄)小鼠接种,并加强免疫。加强免疫后2周,通过尾静脉用鲍氏不动杆菌HUMC1感染DKA小鼠。与佐剂对照小鼠相比,接种小鼠的存活显著提高(图5A)。使用年幼小鼠,用多次疫苗剂量,重复实验。与佐剂对照小鼠相比,所有疫苗剂量均提高存活,发现用100μg的剂量,反应具有最大功效,其显著优于3μg剂量(图5B)。
为了测定接种对细菌负荷的影响,如上所述给年幼小鼠接种,使得糖尿病,并感染。感染后第2天(根据之前的实验预测对照小鼠死亡的日子),使小鼠安乐死,收获器官以测定组织细菌负荷。除肺以外,其细菌负荷具有不显著的(p=0.08)3倍减少,在所评价的所有器官中,接种减少组织细菌负荷约10倍(对于所有其它器官,接种小鼠相对于对照小鼠的细菌负荷p<0.01)(图5C)。
为了证实在第二动物模型中的功效,使用已建立的大鼠鲍氏不动杆菌肺炎模型(Russo等,Infect Immun76:3577-3586(2008);Russo等,JInfect Dis199:513-521(2009)。简单地说,Long-Evans大鼠(250-300g)用3.5%氟烷/100%氧气麻醉直到无意识,然后在3.5%氟烷下维持。经手术暴露出气管,将1-0丝的4英寸段移至气管下以利于接种物的滴注。将动物以仰卧位悬挂在60°斜度板上。通过1-ml注射器和26规格针,在气管内,将含细菌的PBS的经肺滴注(1.2ml/kg体重)引入,切口用手术缝合钉闭合。在24和48小时时收获肺,匀浆,定量培养以测定细菌负荷。该模型扼要重述了通过上气道的吸入,这是在重症监护室中在不需要免疫抑制的情况下鲍氏不动杆菌临床肺炎的普遍模式(Russo等,InfectImmun76:3577-3586(2008)。给大鼠接种,加强免疫,加强免疫后2周气管内受感染。在24和48小时时评价肺细菌负荷。(图5D)。
实施例III
疫苗介导保护中的抗体
对接种小鼠中抗体效价和存活间的关系进行了评价。假如在接种3μg的小鼠中观察到接近50%存活,则选择该剂量用于抗体-存活分析,使经历感染后存活或不存活的接种小鼠的混合成为可能。在2个独立的实验中,给小鼠接种3μg或仅佐剂,加强免疫,并测定感染前的抗体效价。接种引起抗rOmpA IgG抗体效价显著提高(对于接种小鼠相对于对照小鼠,中位值[范围]效价=204,800[102,400-409,600]相对于800[400-1,000],p<0.0001)。因为感染性接种物在这些实验中比之前的实验(2x107)略微较低(1.4x107和1.6x107),所以大于50%的接种小鼠存活,尽管使用3μg疫苗剂量(图6A)。当分析混合的接种和对照小鼠两种(p<0.0001,rho=0.6)或只分析无对照小鼠的接种小鼠时(p=0.0009,rho=0.6,通过Spearman Rank检验,图6B),抗体效价与存活相关。当分析接种小鼠和对照小鼠两者时,在将生还者与非生还者区分开的≥204,800的IgG效价阈值为最大精度的(98%),而当只分析接种小鼠时,≥102,400或204,800两者的效价具有相同的最大精度(85%)(表3)。
抗体效价与存活的关系表明,抗体是rOmpA疫苗效应物。使B细胞缺陷型小鼠感染鲍氏不动杆菌HUMC1以确定这些细胞类型的小鼠缺陷型是否易受感染,但无死亡发生,且小鼠从不出现临床疾病。此外,B细胞缺陷型小鼠对糖尿病诱导有抗性,使得B细胞缺陷型和野生型小鼠间的比较成问题。因此,与其破坏B淋巴细胞功能,不如给供体小鼠接种rOmpA或仅佐剂,并通过终末放血收获免疫血清或对照血清。免疫血清中的rOmpA效价高于对照血清(1:409,600与1:3200)。用0.5ml免疫血清或对照血清对DKA小鼠进行ip治疗,2小时后用鲍氏不动杆菌HUMC1感染。相对于用对照血清治疗的小鼠,用免疫血清治疗的小鼠的存活显著提高(图7A)。
为了确定抗体诱导保护的机制,在免疫血清与非免疫血清存在下培养鲍氏不动杆菌。在两种血清中培养1小时后,鲍氏不动杆菌数增加,排除了补体介导的杀灭作用作为保护的机制。然而,免疫血清不提高鲍氏不动杆菌的巨噬细胞调理吞噬性杀灭(图7B)。

实施例IV
疫苗剂量对免疫原性的影响
研究了疫苗剂量对rOmpA疫苗的免疫应答性质的影响。如上所述给小鼠接种。加强免疫后2周,收获血清和脾细胞。对于对照、3、30和100μg剂量接种的小鼠的抗体效价中位值[四分位数间距]为2,400[800-3,200]、51,200[51,200-102,400]、204,800[102,400-204,800]和204,800[89,600-512,000](对于所有接种剂量相对于对照p<0.001,对于30和100μg剂量两者相对于3μg剂量<0.05)(图8A)。
在响应30和100μg剂量时,IgM应答显著高于3μg剂量(两种较高剂量的效价中位值1:12,000相对于3μg剂量和佐剂对照小鼠的1:800,p<0.05)(图6B)。IgG1是所发现的主要Ig亚型,接种小鼠1:320,000-1:1,600,000的效价中位值相对于对照小鼠的1:400(对于所有小鼠,相对于对照p<0.05)。接种100μg的小鼠的IgG1效价比接种3μg的显著较高(p=0.02)。IgG2a和2b效价中位值比IgG1效价显著较低,但仍显著高于对照小鼠的效价(图8B)。IgG3效价更低得多,其中所有3个接种组的效价中位值为1:800,但仍显著高于对照小鼠(中位值1:200)。
与抗体应答类似,相对于来自对照小鼠的脾细胞,疫苗的所有剂量均介导通过脾细胞产生的IFNγ、IL-4和IL-17显著增加(图9A)。在最高(100μg)剂量的疫苗下,IL-4产生最大。与对照(未接种)脾细胞通过rOmpA刺激后的基线IFNγ占主导的IFNγ:IL-4比率相比,疫苗的所有剂量介导更均衡的比率(对于对照相对于3、30和100μg剂量,对照的比率中位值[四分位数间距]=对照的3.2[1.3-5.8]分别相对于1.0[0.8-1.3]、0.9[0.7-1.1]和0.5[0.5-0.7])。与所有其它组相比,100μg剂量的Th1:Th2比率显著较低(对于所有比较p<0.02)。
使用重叠肽确定T细胞和B细胞免疫显性表位。免疫显性T细胞表位定义为在所测的所有15聚体间引起高于第3四分位数的细胞因子应答的表位。在接种3μg的小鼠中,发现仅4、5和5种肽对于IFN-γ、IL-4和IL-17分别符合这个标准(图10)。发现截然不同的肽诱导来自脾细胞的3种细胞因子。感兴趣的是肽1是IFN-γ产生的最有效的诱导物,肽2,以5个氨基酸之差与肽1重叠,诱导实质上更多的IL-4。发现仅2种共有表位诱导脾细胞(自接种3μg(肽23和30)的小鼠收获)的所有3种细胞因子。
实施例V
抗OmpA多克隆免疫血清的表位作图
为了鉴定B细胞表位,使用含有重叠肽的免疫血清和膜进行了免疫点印迹法。简单地说,合成了偏移5个氨基酸的重叠12-聚体肽,在C端与Whatman50纤维素膜共价结合,用免疫血清直接探测。膜用第二抗小鼠IgG抗体复染,在T-TBS(含有0.05%Tween20的TBS)中洗涤4次,与1:3,000稀释的辣根过氧化物酶缀合的G蛋白(Bio-Rad,Hercules,Calif.)一起在封闭缓冲液中温育。膜用Amersham Pharmacia Biotech ECL试剂盒(Piscataway,N.J.)处理用于胶片显影(化学发光检测)。TIF图像用Bio-Rad Gel Doc2000成像系统生成,光密度测定法用来确定定量反应性。鉴定出许多特异性B细胞表位(图11)。发现仅3种肽表现B细胞和T细胞表位两者(2、16和23)。同源性建模显示,优势B细胞表位位于表面暴露的α螺旋和β片层上,但预料不到地,在发夹环结构处的蛋白质胞质面上还有显性B细胞表位(图12)。
通过经ExPASy网络服务器可进入的SWISS-模型全自动蛋白质结构同源性建模服务器,对rOmpA进行了计算机建模。应用DiscoveryStudio2.1版(Accelrys,San Diego,CA),通过能量最小化使模型优化。采用最陡下降法和共轭梯度算法,以几个步骤进行最小化,以达到最小收敛性(0.02kcal mol-1A-1)。表位对应残基是用不同色彩作标记的(图11):

a.现有技术序列与1951年(ATCC17978)和2009年(HUMC菌株)间收获 的6个临床分离株的比对
以53/350个以上氨基酸而不同的现有技术序列在序列开始处具有另外的28个氨基酸,其不出现在任何鲍氏不动杆菌OmpA序列中。因此,相对于用于感染小鼠的鲍氏不动杆菌的全部6个临床分离株,现有技术序列共以81个氨基酸(23%序列趋异性)之差而不同。最后,当与PubMedGenbank中的12个其它鲍氏不动杆菌分离株的序列比较时,现有技术序列保持趋异(现有技术序列与表4中的12个比对序列进行比较)。
b.已知B细胞和T细胞表位的序列相对于来自用于感染小鼠的 ATCC17978和HUMC菌株的鲍氏不动杆菌OmpA序列。

实施例VII
针对OmpA的单克隆抗体(MAb)有效治疗致死鲍氏不动杆菌血流感染
针对OmpA和通过鉴定可与鲍氏不动杆菌表面上的天然OmpA结合的前克隆(pre-clone)而选用于亚克隆的前克隆产生多种MAb。在通过ELISA选择和用于细胞表面染色的流式细胞术后,获得5个杂交瘤亚克隆,3个IgM和2个IgG。将杂交瘤上清液针对PBS透析。阴性对照是IgG同种型对照MAb。使用C3H/FeJ小鼠通过尾静脉感染鲍氏不动杆菌HUMC1,感染后几小时用50μg MAb进行ip治疗。4种MAb显著提高感染小鼠的存活,而1种MAb无益(IgM#1)(图15)。这些数据证实,MAb疗法针对这些感染是有效的,证实了针对鲍氏不动杆菌的被动接种和有关MAb的组合物的构思。
