Abstract
本发明属于恶性胸腔积液检测方法技术领域,具体涉及一种利用CD14+?CD163+细胞来鉴定恶性胸腔积液的方法。该方法包括提取单个核细胞、抗体标记、利用流式细胞仪进行免疫表型分析三个步骤。本方法与现有技术相比,具有简便、快速、灵敏度高、特异性高的优点;同时该方法可以减少人为主观因素的干扰,较为客观,能够针对潜在的恶性胸腔积液患者及早做出判断、鉴定,从而为后续的及时干预治疗提供便利。
Description
技术领域
本发明属于恶性胸腔积液检测方法技术领域,具体涉及一种利用CD14+CD163+细胞鉴定恶性胸腔积液的方法。
背景技术
胸腔积液是一种常见的临床表现。胸腔积液产生的原因很多,有由肿瘤细胞直接侵犯或血性扩散引起的胸膜转移而形成的恶性胸腔积液,也有由其他良性原因出现,如:肿瘤合并心衰,结核等引起的炎性胸腔积液。恶性胸腔积液是肿瘤病人的一个常见的临床表现,因此对胸腔积液良恶性质的判断与治疗和预后密切相关。
目前临床上诊断和鉴定胸腔积液的方法包括临床表现、胸腔穿刺的化验结果、胸膜活检、气管镜、外科活检等,其中最常用的方法是采用胸腔穿刺进行细胞学检查,如果在胸腔积液中发现肿瘤细胞即可确诊为恶性,同时其它方法可作为辅助性手段帮助确诊。但是由于胸腔积液中细胞成分复杂,利用细胞学检测法来鉴定胸腔积液是良恶性存在敏感性较低、主观性较强的弊端,容易造成漏诊或误诊,因此需要寻找新的更敏感可靠的检测指标。
现有研究一般认为,恶性胸腔积液是一种特殊的肿瘤微环境,除肿瘤细胞外,还含有大量的淋巴细胞、巨噬细胞以及成纤维细胞等。浸润在肿瘤组织中的巨噬细胞被称为肿瘤相关性巨噬细胞(Tumor-associatedmacrophages,TAMs)。既往研究认为TAMs具有重要的抗肿瘤免疫调节作用,可以直接杀伤肿瘤细胞,或者通过递呈肿瘤相关抗原诱导机体免疫应答清除肿瘤细胞。早期研究发现肿瘤组织浸润的TAMs释放多种细胞因子、生长因子、趋化因子,参与基质重建、肿瘤微血管、微淋巴管生成,发挥免疫抑制和逃逸等多种机制促进肿瘤的发展、浸润和转移,与生存密切相关。随着研究深入,学者们认识到TAMs在不同的肿瘤微环境刺激下发生不同性质的活化,主要包括2种类型:(1)经典激活型的巨噬细胞,又称M1型巨噬细胞,具有抑制和杀伤肿瘤细胞的作用;(2)诱导型活化型的巨噬细胞,又称M2型巨噬细胞,主要分泌抑炎性细胞因子如:Arginase-I、IL-10(Interleukin10,IL-10)、TGF-β(Transforminggrowthfactor-beta1),具有促进血管生成、有助于肿瘤增长及免疫抑制功能。
CD163原名血红蛋白清道夫受体,最早于1987年被Zwadlo在体外培养的单核细胞中发现,并被认为是一种主要与炎症反应有关的急性时相反应蛋白。目前的研究认为,CD163是一种仅在单核/巨噬细胞系统细胞膜上特异性表达的单链跨膜糖蛋白分子,为富含半胱氨酸的清道夫受体超家族(scavengerreceptorcysteine-richsuperfamily,SRCR-SF)中的一员,其分子量为130kDa。CD163的结构由胞外段、跨膜区和胞质内段三部分构成,其胞外段有两种变异型,其中一种含9个SRCR结构,另一种则仅含前3个SRCR结构。
CD163分子作为一种单核/巨噬细胞系表面受体,已有大量文献报道其是诱导型巨噬细胞标志,并且与多种肿瘤的不良预后相关。但尚未见到关于CD163分子与恶性胸腔积液的相关性及其在鉴定恶性胸腔积液应用的研究报道。
发明内容
本发明目的在于提供一种利用CD14+CD163+细胞来鉴定恶性胸腔积液的方法。
本发明采用的技术方案如下:
一种利用CD14+CD163+细胞鉴定恶性胸腔积液的方法,包括以下步骤:
(1)提取单个核细胞:采集100ml胸腔积液;1500~2000转/min,离心7~10min,优选1500转/min,离心10min,弃上清;PBS缓冲液冲洗一次,弃上清;将沉淀细胞用30mlPBS缓冲液重悬细胞后置于15ml淋巴细胞分离液上,进行密度梯度离心,收集单个核细胞并用PBS缓冲液冲洗三次;
所述PBS缓冲液为无钙离子、无镁离子的磷酸盐缓冲液;
所述PBS缓冲液冲洗为1500~2000转/min条件下,离心7~10min,优选1500转/min,离心10min;
所述密度梯度离心为2500转/min条件下,离心20~25min,优选离心25min;
(2)抗体标记:将步骤(1)获得的胸腔积液单个核细胞,取1×105~1×106个细胞,加入细胞表面荧光抗体进行染色,室温避光孵育15min;
所述细胞表面荧光抗体为PE标记抗人-CD163单克隆抗体、APC-Cy7标记抗人CD14单克隆抗体;
(3)利用流式细胞仪进行免疫表型分析,检测计算CD163分子在单核细胞上的表达水平。
所述利用CD14+CD163+细胞鉴定恶性胸腔积液的方法,用于鉴定由肺癌引起的恶性胸腔积液。
所述利用CD14+CD163+细胞鉴定恶性胸腔积液的方法,以CD163分子在单核细胞上的表达水平为3.65%作为诊断指数时,其灵敏度为86%,特异性为100%。
有关CD163+巨噬细胞(即CD163阳性M2型巨噬细胞)与肿瘤的研究已有较多报道,但是关于CD163+巨噬细胞在胸腔积液中的表达及在鉴定恶性胸腔积液中的用途尚未见有关报道。本发明所提供的利用CD14+CD163+细胞来鉴定恶性胸腔积液的方法,实际应用时,由于CD14+CD163+细胞在良恶性胸腔积液患者的外周血中均未检出,而当以CD163分子在单核细胞上的表达水平为3.65%作为诊断指数时,其灵敏度为86%,特异性为100%,因而该方法具有简便、快速、灵敏度高、特异性好的优点;同时可以减少人为主观因素的干扰,较为客观,能够针对潜在的恶性胸腔积液患者及早做出判断、鉴定,从而为后续的及时干预治疗提供便利。

具体实施方式
下面结合实施例对本发明做进一步的解释说明。
实施例
为检验本发明所提供的检测恶性胸腔积液的准确性,发明人做了如下的对比实验。
实验标本来源:
标本来自于郑州大学第一附属医院肿瘤科及呼吸科患者,共100例。其中采集肺癌患者恶性胸腔积液50例,采集炎症患者炎性胸腔积液50例。所有患者的胸腔积液均经病理科细胞学检测确诊,患者临床资料与分析的统计见下表。标本均征得患者同意后采集,并签订知情同意书。

相关实验仪器及耗材来源:
流式细胞仪FASCanto购自美国BD公司,PE标记抗人-CD163单克隆抗体、APC-Cy7标记抗人CD14单克隆抗体均购自美国BDPharmingen公司,淋巴细胞分离液购自上海华精生物高科技术有限公司。
检测方法及步骤:
1、提取单个核细胞
分别采集炎性胸腔积液与恶性胸腔积液患者各100ml胸腔积液,按以下步骤收集胸腔积液的单个核细胞:1500转/min离心10min,弃上清;PBS缓冲液冲洗一次,弃上清;将沉淀细胞用30mlPBS缓冲液重悬细胞后轻置于15ml淋巴细胞分离液上,进行密度梯度离心,收集单个核细胞并用PBS缓冲液冲洗三次;
所述PBS缓冲液为无钙离子、无镁离子的磷酸盐缓冲液;
所述PBS缓冲液冲洗为1500转/min条件下,离心10min;
所述密度梯度离心为2500转/min条件下,离心25min。
2.标记抗体及免疫表型分析
各取步骤1获得的炎性胸腔积液与恶性胸腔积液患者的1×106个单个核细胞,加入细胞表面荧光抗体进行染色,室温避光孵育15min;
所述细胞表面荧光抗体为PE标记抗人-CD163单克隆抗体、APC-Cy7标记抗人CD14单克隆抗体。
采用BDCantonII流式细胞分析仪进行免疫表型分析,检测计算CD163分子在单核细胞上的表达水平,统计结果如图2所示,图1为随机抽取的一例恶性胸腔积液和一例炎性胸腔积液的细胞流式结果。
3.ROC分析
将100例标本分为恶性胸腔积液与炎性胸腔积液两组,分别统计CD163分子在单核细胞上的表达水平,采用SPSS17.0软件分析CD163分子在单核细胞上的表达水平作为诊断指标的敏感度与特异性,统计结果如下表所示;同时绘制ROC曲线,如图3所示。

利用流式细胞仪进行免疫表型分析时,针对表面抗体染色标记的目的细胞,利用CD14抗体标记出目的细胞即巨噬细胞,针对CD14、CD163双表达阳性细胞分析其中CD163分子在单核细胞表达水平。利用SPSS软件统计表明,在恶性胸腔积液中CD163分子在单核细胞表达比例是(均数±标准差)19.08%±13.96%,而在炎性胸腔积液中其比例是0.918%±0.936%。图1为随机抽取的其中一例恶性胸腔积液和一例炎性胸腔积液的细胞流式结果,从其对比可以看出,在恶性胸腔积液中,CD14+CD163+细胞(CD14、CD163双表达阳性细胞)表达比例高达53.2%,而在炎性胸腔积液中,CD14+CD163+细胞(CD14、CD163双表达阳性细胞)表达比例仅有0.2%。进一步进行独立样本t-检验,如图2所示,结果显示CD163分子在单核细胞表达水平在恶性胸腔积液与炎性胸腔积液中具有极显著差异。此结果提示CD163分子在单核细胞表达水平可作为鉴别良、恶性胸腔积液的一个新指标。
如图3所示,通过对ROC曲线分析,利用SPSS软件计算ROC曲线下面积为0.941,与0.5相比具有统计学意义(P<0.001)。当选取CD163分子在单核细胞表达水平为3.65%作为诊断指数时,其敏感度为86%,特异性为100%。这些结果提示CD163分子可作为一种灵敏度好、特异性高的检测指标,具有潜在的临床应用价值。
综上所述,我们认为,恶性胸腔积液中单核/巨噬细胞特异性高表达CD163分子,在炎性胸腔积液中几乎不表达,通过ROC曲线的进一步分析,当选用CD163分子在单核细胞表达水平为3.65%时,检测方法的灵敏度为86%,特异性为100%,因此本发明所提供的利用CD14+CD163+细胞鉴定恶性胸腔积液的方法具有较高的灵敏度、较好的特异性,具有潜在的临床应用价值。
Claims (2)
1.CD163抗体在制备胸腔积液性质判定试剂中的应用,其特征在于,通过检测胸腔积液中CD14+CD163+双表达阳性细胞分析CD163分子在单核细胞上的表达水平来判定胸腔积液的性质,具体包括以下步骤:
(1)提取单个核细胞:采集100mL胸腔积液;1500~2000转/min,离心7~10min,弃上清;PBS缓冲液冲洗一次,弃上清;将沉淀细胞用30mLPBS缓冲液重悬细胞后置于15mL淋巴细胞分离液上,进行密度梯度离心,收集单个核细胞并用PBS缓冲液冲洗三次;
所述PBS缓冲液为无钙离子、无镁离子的磷酸盐缓冲液;
所述PBS缓冲液冲洗为1500~2000转/min条件下,离心7~10min;
所述密度梯度离心为2500转/min条件下,离心20~25min;
(2)抗体标记:将步骤(1)获得的胸腔积液单个核细胞,取1×105~1×106个细胞,加入细胞表面荧光抗体进行染色,室温避光孵育15min;
所述细胞表面荧光抗体为PE标记抗人-CD163单克隆抗体、APC-Cy7标记抗人CD14单克隆抗体;
(3)利用流式细胞仪进行免疫表型分析,检测计算CD163分子在单核细胞上的表达水平;
以CD163分子在单核细胞上的表达水平为3.65%作为诊断指数。
2.如权利要求1所述CD163抗体在制备胸腔积液性质判定试剂中的应用,其特征在于,用于鉴定由肺癌引起的恶性胸腔积液。
