Abstract

本发明涉及一种医用试剂的配方,为使AVL983钾钠氯分析仪在国内应用于临床而对原A标准液试剂配方作出改进。先按原配方配制混合溶液,再按每升混合液加入1.0m mol/l NaCl 17.0ml,1.0m mol/l KCl 0.25ml,0.5m mol/l CaCl2 2.5ml。经用原配方和改进配方的试剂分别对采样血清标本和标准定值血清作比较试验,原试剂测定的K+、Na+、Cl-系统性偏高,而改进配方试剂测定的值均接近真值。有满意的临床应用价值。

Description

一种钾钠氯分析仪的A标准试剂
本发明涉及一种医用试剂的配方,更确切地说是涉及AVL983钾钠氯分析仪试剂的配方。
用火焰光度计法测试人体新鲜不溶血、无乳靡血清中钾、纳、氯离子K+、Na+、Cl-定值,存在操作繁琐、测定结果不准确,及用煤气点火不安全等弊端,因而已趋淘汰。
一种先进的测量方法是离子选择电极法,如根据这一方法设计制造的AVL983钾纳氯分析仪(瑞士产),目前正为国内引进使用。该分析仪要使用三种溶液作试剂,包括A标准液、B标准液和参比电极液。其中A标准液的配方是:
NaCl     110mmol/l   KCl              15mmol/l,
CH3COONa 40mmol/l  TEA(防腐剂乙二醇)0.2ml/l。
在使用该配方标准液试剂的过程中,发现测定结果的K+、Na+、Cl-定值均呈不同程度的系统性偏高,显然该配方试剂不适合中国国情,致使分析仪不能用于临床。
本发明的目的是对A标准液配方作出改进,使采用改进配方A标准准液试剂的分析仪可用于临床。
本发明的AVL983钾纳氯分析仪改进A标准液试剂的配方是,先按原A标准液试剂配方配制混合溶液,用弱酸调节PH值为7.0,然后按每升混合溶液加入下述成份:
1.0毫摩尔/升的  NaCl     17.0毫升;
1.0毫摩尔/升的  KCl       0.25毫升;
0.5毫摩尔/升的  CaCl2    2.5毫升。
上述溶液充分混合后,在4℃环境中保存,在室温环境下可保存一周。
本发明在原A标准液试剂中通过加入NaCl、KCl和CaCl2,增加了K+、Na+、Cl-的浓度,从而改变了离子选择电极测量K+、Na+、CL-的斜率,使测量结果接近真值。所加入的NaCl、KCl、CaCl2溶液中,均含有Cl-,很显然改进配方后的A标准液试剂与原配方A标准液试剂相比,有明显的差异,但改进配方后的A标准液试剂经试验能获得满意的测定结果。
随机抽取当日上午病人(或人)的新鲜不溶血、无乳糜血清73份,随机抽取当日上午健康成人的新鲜不溶血、无乳靡血清28份,共计101例标本,分别采用原配方A标准溶液试剂和改进配方的A标准液试剂进行K+、Na+、Cl-测定;由于进口定值血清不含电解质,而检验学界普遍公认以上海产定值血清为最佳,因此试验时以上海产定值血清作标准(批号900456),使用前一小时溶解,分别采用原配方A标准液试剂和改进配方的A标准液试剂进行K+、Na+、Cl-测定,测定的平均结果见下表所示。

上述结果表明,采用原配方A标准液试剂,无论病人、健康人组血清标本还是标准上海定值血清,测得的K+、Na+、Cl-结果偏高,除K+外与改进配方的A标准液试剂测定结果比较,P值都小于0.05,存在显著性差异;在K+、Na+、Cl-三种电解质测定中,用原配方A标准液试剂时,最明显偏高的是Cl-,P值小于0.01,存在极显著的差异;上海产定值血清的定值为K+ 4.92mmol/l、Na+ 140.3mmol/l、Cl- 99mmol/l,从重复10次的均值可看出,应用原配方A标准液试剂与改进配方A标准液试剂比较,三种电解值P值都小于0.05,存在显著性差异。
改进配方的A标准液试剂临床应用近两年,结果全部符合临床,在参加北京市临床生化室间质控活动中均获得优秀,结果满意,值得推广应用。

Claims (2)

1、一种钾钠氯分析仪的A标准液试剂,按下述配方制备混合溶液,
NaCl:    110mmol/l,KCl:15mmol/l,
CH3COONa:40mmol/l,TEA:0.2ml/l,其特征在于还在每升混合溶液中加入下述成份:
1.0mmol/l    NaCl    17.0ml,
1.0mmol/l    KCl     0.25ml,
0.5mmol/l    CaCl2    2.5ml。
2、根据权利要求1所述的钾钠氯分析仪的A标准液试剂,其特征在于所述混合溶液用弱酸调正PH值为7.0。
