Abstract

Description
技术领域
本发明涉及用于校正来自对象的医学成像扫描的图像数据的方法和装置,其中已将规定数量的成像物质引入到该对象中,特别是用于校正PET成像中的剂量浸润(dose infiltration)。
背景技术
在医学成像领域,已知各种成像方案。例如,PET(Positron EmissionTomography,正电子发射断层成像)是一种利用在体内所注射的放射性的(radio-active)物质对对象进行3D成像的方法,通常生成表示一个或多个生理功能的图像。其他这样的功能成像模态也是已知的,例如SPECT。
在这样的功能图像中,很多重要的病理和解剖结构表现出很高(或低)的亮度。例如,FDG-PET图像中的肿瘤通常表现为亮区。
当PET放射性示踪剂被注射入患者时,在放射示踪剂还没有全部注射入静脉时就会发生剂量浸润。图1中所示的最大密度投影(100)阐明了剂量浸润在注射部位(102)的情形。浸润能够被视为注射部位周围区域(104)中高度增加的吸收。在这种情形下,浸润部位的最大SUV超过4000。
这对于临床工作流程的潜在影响如下:
1.由于DICOM中编码的重新调节的斜率和截距(rescale slope andintercept,RSI),其中存在浸润部位的轴向切片承受减小的强度粒度,除了影响这些切片的临床可读性的其他图像伪迹之外。
2.由于浸润剂量不能环绕身体,对于整个患者而言,标准化吸收值(Standardised Uptake Value,SUV)不再可靠,因此只有一部分注射剂量可用于通过组织的吸收。
关于前面列出的潜在的临床影响的第一个,如果在患者离开成像设备前检测到了剂量浸润,受浸润部位影响的床体位置能够随着浸润部位移动到视野(FOV)之外而重新扫描。
然而,重新扫描受影响的床体位置无法解决不可靠SUV的第二个问题。这仅可通过在以后的时间重复整个成像过程、使患者暴露在来自放射示踪剂的额外剂量和用于衰减校正的CT扫描中来解决。
剂量浸润还影响其他成像模态(例如MR,或超声),其中将成像物质引入对象中,这里,并没有通过注射或插入而将所有的成像物质正确地引入,从而成像物质的初始数量减少了一个未知的部分。
发明内容
本发明的目的在于,解决这些问题并提供对已知装置和方法的改进。

优选地,成像物质是药物示踪剂。更优选地,变量为示踪剂的活度。而更优选地,确定步骤包括测量该区域的SUV值。
相称地,该方法还包括确定所识别区域内的平均活度浓度以及体积,以确定所识别区域内的总活度。

本发明提供了一种通过评估包含某成像物质(无意引入)的区域以及将它与所引入的物质的初始量相比较,而用于校正对象中某部位上该成像物质无意引入的方法。
本发明的实施例具有诸如以下的特征:
-自动检测注射部位的非流通放射示踪剂(浸润)并校正放射示踪剂吸收的注射剂量标准化测量
-基于异常高活度浓度的存在来检测剂量浸润的存在
-利用等高线描绘的子体积来分类和测量在浸润部位所捕获的活度
-从(衰减校正的)总注射活度中减去所测得的捕获活度,用于校正放射示踪剂吸收的注射剂量标准化测量的重新计算
-利用解剖信息对剂量浸润的检测进行精炼,例如,配准到解剖图集上以限定为搜索臂部。
所提出的本发明的实施例允许该系统提供校正后的SUV,其解释了浸润部位所捕获的的非流通示踪剂。在该实施例中,其参照图2进行如下过程:
1.加载患者数据(202),并且使用者在图像回放软件中手动识别并描绘的浸润部位。替代地,可由系统通过检测(204)SUV值超过给定阈值(例如500)的任意体素、然后利用等高线(例如在SUV最大值的某个固定百分比)分割(206)周围区域来自动识别浸润部位。
2.计算描绘的浸润部位的总活度(208)(即,平均活度浓度(Bq.ml-1)×体积(ml))。
3.然后从用于计算SUV的衰减校正的注射剂量值中减去(210)浸润活度:
这里,C是在感兴趣体素或区域内测得的活度浓度(Bq.ml-1),D是在衰减校正后至图像开始时间的注射剂量(Bq),I是所测得的浸润活度(Bq),并且W是患者体重(g)。
图2中的第二幅流程图给出了实例,其基于图1中获得和阐明的对象图像数据记录。加载的患者数据(212)返回4062的最大体素SUV值(214),其不仅指示了部位102的异常,而且指示了部位的位置。
所分割的等高线具有该部位周围的530个体素(216),并且之后计算并减掉总活度以找到校正剂量(218,220)。
当使用自动寻找浸润部位的方法时,值得注意的是,到图像数据中浸润很清晰时(数据集中的时间点),已经正确地进入静脉的注射的示踪剂已经从注射部位穿行了很长距离进入对象,并且因此该区域任何残留的活度都应当是基于剂量浸润。必要时能够执行对任何本底活度的校正。
浸润部位的自动检测可利用解剖图集进行精炼,例如,通过将搜索限制到诸如臂部的可能注射部位。
如果浸润活度非常高,成像系统会低估所测得的真实数值(由于饱和/停滞时间等)。在这样的情形下,如果可用则可以采用对此的额外校正。
在图像采集点可检测浸润部位(通过与特定区域对应的正弦图集中异常高的数值)。然后可通过在用于SUV计算的DICOM数据头中提供校正的注射剂量在该点进行自动校正(如上所述)。
此外,存储在重建图像体素中浸润部位处的活度可减小至一个不那么极端的数值,从而改善RSI编码后那些图像切片中强度值的粒度。
在本发明的其他实施例中,对于成像物质异常引入的评估可应用到使用成像物质的其他成像模态中,诸如MR或超声。例如,在MR的情形下,造影剂的使用可能受到药剂浸润或渗漏到计划外区域的影响。以与前述方法类似的方式,可获得受影响的对象的成像数据的区域,并识别区域中药剂的数量,并从初始引入量中减去。例如,可以直接或与背景进行对比地测量选定区域内的额外对比度,并且这与使用标准量药剂期望的对比度相对比,并且这用于计算受影响区域内药剂的量。
参见图3,本发明的上述实施例可容易地实现以作为适于用指令编程而执行根据本发明的方法的步骤的计算机系统。
例如,中央处理单元304能够接收代表通过端口305进行的医学扫描的数据,端口305可以是用于移动数据存储介质(例如CD-ROM)的阅读器;与诸如医用扫描器的装置的直接连接(未示出)或连接至网络。例如,在一个实施例中,处理器执行如下步骤:识别包含所引入的成像物质的异常部分的图像数据区域;为识别的区域确定与成像物质相关联的图像数据中变量的区域值;使用该区域值来确定该区域中物质的部分;以及从成像物质的规定数量中减去该部分。

为所识别的区域确定与成像物质相关联的图像数据中变量的区域值;
使用该区域值来确定该区域中物质的部分;以及
从成像物质的规定数量中减去该部分。
