本发明的促血管生成组合物为新的细胞治疗策略开创了途径。
在下面的说明书中,在方括号[]之间的参考数字是指本文结尾处所列的参考文献。
现有技术
关于存在循环系统内皮祖细胞(EPC)及其用于刺激血管生成和血管修复的潜力的证实,是血管生物学领域的重要进步。
这些细胞可存在于一般的循环系统中,其既可以是自发的,也可以是对于各种刺激(促炎性细胞因子,生长因子,缺血,他汀类药物(statins)等等)的响应。这些参与血管修复机理的细胞,为缺血性心血管疾病的治疗开创了一种先进的治疗途径。
已公开了细胞标记物/特异性配体型的促血管生成系统,其包括内皮祖细胞,其中包含Eph细胞标记物,特别是EphB4或EphB1[1]。所述系统中,为了刺激血管生成,包含Eph标记物的内皮祖细胞由属于肝配蛋白(ephrin)家族的特异性配体活化。内皮祖细胞可从来自骨髓、外周血液或脐带血液的单核细胞获得。分化后,骨髓或脐带的单核细胞提供相对大量的内皮祖细胞,其表达或包含Eph标记物。外周血液中,循环EPC只占单核细胞很小的百分比。然而,经若干步骤(通过细胞计量术预选和分选、离体培养、分化和成熟以及膨胀)获得内皮祖细胞却需要复杂和漫长的操作;因此其费用高并且产率低。
另外,已知心血管疾病的风险因子可通过内皮功能异常诱导血管和心血管障碍,所述功能异常可能导致参与组织修复的EPC数量和功能的下降[2]。循环EPC的功能异常和低于正常值的量,已在具有心血管疾病风险因子的某些个体中观察到,所述心血管疾病例如高胆固醇血症、糖尿病或高血压[3]。
因此确实需要提供可刺激血管生成的新的组合物,所述刺激至少具有与各种来源的EPC相同的功效,或甚至具有与血管生成因子例如VEGF相同的强度,并且这些新组合物包含容易获得的细胞,所述细胞不必然要求:
-通过细胞计量术预选和分选的步骤、离体培养的步骤、分化和/或成熟的步骤
或
-作为与用于刺激血管生成的其他细胞的混合物。
还实际需要提供新的组合物,其能够刺激血管生成功能,该组合物包含数量保持稳定的细胞。
另外,实际需要新的促血管生成组合物,其使用简单、可再现且便宜。
发明内容
本发明的目的正是满足了这些需要,这通过提供包含源自外周血液的单核细胞(MNC)和能够使所述外周血液单核细胞(MNC)活化的配体L的促血管生成组合物而实现,其中循环血液单核细胞(MNC)包含能够接受配体L以使所述细胞活化的受体。
由配体L激活后,循环血液MNC获得促血管生成性质,其相当于或大于源自骨髓或脐带血液的EPC的作用。
对于本发明的目的,术语“循环血液单核细胞”是指达到成熟状态并分化为单核细胞和淋巴细胞的单核细胞。
根据本发明的术语“外周血液”等同于术语“循环血液”。
相比于EPC,所述单核细胞更直接地和更容易获得,因为其从循环血液的分离并不需要通过细胞计量术的预选和分选的步骤、离体培养的步骤、分化和/或成熟的步骤。因此这不但促进了本发明的促血管生成组合物的使用,还可降低所述组合物的生产成本。
在病理症状需要紧急治疗而不能拖延的情况下,本发明的组合物可迅速获得,这也成为其重要优势。
本发明的组合物避免了异源性血液细胞输入所带来的问题,并且构成一种有希望的方法。事实上,本发明的组合物适用于自体输血。
迄今为止,从未考虑到循环血液单核细胞(MNC)可直接活化以刺激促血管生成活性。另外,现有技术仅教导了基于内皮祖细胞的促血管生成组合物,其为骨髓单核细胞分化的结果。
本发明的组合物中,单核细胞(MNC)可包含能够接受配体L以使所述细胞活化的受体。
本发明的组合物的形式可为不同性质和来源的细胞和分子的粗混合物,且其性质可改变,或其数量可增加,以使MNC的量足以用于所述组合物预期的应用中。存在于所用组合物和/或细胞批料(或混合物)中的单核细胞的量可为例如106至1012。
本发明的组合物还可为“次粗品(less crude)”形式,其中通过本领域技术人员已知的纯化方法移除了某些细胞和/或分子。这些方法不得对所述细胞(MNC)有害。相比于粗混合物的细胞(MNC)的浓度,所述“次粗品”组合物通常含有更高浓度的能够被刺激的细胞(MNC)。存在于所用组合物和/或细胞批料(或混合物)中的细胞的量可为例如106至1011个单核细胞。
本发明的组合物还可包含脐带血液或骨髓,或两者的混合物。
更具体地,循环血液单核细胞(MNC)可源自哺乳动物的外周血液。
术语“哺乳动物”表示任何哺乳动物。例如可提及驯养的动物,例如狗和猫;饲养动物,例如猪、牛、绵羊和山羊;动物,例如小鼠和大鼠;灵长类,例如猴和猩猩;和人。
在本发明的上下文中,“刺激”促血管生成活性是指引起体内和体外促血管生成性质的增加和改善。
在本发明的上下文中,使得循环血液单核细胞(MNC)“活化”的术语,是指开始对分化细胞(MNC)的部分产生细胞响应。
如前文所述,本发明的组合物包含循环血液单核细胞(MNC),该细胞包含能够接受配体L以使所述细胞活化的受体。即使在过量存在的情况下,所述配体L也仅仅结合至能够接受它们的受体,而不是结合至所有可存在于循环血液单核细胞(MNC)上的受体。
循环血液单核细胞(MNC)因此包含能够通过非循环膜配体(例如肝配蛋白)刺激的受体。
这些受体可例如为选自EphA和EphB的跨膜受体酪氨酸激酶。
所述配体L可为肝配蛋白家族的配体。肝配蛋白可分为两个亚家族,即A型配体通过糖基化磷酯酰肌醇(GPI)简单地结合至质膜,B型配体是跨膜的且具有胞质内域。
在本发明的一个实施方式中,所述配体L为具有肝配蛋白-B活性的分子,更具体地为具有肝配蛋白-B2活性的分子。所述配体L可为肝配蛋白-B2。
所述配体L可任选与稳定化和/或结合分子缔合(associated)。在本发明的上下文中,术语“缔合的”包涵了不同强度和性质的任何类型的结合:即,共价、离子、极性、范德华氏和疏水结合。所述稳定化和/或结合分子可稳定化和/或保护所述配体。其可结合所述配体但不能结合所述受体。
本发明的组合物中,所述稳定化和/或结合分子可为例如免疫球蛋白的Fc片段。该Fc片段是免疫球蛋白分子的肽键断裂的结果,其不具有抗原-结合位点。因此其不具有抗体活性。所述Fc片段(或Fc域)是由两个重链的恒定区形成的免疫球蛋白分子的部分。该Fc域给予了免疫球蛋白其性质。Fc片段可由免疫球蛋白获得,例如通过木瓜蛋白酶、胃蛋白酶或其他任何适当的酶的蛋白水解作用。
本发明的另一主题是促血管生成物质,其包含通过配体L活化的循环血液单核细胞(MNC)以刺激血管生成,所述配体L任选与稳定化和/或结合分子缔合。
在本发明的一个实施方式中,该促血管生成物质包含由肝配蛋白-B2分子作为配体所刺激的循环血液单核细胞(MNC),所述配体L任选与稳定化和/或结合分子缔合。在该实施方式中,所述循环血液单核细胞(MNC)包含选自EphA和EphB的受体,其能够由肝配蛋白-B2分子刺激。
本发明还涉及制备本发明的促血管生成组合物的方法,其包括以下步骤:
(i)获得外周血液单核细胞(MNC)并放在适当的介质中;
(ii)步骤(i)的单核细胞与配体L接触,所述配体任选与稳定化和/或结合分子缔合。
根据本发明的术语“外周血液”等同于术语“循环血液”。单核细胞源自外周血液(PBMNC)。
此外,脐带血液或骨髓或两者的混合物,可加入该步骤的包含单核细胞的外周血液中。
可通过本领域技术人员已知的任何适当的方法从外周血液分离MNC,所述方法例如差速离心,或通过流式细胞计量术分选。
步骤(i)的适当的介质可为主要含水的生理液体。适当的介质可包含至少80%,有利地85%和甚至更有利地至少90%的水。
除水之外,所述适当的介质可包含一种或多种选自以下的要素:
-无机溶质,例如痕量元素和离子,尤其是Na+,Cl-,K+,PO4 3-,Ca 2+,SO4 2-;
-有机溶质,例如脂质,碳水化合物,氨基酸,尿素,尿酸,胆红素,激素;
-血浆蛋白,例如白蛋白,免疫球蛋白,纤维蛋白原,脂蛋白等等。
所述适当的介质更具体地为血浆。
特定情况下,所述血浆可为自体血浆。
将该循环血液单核细胞(MNC)与肝配蛋白家族的配体接触。
所述配体L有利地为具有肝配蛋白-B活性的分子,更有利地为具有肝配蛋白-B2活性的分子。步骤(ii)中所述的配体L可为肝配蛋白-B2。
所述配体L可任选与前述稳定化/结合分子缔合。
所用单核细胞(MNC)包含能够由肝配蛋白家族的配体刺激的受体。这些受体可为例如选自EphA和EphB的跨膜受体酪氨酸激酶。
所述配体L可任选与前述稳定化/结合分子缔合。该稳定化/结合分子有利地为免疫球蛋白Fc片段。
步骤(ii)中,循环血液单核细胞(MNC)和所述配体L接触的时间可为5分钟至6小时,例如20分钟至4小时,例如30分钟至1小时。
步骤(ii)中的接触可在20至50°C,例如25至40°C,例如37°C的温度进行。
步骤(ii)中的接触可在促进所述配体L结合结合至循环血液细胞(MNC)受体的气氛中进行,例如CO2和氧气的百分比分别为2%至10%和98%至90%,其中CO2和氧气的百分比之和在所有情况下等于100%。更具体地,所选气氛由5%的CO2和95%的氧气组成。
在本发明的方法中,还可在步骤(ii)之后进行洗涤步骤。可用如前所述的适当的介质来进行所述洗涤。
本发明还涉及可根据本发明的方法获得的促血管生成组合物,所述组合物包含循环血液单核细胞(MNC)和能够活化所述循环血液单核细胞(MNC)以刺激促血管生成活性的配体L;
所述配体L任选与稳定化和/或结合分子缔合。
本发明的促血管生成组合物可直接使用或以药物组合物的形式用作药物。
本发明的促血管生成物质根据还可直接使用或以含有其的药物组合物的形式用作药物。
因此,本发明的一个方面可为本发明的促血管生成组合物或促血管生成物质,其用作药物,特别是用于治疗和再生受损的血管组织,特别是在心脏、脑或者外周水平;或用于疗法中,特别是用于预防或治疗任何引起缺血并发症的病理症状,所述并发症例如糖尿病,糖尿病性神经病,动脉粥样硬化,心肌梗塞,中风,下肢动脉病,衰老,高脂血症,高胆固醇血症,肥胖或高血压。
根据其另一方面,本发明涉及包含本发明的促血管生成组合物或促血管生成物质,和任选一种或多种药学上可接受的赋形剂的药物组合物。
本发明的药物组合物可通过已知方法制备。
所述赋形剂通常是无治疗活性的物质,其形成药物的组合物的一部分,或者其用于该组合物的制备。赋形剂的功能是改善外观、保证储存和促进药物的形成和给药。其作用还有传递活性成分至其作用位点并控制其被机体吸收。取决于所需的药物形式和给药模式,可使用各种赋形剂。本领域技术人员能够根据所需的药物形式和给药模式选择合适的赋形剂[6]。
直接给药或包括在药物组合物中的促血管生成组合物或促血管生成物质的量为治疗有效量。术语“有效量”或“治疗有效量”是指产生所需预防或治疗效果的促血管生成物质或药物组合物中的促血管生成组合物的量。所述有效量可通过标准药物操作的手段以实验的方式确定。
为了获得所需治疗效果,活化细胞所需的活性成分(例如肝配蛋白B2-Fc)的剂量的范围可为10至200微克/ml,且所用的批料中活化的细胞的数量为每次治疗108至1011个细胞。尽管这些剂量是平均情况的实例,但也可能存在特殊的情况,其中更高或更低的剂量是合适的。所述剂量也是本发明的一部分。根据通常的实践,对于每位患者的合适的剂量由医师根据给药模式以及患者的体重和响应来确定。
给药模式可有利地为非经肠。非经肠给药可包括例如静脉内,肌内,皮下,腹膜内,动脉内,关节内,池内(intra-cisternal),皮内,病变内,眼内,胸膜内,鞘内,子宫内和心室内给药。待治疗的治疗性适应症以及组合物的生理化学和生物学性质使其能够确定给药途径以及最合适的制剂。各种制剂和给药系统公开于现有技术中[5]。
本发明的促血管生成组合物或促血管生成物质,可用于制备预期用于治疗和再生受损的血管组织的药物,特别是在心脏、脑或者外周水平。其还可用于制备预期用于预防或治疗任何引起缺血并发症的病理症状的药物,所述并发症例如糖尿病,糖尿病性神经病,动脉粥样硬化,心肌梗塞,中风,下肢动脉病,衰老,高脂血症,高胆固醇血症,肥胖或高血压。
根据其一个方面,本发明涉及本发明的促血管生成组合物或促血管生成物质,其用于治疗和再生受损的血管组织,特别是在心脏、脑或者外周水平。

本发明还涉及具有肝配蛋白-B2活性的分子作为能够结合至单核细胞(MNC)的受体以触发其活化的配体的用途。
本发明的促血管生成组合物可用于哺乳动物、特别是人的细胞治疗。
根据其另一方面,本发明还涉及预防或治疗上述病理症状的方法,其包括给药促血管生成组合物或包含其的药物组合物,或给药促血管生成物质或包含其的药物组合物。
第一种方法可在于:
-从循环血液和/或骨髓分离单核细胞(MNC);
-通过将配体L结合至在MNC上存在的它们的特异性受体而体外活化MNC,以改善它们的促血管生成性质;和
-向血液或直接向缺血组织中引入由此活化的细胞(MNC)。
该方法使得可显著地提高活化细胞(MNC)的数量。MNC的体外活化还可用于例如接种人工血管。
第二种方法可在于:
-直接使用含有细胞(MNC)以及任选含有其他细胞和细胞因子的整体的血液和/或骨髓样品;
-将配体L添加至所述样品;
-直接引入整个混合物而不分离(fractionation)细胞。
在人中,该方法可描述为自体方法并且使得可进行自体输血,因而使得可能例如消除免疫学相容性和病毒安全性的所有问题。

实施例
方法和材料
根据法国的动物实验中的动物保护立法和其他法律或法规,以及根据涉及实验室动物的看护和使用的欧洲委员会指令No.07430,进行实验。该研究方案经法国农业部的动物保护和健康部以及环境保护部批准。
实施例1:本发明的促血管生成组合物的制备
MNC分离
将30至40ml的人外周血液新鲜收集于肝素涂覆的管(Vacuette 455051,Greiner Bio-One GmbH公司销售)中。单核细胞(MNC)从外周血液通过Ficoll密度梯度离心分离。在静脉(iv)注射细胞(MNC)于动物中2小时之前,将血液样品用磷酸盐缓冲盐水或PBS(14040,Gibco公司销售)稀释2倍。30至35ml的稀释的血液样品置于10ml的预先制备的Ficoll带(1.077g/ml;P04-60500,Pan Biotech公司销售)上成为一层。在环境温度(大约20°C)以700×g(或重力加速度的700倍)离心30分钟。从血浆-Ficoll界面除去细胞(MNC)的白色层。用50ml的M199(31153,Gibco公司销售)洗涤两次后,将单核细胞计数,并再混悬于含有2%胎牛血清或FBS的M199培养基中,浓度为3×106细胞/ml。
MNC刺激
对于所述刺激,在含有5%的CO2和95%的氧气的气氛下在37°C用15μg的肝配蛋白-B2(496-EB,R&D SYSTEMS)培养1ml的含有2%胎牛血清或FBS的M199培养基中的3×106单核细胞(MNC)30分钟。然后用10ml的含有2%FBS的M199培养基洗涤所刺激的细胞(MNC)两次以除去游离的肝配蛋白-B2。
MNC注射
所洗涤的MNC以103、104和105个细胞/100μl的PBS的浓度重新混悬,然后在无菌条件下结扎股动脉5小时后注射入所实验的小鼠尾静脉中。
实施例2:本发明的组合物的促血管生成活性
无胸腺裸小鼠的准备
饲养雄性无胸腺成年裸小鼠(6至7周龄,Harlan)至少7天,自由饮食、饮水,然后结扎右侧股动脉。将小鼠保持在倒转的12小时夜/昼循环中:早7时至晚7时将小鼠置于黑暗中,在晚7时到早7时将小鼠置于光线下,处于环境温度(22℃)。所有操作均在无病原、无微生物的条件下进行。将小鼠置于一次性塑料笼中,其在上部包含过滤器。事先用UV照射该笼、上部滤器部分、水瓶、进食托盘(Innovive)和饲料(Harlan)。使用前用高压灭菌锅处理其所饮用的水。
右侧股动脉的结扎
在无菌条件下进行手术操作以创造出后腿的单侧缺血。通过吸入异氟烷(1.5至2.0%)麻醉小鼠。腿部退避反射和眨眼反射在操作之前完全抑制。通过切开右腿的覆盖皮肤中心部分而暴露股动脉。使用显微镜,刚好在旋股外侧(circomflexa femoris lateralis)起始点上方处,从静脉分开并分离股动脉。覆盖的皮肤随后用外科缝合器缝合。
腿部皮肤灌注评价
经由肝配蛋白-B2-刺激的细胞(MNC)的腿部皮肤血流的评价使得能够评价刺激的细胞(MNC)改善缺血肢体灌注的能力
在结扎后第14天,通过腹膜内(ip)注射苯巴比妥(10mg/kg)麻醉动物。然后将小鼠放在热毯上,直肠温度保持在36.5至37°C以使体温对腿部皮肤灌注的影响最小。然后通过灌注成像多普勒激光仪(MLDI 5078,MoonInstruments公司销售)进行评价。每只动物的皮肤灌注结果表示为缺血的右腿与不缺血的左腿之比。
动脉密度测定
测定经由肝配蛋白-B2-刺激的细胞(MNC)的血管密度使得能够评价所述刺激的细胞诱导原有血管网络重塑(血管重塑)的能力。
评价腿部皮肤灌注之后,血管密度通过高清晰度微血管造影法(2200,Kodak Dental Systems,France销售)确定。进行纵向剖腹术,向腹主动脉插入聚乙烯(PE-10)导管,并通过其注射大约150μl的造影产品(1.67g/ml的硫酸钡在盐水中的溶液)(Fluka-11845,Sigma,France公司销售)。然后建立后腿的微血管造影法,并通过数字X-射线传感器获得图像(每只动物2至3个图像)。每只后腿的动脉密度通过在动脉所占据的定量区域中每张图像的像素百分比估算,其使用常规定量图像分析。每只动物的血管密度结果表示为右腿(缺血)与左腿(不缺血)之比。
为进行免疫组织化学分析,收集了所有动物的一对肱三头肌,将其嵌入Tissue-Tek O.C.T基质(Finetek Europe公司销售)并迅速使用液氮中预先冷却的异戊醇(isopentyl)冷冻。
毛细血管密度定量
经由肝配蛋白-B2-刺激的细胞(MNC)的毛细血管密度定量,使得能够评价所述刺激的细胞诱导血管系统从毛细血管网络发展(血管发生和/或血管生成)的能力。
为评价毛细血管密度,使用低温恒温器(Leica公司销售)切开各个冷冻的肌肉样品。切片厚度为7μm。切片在三个不同水平的样品下进行,间隔距离大约300μm。切片用缀合至异凝集素B4的异氰酸荧光素或FITC(100μl/片,10μg/ml;L2895,Sigma公司销售)在环境温度(20°C)培养一小时。
使用配有适当的滤光片的荧光显微镜(Observer ZI,Zeiss公司销售),对各个缺血或非缺血肌肉样品得到六张图像(每个水平两张图像×三个水平)。使用HistoLab软件(版本7.0,Microvision Instrument公司销售)以定量毛细血管密度。对于各个动物的结果表示为缺血的后腿与不缺血的后腿的毛细血管密度之比。
统计学分析
腿部皮肤灌注、动脉密度和毛细血管密度结果表示为用PBS(磷酸盐缓冲盐水)处理的对照组的平均百分比±SEM(平均值的标准差)。各实验组之间的对比,通过单向方差分析(ANOVA),和随后的Fischer’s PLSD法(Fischer’s保护的最小显著差异法)的补充分析而进行。若P值低于0.05的显著性阈值(P<0.05),则被认为是显著的。

在缺血后第14天,相比于PBS,源自糖尿病患者静脉血液的EFNB2刺激的细胞(MNC)(细胞浓度为103,104和105)显著地改变了腿部皮肤灌注(对于浓度为103、104和105个细胞的EFNB2刺激的MNC,分别为PBS组的97±7%,132±14%和163±10%,如图1)。
血管造影结果
在缺血后第14天,相比于PBS,源自糖尿病患者的EFNB2刺激的MNC(细胞浓度为103,104和105)显著地提高了血管造影评分(对于浓度为103、104和105个细胞的EFNB2刺激的MNC,分别为PBS组的142±19%,157±19%和189±17%,如图2)
毛细血管密度
在结扎后第14天,浓度为103、104和105的EFNB2刺激的MNC细胞的静脉内递送,显著地增加了毛细血管密度(对于浓度为103、104和105个细胞的EFNB2刺激的MNC,分别为PBS组的163±14%,156±11%和171±23%,如图3)。
总之,肝配蛋白-B2-Fc活化的糖尿病患者外周血液单核细胞具有有益的促血管生成的作用,其从功能的观点(皮肤血流测量)和结构的观点(动脉微血管造影和毛细血管密度测量)来看作用非常显著。

Claims (11)
1.促血管生成组合物,其通过包括以下步骤的方法获得:
(i)获得外周血液单核细胞(MNC)并放在适当的介质中;
(ii)通过将步骤(i)的外周血液单核细胞(MNC)与肝配蛋白-B接触以使所述外周血液单核细胞活化,以直接刺激该外周血液单核细胞的促血管生成活性,而没有任何分化和/或成熟的离体培养步骤,所述肝配蛋白-B与稳定化和/或结合分子缔合。
2.权利要求1的组合物,其还包含脐带血液或骨髓,或两者的混合物。
3.权利要求1或2的组合物,其中所述肝配蛋白-B为肝配蛋白-B2。
4.权利要求1或2的组合物,其中所述稳定化和/或结合分子为免疫球蛋白Fc片段。
5.制备促血管生成组合物的方法,其包括以下步骤:
(i)获得外周血液单核细胞(MNC)并放在适当的介质中;
(ii)通过将步骤(i)的外周血液单核细胞(MNC)与肝配蛋白-B接触以使所述外周血液单核细胞活化,以直接刺激该外周血液单核细胞的促血管生成活性,而没有任何分化和/或成熟的离体培养步骤,所述肝配蛋白-B与稳定化和/或结合分子缔合。
6.权利要求5的方法,其中将脐带血液或骨髓或该两者的混合物加入步骤(i)的包含单核细胞的外周血液中。
7.权利要求5或6的方法,其中步骤(i)中所述适当的介质为血浆。
8.权利要求5或6的方法,其中所述肝配蛋白-B为肝配蛋白-B2。
9.药物,其包含权利要求1至4中任一项的促血管生成组合物。
10.药物组合物,其包含权利要求1的促血管生成组合物,以及任选一种或多种药学上可接受的赋形剂。
11.权利要求1所述的促血管生成组合物在制备用于预防或治疗任何引起缺血型并发症的病理症状的药物中的用途。
