背景技术
在环境中,腐殖物质在植物和动物遗体的腐败期间形成(MacCarthy et al.,1985)。在以水中的溶解度作为pH值的函数的基础上,这些物质可以分为腐殖酸、富里酸和胡敏素。其中富里酸是在所有pH值条件下都易溶于水的那部分物质,并且与腐殖酸相比,通常具有较低的分子大小和分子量,以及较低的色度。

发明内容
根据本发明的一个方面,本发明提供一种包含富里酸、盐、酯或其衍生物以及一种或多种抗生素的组合物,该抗生素源自青霉素类和氨基糖苷类。

富里酸、盐、酯或其衍生物可以具有从酸性到碱性的任何pH值,pH值通常为从酸性到中性。富里酸可以以缓冲至合适pH值的溶液的形式存在。优选地,富里酸以酸的形式或以盐如钾盐的形式存在。

所述疾病或症状可以是细菌感染,优选该细菌为抗生素耐药菌,更优选该细菌为多重耐药菌,如MRSA(多重耐药金黄色葡萄球菌)。
所述细菌对一种或多种源自青霉素类和氨基糖苷类的抗生素具有耐药性。优选地,该抗生素选自苯唑西林和庆大霉素,以及其组合。
所述服用可以为口服、外用或任何其它合适的给药形式。
受体可以是动物或者人。
服用或使用本发明组合物可以比单独使用或服用富里酸、盐、酯或其衍生物,或抗生素更有效。
本发明的一种形式中,已经发现该细菌暴露于所述组合物多达约5天并不对富里酸、盐、酯或其衍生物与苯唑西林的组合物产生耐药性,更优选暴露于所述组合物多达约10天,甚至更优选暴露于所述组合物多达约20天。
已经发现使用富里酸、盐、酯或其衍生物能提高两种抗生素的抗生素特性,特别是苯唑西林和庆大霉素,或其任意组合物。

具体实施方式
耐药性,特别是多重耐药性(MRA)已经成为治疗各种疾病和症状特别是由细菌剂引起的疾病和症状的主要问题。因此需要对这些抗药菌株有效的治疗策略。
进行了两项研究以评估单独的富里酸或与源自青霉素类和氨基糖苷类的抗生素一起的富里酸对特定微生物,特别是细菌,更特别是一些耐抗生素细菌的抗菌特性。富里酸是WO2007/125492中描述的富里酸且由WO2007/125492中描述的方法生产,下文将其称为CHD-FA。
第一项研究中,评估了单独的富里酸或与苯唑西林或庆大霉素一起的富里酸的抗菌效果。
第二项研究评估了经过一段时间后,对有抗生素富里酸或无抗生素富里酸的细菌耐药性的产生。
以下举例仅限于示例的目的,且不以任何形式对本发明进行限制。
例1:-富里酸的抗菌特性
进行研究以测定富里酸的抗菌特性。用抑制微生物生长的径向区域法和肉汤系列稀释法测定这种产品的抗菌活性。结果显示富里酸具有对粪肠球菌(ATCC 51575)、沙门氏菌(ATCC 700565)金黄色葡萄球菌(ATCC 12600)、多重耐药金黄色葡萄球菌(MRSA)(ATCC)(P3938)、大肠杆菌(ATCC1173)和绿脓杆菌(ATCC 6027)的抗菌活性。CHD-FA的抗菌谱显示了与庆大霉素和苯唑西林(甲氧西林)的协同作用(图1~3)。在富里酸的存在下白色念珠菌的革兰氏染色生长显示圆形扩大的非分裂体,其表明这种产品在微生物细胞壁上起作用(图4)。
方法
测试的样品
所使用的样品为从碳水化合物衍生的富里酸(CHD-FA)。在pH值为2.14下,该CHD-FA为含有3.5%富里酸的棕色液体。富里酸中铝、汞、镉、铬和铅元素的含量低于20ppm且通过WO2007/125492中描述的方法生产。
使用的微生物菌株
使用以下菌株:
大肠杆菌(ATCC 1173)、绿脓杆菌(ATCC 6027)、金黄色葡萄球菌(ATCC 12600)、最新的分离凝固酶阴性的金黄色葡萄球菌、表皮葡萄球菌(最新分离)及大肠杆菌(ATCC 1173)。与目前规定的抗生素一起用于对比研究的金黄色葡萄球菌为标记22225的最新临床菌株(耐红霉素)和两种耐甲氧西林的金黄色葡萄球菌(MRSA)菌株2606(耐氨苄青霉素、苯唑西林(甲氧西林),对庆大霉素轻微敏感)和P3938(MRSA菌株;耐红霉素、苯唑西林(甲氧西林)、克林霉素,对富里酸轻微敏感)。对两种白色念珠菌(ATCC 10231和ATCC 9027)和五种不同的我们标记为42、43、44、U1和U7的最新分离的白色念珠菌也进行了测试。
通过径向扩散抑制法测定的抗菌活性
通过基本的扩散法药敏试验(Kirby-Bauer法)测定由CHD-FA抑制微生物生长的径向区域。简言之,将用于定量分析的50μL纯CHD-FA添加到直径4.5mm的孔中的4mm深米勒-海顿(Mueller Hinton)琼脂(Mast Group Ltd,Merseyside,UK)中,并在琼脂上接种相关微生物的菌苔,然后在37℃下培养16小时。在生理盐水中通过将50μL标准微生物悬浮液接种到琼脂板的表面以产生所述菌苔。在带有520光密度过滤器的比色计(Sherwood,254)上读数为0.07时得到这种悬浮液,该悬浮液使每ml单元形成35×106的菌落。在彼此成直角的情况下测量抑制区域尺寸两次并记录平均值。
在如常规实验室抗菌谱测定中所使用的那样,将抗生素施用到带有抗生素涂层片(Mast Group Ltd,Merseyside,UK)的琼脂板上。
通过系列肉汤稀释法测定抗菌活性
通过在生理盐水中两倍系列稀释0.5ml的CHD-FA,并随后加入0.5ml双倍强度的Mueller Hinton肉汤(Mast Group Ltd,Merseyside,UK)来测定CHD-FA对于不同微生物的最低致死浓度(MLC)。如上所述,用25μL的标准微生物悬浮液接种该肉汤并培养16小时。当将最后管的内容物滴在Mueller Hinton琼脂上并培养16小时时,最后管的内容物显示没有生长,则端点被评估为稀释最后管的内容物。包括合适的阳性和阴性对照。
协同评价
在Mueller Hinton琼脂板上相互间以不同的距离放置含有CHD-FA的套管和抗生素片,如上所述,在该琼脂板上接种相关微生物;抑制方式与Lorian(1991)描述的抑制方式相比较。
以上所有的研究进行三次,结果取平均值。
白色念珠菌的形态研究
用带有与显微镜目镜连接的数码相机(微粒图像正版本2.0ML)的卫永(Reichert Jung)显微镜拍摄显微照片。该照片经过相同程度的显微镜和软件处理。
结果
CHD-FA与各种微生物的协同活性
图1显示在金黄色葡萄球菌(ATCC 12600)的菌苔上衍生富里酸的碳水化合物和苯唑西林(甲氧西林)间的协同活性。图2显示MRSA菌株(P3938)的菌苔上CHD-FA和苯唑西林(甲氧西林)间的协同活性,图3显示绿脓杆菌(ATCC 9027)的菌苔上富里酸和庆大霉素间的协同活性。
白色念珠菌上的CHD-FA的形态效果
图4b显示由CHD-FA抑制的区域内得到的白色念珠菌的革兰氏染色,其可以与图4a中没有经过CHD-FA的抗菌作用的白色念珠菌的革兰氏染色相比较。
讨论
通过径向扩散抑制法和系列双倍稀释肉汤法得到的结果显示CHD-FA为广谱抗菌剂,包括革兰氏阳性球菌、革兰氏阴性菌和酵母菌。目前的常规规定的抗菌剂没有这种抑制谱。也发现CHD-FA有效抵抗三种最近分离的金黄色葡萄球菌菌株,其中两种为MRSA。三种最近分离的金黄色葡萄球菌菌株可累积耐受红霉素、氨苄青霉素、苯唑西林(甲氧西林)、克林霉素以及庆大霉素且对CHD-FA轻微敏感。
富里酸与苯唑西林(甲氧西林)和庆大霉素起协同作用,因此可能通过允许抗生素通过微生物的细胞壁上的非特定弱化作用向微生物较好渗透的方式来增加其抗菌效果,如图4所示。
例2-CHD-FA的抗药性研究
CHD-FA也称为富里酸的物理性能
例2中的CHD-FA重组为4%的溶液。所述溶液在室温下避光保存。该报道中的所有实验用2008年12月从南非直接收到的第三瓶来完成。
4%的CHD-FA溶液是黄色/棕色的具有强烈气味的微粘溶液,在25℃的pH值为1.9。在该研究中,所述CHD-FA用10M的氢氧化钠调整以得到具有缓冲pH值为3、5或7的4%CHD-FA原液。
方法
细菌性分离菌
对细菌性分离菌,耐甲氧西林金黄色葡萄球菌,以及EMRSA 16进行药敏试验。在珠子上的EMRSA 16菌株从-80℃下的长期贮藏中恢复并在MuellerHinton琼脂(Oxoid)上37℃生长24小时。
培养基
根据制造商的指示,所有实验都采用重组Mueller Hinton培养基(Oxoid)的肉汤或琼脂配方进行。
接种物的制备
a)在测试前,将在Mueller Hinton琼脂上的EMRSA 16菌株在环境空气中37℃培养24小时。
b)每一菌株的接种物通过从培养板中挑选分离的菌落并将它们悬浮在2ml Mueller Hinton肉汤中来制备。然后将浊度调整为麦氏(McFarland)标准0.5。
c)所述接种物通过在旋涡混合器上剧烈振动15s完全重悬浮。
d)然后在Mueller Hinton肉汤中以1∶100的比例稀释调整接种物用于MIC测试。
实验条件
采用96孔平底一次性无菌塑料微量滴定板。
步骤1加入CHD-FA
CHD-FA原液含有4%的天然化合物。针对每一测试菌株,2~12每孔加入100μL培养基。然后在第1列的孔中加入200μL 4%的CHD-FA。然后从第1列的孔中取出100μL的量,用多通道移液器(±2%的变动系数)转移至第2列稀释2倍。然后从第2列的孔中100μL样品并转移到第3列,依此类推一直到第10列。舍弃最后100μL的药物。第11列是不含有CHD-FA的阳性对照,第12列是只含有稀释剂的阴性对照。
步骤2加入苯唑西林
向8mg苯唑西林中加入5ml无菌蒸馏水制得1600mg/L苯唑西林原液。然后制备1∶100的原液稀释液,随后按1∶2的系列稀释制备16~0.06mg/L范围的苯唑西林稀释液。然后将50μL稀释的苯唑西林加入稀释的CHD-FA中在适当情况下制得最终浓度为4~0.015mg/L的苯唑西林。
步骤3加入EMRSA 16菌株
在合适的孔中加入100μL或50μL体积的Muller Hinton肉汤中的稀释接种物悬浮液,分别用于CHD-FA或CHD-FA+苯唑西林效能测试。这产生含有200μL最终体积的孔(由100μL稀释的CHD-FA或稀释剂以及100μL在用于菌株的合适肉汤中的接种物或单独的肉汤中的接种物组成)。
步骤4培养板
将所有板在通风避光的培养器中37℃培养48小时。
步骤5读取板
用抑制50%接种物的药物空白对照的最低药物浓度作为端点肉眼读取板。
步骤6传代耐药性
为了确定CHD-FA+/-苯唑西林对EMRSA 16的抑菌效果上的传代耐药性的效果,将100μL在MIC(最低抑菌浓度)板上的最后孔的生长物接种到半块Mueller Hinton琼脂板上并在37℃培养24小时。然后将该板的生长物用作下一个传代的新悬浮液。总共重复10次传代,CHD-FA+/-苯唑西林对EMRSA16的功效在每次传代后测定。
结果
抗CHD-FA+/-苯唑西林的MICs
MICs表明CHD-FA+/-苯唑西林的功效可稳定多达10代。CHD-FA+/-苯唑西林的MIC值详列于表1中。

在pH 7.0时CHD-FA的端点MICs
端点MICs表明CHD-FA(pH 7.0)+/-苯唑西林的功效可稳定多达10代,例如,与开始的MIC测量值(第1代)相比,每代之间的MIC结果不超过1孔的差别。CHD-FA(pH 7.0)+/-苯唑西林的端点MIC值详列于图7中。
结论:
·无论是在缓冲pH值为3.0、5.0或7.0下测试,CHD-FA在体外抗EMRSA16都是有效的。
·无论是在缓冲pH值为3.0、5.0或7.0下测试,CHD-FA加苯唑西林在体外抗EMRSA 16都是有效的。
·在适当的亚抑菌水平下的CDH-FA对EMRSA 16的连续传代对MIC没有影响。
·在适当的亚抑菌水平下的CDH-FA加苯唑西林对EMRSA 16的连续传代对组合物的MIC没有影响。
·在连续传代中,暴露于所述组合物大于20天后,抗EMRSA 16的CHD-FA的MIC是稳定的。
·目前还没有观察到CHD-FA单药或CHD-FA与苯唑西林的组合物对MRSA 16产生耐药性。

Claims (9)
1.一种用于治疗或抑制动物或人细菌感染的方法的组合物,由富里酸或其盐,以及一种或多种选自苯唑西林和庆大霉素,以及其组合的抗生素组成。
2.根据权利要求1所述的组合物,其中所述富里酸或其盐具有从酸性到中性的pH值。
3.根据权利要求1所述的组合物,其中所述富里酸为衍生自碳水化合物的富里酸。
4.根据权利要求1所述的组合物,其被配制成药物剂型。
5.根据权利要求4所述的组合物,其中所述药物剂型为液体、片剂、胶囊、软膏、凝胶等。
6.一种根据权利要求1所述的组合物在药剂的制造中的用途,所述药剂用于治疗或抑制受体的金黄色葡萄球菌或绿脓杆菌感染的方法,该方法包括将所述药剂给受体服用,所述受体包括动物或人。
7.根据权利要求6所述的用途,其中所述细菌为包括多重耐药菌的抗生素耐药菌。
8.根据权利要求6所述的用途,其中所述服用为口服或外用。
9.根据权利要求6所述的用途,其中所述抗生素为苯唑西林并且所述细菌暴露于所述药剂多达约20天并不对所述药剂产生耐药性。
