背景技术

下面将参考附图介绍本发明的某些示例实施例。通常,本发明涉及一种用于将低承载假体10固定在患者的骨上的外科手术方法或过程。更具体地说,本发明的优选实施例涉及一种用于将低承载假体10固定在患者的脊柱S中的一个或多个椎骨本体上的外科手术方法或过程,该方法或过程用于最小化或基本防止疤痕组织粘附在周围脉管V上。低承载假体10还可以提供在修正外科手术(如果需要)中的解剖平面。本发明还涉及钉或驱动螺钉(本文中统称为钉)50以及用于将钉50驱动到患者的椎骨本体内、以便将低承载假体10固定在患者的脊柱S上的相关仪器100、100′、300、400的各种示例性实施例。钉50设置成通过捶击类型或推动作用被冲击驱动到患者的脊柱S内(与旋转类型的作用相反)。
如后面更详细所述,当本发明优选的钉50、仪器100、100′、300、400和外科手术方法或过程用于将低承载假体10固定在患者的脊柱上时,本领域普通技术人员通常知道,该钉50、仪器100、100′、300、400和外科手术方法或过程同样可用于其它外科手术过程(在这些其它外科手术过程中,外科医生希望将假体10固定在骨上,包括但不局限于:用于创伤外科手术、颅颌面外科手术、整形和重建外科手术等。本发明的优选实施例也可以用于将较大承载的假体(例如骨板)固定在骨上。
在使用时,如后面更详细所述,优选地,低承载假体10保护外科手术部位防止疤痕组织发展,该疤痕组织可能在外科手术过程后粘附在患者的周围脉管V上。也就是,低承载假体10优选地用作在外科手术部位和一个或多个患者脉管V之间的屏障,以便最小化和/或防止疤痕组织粘附在患者的脉管V上。这样,低承载假体10可以为正方形、矩形、圆形等形状。
此外,低承载假体10可以在修正外科手术过程中引导外科医生通过疤痕充满组织。低承载假体10便于识别外科手术平面和/或环绕关键脉管V的安全导航通路。低承载假体10优选地操作成产生一个或多个穿过疤痕充满区域的平面,这样,外科医生可以在随后的修正外科手术过程中沿假体10进行引导。因此,低承载假体10在例如前椎骨外科手术之后用作脉管V的屏障,以便在修正外科手术过程中通过提供解剖平面而减小脉管V可能受损的危险。
通常,低承载假体10可以是柔性织物、屏障或隔膜、薄金属板、扁平板形假体、用作遮盖物或骨架的柔软植入件(以便在骨、椎骨本体和周围脉管V之间进行保护和起作用)、用于固定较小骨段的假体、设计成优化组织向内生长的骨架型假体、用于防止组织向内生长的粘接屏障型假体、用于使得较小骨段、较小腱和/或软组织保持就位的假体等。优选地,如共同待审的国际专利申请No.PCT/US08/88444(申请日为2008年12月29日,标题为“A METHOD OF FORMING ANDTHE RESULTING MEMBRANE COMPOSITION FOR SURGICALSITE PRESERVATION”,该文献整个被本文参引)中所述和如图2中大致所示,低承载假体10是涂覆水凝胶的网。优选地,低承载假体10包括第一端12、第二端14和中间部分16。假体10还可以包括一个或多个不透射线的指示器18和/或一个或多个预制的折叠线20。
如美国专利申请No.11/219966(标题为“Methods and Apparatusfor Vascular Protection in Spinal Surgery”,该文献整个被本文参引)中所述,已经开发了用于进入前脊柱的多种外科手术过程。该过程允许外科医生在脊柱的多个部分进行修补和矫正外科手术,例如修补脊柱S的运动段。例如通向脊柱S的前腰区域中的部位的普通外科手术方法必然形成穿过患者筋膜和穿过或环绕一个或多个肌肉平面的进入切口。露出受伤的脊柱部位也会使得患者的脉管V(该脉管V直接布置在例如脊柱S的腰区域的前部)运动。例如,除去退化的盘和用熔合骨架结构(cage)或假体来代替需要使得脉管V运动,以便暴露椎间盘空间。然后,脉管V能够在脊柱处理过程完成后返回它们的初始位置。根据外科手术部位的解剖学位置,疤痕组织可以粘附在周围脉管V上。
通常,手术后疤痕组织粘附在患者脊柱S周围的脉管V上,且疤痕组织遮挡了脉管V以及其它关键的解剖学标志。由于不能通过视觉来识别解剖学标志,形成的疤痕组织通常在修正外科手术过程中产生了几乎盲目导航的区域。因此,形成的疤痕组织可能在修正外科手术过程中成为问题。当前面脊柱的任何外科手术需要很仔细地识别关键解剖学标志作为初步的努力时,前部的修正外科手术需要引导(通常是盲目地)通过不同程度的强韧的疤痕组织。识别脉管结构和其它关键标志以便安全开始修正外科手术引起了伤害敏感结构(特别是脉管解剖结构)的明显的危险。
因为修正外科手术很可能需要一定数目的患者接受前脊柱外科手术,因此,本领域需要新颖的方法和装置用于在外科手术过程中特别是对脊柱S的修正前部手术中保护脉管结构。
参考图1A-1H,根据本发明的优选外科手术方法或过程,低承载假体10在封闭进入切口之前通过一个或多个钉50固定在患者的脊柱S上。钉50大致在图1A-1H中表示,而钉50的多种优选实施例在图3-9B中表示,并用多个基本符号(′),以便区别不同实施例。在普通的外科手术过程中,外科医生通常在患者的脊柱修补后开始封闭过程。不过,根据本发明,外科医生将首先通过一个或多个钉50来植入低承载假体10,这样,一旦植入,该低承载假体10用于至少部分防止疤痕组织粘附在患者的脉管V上和/或在随后的外科手术(如果需要)中帮助外科医生至相同的运动段。
如图1A中所示,假体10布置在外科手术修补部位的至少一部分上面。然后,如图1B和1C所示,假体10的第一端12通过一个或多个冲击驱动钉50而钉在患者的脊柱S的至少一个椎骨本体上,如后面更详细所述。更优选地,第一钉50a用于将假体10附接在第一椎骨本体上,而第二钉50b用于将假体10附接在第二相邻的椎骨本体上。然后,假体10在假体10的中间部分16处分别通过第三和第四钉50c、50d而钉在第一和第二椎骨本体上,如图1D和1E中所示。参考图1F和1G,假体10优选地沿一个或多个折叠线20至少局部折叠在它自身上一次或多次。这样,外科医生可以根据患者的解剖结构而确定在第一和第二端12、14中间的一个或多个折叠位置。也可选择或者另外,假体10可以包括一个或多个可见的、预先形成的折叠线20,该折叠线20向外科医生建议在何处折叠假体10,以便适应合适的植入位置。
假体10的第二端14优选地离开患者的脊柱S延伸,优选地与患者的、沿离开患者的脊柱S朝着进入切口的方向间隔开的结构连接。例如,假体10的第二端14可以例如通过缝合、夹子等而与后直肌鞘、腰肌等连接。
一旦植入,假体10初期布置,直到进行修正处理过程。在修正外科手术中,假体10使得外科医生能够降低与对初始外科手术部位的疤痕相关引导错误相关联的可能的并发症。
优选地,假体10还包括一个或多个不透射线的指示器18,这些指示器18优选地能够在修正外科手术之前和/或过程中使用机器来读出和/或定位一个或多个指示器18,用于定位和重新进入在先的外科手术部位。优选地,一个或多个不透射线的指示器18布置成靠近假体10的第二端14,这样,外科医生可以在切开处理之前识别第二端14的位置。
不管机器是否使用不透射线的指示器18,重新进入的过程优选地包括利用在先的进入切口、先前的筋膜切口以及假体10的第二端14的位置。接着,外科医生优选地沿假体10向下剖开,其识别敏感解剖学结构、外科手术平面以及环绕关键脉管V和穿过疤痕充满区域的安全引导通路。
当重新进入该部位时,假体10可以沿前部方向被拉动,这样,假体10可以以手风琴的方式展开,并露出该部位。因此,外科医生可以仔细打开假体10或将假体10从脊柱S拉出,以便露出该部位。然后可以进行脉管V退回、取出假体10和在脊柱S上进行修正外科手术。在完成对脊柱S的修正外科手术之后,可以利用上述技术植入新的假体10。
钉的示例实施例
总的来说,参考图3-9B,钉50优选地设置成通过捶击类型的运动而冲击驱动到患者的骨内,与旋转成与患者的骨接合不同。优选地,钉50可以通过一个或多个冲击类型的驱动动作来传送。钉50优选地包括头部部分54和杆部分52。头部部分54可以包括驱动机构56,用于便于在需要时通过取出仪器(未示出)从患者的骨中取出钉50。杆部分52可以包括尖锐的远侧尖端53,并有相对较小的外径,以便于在并不预先钻孔的情况下将钉50驱动到患者的脊柱S内。
在使用时,本发明的钉50优选地包括能够将钉50冲击驱动到患者的骨内的两种设计原理中的一种。第一设计原理是钉50包括从钉50的外表面伸出的外部凸缘58,这样,当钉50被驱动到患者的骨内时该钉50局部旋转(例如类似于螺旋叶片)。第二设计原理是钉50可以包括一个或多个倒钩60,该倒钩60可以沿一个方向被冲击驱动,但是阻止沿相反方向的运动。倒钩60可以以多种方式环绕钉50在钉50的长度上形成图形。
优选地,钉50的直径为大约三又二分之一毫米(3.5mm)或更小。更优选地,钉50的外径为大约一又十分之二毫米(1.2mm)至大约一又十分之八毫米(1.8mm)。当钉50包括凸缘58时,该凸缘58优选地高度为大约十分之一毫米(0.1mm)至大约二分之一毫米(0.5mm),且长度为大约3毫米(3mm)至大约16毫米(16mm)。不过,钉50并不局限于上述优选尺寸,并可以有优选用于特定患者和/或处理过程的几乎任意尺寸和形状。
参考图3,优选地,用于本发明的钉50′的第一优选实施例成螺旋钉50′的形式。螺旋钉50′包括头部部分54′和杆部分52′。头部部分54′包括驱动机构56′,用于方便在需要时通过取出仪器(未示出)从患者的骨中取出螺旋钉50′。杆部分52′包括尖锐的远侧尖端53′,并有较小的外径,以便于在并不预先钻孔的情况下将螺旋钉50′冲击驱动到患者的椎骨本体内。螺旋钉50′优选地还包括从钉50′的外表面伸出的外部凸缘58′。该外部凸缘58′优选地具有较大的凸缘节距,这使得螺旋钉50′能够通过直接轴向冲击(例如通过捶击运动)而被驱动到椎骨本体内。不过,凸缘节距设置成使得螺旋钉50′在它被冲击驱动到患者的骨内时旋转。螺旋钉50′可以包含1至7个凸缘转,尽管可以设想任意其它的转数,只要螺旋钉50′能够旋转,以便当螺旋钉50′被冲击驱动到患者的骨内时限制骨的裂开。优选地,螺旋钉50′包括凸缘节距,该凸缘节距使得螺旋钉50′能够旋转大约30度(30°)至大约360度(360°)。
参考图4,优选地,用于本发明的钉50″的第二优选实施例成有倒钩的钉50″的形式。有倒钩的钉50″优选地包括一个或多个形成于钉50″的外表面上的倒钩60″,以便防止拉出。优选地,一个或多个倒钩60″切入钉50″中。此外,倒钩60″可以交错,以便帮助在套筒中对齐,如后面更详细所述。

钉50可以通过目前或以后已知的任意方法来制造,这些方法包括但不局限于:冲压(heading)、滚丝、铣削等。倒钩60和/或肋65可以通过切割、机械加工等来制造。钉50可以由可生物相容的材料来制造,该材料包括但不局限于:不锈钢、钛、钛合金、骨(包括同种移植骨)、一种或多种聚合物例如聚醚醚酮(PEEK)、聚L-丙交酯(PLLA)、形状记忆合金例如镍钛合金(Nitionol)、一种或多种可生物吸收的材料例如聚乳酸(PLA)等。钉50还可以涂覆有例如羟磷灰石,以便促进骨向内生长,或者例如通过等离子体涂覆等来处理用于骨结合。也可选择和/或另外,钉50可以通过例如喷砂处理进行表面加工,以便增大拉出强度。也可选择或者另外,外科手术粘接剂可以施加在钉50上,以便提高拉出强度,且还可以进行防止感染或允许用于抗菌表面安装的表面处理。
用于将钉驱动到患者的骨内的插入仪器的示例实施例
优选地,本发明还涉及一种用于将钉50驱动到患者的骨内的插入仪器100、100′。如下面更详细所述,插入仪器100、100′优选地能够接收冲击力F,该冲击力F又使得内部活塞140、140′向远端运动,以便与钉50接合,这样,钉50可以被驱动到患者的骨内。冲击力F可以由外科医生的手、锤或短锤来产生,或者自动产生,类似钉枪或自动钉驱动器等。
插入仪器100、100′优选地(i)使钉50适当地对齐和定位;(ii)减少或防止钉50错过敲击的可能性,这限制了刮削和偏心骨冲击;(iii)减少或防止钉50的弯曲;以及(iv)以优化钉50植入患者的骨内的方式来将负载引导至钉50上。优选地,插入仪器100的基底面设计成使得任何附加冲击分配在假体10的整个表面上,以便最小化对下面的骨、组织或假体10的损害。插入仪器100、100′优选地设计成将钉50的头部54定位在椎骨本体内至精确的深度,或者将头部54定位在高于假体10的精确高度。也就是,内部活塞140、140′优选地停在预定点,以便将钉50传送至合适深度。
参考图10,插入仪器100的第一优选实施例包括用于向钉50施加和/或传递冲击力F的内部活塞140、冲击端部130、手柄120和远端的钉接合端部110。内部活塞140可以与冲击端部130形成一体。也可选择,内部活塞140可以与冲击端部130操作连接。在使用时,冲击力F优选地使得内部活塞140相对于手柄120向远侧运动,从而将钉50驱动到患者的骨内。

内部活塞140优选地通过弹簧力而被偏压成返回它的初始位置(例如,在图13A-13C中偏压成从左向右运动),形成于内部活塞140上的第二凸肩294与阻挡机构260接触,从而使得阻挡机构260向近侧返回它的初始位置,这又除去由阻挡机构260施加在小球270上的偏压力。因此,小球270接触形成于较小直径部分280和较大直径部分290之间产生的槽形边缘291,这产生了沿朝着内部活塞140返回方向的力矢量,从而使得小球270离开较大直径部分290的内表面292,并导致小球270返回较小直径部分280。一旦小球270处于较小直径部分280中,壳体230能够通过弹簧力S而返回它的初始位置。
在使用时,位置锁定机构250和单向制动机构210可以组合,以便产生具有自动复位特征的棘轮机构。在该结构中,内部活塞140通过弹簧力S被偏压,从而向近侧推压活塞140(例如图12A-13C中从左向右)。当活塞140通过冲击力F而从右向左运动或从冲击端130朝着接合端110运动时,单向制动机构210大致防止活塞140向近侧返回,从而能够使活塞140渐增运动,直到活塞140达到预定深度。随后,形成于活塞140上的凸肩232接触壳体230,从而使得壳体230运动离开锥形或楔形内表面212,从而能够使得位置锁定机构250接合。这时,单向制动机构210不能起作用,因为壳体230锁定在该位置。在活塞140上的弹簧力使得活塞140返回它的初始位置。当活塞140达到它的初始位置时,形成于活塞140上的第二凸肩294与阻挡机构260接合,并脱开位置锁定机构250。这时,壳体230能够返回它的初始位置,这又使得单向锁定机构210重新接合。
固定套筒驱动器
钉50可以通过使得钉50插入通过形成于插入仪器100、100’中的插管套管而装入插入仪器100、100’中。也可选择和/或另外,钉50可以通过任意已知装置(包括但不局限于通过O形环)而与插入仪器100、100’的远端的钉接合端部110、110’操作连接。O形环用作干涉配合,这使得钉50的头部54能够在冲击时通过。类似的方案可以通过合适公差的聚合物部件来实现。

Claims (6)
1.一种用于将钉(50)驱动到患者的骨内的组件,所述组件包括:
A)具有插管轴的插入仪器(100);
B)钉(50),所述钉具有中心轴线(55)、头部部分(54)和杆部分(52),其中:
C)所述钉(50)被沿着所述插管轴插入;
D)所述插入仪器(100)还包括多个辊(400),所述多个辊的每一个都具有纵向轴线(405),所述多个辊绕所述钉(50)沿周向布置,其中,所述多个辊(400)的外径与钉(50)的杆部分(52)相切,所述多个辊(400)对通过所述插管轴插入的所述钉(50)提供轴向限制;以及
E)所述辊(400)中的每一个都为圆柱形部件,所述圆柱形部件具有螺旋形切口(402),用于接收所述钉(50)的所述头部部分(54)的至少一部分。
2.根据权利要求1所述的组件,还包括:内部活塞(140),用于将冲击力F施加和/或传送给所述钉(50)。
3.根据权利要求2所述的组件,还包括:自动复位机构(200),使得在钉(50)被驱动到患者的骨内之后,所述内部活塞(140)返回它的初始位置。
4.根据权利要求3所述的组件,其中:所述自动复位机构(200)包括单向制动机构和位置锁定机构。
5.根据权利要求1所述的组件,还包括:附接在所述辊(400)上的马达或人工机构,以便使所述辊旋转。
6.根据权利要求1至5中任意一个所述的组件,其中:所述钉(50)设置为螺旋钉,其包括外部凸缘(58),所述外部凸缘的凸缘节距设置成使得当钉被冲击驱动到患者的骨内时,所述钉(50)旋转。
