Abstract
本发明公开了一组胃癌辅助诊断的基因序列,以及针对该组基因序列的基因芯片和试剂盒,本发明还公开了该组基因序列在胃癌辅助诊断中的应用。利用本发明的胃癌辅助诊断的基因序列,可从分子水平对胃癌进行分型,帮助临床进行诊断,从而更加有效地治愈患者且最大限度地降低治疗毒副作用。
Description
技术领域
本发明涉及分子诊断领域,尤其涉及一组胃癌辅助诊断的基因序列及其应用。
背景技术
肿瘤治疗的挑战之一,是如何对肿瘤进行比较准确的分型,在此基础上选择合适的治疗方案。目前,肿瘤分型或分类主要是依据于形态学标准,有一定的局限性,肿瘤在临床上的异质性提示可能存在不同的分子亚型,因为相同形态的肿瘤在分子水平可能具有明显不同的基因表达特征,因而具有不同的细胞生物学和分子生物学行为,从而可能对相同治疗产生不同的反应。随着人类生物医学进入分子水平时代,普通病理形态学研究已不能完全适应肿瘤治疗的需要,因此迫切需要应用分子诊断技术,对肿瘤发生发展从分子水平上加以认识,并重新分型。
为了最有效地治疗且最大限度地降低治疗毒副作用,癌症治疗已经越来越趋向于不同型别采取不同的治疗手段。表达谱基因芯片为从分子水平进行肿瘤分类提供有力工具,通过平行考察上万种基因的表达情况,得到每个癌症患者的肿瘤基因表达的分子图谱,然后利用多种聚类分析,如层次聚类、自组织分析、主成分分析等方法就可以将具有相似基因表达谱的肿瘤归并在一种类型里。这种对肿瘤分类方法的研究可以分为二类,一类是发掘肿瘤类型,即确定以前未被认识的肿瘤亚型,另一类是预测肿瘤类型,即将特定的肿瘤标本归于已定义的可以反映当前状况和预后的肿瘤类型中。
目前国际上已经有文献报道通过基因表达谱的研究,选出一些相关的基因,通过检测这些基因在癌组织和癌旁组织表达改变的情况,可以对肿瘤进行分期和分型。Golub等阐述了将基因芯片技术应用于白血病分型,具有快速、简便和高特异性等优点(Science.286:531-7)。他们设计出的DNA芯片成功地分析了急性髓细胞性白血病(AML)和急性淋巴细胞性白血病(ALL),为人类的肿瘤疾病分型及早期诊断奠定了基础。Van’t Veer等通过基因芯片对乳腺癌进行研究(Nature 415:530),选出了70个标志(Marker)基因,通过检测这些基因表达的改变,区分出肿瘤的恶性程度,有可能使一部分患者在术后化疗过程中减少药物的使用量。另外在肺癌(Nat Med 8:8162002)和前列腺癌(J Clin Invest 113:913 2004)中也有类似的报道。多个研究结果表明,有可能通过检测基因表达谱的改变来对肿瘤进行分型,从而在治疗方案的确定中提供有力的依据,根据不同的分子分型,采用不同的化疗和放疗手段,提高肿瘤病人的生存率。
胃癌是最常见的恶性肿瘤之一,目前在我国的发病率占肿瘤中的首位,每年约有16万人死于胃癌。对于胃癌基因表达谱的研究,已有不少报道(如Cancer Res 62:7012;Cancer Res 62,233-240等),找到了一些与胃癌的发生和发展相关的基因,对于不同基因与胃癌的关系有了更深入的了解。但对利用基因表达谱的改变来对胃癌进行分子分型仍未见有相关报道。
对于胃癌的分型,病理上主要是Lauren分型(分为肠型和弥漫型)或者TNM分型。前者主要是基于组织学的改变,后者主要根据肿瘤的形态大小和部位并结合淋巴结和远处转移的情况来,这些分型方法有时候与临床表现和疾病的发展不太吻合,同一种类型,采用相同的治疗方法,最后的结果可能完全不同。另外通过形态学分型,有可能会发生一些比较难于判断的情况,并且目前的组织病理分型依赖于病理诊断医师的经验和知识,不同的病理医生可能会得出不同的结果。
发明内容
本发明要解决的技术问题之一是提供一组胃癌辅助诊断的基因序列,通过分子杂交或者PCR手段,对疑似胃癌组织中该些基因表达改变状况进行检测,从而辅助临床病理诊断。
本发明要解决的技术问题之二是提供一种用于胃癌辅助诊断的基因芯片。
本发明要解决的技术问题之三是提供一种利用上述基因芯片进行胃癌辅助诊断的方法。
本发明要解决的技术问题之四是提供一种用于胃癌辅助诊断的试剂盒。
本发明要解决的技术问题之五是提供本发明胃癌辅助诊断的基因序列的应用。
为了解决上述技术问题,本发明通过如下技术方案实现:
在本发明的一个方面,提供了一组胃癌辅助诊断的基因序列,它包括:
(a)SEQ ID No.1~SEQ ID No.45所示的序列;
(b)SEQ ID No.1~SEQ ID No.45所示的序列中每条序列的互补序列;
(c)与SEQ ID No.1~SEQ ID No.45所示的序列中每条序列有至少70%同源性的序列。
优选地,所述基因序列包括SEQ ID No.1~SEQ ID No.45所示的序列。
在本发明的另一方面,提供了一种用于胃癌辅助诊断的基因芯片,它包括:固定在固相载体上的与待测SEQ ID No.1~SEQ ID No.45所示基因序列和/或其互补序列的全部或连续15个以上核苷酸片段进行杂交的探针。
本发明中所述固相载体可选用领域周知的载体,只要所述载体与所述反应物相容,不会影响检测结果就可以。优选的,本发明所述固相载体选材为玻片、硅片、硝酸纤维素膜、尼龙膜和高分子材料中的一种或它们的任意组合。
在本发明的另一方面,提供了一种利用上述基因芯片进行胃癌辅助诊断的方法,包括如下步骤:
1)从合适样品中抽提待测样品核酸;2)标记样品核酸;3)选取权利要求3所述的基因芯片,在适于与所选芯片进行杂交的条件下,加入经标记的样品核酸,并使其反应足够时间;4)洗涤后检测杂交反应的结果。其中,步骤1)所述的合适样品包括手术切除的胃组织和胃镜获取的胃粘膜上皮组织。
在本发明的另一方面,提供了一种用于胃癌辅助诊断的试剂盒,所述试剂盒包含SEQ ID No.1~SEQ ID No.45所示基因序列或其互补序列的至少3个基因序列。
在本发明的另一方面,还提供了本发明的基因序列在胃癌辅助诊断中的应用,它是通过检测SEQ ID No.1~SEQ ID No.45所示序列的基因的表达变化对胃癌进行分子分型,所述的对胃癌进行分子分型包括区分开胃癌和胃炎。
所述检测基因的表达变化采用包括基因芯片、PCR扩增反应的方法。
本发明的胃癌辅助诊断的基因序列,从分子水平,而不是形态学水平,对胃癌进行分型,帮助临床进行诊断,从而更加有效地治愈患者且最大限度地降低治疗毒副作用。

具体实施方式
下面结合附图和实施例对本发明作进一步详细的说明。
实施例1临床样品的准备
47份胃癌疑似病例行内窥镜检查取样活检,经病理学诊断确诊为28例胃癌和19例非癌炎症或其它病变胃部疾患,胃癌的诊断标准参照全国胃癌病理协作组拟定的胃粘膜活检病理诊断的统一标准。留取病变及对侧非病变胃粘膜上皮的组织,液氮冻存。此步骤由上海交通大学附属瑞金医院和新华医院消化科完成。
实施例2总RNA的提取、质量检测和纯化
(1)总RNA的提取
47对胃癌疑似病例的病变胃粘膜和远端非病变胃粘膜分别进行RNA抽提,用MRC的TRIzol试剂盒,常规哺乳动物组织RNA的提取方法。每100mg组织加入1ml TRIzol,用电动匀浆器充分打碎组织块。加入约1/5体积的氯仿,上下颠倒充分混匀1分钟左右,室温下静置5分钟。4℃,12,000rpm离心15分钟后小心取出上清液,转入新的1.5ml离心管,加入等体积的异丙醇,轻轻颠倒混匀,室温静置5分钟。4℃,12000rpm离心10分钟后,去上清,向沉淀中加入2/5体积的70%乙醇,4℃,12000rpm离心洗涤沉淀15分钟。去上清,沉淀室温晾干后加入适量无RNA酶的水充分溶解沉淀,测定OD260和OD280值。采用无RNA酶的DNA酶I处理去除基因组DNA的污染,从而提高样品纯度。
(2)总RNA的质量检测
对提取的47对样本的总RNA进行琼脂糖凝胶电泳:配置用DEPC处理的电泳缓冲液50×TAE,高压灭菌后待用。使用电泳槽前用3%H2O2浸泡15min,然后用DEPC处理的水冲洗,倒入适量1×TAE电泳缓冲液。称取适量琼脂糖,加入1×TAE电泳缓冲液,制备1%的胶,用专用6×Loading buffer做指示剂,取约700ngRNA配成10μl上样电泳(电压100伏)15min。取出电泳胶,在凝胶成像仪上观测、拍照,得图1。通过目测28S和18S条带清晰且其亮度或宽度比例28S∶18S≥2初步评价总RNA的质量较好。用Agilent 2100 bioanalyzer分析提取的RNA,得2100峰图(见图2),清晰的峰形和最低的背景荧光显示完整、未降解的RNA。
(3)总RNA的纯化
对通过质量检测的47对总RNA使用QIAGEN RNeasyKit纯化,详细方法见RNeasyMini Protocol。
实施例3基因表达谱实验
(1)体外转录
47对样本提取的总RNA各取2μg,采用Agilent linear amp kit体外转录来扩增出cRNA,即在1.5ml离心管中首先配置2μg RNA、5μl T7 Promotor primer、Rnase-free Water合11.5μl,65℃变性10分钟,冰浴5分钟。然后加入4μl 5×FirstStrand Buffer、2μl 0.1M DTT、1μl 10mM dNTP mix、1μl MMLV RT、0.5μl RNase OUT,40℃保温2h(热盖65℃),反应合成大量的cDNA一链和二链,然后在此基础上加入20μl4×Transcription Buffer、6μl 0.1M DTT、16μl NTP、4μl25mM aa-UTP、6.4μl 50%PEG、0.5μl Rnase OUT、0.6μl Inorganic Pyrophosphatase(无机焦磷酸酶)、0.8μlT7 RNA Polymerase(聚合酶)、5.7μl Rnase-free Water合80μl体系40℃保温2h转录出cRNA链。应用QIAGEN RNeasyMini kit纯化。操作同上。
(2)荧光探针制备、纯化和定量
cRNA荧光探针采用间接标记法,荧光试剂为GE Life Science提供的Cy3/Cy5NHS Ester,就47对样本cRNA,Cy3标记病变组织,Cy5标记对侧非病变组织。上步47对纯化的cRNA各取4μg并浓缩至6.66μl,依次加入10μl DMSO、3.4μl0.3Msodium bicarbonate buffer(碳酸氢钠缓冲液,pH9.0)并分别混匀。然后将此20μlcRNA混合物加入到浓缩抽干的Cy3/Cy5 NHS Ester中并混匀。室温25℃保温1h后再加9μl 4M Hydroxylamine(羟胺)混匀后室温25℃保温15min。经QIAGEN RNeasyMini Kit纯化后通过酶标仪OD552、OD650的测定计算相应Cy3、Cy5的量。
(3)芯片杂交
基因表达谱芯片采用上海生物芯片有限公司制备的SBC-Human-cDNA-V2.3芯片,覆盖有14592多个点,除768个质控点外,代表13424个人类的基因或EST。就每对探针,取85pmol Cy3-cRNA、45pmol Cy5-cRNA,再加入1μl PolyA、9μl 4×hybridizationBuffer(杂交液)、12μl甲酰胺、Xμl Rnase-free Water合36μl体系轻轻混匀并稍微离心。然后将36μl杂交液用微量加样器小心滴加在芯片上,盖上盖玻片并密封芯片于含PBS的杂交湿盒中,42℃保温16-18小时。
(4)芯片洗涤与扫描
取出杂交好的芯片先后在50℃ 2×SSC/0.1%SDS,室温1×SSC/0.1%SDS,室温0.5×SSC各洗涤10分钟,然后再置于室温DEPC水中1分钟洗涤后1500rpm离心5分钟干片,最后洗涤后的芯片装入芯片holder置于共聚焦扫描仪扫描。
(5)芯片数据读取与检测信号的确定
采用ImaGene5.0读图软件,并取阴性质控点,计算其Cy3、Cy5_signal的平均信号强度与2倍标准方差的和(mean+2*STDEV),以双通道Cy3、Cy5_signal大于mean+2*STDEV,且信噪比大于2.0为标准确定检测信号,否则为缺失点或污染点。
(6)数据标准化
数据导入ArrayLIMS,采用LOWESS方法进行标准化消除系统误差,使Cy3和Cy5两信道的信号总值趋于等同。
(7)差异筛选和统计分析
差异基因筛选,以80%样本中有检测信号为标准,应用SAM方法分别作胃癌组和非胃癌组的组内差异显著性分析,False Discovery Rate(FDR)值取0.01或0.1。
(8)基因表达相似性分析及基因功能学分析
采用层次聚类法(Hierarchical clustering)对所有差异表达基因在28例胃癌和19例非胃癌组织中的表达进行相似性分析。Treeview进行聚类图像展示。通过GO进行生理功能分类。
(9)结果
从所有芯片中,挑选数据质量合格的47张芯片(28例胃癌,19例胃炎),进行片内和片间标准化之后,应用SAM软件(PNAS 98:5116-5121)筛选在胃癌和胃炎中差异表达的基因(取FDR=0.01),共找出53个基因,进一步选出这些基因中差异表达幅度较大的45个基因(平均fold change>1.3)对样本进行聚类分析(使用软件Cluster3.0,PNAS 95:14863-14868),得图3,其中,图3最上方是样本编号,如Ca_xh30代表编号为xh30的胃癌(Ca)样本,G_xh42代表编号为xh42的胃炎(G)样本,样本编号下面是聚类树状图,可以看出癌症样本和炎症样本各自聚成一类,说明这45个基因可以很好的把癌症和炎症样本区分开(正确率为95.7%),再下面是表达图谱,红色表示基因上调(相对于该基因的平均数),绿色表示下调,黑色表示未变化,灰色表示数据缺失,右列是这些差异基因的Genbank号码。所以这些基因可以作为胃癌(区别于胃炎)的标志基因。
实施例4PCR方法检测基因表达的改变
(1)内参设定及引物设计、合成
内参选用β-actin,引物设计采用Primer Express2.0软件以序列19-22bp且Tm59℃为优化条件,Invitrogen合成。
(2)RNA提取和逆转录cDNA合成
RNA的提取、质量检测和纯化步骤同实施例2所述。
在Eppendorf管中加入下列试剂,然后70℃温浴10分钟,稍微离心,立即放置冰上至少2分钟。

(3)实时PCR
反应采用ABI 7000型定量PCR仪。上述逆转录产物稀释100倍,取其中1μl进行反应。Premix Ex Taq 12.5μl+ROX 0.5μl +Primer 1ul+ddH2O合每管25μl。

Claims (10)
1.一组胃癌辅助诊断的基因序列,其特征在于,它包括:
(a)SEQ ID No.1~SEQ ID No.45所示的序列;
(b)SEQ ID No.1~SEQ ID No.45所示的序列中每条序列的互补序列;
(c)与SEQ ID No.1~SEQ ID No.45所示的序列中每条序列有至少70%同源性的序列。
2.如权利要求1所述的基因序列,其特征在于,它包括SEQ ID No.1~SEQ ID No.45所示的序列。
3.一种用于胃癌辅助诊断的基因芯片,其特征在于,它包括:固定在固相载体上的与待测SEQ ID No.1~SEQ ID No.45所示基因序列和/或其互补序列的全部或连续15个以上核苷酸片段进行杂交的探针。
4.如权利要求3所述的检测芯片,其特征在于,所述固相载体选材为玻片、硅片、硝酸纤维素膜、尼龙膜和高分子材料中的一种或它们的任意组合。
5.一种利用权利要求3所述的基因芯片进行胃癌辅助诊断的方法,其特征在于,包括如下步骤:
1)从合适样品中抽提待测样品核酸;2)标记样品核酸;3)选取权利要求3所述的基因芯片,在适于与所选芯片进行杂交的条件下,加入经标记的样品核酸,并使其反应足够时间;4)洗涤后检测杂交反应的结果。
6.如权利要求5所述的方法,其特征在于,步骤1)所述的合适样品包括手术切除的胃组织和胃镜获取的胃粘膜上皮组织。
7.一种用于胃癌辅助诊断的试剂盒,其特征在于,所述试剂盒包含权利要求1或2中的至少3个基因序列。
8.权利要求1或2所述的基因序列在胃癌辅助诊断中的应用,其特征在于,它是通过检测SEQ ID No.1~SEQ ID No.45所示序列的基因的表达变化对胃癌进行分子分型。
9.如权利要求8所述的应用,其特征在于,所述检测基因的表达变化采用包括基因芯片、PCR扩增反应的方法。
10.如权利要求8所述的应用,其特征在于,所述的对胃癌进行分子分型包括区分开胃癌和胃炎。
