Abstract
Positioning the patient in an open or closed tubular system (1) with windows allows a caregiver and the patient (2) to mechanically adjust any position between supine, lateral and, if the tubular system is closed, the prone position without the need for additional assistants. The inclination of the patient to the vertical also makes it easier to eat and make eye contact with visitors. The main advantages of the pipe system for patients and nursing staff are the simplified treatment and the automatic toilet. Medical benefits include reduction of lung problems associated with easier breathing and prevention of bedsore problems. Muscle breakdown is slowed down by vibration and massage and the subsequent REHA phase is often shortened. Computer, Internet, and television occupations and some limb exercise may help partially maintain or revive the patient's spirits.
Description
The present invention relates to a rotatable tube system with a patient couch, which relieves the nursing staff in hospital intensive care units and reduces lung problems for the long-term patient, avoids decubitus ulcers and brings relief.
With the corona pandemic, the intensive care units of the hospitals are heavily burdened with COVID19 patients, some of whom have to be treated in a coma in beds for longer periods of time. For the treatment of the ARDS syndrome and for the reduced load on the lungs, the patients have to be turned from the supine to the prone position and back on a regular basis. This means a lot of effort for the nursing staff and often requires 4 to 5 nurses. A major problem for bedridden patients with pressure ulcers is damage to the skin and underlying tissues from prolonged pressure and lack of blood flow, leading to ulcers.
In the following, a patient bed within a rotatable hollow body (pipe system) is described, which avoids the effort required to turn the patient, which prevents the development of decubitus ulcers and makes the patient's life easier with an automatic toilet and the possibility of a computer and TV .
Stand der Technik:State of the art:
Normally, long-term patients are stored in beds with mattresses and blankets, and they have to be rotated periodically about the longitudinal axis in the lateral position or, better, in the prone position to prevent storage damage and to avoid lung respiratory damage. An improvement is provided by hospital beds that are rotated at a limited angle around the horizontal axis mechanically or by means of mattresses with inflatable balloon tubes ("Freedom Bed"; "PressureGuard CFT® Mattress"; "Smart Hospital Bed" at North York General Hospital & Toronto hospitals allows 30° patient angle and costs > $80,000 An open full rotating hospital bed RotoProne® Therapy System was developed by KCl San Antonio, Texas and is distributed by ARJO Inc. , Addison Illinois, USA It also has rocking Possibility and was published in the Journal of the American Medical Association November 2009. RotoProne is successful, but complex with packing the patient - The new invention with the tube system solves the problems and has many advantages, which greatly simplify the treatment, create convenience for the patient and which are described below.

Medizinische Voraussetzungen:Medical requirements:
The tube system (1) consists of two longitudinal halves (or three to four longitudinal parts), the upper half being folded open or mechanically lifted to allow patient loading and unloading. Within the stable tube system there are fixing options (as tubes or as pins) for curved support plates (4) which are adapted to the body shape of the patient and which are provided with an elastic, skin-friendly surface (5). The patient is fixed by holders on his back and his front in such a way that, depending on the angle of rotation of the tube system, he is held in a supine, prone or side position. The head, legs and arms are also held in this way. The inner wall of the pipe system is not touched by the body. The tube system itself can assume the horizontal position, but can also be tilted and fixed to the vertical position (Fig. 2) and rotated about 180 degrees for the prone position. Because of the automatic toilet, a slightly inclined position is preferred. In a luxury version, the curved support plates are equipped with vibrators that reduce muscle breakdown and promote blood circulation. As an alternative, the whole pipe system can be vibrated. There are numerous windows in the pipe system that can be easily opened and closed with a simple mechanism. These allow treatment, change of clothing and control of the patient. The clothing is split lengthwise and held together with an elastic band, or it is replaced by duvets for the front and back. In the lower area of the pipe system, this is specially shaped (7) to collect body fluids, in order to enable them to flow into a separate collection container (with odor-reducing flap (12)). Automatically or at the direction of the patient or caregiver, a wide jet of water or washing solution (9) is released that washes the patient's pubic area and flushes feces and urine into the container. The pipe system is automatically inclined slightly, which facilitates drainage. The garment or blankets are automatically lifted to keep them dry. The drainage of liquid along the legs is prevented with a ring around the legs. Warm compressed air (10), which dries the patient and the environment, is supplied above this toilet area on instruction or automatically. Temperature and humidity within the pipe system are regulated. This data is displayed along with oxygen and CO2 levels at the foot end. At the head end of the pipe system, the respiratory air as well as the patient's coughing and sneezing sputum are drawn off (11) and drained off separately, so that infection of the nursing staff and visitors is ruled out when the pipe system is closed. The patient can be treated and washed relatively easily by opening the pipe system or through large windows or by lifting the pipe system cover. Parts of the patient can be raised slightly for changing clothes or bed covers, or individual holders can be lowered so that the patient is held by the remaining majority of holders. Normally, one caregiver will be sufficient temporarily for the patient, avoiding the physical exertion of turning the patient. If a special crane is not available, several caregivers are only required when inserting the patient into the pipe system and removing it. The advantage of this action is that the upper half of the pipe system is lifted off in a mechanized manner. All supply hoses and measuring probes (8) are routed to the head end (or to the foot end), passed through the end plates or a window of the pipe system and connected to external devices and breathing apparatus. Signal switches can give the patient the opportunity to control their position and inclination themselves, to activate the toilet flush and air drying in a slightly inclined supine position, and to open the cover of the pipe system. Of course, the patient can also press an emergency switch.
Konstruktion des Rohrsystems:Construction of the pipe system:
The tube system (1) is either placed on intensive care beds or it can stand on the floor as an independent unit, in which case rollers are folded down for transport. – Various tube system models are adapted to height and girth, with lengths ranging from 100cm to 250cm, the latter for individuals of 2m in length, and the typical diameter is between 50cm and 120cm. The stability is adjusted to the maximum weight of the patient. – The material is stainless steel and combination with transparent hard plastic/plexiglass. However, to save weight, materials such as hard aluminum, composite materials, light ceramics or wood can also be considered. In humid areas, a hydrophobic surface (through surface treatment / coating) can simplify cleaning. - The inner surface of the pipe system (1) has numerous locations with short rods or pipes on which the supports (4) can be placed and fixed. The length of the holders of the straps is adjusted to the patient's body. The curvature of the carrier surface also fits the body surface. This requires malleable surfaces (rapid setting plastic) or bearing on various support surface structures. - The carrier surfaces have a layer of elastic, skin-friendly material (5) as a soft support and/or soft special cushions are fixed. - Transparent plastic such as plexiglass or shatterproof glass is preferred for the large, easily removable, hinged and customized windows (not shown). The positions and sizes of the windows are coordinated with the carriers so that when the windows are open, the entire surface of the patient can be reached by the nursing staff for treatment and washing without opening the pipe system and partially displacing the carriers. – The patient's position and inclination can be adjusted by means of an operating/keyboard panel which can be seen and operated by the patient. The same settings can also be made outside, for example at the foot end, by the nursing staff. - In the area of the upper body and the head, holders for the keyboard and controls are provided, so that in the prone position, computers, smartphones and TVs can be operated and seen through a clear window (not shown in the figures). Connections for headphones can be reached by the patient. – Using vibrators on the straps can improve local blood circulation and reduce muscle breakdown. This biomechanical stimulation (BMS) was developed by the Russians in the 1970s for top athletes and astronauts. Low frequency (up to 12 Hz) is used for muscle relaxation, medium frequency up to 20 Hz improves muscle function, and high frequency 20 to 35 Hz increases muscle performance. Movement exercises for the legs (when withdrawing or removing the straps) and massaging them with the pipe system open will also be helpful. Vibration can be activated and controlled by both patient and caregiver. - The production of such a pipe system and its control mechanisms presents factories for medical equipment and hospital beds and sanitary facilities with solvable problems, whereby design details such as size, window support arrangement and special requests of the interested hospitals, intensive care physicians and nursing specialists are taken into account.
Figures 1 and 2 show schematically the longitudinal cross section and the vertical cross section of the tube system (1) with the patient (2) and the rotation mechanism (3), which can also be set up axially (not shown). The patient is fixed by the holders (4), the length of which is adjusted, while the elastic-soft surface (5) is in contact with the patient. The toilet can be activated with the patient in a supine or prone position. The sump and sanitary drain are not shown. The breathing air is sucked out at the head end (11), and this is also where the measuring and supply lines (8) are fed to the devices. The stands (6) of the pipe system allow the motorized (e.g. hydraulic) erection of the pipe system (1) from the horizontal position (Fig. 1) to the vertical position (Fig. 2), whereby the angle of inclination can be adjusted in the range from 0° to 90° . A steep supine position facilitates feeding and allows visual contact with visitors.
