La presente invenzione riguarda derivati salini stabili solubili in acqua della 2-N-metilol-cefalexina in forma liofilizzata e le loro composizioni farmaceutiche.
La cefalexina è un antibiotico ad ampio spettro insolubile in acqua avente formula:

A causa di questa sua insolubilità, la cefalexina viene abitualmente somministrata sotto forma dei suoi derivati salini che, tuttavia, sono solubili in acqua solamente a pH nettamente alcalini, cosa che porta ad una notevole limitazione dei possibili impieghi in campo terapeutico.
Per risolvere questo inconveniente, la ricerca farmaceutica si è pertanto dedicata allo studio di derivati della cefalexina dotati delle stesse proprietà antibiotiche, ma caratterizzati da una maggiore solubilità acquosa delle corrispondenti forme saline.
Nel brevetto statunitense n. 4 471 115 vengono descritti derivati salini della 2-N-metilol-cefalexina, di formula:

dove X è un metallo alcalino o un catione organico, contenente azoto, di un generico amminoacido basico. Detti derivati non solo mantengono l'attività antibiotica ad ampio spettro tipica della cefalexina, ma sono caratterizzati da una perfetta solubilità in acqua, fornendo tra l'altro soluzioni praticamente neutre e quindi iniettabili.
Detti derivati salini della 2-N-metilol-cefalexina, una volta portati in soluzione, sono tuttavia contraddistinti da una diminuzione dell'attività antibatterica, probabilmente legata all'apertura dell'anello beta -Iattamico, che ne limita l'utilizzo, rendendo necessaria la solubilizzazione in loco del principio attivo ed impedendo la commercializzazione di fiale contenenti soluzioni preconfezionate dello stesso.
È stato ora sorprendentemente trovato e costituisce l'oggetto della presente invenzione che, detti derivati salini della 2-N-metilol-cefalexina, nel caso in cui vengano sottoposti a liofilizzazione, controllando il contenuto di umidità residua del prodotto liofilizzato, assumono una attività antibatterica praticamente costante nel tempo, con i conseguenti immaginabili vantaggi in campo terapeutico.
Un secondo oggetto della presente invenzione, è inoltre costituito dal fatto che detti derivati salini, una volta liofilizzati, sono presenti come una miscela di due distinti composti in equilibrio fra loro, di cui qui di seguito si riportano le formule:

Senza voler porre indebite limitazioni alla presente invenzione, sembra plausibile che il composto (III) sia il risultato di una ciclizzazione intramolecolare del composto (II), determinata dalla perdita di una molecola di acqua durante la fase di liofilizzazione.
Un terzo oggetto della presente invenzione è pertanto costituito dai derivati salini di formula (III) prodotti durante il procedimento di liofilizzazione dei derivati salini della 2-N-metilol-cefalexina di formula (Il). Un quarto oggetto della presente invenzione è inoltre costituito da composizioni farmaceutiche contenti i derivati salini di formula (Il) e (III).
Allo scopo di valutare il differente comportamento nel tempo dell'attività antibatterica dei derivati salini oggetto della presente invenzione, sono stati effettuati dei test comparativi utilizzando il sale di lisina della 2-N-metilol- cefalexina, prodotto secondo la metodica descritta nel già citato brevetto statunitense n. 4 471 115. La valutazione dell'attività antibatterica del prodotto sciolto in acqua demineralizzata è stata determinata con metodologia HPLC (titolo espresso in mu g/mg come cefalexina), con metodo dello standard esterno, eseguito utilizzando il sale sodico di cefalexina come composto di riferimento standard.
È stato utilizzato un cromatografo ad alta pressione Varian mod. 5000, dotato di rivelatore ad ultravioletto Merck-Hitachi mod. 655A ed integratore Merck-Hitachi mod. D2000, con le seguenti condizioni operative:

La perdita dell'attività antibiotica (come cefalexina) del sale di lisina della 2-N-metilol-cefalexina disciolto in acqua demineralizzata ad una concentrazione di 2,5 mg/ml e ad un pH = 7,5 è stata monitorata operando ad una temperatura di 25 DEG C; i risultati sono riportati in tabella 1:

Dal momento che la cefalexina è un principio attivo accreditato in letteratura di una attività antibiotica compresa tra 696 e 630 mu g/mg, si può notare come già dopo due ore dalla dissoluzione, la perdita di attività sia rilevante. La stessa notevole diminuzione di attività è stata poi rilevata, sempre a 25 DEG C, operando in campo neutro o con un pH leggermente acido (soluzione 0,1 M di H3PO4).
La perdita di attività antibiotica del sale di lisina della 2-N-metilol-cefalexina è stata valutata anche operando ad una temperatura di 37 DEG C; i risultati sono riportati in tabella 2:

Come si può notare dai risultati riportati nelle tabelle 1 e 2, la diminuzione dell'attività antibiotica è pertanto direttamente proporzionale alla temperatura e, già a 37 DEG C, la perdita di attività è così rilevante da compromettere l'utilizzo dei principio attivo dopo solamente un'ora dalla dissoluzione.
La seconda prova effettuata ha riguardato lo studio della perdita di attività antibatterica del sale di lisina della 2-N-metilol-cefalexina (sempre preparata secondo il brevetto statunitense n. 4 471 115) dopo un processo di liofilizzazione dove è stata opportunamente lasciata una percentuale di umidità residua del 4,5% in un primo caso ed inferiore o uguale all'1,5% nei rimanenti. Il campione liofilizzato contenente una umidità residua del 4,5% ha evidenziato una perdita di attività antibatterica (calcolata con la metodologia HPLC sopra riportata) dei 10% dopo 60 giorni; al contrario l'attività antibatterica è risultata rimanere invariata, sempre dopo un periodo di 60 giorni, nel caso di campioni contenenti un'umidità residua inferiore od uguale all'1.5%.
Per valutare le caratteristiche fisico-chimiche del sale di lisina della 2-N-metilol-cefalexina liofilizzato, sono state condotte analisi spettrofotometriche (l.R.) e spettrometriche (<1>H-NMR e massa), su di un campione contenente un quantitativo di acqua pari all'1,5%, dalle quali è emerso chiaramente come il sale sia in realtà presente come miscela dei sali (IV) e (V), in equilibrio tra loro, come qui di seguito raffigurato (lo spettro I.R. è riportato in fig. 1; lo spettro <1>H-NMR è riportato interamente in fig. 2 ed in scala espansa nelle fig. 3-5):

È stato utilizzato uno spettrometro di massa VG Analytical 7070E, equipaggiato con una sorgente FAB ed operante con un'accelerazione potenziale di 6 KV; il campione è stato analizzato in una soluzione di glicerina (0,5 1,0 mu g).
Lo spettro di massa mostra dei picchi a 360 e 348 m/z in accordo con le seguenti frammentazioni:

Inoltre, sono presenti nello spettro due picchi a 706 e 718 m/z appartenenti alle seguenti frammentazioni:
- 718 m/z (ione aggregato)

I picchi a 494 e 506 m/z sono in accordo con la presenza di lisina (C6H4N2O2: M.W. = 146,19) nel principio attivo:
- 348 m/z + lisina = 494 m/z
- 360 m/z + lisina = 506 m/z
Con i sali della 2-N-metilol-cefalexina oggetto della presente invenzione, sono state preparate diverse composizioni farmaceutiche, utilizzabili non solo per via parenterale, ma anche per via orale; i seguenti esempi devono essere considerati illustrativi e non limitativi.
Esempio 1
In questo esempio viene descritta la preparazione di fiale contenenti 2-N-metilol-cefalexina di lisina caratterizzate da una elevata velocità di solubilizzazione del principio attivo, una volta che questo viene messo in contatto con acqua.
Operando in camera sterile, il sale di lisina di 2-N-metilol-cefalexina è stato versato lentamente in acqua sterile preraffreddata a +5-10 DEG C, fino ad una concentrazione del 15% in peso e mantenuto sotto agitazione fino a dissoluzione avvenuta. La soluzione così preparata è stata poi versata in fiale preraffreddate ad una temperatura di -40 DEG C, in modo da avere un congelamento immediato del campione; dopodiché si è iniziato il ciclo di liofilizzazione e si è raffreddato il contenuto delle fiale a circa -45 DEG C; dopodiché si è raffreddato anche il condensatore alla stessa temperatura. Si è quindi dato inizio ad un processo di riscaldamento, applicando costantemente un vuoto residuo di 0,01 mbar; dopo 16 ore si è ottenuta una temperatura interna del prodotto di +28 DEG C.
Il prodotto è stato quindi lasciato a 0.01 mbar e a +28 DEG C fino ad ottenere un residuo di umidità </= 1,5% (le fiale così ottenute devono essere chiuse direttamente nel liofilizzatore o in camera deumidificata sterile).
Il principio attivo liofilizzato ottenuto secondo questa metodica si distingue dal principio attivo come tale, oltre che per le ragioni precedentemente descritte, anche per i rapidissimi tempi di solubilizzazione, che risultano essere pari a 10 secondi, contro i 90-120 secondi del campione come tale microdosato in fiala, con i conseguenti vantaggi per quello che riguarda le possibili applicazioni.
Esempio 2
Sono state realizzate, utilizzando i sali di metilolcefalexina oggetto della presente invenzione, delle compresse gommose masticabili (tipo chewing gum) e delle compresse da masticare o da sciogliere in bocca. Per tali forme farmaceutiche possono essere utilizzati direttamente i sali di metilolcefalexina liofilizzati, oppure un prodotto liofilizzato ottenuto con l'utilizzo di prodotti filmanti sui cristalli del principio attivo liofilizzato;
possono essere usati i composti filmanti normalmente utilizzati nell'industria farmaceutica, quali il polivinilpirrolidone, la cellulosa, la carragenina, la cera, il carbossipolimetilene e, più in generale, tutti quei prodotti che possono dare luogo a gel e che, dopo liofilizzazione, danno luogo ad una filmatura stabile dei cristalli del prodotto finito, preservandolo dall'attacco di agenti esterni la formulazione di una polvere filmata di un sale di metilolcefalexina, può avere, a titolo puramente esemplificativo, la seguente composizione:

The present invention relates to water-soluble stable saline derivatives of 2-N-methylol-cepalexine in lyophilized form and their pharmaceutical compositions.
Cefalexin is a water-insoluble broad-spectrum antibiotic with the formula:

Because of this insolubility, cefalexin is usually administered in the form of its saline derivatives which, however, are soluble in water only at clearly alkaline pH, which leads to a considerable limitation of the possible uses in the therapeutic field.
To solve this drawback, pharmaceutical research has therefore dedicated itself to the study of derivatives of cefalexin having the same antibiotic properties, but characterized by a greater aqueous solubility of the corresponding saline forms.
In U.S. Pat. 4 471 115 saline derivatives of 2-N-methylol-cepalexine, of formula:

where X is an alkaline metal or an organic cation, containing nitrogen, of a generic basic amino acid. Said derivatives not only maintain the broad-spectrum antibiotic activity typical of cefalexin, but are characterized by perfect solubility in water, providing, among other things, practically neutral and therefore injectable solutions.
Said 2-N-methylol-cefalexin saline derivatives, once brought into solution, are however characterized by a decrease in antibacterial activity, probably linked to the opening of the beta-Iactamic ring, which limits their use, making it necessary the on-site solubilization of the active ingredient and preventing the marketing of vials containing pre-packaged solutions of the same.
It has now been surprisingly found and constitutes the object of the present invention that, said saline derivatives of 2-N-methylol-cefalexin, in the case in which they are subjected to freeze-drying, controlling the residual moisture content of the freeze-dried product, assume an antibacterial activity practically constant over time, with the consequent imaginable advantages in the therapeutic field.
A second object of the present invention is further constituted by the fact that said saline derivatives, once freeze-dried, are present as a mixture of two distinct compounds in equilibrium with each other, of which the formulas are reported below:

Without intending to unduly limit the present invention, it seems plausible that compound (III) is the result of an intramolecular cyclization of compound (II), determined by the loss of a water molecule during the freeze-drying phase.
A third object of the present invention is therefore constituted by the saline derivatives of formula (III) produced during the lyophilization process of the saline derivatives of the 2-N-methylol-cephalexin of formula (II). A fourth object of the present invention is further constituted by pharmaceutical compositions containing the saline derivatives of formula (Il) and (III).
In order to evaluate the different behavior over time of the antibacterial activity of the saline derivatives object of the present invention, comparative tests were carried out using the lysine salt of 2-N-methylol-cefalexine, produced according to the method described in the aforementioned patent U.S. n. 4 471 115. The evaluation of the antibacterial activity of the product dissolved in demineralized water was determined by HPLC method (titre expressed in mu g / mg as cefalexin), by the external standard method, performed using the sodium salt of cefalexin as a compound of standard reference.
A Varian mod. High pressure chromatograph was used. 5000, equipped with Merck-Hitachi ultraviolet detector mod. 655A and Merck-Hitachi integrator mod. D2000, with the following operating conditions:

The loss of the antibiotic activity (such as cefalexin) of the lysine salt of 2-N-methylol-cefalexine dissolved in demineralized water at a concentration of 2.5 mg / ml and at a pH = 7.5 was monitored by operating at a temperature of 25 DEG C; the results are shown in table 1:

Since cefalexin is an active ingredient accredited in the literature of an antibiotic activity between 696 and 630 mu g / mg, it can be noted that already after two hours from dissolution, the loss of activity is significant. The same notable decrease in activity was then detected, always at 25 DEG C, operating in the neutral field or with a slightly acidic pH (0.1 M solution of H3PO4).
The loss of antibiotic activity of the lysine salt of 2-N-methylol-cefalexin was also evaluated by operating at a temperature of 37 DEG C; the results are shown in table 2:

As can be seen from the results reported in tables 1 and 2, the decrease in antibiotic activity is therefore directly proportional to the temperature and, already at 37 DEG C, the loss of activity is so significant as to compromise the use of the active principle after only one hour after dissolution.
The second test carried out concerned the study of the loss of antibacterial activity of the lysine salt of 2-N-methylol-cefalexin (always prepared according to U.S. Patent No. 4 471 115) after a freeze-drying process where a percentage was appropriately left of residual humidity of 4.5% in the first case and less than or equal to 1.5% in the remaining ones. The lyophilized sample containing a residual humidity of 4.5% showed a loss of antibacterial activity (calculated with the HPLC method above) of 10% after 60 days; on the contrary, the antibacterial activity was found to remain unchanged, always after a period of 60 days, in the case of samples containing a residual humidity less than or equal to 1.5%.
To assess the physico-chemical characteristics of the lysine salt of lyophilized 2-N-methylol-cepalexin, spectrophotometric (lR) and spectrometric analyzes (<1> H-NMR and mass) were carried out on a sample containing a quantity of water equal to 1.5%, from which it emerged clearly that the salt is actually present as a mixture of the salts (IV) and (V), in balance with each other, as shown below (the IR spectrum is shown in fig. 1; the spectrum <1> H-NMR is shown entirely in fig. 2 and on an expanded scale in fig. 3-5):

Mass spectrum - Fast atom bombardment
A VG Analytical 7070E mass spectrometer was used, equipped with a FAB source and operating with a potential acceleration of 6 KV; the sample was analyzed in a glycerin solution (0.5 1.0 mu g).
The mass spectrum shows peaks at 360 and 348 m / z according to the following fragments:

The peaks at 494 and 506 m / z are in agreement with the presence of lysine (C6H4N2O2: M.W. = 146.19) in the active substance:
- 348 m / z + lysine = 494 m / z
- 360 m / z + lysine = 506 m / z
With the salts of 2-N-methylol-cefalexin object of the present invention, various pharmaceutical compositions have been prepared, which can be used not only parenterally, but also orally; the following examples are to be considered illustrative and not limiting.
Example 1
In this example the preparation of ampoules containing lysine 2-N-methylol-cepalexin characterized by a high rate of solubilization of the active principle, once it is put in contact with water, is described.
Working in a sterile chamber, the 2-N-methylol-cepalexin lysine salt was slowly poured into sterile water pre-cooled to + 5-10 DEG C, up to a concentration of 15% by weight and kept under stirring until dissolution took place . The solution thus prepared was then poured into pre-cooled vials at a temperature of -40 DEG C, so as to have an immediate freezing of the sample; after which the lyophilization cycle was started and the contents of the vials were cooled down to about -45 DEG C; after which the condenser also cooled to the same temperature. A heating process was then started, constantly applying a residual vacuum of 0.01 mbar; after 16 hours an internal product temperature of +28 DEG C was obtained.
The product was then left at 0.01 mbar and at +28 DEG C until a residual of moisture </ = 1.5% was obtained (the vials thus obtained must be closed directly in the freeze dryer or in a sterile dehumidified chamber).
The freeze-dried active principle obtained according to this method differs from the active principle as such, as well as for the reasons previously described, also for the very rapid solubilization times, which are equal to 10 seconds, against 90-120 seconds of the sample as such microdosed in vial, with the consequent advantages for what concerns the possible applications.
Example 2
Chewable gum tablets (chewing gum type) and chewing or dissolving tablets in the mouth have been made using the methylolcefalexin salts object of the present invention. For these pharmaceutical forms, the freeze-dried methylolcefalexine salts or a freeze-dried product obtained by using film-forming products on the crystals of the freeze-dried active principle can be used directly;
the film-forming compounds normally used in the pharmaceutical industry, such as polyvinylpyrrolidone, cellulose, carrageenan, wax, carboxypolimethylene and, more generally, all those products which can give rise to gel and which, after freeze-drying, can be used The formation of a filmed powder of a methylolcefalexine salt can result in a stable filming of the crystals of the finished product, preserving it from attack by external agents, may have, by way of example, the following composition:

The salts of methylolcefalexine, filmed or not, can be used for the preparation of pharmaceutical compositions for oral use, in combination with sweeteners, excipients, flavorings and preservatives which can be varied qualitatively and quantitatively, without affecting the antibacterial properties of the finished product.
Four different possible pharmaceutical compositions based on lysine methylolphaphaloxin are described below, purely by way of example.
