Description
Die vorliegende Erfindung betrifft eine Humeruskopfprothese, bestehend im Wesentlichen aus einem Schaft, einem Winkeladapter und einem Kopfteil.
Mit der Entwicklung einer Schulterprothese zur Behandlung von Humeruskopffrakturen, hat der New Yorker Orthopäde, Dr. Charles Neer, im Jahre 1951 den Grundstein für die moderne Schulterprothetik gelegt. Die aus einem Schaft mit darauf festfixiertem Kopf bestehende Prothese, die in mehreren Schaft- und Kopfgrössen erhältlich war, wurde unverändert bis zum Jahre 1973 zur Anwendung gebracht. In jenem Jahr erfolgte die Einführung des "Neer-II"-Systems. Dies bestand aus einem modifizierten Schaft sowie abgeändertem Kopfdesign. Das "Neer-II"-System als unverblockte, kraftschlüs-sige Prothese besteht aus einer Kopfprothese mit integriertem nicht entkoppelbarem Schaftanteil, welche mit verschiedenen Schaftlängen, Schaftdicken und Kopfgrössen vorrätig ist.
Die erste Generation von Neuentwicklungen (Biomed, Richards, Intermedics, DePuy, 3M) weist nun abnehmbare, modulare Kalotten auf, die auf eine Schafthalskomponente aufgesteckt werden. Hierbei handelt es sich um ein modulares Konzept, das durch die aufsteckbare bzw. abnehmbare Kalotte operationstechnische Vorteile bei der Implantation bzw. Revision bildet.
Eine zweite Generation von Neuentwicklungen (Tornier) berücksichtigt durch ein mehrfach modulares System die konstruktiven Eigenheiten des Schultergelenkes. Diese Humeruskopfpro these besteht aus einem 3fach modularen System, zusammengesetzt aus Schaft, Winkeladapter und Kopfkalotte. Die drei Anteile Schaft, Winkeladapter und Kopfkalotte sind in jeweils verschiedenen Dimensionen vorrätig, sodass individuelle Unterschiede der Schaftdicke, des Inklinationswinkels (Schafthalswinkel) sowie Kopfgrösse berücksichtigt werden können. Die Kopfkalotte weist darüber hinaus ein exzentrisches Verankerungssystem auf dem Winkeladapter auf. Dadurch können verschiedene Rotationsstellungen des Kopfes gegenüber dem Winkeladapter eingenommen werden. Dieses Konstruktionselement ermöglicht den interindividuellen unterschiedlichen Humeruskopfmittelpunkt gegenüber der Schaftachse nachzuempfinden.
Durch exzentrisches variables Aufstecken der Kopfkalotte lässt sich dem individuellen mediadorsalen "Offset" des Humeruskopfmittelpunktes gegenüber der Schaftachse Rechnung tragen.
Mit der von der Firma Tornier vorgestellten 3fachen modularen Humeruskopfprothese ist der gegenwärtige Stand der Technik formuliert. Dennoch weist diese Prothese für den Operateur noch konstruktive Mängel auf.
Besonders im Rahmen der Traumatologie des Schultergelenkes und insbesondere bei der Versorgung von veralteten Humeruskopffrakturen ergibt sich das Problem, dass es insbesondere zu Ante- bzw. Retrotorsionsfehlstellungen der Kopfkalotte kommt, das heisst, dass der Kopf einen grösseren Drehfehler gegenüber der Schaftachse aufweist. Ein zweites Problem besteht darin, dass bei, vom Chirurgen nicht einwandfrei durchgeführter Resektion des Humeruskopfes, Ante- bzw. Retrotorsionsfehlstellungen verursacht werden. Dies führt jedoch zu einem Höhenverlust an Knochen und damit zu einer zu tiefen Implantation der Humeruskopfkalotte. Daraus resultiert eine Armabduktionsschwäche, da die Vorspannung für den M. deltoideus geschwächt bzw. verkürzt wird.
Die Aufgabe der vorliegenden Erfindung besteht im Lösen des geschilderten Problemes, indem Änderungen des Ante- bzw. Retorsionswinkels stufenlos möglich sein sollen, die Schulterprothese eine stufenlos verstellbare, exzentrische Einstellung des Humeruskopfes gegenüber der Schafthalsachse der Prothese ermöglicht, und der Inklinationswinkel stufenlos wählbar sein soll, wobei diese Forderungen ohne die Notwendigkeit des Austausches von Teilen der Prothese zu erfüllen sind.
Die erfindungsgemäss gestellte Aufgabe wird mittels einer Humeruskopfprothese gemäss dem Wortlaut insbesondere nach Anspruch 1 gelöst.
Die technische Lösung der Erfindung weist die folgenden konstruktiven Merkmale auf.
1. Vorgeschlagen wird, dass der Winkeladapter (Halselement) im Prinzip fest mit dem Schaft bzw. Schaftteil verbunden ist, jedoch um mindestens eine senkrecht zum Schaftteil verlaufende Drehachse schwenkbar mit dem Schaft verbunden ist, sodass der Winkeladapter in einem Inklinationsbereich von 125-140 DEG stufenlos einstellbar ist resp. um einen mit der Schaftachse eingeschlossenen, spitzen Winkel im Bereich von ca. 40-55 DEG stufenlos schwenkbar und fest mit dem Schaft arretierbar verbindbar ist.
Ein mit dem Winkeladapter integral verbundenes Kopfhalsteil ist dergestalt ausgearbeitet, dass es eine Torsion der Kopfkalotte gegenüber des Schaftes von jeweils 20 DEG um eine Rota tionsachse in der Rotationsebene zulässt. Dies ermöglicht eine stufenlose Antetorsion bzw. Retrotorsion des Humeruskopfes um 20 DEG nach vorn bzw. 20 DEG nach hinten.
2. Das Kopfhalsteil, vorgesehen für die Aufnahme der modularen Humeruskopfkalotte, ist zudem stufenfrei exzentrisch drehbar ausgestaltet, sodass durch eine exzentrische Anordnung der Prothesenkopf stufenfrei über einen Exzenter in einer 360 DEG -Bahn versetzt gegenüber dem Winkeladapter angeordnet werden kann.
3. Der stufenfrei verstellbare Winkeladapter zusammen mit dem Kopfhalsteil, welche zusammen sowohl die Inklination als auch die Retro- und Antetorsion sowie eine exzentrische Rotation des Kopfes zulassen, können durch je einen zentralen Verschlussmechanismus geblockt werden, sodass eine einmal gewählte Position arretiert werden kann. Die auf diese Weise erzeugte dreidimensionale, stufenfreie Einstellbarkeit des Kopfes, verblockbar durch je einen zentralen Verriegelungsmechanismus, ermöglicht, dass nach Inplantation eine stabile Prothesenverklemmung verbleibt.
Weitere bevorzugte Ausführungsvarianten des erfindungsgemässen Humeruskopfes sind in den abhängigen Ansprüchen 2-11 charakterisiert.
Die Erfindung wird nun beispielsweise und unter Bezug auf die beigefügten Figuren näher erläutert. Dabei zeigen:

Fig. 1 zeigt im Längsschnitt, schematisch dargestellt, eine erfindungsgemässe Schulterprothese bzw. eine Humeruskopfprothese 1. Die Prothese umfasst einen Schaft 3, umfassend das untere längsausgebildete Einsteckteil 5, sowie eine obere Partie 7, vorgesehen für das Anordnen des Winkeladapters mit Kopfhalsteil. An der oberen Partie 7 angeordnet ist der erwähnte Winkeladapter 13, welcher über eine Drehachse 11 fest mit dem Schaft verbunden ist. Der Winkeladapter 13 liegt über einer Längspartie 9 auf, welche fest mit der oberen Partie 7 des Schaftes 3 verbunden ist. An einer zum Schaft gegenüberliegenden Auflagefläche 15 des Winkeladapters 13 angeordnet ist das Kopfhalsteil 17, auf welchem aufgesteckt die Humeruskopfkalotte 19 angeordnet ist.
In Fig. 2 ist ausschnittsweise die Verbindung zwischen dem Winkeladapter 13 und dem oberen Schaftende 7 dargestellt, wobei im Vergleich zu Fig. 1 in Fig. 2 der Winkeladapter 13 etwas um die Schwenkachse 11 vom oberen Schaftende 7 weggeschwenkt dargestellt ist. Dadurch wird die mit dem oberen Schaftende 7 fest verbundene schaftähnliche, längsausgebildete Rippe 9 erkennbar, auf welcher je seitlich, diese überlappend, der Winkeladapter 13 angeordnet ist, wie insbesondere aus Fig. 3 erkennbar ist. In dieser Längsrippe, je durch Einkerbungen 10 min vom übrigen Teil getrennt, ist je eine Mittelpartie 10 ausgebildet, zwischen welchen beiden Partien 10 eine keilförmig, einschraubbare Schraube 12 angeordnet ist.
Die mit einer innenseitig sechskantig ausgebildeten \ffnung versehene Schraube 12 treibt beim Einschrauben die beiden Mittelpartien 10 auseinander, sodass der Winkeladapter 13 mit den beiden je seitlich die Rippe 9 überlappenden Partien 13a (siehe Fig. 4c) klemmend fest arretiert wird.
Dies wird dadurch möglich, dass durch den Winkeladapter hindurchverlaufend eine \ffnung 22 vorgesehen ist, durch welche hindurch mittels eines Sechskantschlüssels die Schraube 12 ein- oder ausgeschraubt werden kann.
In Fig. 4a ist der Winkeladapter 13 in Obendraufsicht auf die Auflagefläche 15 dargestellt, auf welcher aufliegend das Kopfhalsteil 17 angeordnet wird. Dabei ist nun deutlich die \ffnung 22 erkennbar, durch welche hindurch mittels des erwähnten Sechskantschlüssels die Schraube 12 betätigbar ist.
Die Auflagefläche 15 ist konvex ausgebildet, wie der Seitenschnitt in Fig. 4c zeigt, d.h. die mittige Längsachse 15 min der Auflagefläche 15 ist gegenüber den seitlichen Begrenzungen der Auflagefläche vorstehend ausgebildet. Durch dieses konvexe Ausbilden der Auflagefläche wird es nun möglich, dass das Kopfhalsteil 17 je seitlich von der Längsmittelachse der Auflagefläche 15 des Winkeladapters 13 verschwenkt aufliegend angeordnet werden kann. Die Auflagefläche 27 des Kopfhalsteils 17 ist, wie Fig. 5 zeigt, ihrerseits, im Gegensatz zur Auflagefläche 15 des Winkeladapters 13, eben bzw. kreisrund flach ausgebildet.
Von der Auflagefläche 27 des Kopfhalsteils 17 vorstehend ist ein kugelartiges Gelenkteil 31 angeordnet, vorgesehen, um in einer weiteren \ffnung 21 des Winkeladapters 13 eingesteckt angeordnet zu werden. Durch Betätigen einer Längsschraube 23 in einem Gewinde 25 des Winkeladapters 13, kann der Gelenkkopf 31 in der \ffnung 21 festklemmend arretiert werden. Durch die exzentrische Anordnung des Gelenkkopfes 31 an der Auflagefläche 27 des Kopfhalsteils 17 ist es möglich, das Teil 17 exzentrisch, d.h. zentrumsversetzt, in Bezug auf die Auflagefläche des Winkeladapters 13 anzuordnen.
Schlussendlich weist das Kopfhalsteil 17 einen mittigen Körper 29 auf, welcher dazu vorgesehen ist, um eine Humeruskopfkalotte 19 zu tragen. Diese Kalotte kann in verschiedenen Ausführungen, angepasst an die Anforderungen bzw. Dimensionen des zu ersetzenden Schultergelenkes auf dem Kopfhalsteil, angeordnet werden.
In den Fig. 6a und 6b ist schematisch das Spannprinzip dargestellt, um das Kopfhalsteil 17 mit dem Gelenkkopf 31, eingespannt im Winkeladapter 13, fest zu blockieren. Dabei zeigt Fig. 6a im Schnitt das Kopfhalsteil 17 zusammen mit der Kopfkalotte 19, angeordnet auf dem Winkeladapter 13, währenddem Fig. 6b den Ausschnitt A aus Fig. 6a vergrössert zeigt.
Dabei zeigt Fig. 6b vergrössert den in der \ffnung 21 des Winkeladapters 13 eingeführten Gelenkkopf 31, wobei insbesondere die Frontseite 31 der Längsschraube 23 und die in der \ffnung gegenüberliegende Auflagefläche für das Aufliegen des Gelenkkopfes 31 dergestalt ausgebildet ist, dass der eingeführte Gelenkkopf 31 beim Einspannen durch Einschrauben der Längsschraube 23 und durch Erzeugen der in Längsrichtung des Winkeladapters 13 wirkenden Kraft F1 durch die Versetzung V des Kugelgelenkes an den beiden Partien 32 bzw. 32a anliegt bzw. eingespannt wird. Somit wird an den Partien 32 bzw. 32a die Spannkraft F2 erzeugt, um den kugelförmigen Gelenkkopf 31 in der \ffnung 21 des Winkeladapters 13 festzuklemmen. Damit resultiert die Spannkraft F3, mittels welcher das Kopfhalsteil 17 in der vorgegebenen Einstellung blockiert wird.
In den Fig. 7-9 schematisch dargestellt sind die verschiedenen Verstellungsmöglichkeiten der erfindungsgemäss vorgeschlagenen Humeruskopfprothese. Fig. 7 zeigt die Möglichkeit des Einstellens eines unterschiedlichen Inklinationswinkels, durch Schwenken des Winkeladapters 13 um die Drehachse 11. Dabei zeigt Position 13 den Winkeladapter in Ausgangsposition, beim Setzen des Schaftes im Oberarmknochen. Durch Schwenken des Winkeladapters 13 um die Achse 11 kann dieser in die gewünschte Position 13 min geschwenkt werden und mittels der Schraube 12 fest arretiert positioniert werden. In der Regel ist es ausreichend, wenn der Winkeladapter 13 um einen Winkel alpha von ca. 15 DEG verstellbar gegenüber dem Schaft angeordnet werden kann. Durch das Wegschwenken des Winkeladapters 13 in Position 13 min wird auch die Kalotte 19 in die gestrichelte Position 19 min weggeschwenkt.
in Fig. 8 nun wird die Verstellbarkeit um einen Torsionswinkel b (beispielsweise von je 20 DEG ) schematisch dargestellt, welche Torsionsbewegung durch das konvexe Ausbilden der Auflagefläche 15 des Winkeladapters 13 ermöglicht wird. Dabei zeigt wiederum die ausgezogene Position 19 die Ausgangslage, beim Setzen der Prothese. Durch torsionsmässiges Verschwenken des Kopfes 19 kann dieser entweder in die gestrichelt dargestellte Position 19 min min oder in die gestrichelte Position 19 min min min weggeschwenkt werden, wobei die Arretierung in der entsprechenden Position mittels der Schraube 23 im Winkeladapter 13 erfolgt.
Fig. 9 schlussendlich zeigt in Obendraufsicht das Kopfteil 19, welches durch das exzentrische Anordnen des Kugelgelenkes 31 in der Auflagefläche 15 des Winkeladapters 13 zentrumsversetzt angeordnet werden kann. Dabei zeigt Fig. 9 erneut die Ausgangsposition 19 und drei weitere, mögliche, gestrichelt dargestellte Positionen 19<IV>-19<VI>. Die Punkte 20<IV>-20<VI> zeigen dabei die entsprechenden Mittelachsen des Humeruskopfes 19 bzw. 19<IV>-19<VI>. Erneut erfolgt die Arretierung in der entsprechenden Position durch Festklemmen mittels der Schraube 23.
In den Fig. 1-9 ist ein Beispiel einer Humeruskopfprothese dargestellt, welche selbstverständlich in x-beliebiger Art und Weise abgeändert, variiert oder modifiziert werden kann. Wesentlich dabei ist, dass beim Setzen der Prothese sowohl ein stufenloses Verändern des Inklinationswinkels, eine stufenlose Torsionsbewegung sowie ein stufenloser Zentrumsversatz möglich ist und die Prothese bzw. die verschiedenen Teile in der einmal eingestellten Position fest arretiert werden kann (bzw. können). Auch wird in den Beispielen nicht auf die Materialwahl der Prothese eingegangen, da die für die Prothese verwendeten Materialen an sich bestens aus dem Stand der Technik bekannt sind.
Bekannt ist auch die unterschiedliche Materialwahl, beispielsweise für das Herstellen des Humeruskopfes selbst, welcher beispielsweise aus Metall, einer Legierung oder aus einem Keramikmaterial hergestellt werden kann.
The present invention relates to a humeral head prosthesis, consisting essentially of a shaft, an angle adapter and a head part.
With the development of a shoulder prosthesis for the treatment of humeral head fractures, the New York orthopedist, Dr. Charles Neer laid the foundation stone for modern shoulder prosthetics in 1951. The prosthesis, which consisted of a shaft with a head firmly attached to it and was available in several shaft and head sizes, was used unchanged until 1973. That year the "Neer-II" system was introduced. This consisted of a modified shaft and a modified head design. The "Neer-II" system as an unblocked, non-positive prosthesis consists of a head prosthesis with an integrated, uncouplable shaft portion, which is available with different shaft lengths, shaft thicknesses and head sizes.
The first generation of new developments (Biomed, Richards, Intermedics, DePuy, 3M) now has removable, modular domes that are attached to a stem neck component. This is a modular concept which, thanks to the clip-on or detachable calotte, provides operational advantages during implantation or revision.
A second generation of new developments (Tornier) takes into account the structural characteristics of the shoulder joint through a multi-modular system. This humeral head prosthesis consists of a 3-fold modular system, composed of a shaft, angled adapter and a spherical cap. The three parts of shaft, angle adapter and head cap are available in different dimensions, so that individual differences in shaft thickness, inclination angle (shaft neck angle) and head size can be taken into account. The spherical cap also has an eccentric anchoring system on the angle adapter. As a result, different rotational positions of the head relative to the angle adapter can be assumed. This construction element enables the inter-individual different center of the humeral head to be reproduced in relation to the shaft axis.
The eccentric variable attachment of the calotte allows the individual mediadorsal "offset" of the center of the humeral head with respect to the shaft axis to be taken into account.
The current state of the art is formulated with the 3-fold modular humeral head prosthesis presented by Tornier. Nevertheless, this prosthesis still has structural defects for the surgeon.
Particularly in the context of the traumatology of the shoulder joint and in particular in the treatment of outdated humeral head fractures, the problem arises that the calotte is particularly ante or retrotorsionally misaligned, which means that the head has a larger rotational error with respect to the shaft axis. A second problem is that malformations of the ante or retrotorsion are caused when the humeral head is not properly resected by the surgeon. However, this leads to a loss of height in the bones and thus to a too deep implantation of the humeral head cap. This results in an arm abduction weakness because the preload for the deltoid muscle is weakened or shortened.
The object of the present invention is to solve the described problem by making it possible to continuously change the ante or retorsion angle, the shoulder prosthesis allows an infinitely adjustable, eccentric adjustment of the humeral head relative to the shaft neck axis of the prosthesis, and the inclination angle should be infinitely variable, these requirements being met without the need to replace parts of the prosthesis.
The object of the invention is achieved by means of a humeral head prosthesis according to the wording, in particular according to claim 1.
The technical solution of the invention has the following design features.
1. It is proposed that the angle adapter (neck element) is in principle firmly connected to the shaft or shaft part, but is pivotably connected to the shaft about at least one axis of rotation running perpendicular to the shaft part, so that the angle adapter is in an inclination range of 125-140 ° is infinitely adjustable. can be steplessly swiveled around an acute angle enclosed with the shaft axis in the range of approx. 40-55 ° and securely connected to the shaft in a lockable manner.
A head neck part integrally connected to the angle adapter is designed in such a way that it allows torsion of the spherical cap relative to the shaft of 20 ° in each case about a rotation axis in the plane of rotation. This enables a stepless antetorsion or retrotorsion of the humeral head by 20 ° to the front or 20 ° to the rear.
2. The head neck part, which is intended to accommodate the modular humeral head cap, is also designed to be steplessly eccentrically rotatable, so that an eccentric arrangement of the prosthesis head can be arranged steplessly via an eccentric in a 360 ° path relative to the angle adapter.
3. The steplessly adjustable angle adapter together with the head neck part, which together allow inclination as well as retro and antetorsion as well as an eccentric rotation of the head, can be blocked by a central locking mechanism so that a selected position can be locked. The three-dimensional, stepless adjustability of the head generated in this way, which can be blocked by a central locking mechanism, enables a stable prosthesis jam to remain after implantation.
Further preferred embodiments of the humeral head according to the invention are characterized in the dependent claims 2-11.

1 shows a longitudinal section, shown schematically, of a shoulder prosthesis or a humeral head prosthesis 1 according to the invention. The prosthesis comprises a shaft 3, comprising the lower longitudinal insertion part 5, and an upper part 7, provided for arranging the angle adapter with a head neck part. Arranged on the upper part 7 is the aforementioned angle adapter 13, which is firmly connected to the shaft via an axis of rotation 11. The angle adapter 13 lies over a longitudinal section 9 which is firmly connected to the upper section 7 of the shaft 3. The head neck part 17, on which the humeral head cap 19 is arranged, is arranged on a bearing surface 15 of the angle adapter 13 opposite the shaft.
2 shows a section of the connection between the angle adapter 13 and the upper shaft end 7, the angle adapter 13 being shown pivoted away from the upper shaft end 7 somewhat about the pivot axis 11 in FIG. 2 compared to FIG. 1. As a result, the shaft-like, longitudinally shaped rib 9, firmly connected to the upper shaft end 7, can be seen, on each of which the angle adapter 13 is arranged laterally, overlapping them, as can be seen in particular from FIG. 3. In this longitudinal rib, each separated from the remaining part by notches for 10 minutes, a central part 10 is formed, between which two parts 10 a wedge-shaped screw 12 is arranged.
When the screw is screwed in, the screw 12 provided with a hexagonal opening on the inside drives the two central parts 10 apart, so that the angle adapter 13 with the two parts 13a (see FIG. 4c) overlapping the rib 9 is firmly locked in place.
This is made possible by the fact that an opening 22 is provided which extends through the angle adapter and through which the screw 12 can be screwed in or out by means of a hexagon wrench.
4a, the angle adapter 13 is shown in a top view of the support surface 15, on which the head neck part 17 is arranged. The opening 22 is now clearly recognizable, through which the screw 12 can be actuated by means of the hexagon wrench mentioned.
The contact surface 15 is convex, as the side section in Fig. 4c shows, i.e. the central longitudinal axis 15 min of the support surface 15 is formed above the lateral boundaries of the support surface. This convex design of the support surface makes it possible for the head neck part 17 to be arranged so that it lies pivoted laterally from the longitudinal central axis of the support surface 15 of the angle adapter 13. The bearing surface 27 of the head neck part 17 is, as shown in FIG. 5, in turn, in contrast to the bearing surface 15 of the angle adapter 13, flat or circularly flat.
A spherical joint part 31 is arranged above the support surface 27 of the head neck part 17, provided in order to be inserted in a further opening 21 of the angle adapter 13. By actuating a longitudinal screw 23 in a thread 25 of the angle adapter 13, the joint head 31 can be locked in the opening 21 by clamping. The eccentric arrangement of the joint head 31 on the contact surface 27 of the head neck part 17 makes it possible to eccentrically, i.e. to be offset from the center with respect to the support surface of the angle adapter 13.
Finally, the head neck part 17 has a central body 29 which is provided in order to carry a humeral head cap 19. This cap can be arranged in different versions, adapted to the requirements or dimensions of the shoulder joint to be replaced on the head neck part.
6a and 6b, the clamping principle is shown schematically in order to firmly block the head neck part 17 with the joint head 31, clamped in the angle adapter 13. 6a shows in section the head neck part 17 together with the spherical cap 19, arranged on the angle adapter 13, while FIG. 6b shows the detail A from FIG. 6a enlarged.
6b shows an enlarged view of the joint head 31 inserted in the opening 21 of the angle adapter 13, the front side 31 of the longitudinal screw 23 and the bearing surface opposite in the opening being designed for the bearing of the joint head 31 such that the inserted joint head 31 during clamping by screwing in the longitudinal screw 23 and by generating the force F1 acting in the longitudinal direction of the angle adapter 13 due to the displacement V of the ball joint against the two parts 32 and 32a. Thus, the clamping force F2 is generated at the parts 32 and 32a in order to clamp the spherical joint head 31 in the opening 21 of the angle adapter 13. This results in the clamping force F3, by means of which the head neck part 17 is blocked in the predetermined setting.
The various adjustment options of the humeral head prosthesis proposed according to the invention are shown schematically in FIGS. 7-9. 7 shows the possibility of setting a different inclination angle by pivoting the angle adapter 13 about the axis of rotation 11. Position 13 shows the angle adapter in the starting position when the shaft is placed in the upper arm bone. By pivoting the angle adapter 13 about the axis 11, it can be pivoted into the desired position for 13 minutes and can be positioned firmly locked by means of the screw 12. As a rule, it is sufficient if the angle adapter 13 can be arranged so as to be adjustable relative to the shaft by an angle alpha of approximately 15 °. By pivoting away the angle adapter 13 in position 13 min, the cap 19 is also pivoted away into the dashed position 19 min.
8, the adjustability by a torsion angle b (for example of 20 ° in each case) is shown schematically, which torsion movement is made possible by the convex formation of the contact surface 15 of the angle adapter 13. The extended position 19 again shows the starting position when the prosthesis is placed. By pivoting the head 19 in torsion, it can be swiveled away either into the position shown in dashed lines for 19 minutes or into the dashed position for 19 minutes, with locking in the corresponding position by means of screw 23 in angle adapter 13.
Finally, FIG. 9 shows a top view of the head part 19, which can be arranged offset in the center by the eccentric arrangement of the ball joint 31 in the contact surface 15 of the angle adapter 13. 9 again shows the starting position 19 and three further possible positions 19 <IV> -19 <VI>, shown in broken lines. Points 20 <IV> -20 <VI> show the corresponding central axes of the humeral head 19 and 19 <IV> -19 <VI>. The locking is again carried out in the corresponding position by clamping with the screw 23.
An example of a humeral head prosthesis is shown in FIGS. 1-9, which of course can be changed, varied or modified in any desired manner. It is essential that when the prosthesis is set, both a stepless change of the inclination angle, a stepless torsion movement and a stepless center offset are possible and the prosthesis or the various parts can (or can) be firmly locked in the position once set. The examples also do not go into the choice of materials for the prosthesis, since the materials used for the prosthesis are known per se from the prior art.
The different choice of materials is also known, for example for the manufacture of the humeral head itself, which can be made of metal, an alloy or a ceramic material, for example.
