Die Erfindung betrifft eine verstellbare Gelenkuntermatratze für ein Spital- oder Pflegebett, mit einem Grundrahmen und einer Liegefläche, die zumindest in vier Teile unterteilt ist und einen Rückenteil, einen Gesässteil, einen Oberschenkelteil und einen Unterschenkelteil umfasst, und bei welcher der Oberschenkelteil mit dem Gesässteil und der Unterschenkelteil mit dem Oberschenkelteil gelenkig verbunden ist.
Derartige Gelenkuntermatratzen können in neu gefertigten Spezialbetten für Spital- oder Heimpflege integriert, oder auch nachträglich in herkömmliche Betten eingebaut werden. Ihre Liegefläche lässt sich manuell oder motorisch verstellen.
In letzer Zeit wurden hinsichtlich einfacher und leichter Bedienung durch die liegende Person oder das Pflegepersonal, aber auch in bezug auf medizinische und sicherheitstechnische Aspekte vermehrt hohe Anforderungen formuliert, die zu wesentlichen Weiterentwicklungen zwingen. Dabei müssen jedoch auch die Kosten, die bekanntermassen im Gesundheitswesen ein grosses Problem darstellen, möglichst niedrig gehalten werden.
Eine wesentliche Aufgabe der Erfindung ist es, eine Gelenkuntermatratze der eingangs beschriebenen Art zu schaffen, die in Einteilung und Funktion den neusten medizinischen und pflegerischen Anforderungen genügt. Dazu gehört die Möglichkeit, Rücken- und Beinteil je nach Pflegefall entweder gemeinsam oder getrennt einstellen zu können. Bei gemeinsamem Verstellen von Rücken- und Beinteil soll dabei der Oberschenkelteil ab einer bestimmten Neigung des Rückenteils wieder abgesenkt werden, damit in jeder Position eine bequeme Stellung der sitzenden Person gewährleistet ist. Damit soll speziell auch erreicht werden, dass das bei konventionellen Elektrobetten vorkommende, beim Patienten unangenehme Angstgefühle auslösende Zusammendrücken von Oberkörper gegen Oberschenkel verhindert wird.
Zu den sicherheitstechnischen Aufgaben zählt eine selbsttätige Entkopplung des Antriebsorgans im Falle eines im Gestänge eingeklemmten Körperteils sowie eine jederzeit mögliche Schnellabsenkung des Rückenteils in medizinisch indizierten Notfallsituationen.
Überdies ist es aus Kostengründen ein wesentliches Ziel der Erfindung, alle erforderlichen Lagen der Liegefläche mit nur einem einzigen Antriebsaggregat einstellen zu können.
Die erfindungsgemässe Lösung dieser Aufgaben ergibt sich aus dem kennzeichnenden Teil des unabhängigen Patentanspruchs 1, bevorzugte Ausführungsbeispiele sind in den abhängigen Patentansprüchen definiert.
Im folgenden wird anhand der beiliegenden Zeichnungen ein Ausführungsbeispiel einer erfindungsgemässen Gelenkuntermatratze beschrieben.

Die Fig. 1a und 1b dienen zur Erklärung des Aufbaus der Gelenkuntermatratze. Etwas erhöht über einem Grundrahmen 2 positioniert ist die Liegefläche mit einem Rückenteil 4, einem Gesässteil 6, einem Oberschenkelteil 8 und einem Unterschenkelteil 10. Der Gesässteil 6 ist mit Trägern 12 an den beiden seitlichen Längsholmen des Grundrahmens 2 fest angebracht. Der Oberschenkelteil 8 ist am Gesässteil 6 im Lager 14 gelenkig gelagert und liegt in tiefster Stellung auf einem Querholm 16 des Grundrahmens auf. An den Oberschenkelteil 8 ist im Lager 18 der Unterschenkelteil 10 angelenkt, der in tiefster Stellung auf am Grundrahmen 2 angebrachten Stützen 20 abgestützt ist und mit einem Schwenkhebel 22 mit dem Grundrahmen 2 verbunden ist.
Der Rückenteil 4 ist in einer vom Grundrahmen 2 nach unten ragenden Stützplatte 24 gelagert mittels einer im Lager 26 am Rückenteil 4 und im Lager 28 an der Stützplatte befestigten Wippe 30 und liegt in tiefster Stellung auf am Grundrahmen 2 angebrachten Stützen 32 auf.
Im Grundrahmen 2 ist quer zu dessen Längsholmen eine Grundachse 34 drehbar gelagert. Auf dieser Grundachse 34 lagert eine frei drehbare Platte, die in ihrer Funktion einen dreiarmigen Winkelhebel 36 bildet. Weiter ist mit der Grundachse 34 ein Rückenteilhebelarm 38 drehfest verbunden, an welchem im Lager 40 eine schwenkbare Klinke 42 angeordnet ist.
Am einen Hebelarm 36a des dreiarmigen Winkelhebels 36 ist ein Lager 44 zur Aufnahme der einen Seite eines Antriebsmotors 46 angeordnet. Am zweiten Hebelarm 36b des Winkelhebels 36 greift die Klinke 42 an und verbindet in der in Fig. 1a und 1b gezeigten Stellung den Rückenteilhebelarm 38 kraftschlüssig mit dem Winkelhebel 36. Am dritten Hebelarm 36c des Winkelhebels 36 ist das eine Ende einer teleskopartigen Schubstange 48 im Lager 50 angelenkt.
Die Schubstange 48 ist an ihrem anderen Ende mit einem Beinteilhebelarm 52 im Lager 54 gelenkig angebracht, der über eine Querstange 56 fest mit dem Oberschenkelteil 8 verbunden ist. Die Schubstange 48 weist einen als Führungsrohr 48a dienenden Teil und eine in diesem Führungsrohr entlang der Längsachse verschiebbare Stange 48b auf. Eine Sperre 58, die am Führungsrohr 48a ausschwenkbar befestigt ist, kann in eine Aussparung 60 in der Stange 48b einrasten und so die Stange 48b gegen weiteres Einfahren in das Führungsrohr 48a versperren.
Zum Hochheben des Rückenteils 4 in eine geneigte Stellung dient ein Kniehebel 62, der am einen Ende drehfest mit der Grundachse 34 verbunden ist und am anderen Ende im Lager 64 am Rückenteil 4 angelenkt ist.
Ein weiterer Hebelarm, der Matratzenausgleichhebelarm 66, ist ebenfalls drehfest an der Grundachse 34 angebracht, und ein Verbindungshebel 68 reicht vom Lager 70 im Matratzenausgleichhebelarm 66 zum Lager 72 in der Wippe 30.
Zur einfacheren Betätigung der Klinke 42 ist im mit der Grundachse 34 verbundenen Teil 62a des Kniehebels 62 eine Ausklinkstange 74 parallel zur Grundachse 34 drehbar gelagert. Die Ausklinkstange 74 ist im Bereich der Klinke 42 kurbelförmig so abgewinkelt, dass sie um den Arm der Klinke 42 läuft, und weist einen fest mit ihr verbundenen Hebel 76 auf, an welchem ein rohrförmiger Bügel 78 angelenkt ist. In den rohrförmigen Bügel 78 ist eine Stange 80 lose eingetaucht, die an ihrem anderen Ende im Kopfbereich des Rückenteils 4 befestigt ist und auf der der Bügel 78 geführt verschoben werden kann.
Anhand der Fig. 2 bis 5 sollen nun die verschiedenen Funktionen des Verstellmechanismus der Gelenkuntermatratze erläutert werden.
Mit dem Antriebsmotor 46 können der Rückenteil 4 und die Ober- und Unterschenkelteile 8 und 10 angehoben beziehungsweise gesenkt werden und zwar entweder gemeinsam, oder auch nur der eine oder der andere Teil, je nach Stellung der Klinke 42 und der Sperre 58.
In den Fig. 1, 2 und 3 greift die Klinke 42 am plattenförmigen Winkelhebel 36 an, und die Sperre 58 ist in der Stange 48b der Schubstange 48 eingerastet. Stösst nun der Antriebsmotor 46 am Hebelarm 36a des Winkelhebels 36, so dreht sich dieser um die Grundachse 34. Durch den Kraftschluss über die Klinke 42 am zweiten Hebelarm 36b des Winkelhebels 36 wird der Rückenteilhebelarm 38, und mit diesem die Grundachse 34 mitgedreht. Da der Kniehebel 62 fest mit der Grundachse 34 verbunden ist, wird dieser nach oben ausgeschwenkt, und damit der Rückenteil 4 in eine geneigte Stellung gebracht (vgl. Fig. 2). Gleichzeitig mit dem Anheben des Rückenteils 4 stösst der dritte Hebelarm 36c des Winkelhebels 36 die Schubstange 48 in Richtung des Fussendes, womit der Beinteilhebelarm 52 ausgeschwenkt und die Ober- und Unterschenkelteile 8 und 10 angehoben werden.
Der Unterschenkelteil 10 wird dabei durch den Schwenkhebel 22 in eine leicht gegen das Fussende geneigte Lage gebracht.
Durch weiteres Ausfahren des Antriebsmotors 46 lässt sich der Rückenteil 4 in eine beinahe senkrecht geneigte Stellung bringen, wie in Fig. 3 zu sehen. Die Ober- und Unterschenkelteile 8 und 10 jedoch werden nur bis zu einer bestimmten Neigung des Rückenteils 4, bevorzugt etwa bis zu einer Neigung von 70 Grad, weiter angehoben und bei noch stärkerer Neigung des Rückenteils 4 gleichzeitig wieder gesenkt. Dies wird dadurch erreicht, dass die gegenseitige Winkellage des zweiten und dritten Hebelarms 36b und 36c des Winkelhebels 36 so gewählt ist, dass die Längsachse des Hebelarms 36c dann mit der Längsachse der Schubstange 48 einen gestreckten Winkel bildet, wenn der Rückenteil 4 eine Neigung von etwa 70 Grad einnimmt, und demzufolge bei weiterer Neigung des Rückenteils 4 die Schubstange 48 wieder in Richtung Kopfende zurückweichen lässt.
Diese Funktionsweise sorgt bei jeder Neigung des Rückenteils 4 für eine bequeme Stellung der sitzenden Person und verhindert ein zu starkes Anwinkeln der Oberschenkel gegen den Bauch.
Eine weitere Funktion des Verstellmechanismus in einer bevorzugten Ausführungsform ist der sogenannte Matratzenausgleich. Dieser hat die Aufgabe, bei geneigtem Rückenteil 4 den Raum für die Matratze im Gesässbereich so zu vergrössern, dass die Dicke der geknickten Matratze ausgeglichen wird und die sitzende Person nicht gegen das Fussende verschoben wird. Wie in Fig. 2 und 3 zu sehen, wird durch die Drehung der Grundachse 34 ebenfalls der fest mit ihr verbundene Matratzenausgleichhebelarm 66 mitgedreht, der über den Verbindungshebel 68 die Wippe 30 in Richtung Kopfende schwenkt und so das untere Ende des Rückenteils 4 in Abhängigkeit von dessen Neigung vom Gesässteil 6 wegbewegt.
Fig. 4 zeigt eine Stellung der Gelenkuntermatratze, bei welcher der Rückenteil 4 geneigt ist, der Ober- und Unterschenkelteil 8 und 10 jedoch flach liegt. Für diese Funktionsweise wird die Sperre 58 einfach aus der Aussparung 60 in der Stange 48b ausgeschwenkt. Die beiden Teile Führungsrohr 48a und Stange 48b der Schubstange 48 sind damit frei gegeneinander verschiebbar und entkoppeln Ober- und Unterschenkelteil 8 und 10 vom Winkelhebel 36.
Die umgekehrte Funktionsweise mit angehobenem Ober- und Unterschenkelteil 8 und 10, jedoch flach liegendem Rückenteil 4, zeigt Fig. 5. Hier ist durch Ausschwenken der Klinke 42 der Rückenteilhebelarm 38 vom Winkelhebel 36 entkoppelt, sodass dieser auf der Grundachse 34 frei drehen kann, ohne auf den Rückenteil 4 zu wirken.
Diese beiden Entkoppelungsvorrichtungen bieten neben den verschiedenen Einstellmöglichkeiten für die Stellungen der Gelenkuntermatratze auch sicherheitstechnische Vorteile. Sollte nämlich während des Absenkens von Rückenteil oder Ober- und Unterschenkelteil einmal ein Körperteil, beispielsweise eine Hand, zwischen irgendwelchen Teilen eingeklemmt werden, so wird durch Ausklinken der Sperre 58 oder der Klinke 42 der Kraftschluss mit dem zurückfahrenden Antriebsmotor 46 selbsttätig gelöst, womit gravierendere Verletzungen (oder auch eine Beschädigung der Gelenkuntermatratze) verhindert werden.
Die mögliche rasche Entkopplung speziell des Rückenteils 4 entspricht auch aus pflegerischer Sicht einer wichtigen Anforderung. In gewissen kritischen medizinischen Situationen muss der Oberkörper des Patienten so schnell abgesenkt werden, dass ein motorisches Absenken zu langsam wäre. In einem solchen Fall ist es einer Pflegeperson möglich, durch Ausklinken der Klinke 42 von Hand eine sofortige Absenkung zu erreichen. In einer bevorzugten Ausführungsform der Erfindung ist dazu die Ausklinkstange 74 vorgesehen, die vom Kopfbereich aus mit dem Bügel 78 über den Hebel 76 gedreht werden kann, um die Klinke 42 auszuklinken.
Eine bevorzugte Konstruktionsvariante des plattenförmigen dreiarmigen Winkelhebels 36 veranschaulichen die Fig. 6a, 6b und 6c. In dieser Variante ist der Winkelhebel aus drei Einzelteilen zusammengesetzt, die bevorzugt durch Stanzen gefertigt werden können. Fig. 6a zeigt die Form des einen Teils 82, Fig. 6b die Form der beiden anderen, gleichartigen Teile 84. Die drei Teile lassen sich auch erst im Verlauf der Montage der Gelenkuntermatratze zum Winkelhebel 36 zusammenfügen. Dies bietet den Vorteil, dass die Grundachse 34 nicht in den Winkelhebel 36 eingefahren werden muss, sondern dass die Teile zuerst einzeln über die Grundachse 34 gesteckt und anschliessend im Lager 50 und in den Befestigungspunkten 86 und 88 zur Deckung gebracht und miteinander verbunden werden können. Die Grundachse 34 sorgt dabei für die Zentrierung der Einzelteile.
Fig. 6c zeigt die Schmalseite des montierten Winkelhebels 36.
Es sei ausdrücklich erwähnt, dass die beschriebene und in den Zeichnungen dargestellte Art der Gelenkuntermatratze vom Fachmann in mehrfacher Weise im Rahmen des Erfindungsgedankens abgewandelt werden kann. So ist es zum Beispiel möglich, den Schwenkhebel 22 durch einen Teleskopsteller zu ersetzen, um eine zusätzliche Verstellmöglichkeit für die Ober- und Unterschenkelteile zu schaffen. Und selbstverständlich lässt sich die Gelenkuntermatratze auch mit einem oder mehreren zusätzlichen Antriebsmotoren ausrüsten, um weitere Möglichkeiten für getrennte Einstellungen der einzelnen Teile der Liegefläche bereitzustellen.
The invention relates to an adjustable joint mattress for a hospital or nursing bed, with a base frame and a lying surface, which is divided into at least four parts and comprises a back part, a seat part, a thigh part and a lower leg part, and in which the thigh part with the seat part and the lower leg part is articulated to the thigh part.
Such joint base mattresses can be integrated into newly manufactured special beds for hospital or home care, or can also be retrofitted into conventional beds. Your lying surface can be adjusted manually or by motor.
In recent times, high demands have been formulated with regard to simple and easy operation by the person lying down or the nursing staff, but also with regard to medical and safety-related aspects, which force significant further developments. However, the costs, which are known to be a major problem in healthcare, must also be kept as low as possible.
An important object of the invention is to provide an articulated base mattress of the type described at the outset which, in terms of its arrangement and function, meets the latest medical and nursing requirements. This includes the option of being able to adjust the back and leg sections either together or separately, depending on the case of care. When the back and leg sections are adjusted together, the thigh section should be lowered again from a certain inclination of the back section, so that a comfortable position of the seated person is ensured in every position. In particular, this is intended to prevent the compression of the upper body against the thigh, which occurs in conventional electric beds and causes unpleasant feelings of fear in the patient.
The safety-related tasks include automatic decoupling of the drive element in the event of a body part being trapped in the linkage, as well as rapid lowering of the back part at any time in medically indicated emergency situations.
Furthermore, for cost reasons, it is an essential aim of the invention to be able to set all the necessary positions of the lying surface with only a single drive unit.
The achievement of these tasks according to the invention results from the characterizing part of independent patent claim 1, preferred exemplary embodiments are defined in the dependent patent claims.
An exemplary embodiment of a joint mattress under the invention is described below with reference to the accompanying drawings.

1a and 1b serve to explain the structure of the joint mattress. Positioned slightly above a base frame 2, the lying surface is provided with a back part 4, a seat part 6, a thigh part 8 and a lower leg part 10. The seat part 6 is fixedly attached to the two side longitudinal bars of the base frame 2 with supports 12. The thigh part 8 is articulated on the seat part 6 in the bearing 14 and lies in the lowest position on a cross member 16 of the base frame. The lower leg part 10 is articulated to the thigh part 8 in the bearing 18, which is supported in the lowest position on supports 20 attached to the base frame 2 and is connected to the base frame 2 by a pivoting lever 22.
The back part 4 is supported in a support plate 24 projecting downward from the base frame 2 by means of a rocker 30 fastened in the bearing 26 on the back part 4 and in the bearing 28 on the support plate and lies in the lowest position on supports 32 attached to the base frame 2.
A base axis 34 is rotatably mounted in the base frame 2 transversely to its longitudinal spars. A freely rotatable plate, which in its function forms a three-armed angle lever 36, is supported on this basic axis 34. Furthermore, a back part lever arm 38 is non-rotatably connected to the base axis 34, on which a pivotable pawl 42 is arranged in the bearing 40.
A bearing 44 for receiving one side of a drive motor 46 is arranged on a lever arm 36a of the three-armed angle lever 36. The pawl 42 engages the second lever arm 36b of the angle lever 36 and, in the position shown in FIGS. 1a and 1b, non-positively connects the back part lever arm 38 to the angle lever 36. On the third lever arm 36c of the angle lever 36 there is one end of a telescopic push rod 48 in the bearing 50 hinged.
The push rod 48 is articulated at its other end with a leg lever arm 52 in the bearing 54, which is fixedly connected to the thigh part 8 via a cross bar 56. The push rod 48 has a part serving as a guide tube 48a and a rod 48b which can be displaced in the guide tube along the longitudinal axis. A lock 58, which is pivotally attached to the guide tube 48a, can snap into a recess 60 in the rod 48b and thus block the rod 48b against further entry into the guide tube 48a.
A toggle lever 62 is used to lift the back part 4 into an inclined position, which is connected in a rotationally fixed manner to the base axis 34 at one end and is articulated in the bearing 64 on the back part 4 at the other end.
Another lever arm, the mattress compensation lever arm 66, is also attached to the base axis 34 in a rotationally fixed manner, and a connecting lever 68 extends from the bearing 70 in the mattress compensation lever arm 66 to the bearing 72 in the rocker 30.
For easier actuation of the pawl 42, a release rod 74 is rotatably mounted parallel to the base axis 34 in the part 62a of the toggle lever 62 connected to the base axis 34. The release rod 74 is cranked in the region of the pawl 42 so that it runs around the arm of the pawl 42, and has a lever 76 firmly connected to it, to which a tubular bracket 78 is articulated. A rod 80 is loosely immersed in the tubular bracket 78, which is fastened at its other end in the head region of the back part 4 and on which the bracket 78 can be moved in a guided manner.
The various functions of the adjusting mechanism of the joint base mattress will now be explained with reference to FIGS. 2 to 5.
With the drive motor 46, the back part 4 and the upper and lower leg parts 8 and 10 can be raised or lowered, either together, or only one or the other part, depending on the position of the pawl 42 and the lock 58.
1, 2 and 3, the pawl 42 engages the plate-shaped angle lever 36, and the lock 58 is engaged in the rod 48b of the push rod 48. If the drive motor 46 bumps against the lever arm 36a of the angle lever 36, it rotates about the base axis 34. The back part lever arm 38, and with it the base axis 34, is rotated as a result of the frictional engagement via the pawl 42 on the second lever arm 36b of the angle lever 36. Since the toggle lever 62 is firmly connected to the base axis 34, it is swung out upward, and thus the back part 4 is brought into an inclined position (cf. FIG. 2). Simultaneously with the lifting of the back part 4, the third lever arm 36c of the angle lever 36 pushes the push rod 48 in the direction of the foot end, whereby the leg part lever arm 52 is pivoted out and the upper and lower leg parts 8 and 10 are raised.
The lower leg part 10 is brought into a position slightly inclined towards the foot end by the pivot lever 22.
By extending the drive motor 46 further, the back part 4 can be brought into an almost vertically inclined position, as can be seen in FIG. 3. The upper and lower leg parts 8 and 10, however, are only raised further up to a certain inclination of the back part 4, preferably up to an inclination of 70 degrees, and at the same time lowered again with an even greater inclination of the back part 4. This is achieved in that the mutual angular position of the second and third lever arms 36b and 36c of the angle lever 36 is selected such that the longitudinal axis of the lever arm 36c then forms an extended angle with the longitudinal axis of the push rod 48 when the back part 4 has an inclination of approximately 70 degrees, and consequently, if the back part 4 is further inclined, the push rod 48 can move back towards the head end.
This function ensures that the seated person is in a comfortable position with every inclination of the back part 4 and prevents the thighs from being bent too strongly against the stomach.
Another function of the adjustment mechanism in a preferred embodiment is the so-called mattress compensation. This has the task of increasing the space for the mattress in the buttocks area with the back part 4 inclined so that the thickness of the kinked mattress is compensated for and the seated person is not moved against the foot end. As can be seen in FIGS. 2 and 3, the rotation of the base axis 34 also rotates the mattress compensation lever arm 66 which is firmly connected to it and which pivots the rocker 30 towards the head end via the connecting lever 68 and thus the lower end of the back part 4 as a function of whose inclination moves away from the seat part 6.
Fig. 4 shows a position of the joint lower mattress, in which the back part 4 is inclined, but the upper and lower leg parts 8 and 10 lie flat. For this mode of operation, the lock 58 is simply pivoted out of the recess 60 in the rod 48b. The two parts of the guide tube 48a and the rod 48b of the push rod 48 can thus be freely moved relative to one another and decouple the upper and lower leg parts 8 and 10 from the angle lever 36.
The reverse mode of operation with raised upper and lower leg parts 8 and 10, but with the back part 4 lying flat, is shown in FIG. 5. Here, by pivoting the pawl 42, the back part lever arm 38 is decoupled from the angle lever 36, so that it can rotate freely on the basic axis 34 without to act on the back part 4.
In addition to the various setting options for the positions of the joint base mattress, these two decoupling devices also offer safety-related advantages. If, during the lowering of the back or upper and lower leg part, a body part, for example a hand, gets trapped between any parts, the latch 58 or the pawl 42 will automatically release the frictional connection with the retracting drive motor 46, causing serious injuries (or damage to the joint mattress) can be prevented.
The possible rapid decoupling especially of the back part 4 also corresponds to an important requirement from a nursing point of view. In certain critical medical situations, the patient's upper body must be lowered so quickly that lowering the motor would be too slow. In such a case, it is possible for a caregiver to achieve an immediate lowering by disengaging the pawl 42 by hand. In a preferred embodiment of the invention, the release rod 74 is provided for this purpose, which can be rotated from the head region with the bracket 78 via the lever 76 in order to release the pawl 42.
A preferred constructional variant of the plate-shaped three-arm angle lever 36 is illustrated in FIGS. 6a, 6b and 6c. In this variant, the angle lever is composed of three individual parts, which can preferably be manufactured by stamping. FIG. 6a shows the shape of one part 82, FIG. 6b the shape of the two other, similar parts 84. The three parts can also only be joined together to form the angle lever 36 during the assembly of the joint base mattress. This offers the advantage that the base axis 34 does not have to be inserted into the angle lever 36, but rather that the parts can first be inserted individually over the base axis 34 and then covered in the bearing 50 and in the fastening points 86 and 88 and connected to one another. The basic axis 34 ensures the centering of the individual parts.

It should be expressly mentioned that the type of joint base mattress described and shown in the drawings can be modified by the person skilled in the art in several ways within the scope of the inventive concept. For example, it is possible to replace the swivel lever 22 with a telescopic actuator in order to create an additional adjustment option for the upper and lower leg parts. And, of course, the articulated base mattress can also be equipped with one or more additional drive motors to provide further options for separate settings of the individual parts of the lying surface.
