BESCHREIBUNG
Die Erfindung geht aus von einer zweiteiligen Kupplung zum Austausch von flüssigen Medien, mit einem ersten und einem zweiten Kupplungsteil mit je einem Durchflusskanal, wobei mit jedem Durchflusskanai von aussen der Kupplungsteile ein Anschlussschlauch dichtend verbunden ist, und einer Verwendung derselben.
Eine solche zweiteilige Kupplung wird insbesondere in einer Anordnung zum Ausführen eines ambulanten dialytischen Entzuges von durch geschädigte Merken eines Patienten ausgeschiedenen Stoffwechselprodukten eingesetzt. In einem solchen Fall wird aus der Bauchhöhle des Patienten verbrauchtes Dialysat abgeführt und nachher in dieselbe frisches Dialysat eingeführt.
Eine ambulante peritoneale Dialyse kann von einem nierengeschädigten Patienten selber ohne Unterbrechung während 24 Stunden pro Tag und 7 Tage in der Woche ausgeführt werden. Die Heimdialyse wird in Zyklen von etwa 6 Stunden ausgeführt. Das in die Bauchhöhle des Patienten aus einem Beutel einzuführende frische Dialysat hat ein Volumen von meistens zwei Litern. Das verbrauchte Dialysat wird in einen leeren Beutel abgeführt. Durch die Heimdialyse werden von den geschädigten Nieren des Patienten ausgeschiedene Wechselstoffsubstanzen aus dem Körper des Patienten entfernt, wobei er sich während den Wechselzyklen seiner üblichen täglichen Tätigkeit widmen kann.
In der Anordnung zum Ausführen einer peritonealen Heimdialyse transportieren der Katheter (üblicherweise Tenckhoff-Katheter) und der angeschlossene Anschlussschlauch das verbrauchte Dialysat in einer Richtung und das frische Dialysat in der Gegenrichtung. Die Kupplung ist in dieser Anordnung zwischengeschaltet. Der Funktionsablauf ist immer so, dass beim Dialysatwechsel zuerst das verbrauchte Dialysat aus der Bauchhöhle des Patienten abfliesst und nachher über das gleiche Katheter- und Schlauchsystem frisches Dialysat in umgekehrter Richtung einfliesst.
Es ist a priori anzunehmen, dass die Luft ausserhalb der Kupplung kontaminiert ist, während die im Katheter und in der Kupplung befindlichen Medien als steril angenommen werden.
Um das Problem der Verbreitung einer Kontamination durch Bakterienkeime in der bei der Heimdialyse verwendeten Vorrichtung zu verhindern, hat die Firma Fresenius AG, Oberurstel, BRD, ein CAPD-Safe-Lock SB entwickelt. Im ersten Kupplungsteil dieser zweiteiligen Kupplung, in welcher der Katheter bzw. sein Verlängerungsstück endet, ist ein Durchflusskanal gebildet, in welchem eine federbelastete Klappe untergebracht ist. Der zweite Kupplungsteil, in welchem sowohl der Schlauch zum Zuführen eines frischen Dialysats als auch der Schlauch zum Anführen des verbrauchten Dialysats untergebracht sind, ist auf den ersten Kupplungsteil aufschraubbar und mittels eines Bajonettsystems arretierbar.
In der ersten Bajonettstellung werden die Anschlussteile durch ein Volumen des frischen Dialysats umgespült, welches dann in den Schlauch für das verbrauchte Dialysat abgeführt wird. In der zweiten Bajonettstellung wird durch den vorstehenden Endteil des Schlauches zum Zuführen des frischen Dialysats die federbelastete Klappe von ihrer Sitzstelle abgedrückt, so dass das verbrauchte Dialysat aus der Bauchhöhle des Patienten in den zuständigen Schlauch abgeführt werden kann; dabei bleibt die Öffnung des Endteiles des Schlauches zum Zuführen des frischen Dialysats durch die Klappe abgeschlossen. In der dritten Bajonettstellung wird die Klappe von der Mündung des Endteiles des Schlauches zum Zuführen des frischen Dialysats abgeklappt und das frische Dialysat kann in den Katheter fliessen.
Bei dieser Anordnung der zweiteiligen Kupplung werden die Kupplungsteile vorgängig zum Auslaufvorgang des verbrauchten Dialysats durch eine kleinere Menge des Frischdialysats umspült. Der Effekt dieser Spülung ist ungenügend.
Es besteht weiter die Gefahr, dass die in der Kupplung beim Ankoppeln eingeschlossene Luft zusammen mit dem frischen Dialysat in die Bauchhöhle des Patienten gelangt.
Des weiteren werden von manchen Patienten vibröse Substanzen ausgeschieden, die mit dem verbrauchten Dialysat abgeführt werden. Diese Substanzen können sich auf der Schraubenfeder und der Klappe absetzen und den Durchflusskanal verstopfen. Dadurch werden die Feder und die Klappe in ihrer Funktion behindert, was dazu führen kann, dass die Klappe den Ausgang aus dem Endteil des Schlauches zum Führen des frischen Dialysats nicht dicht abschliesst.
Der im Patentanspruch 1 angegebenen Erfindung liegt die Aufgabe zugrunde, eine zweiteilige Kupplung zu schaffen, bei welcher die Folgen einer eventuellen Kontamination der sie durchlaufenden Flüssigkeiten ausgeschlossen werden.
Jegliche Aussenluft, die beim Kupplungsvorgang in die Kupplung gelangt, darf nicht weiter in den Katheter oder in die Bauchhöhle des Patienten gelangen und muss mit dem Auslaufen des verbrauchten Dialysats aus der Kupplung vollständig evakuiert werden. Alle festen Teile, die gegebenenfalls vom Patienten ausgeschieden und mit dem verbrauchten Dialysat transportiert werden, sollen mit Sicherheit die Kupplung passieren. Die Bedienung der Kupplung soll insbesondere für behinderte Patienten (Blinde) erleichtert und eine Fehlmanipulation ausgeschlossen werden.
Die Aufgabe wird erfindungsgemäss durch die Merkmale des kennzeichnenden Teiles des Patentanspruches 1 gelöst.
Die zweiteilige Kupplung wird erfindungsgemäss in einer Anordnung zum Ausführen einer peritonealen Heimdialyse bei Patienten mit geschädigten Nieren verwendet.

Die in den Figuren dargestellte zweiteilige Kupplung zum Austausch von flüssigen Medien besteht aus einem ersten 1 und einem zweiten 2 Kupplungsteil, die miteinander verbindbar sind. Der erste Kupplungsteil 1 weist eine erste, einen Innenraum 8 umsehliessende Hülse 6 und ein sich mit- tig im Innenraum 8 erstreckendes, den ersten Durchflusskanal 3 bildendes erstes Rohr 9 auf. Am freien Ende des ersten Rohres 9 befindet sich eine Öffnung 9a für die Flüssigkeit.
Der zweite Kupplungsteil 2 umfasst eine zweite Hülse 7, die mittig eine Längsbohrung 10 und einen sich an dieselbe anschliessenden Innenraum 11 umschliesst. In diesem Innenraum ist ein elastisch nachgiebiges Mittel 12 untergebracht, das aus einem länglichen Kunststoff- oder Kautschukpfropfen mit einer in demselben eingesetzten Schraubenfeder 17 besteht. Die zweite Hülse 7 kann aus einem vom Kunststoffteil 26 umspritzten Titanteil 25 bestehen.
Der zweite Durchflusskanal 4 im zweiten Kupplungsteil 2 ist durch einen im Querschnitt U-förmigen Trog 19 und ein in den Trog 19 mündendes zweites Rohr 20 gebildet. Der offene Abschnitt des Troges 19 ist von dem Pfropfen 12 abgedeckt. Der Pfropfen 12 besitzt einen konisch auslaufenden Teil 12a, der im offenen Abschnitt des Troges 19 führbar ist.
Der Pfropfen 12 stützt sich einerseits gegen einen Ansatz 18 der Hinterwand der zweiten Hülse 7 ab und ist in Richtung seiner Längsachse zentrierbar (durch seinen auslaufenden Teil 12a) zusammendrückbar.
In der Umfangsfläche der zweiten Hülse 7 (in der dargestellten Ausführungsform in dem Titanteil 25) ist eine im wesentlichen schraubenlinienförmige Steuernut 13 mit zwei Rasterpunkten 14, 14a und Rückdrehsperren 15 eingearbeitet.
Die Steuernut 13 hat in radialer Richtung variierende Abstände vom Zentrum (siehe Fig. 13). Die zwei Rasterpunkte 14, 14a liegen im nächsten Abstand zum Zentrum, wobei die Auslauflinie der Steuernut 13 am weitesten vom Zentrum liegt. An der Innenwand der ersten Hülse ist ein Nocken 16 vorgesehen, der zum Eingreifen in die Steuernut 13 und zum Einrasten in den Rasterpunkten 14, 14a bestimmt ist. Es können mehrere Rasterpunkte vorgesehen sein.
Der Aussendurchmesser der zweiten Hülse 7 ist kleiner als der Innendurchmesser der ersten Hülse 6 und der Innendurchmesser der Längsbohrung 10 ist grösser als der Aus sendurchmesser des ersten Rohres 9. Durch diese Massnahme wird ein teilweises Einführen des zweiten Kupplungsteiles 2 in den ersten Kupplungsteil 1 ermöglicht. Durch das Verdrehen des eingeführten Kupplungsteiles 2 im Kupplungsteil 1 im Uhrzeigersinn wird dann das Einrasten des Nockens 16 in den Rasterpunkten 14 ermöglicht. In dem gekoppelten Zustand der beiden Kupplungsteile 1,2 verläuft der zweite Durchflusskanal 4 zu dem ersten Durchflusskanal 3 exzentrisch.
Durch Verdrehen des zweiten Teiles um annähernd 90 wird der Nocken 16 im ersten Rasterpunkt 14 einrasten.
Durch das Verdrehen des zweiten Teiles 2 um annähernd 1800 im Uhrzeigersinn wird der Nocken 16 im zweiten Rasterpunkt 14a einrasten. Vor dem Weiterdrehen des zweiten Kupplungsteiles 2 im Uhrzeigersinne muss der zweite Kupplungsteil 2 axial nach innen gedrückt werden, um durch die Überwindung der zwangsläufigen Deformation der ersten Hülse 6 an dieser Stelle das weitere Verdrehen des zweiten Kupplungsteiles 2 zu ermöglichen. Durch die Rückdrehsperren 15 wird eine Rückdrehsicherung gewährleistet. Die Form der Steuernut 13 gewährleistet, dass mit dem Nocken 16 nicht in falscher Richtung in die Nut eingefahren werden kann.
In den Fig. 3 bis 5 sind die beiden Kupplungsteile 1, 2 im ungekoppelten Zustand dargestellt. In den Fig. 6 bis 8 sind die beiden Kupplungsteile 1, 2 im gekoppelten Zustand dargestellt, wobei der Nocken 16 im ersten Rasterpunkt 14 einrastet. In dieser Stellung wird der Pfropfen 12 in Richtung der Hinterwand der zweiten Hülse 7 zurückgedrückt, so dass die Flüssigkeit über den ersten Durchflusskanal 3 und die Öffnung 9a im ersten Rohr 9 in den zweiten Durchflusskanal 4 oder umgekehrt gelangen kann. Der zurückgedrückte Pfropfen 12 füllt den gesamten Innenraum 11 des zweiten Kupplungsteiles 2 auf. Dabei nimmt seine Länge ab und gleichzeitig vergrössert sich der Querschnitt des zus ammen- gedrückten Pfropfens 12.
Der Querschnitt des Innenraumes 11, in dem sich der Pfropfen 12 befindet, entspricht auf seiner gesamten Länge dem jeweiligen Querschnitt des auf diese Stelle zusammengedrückten Pfropfens 12. Dadurch ist der Innenraum 11 durch den Pfropfen 12 hermetisch abgedichtet. Dadurch wird auch das vorgängig in diesem Raum befindliche Medium (Flüssigkeit oder Luft) in den zweiten Durchflusskanal 4 evakuiert.
In den Fig. 9 bis 11 ist dann die Phase dargestellt, wenn der Nocken 16 in den zweiten Rasterpunkt 14 einrastet. In dieser Stellung wird der Pfropfen 12 in seine ursprüngliche Länge zurückgebracht und die Verbindung des ersten Durchflusskanals 3 mit dem zweiten Durchflusskanal 4 unterbrochen.
In der Umfangsfläche der zweiten Hülse 7 ist in der Nähe der Steuernut 13 mindestens ein äusserer Dichtungsring 21 eingesetzt. In der Innenwand der Längsbohrung 10 ist mindestens ein innerer Dichtungsring 22 eingesetzt. Diese Ringe 21, 22 bilden einen weiteren Sicherheitsfaktor gegen die Kontamination der Kupplung durch Bakterienkeime. Einen weiteren Sicherheitsfaktor stellt ein Desinfektionsüberzug an der Innenwand der ersten Hülse 6 und der Aussenwand des ersten Rohres 9 sowie an der Aussenwand der zweiten Hülse dar, der als Bakteriensperre wirkt.
Die freien Endteile der ersten Hülse 6 überragen das freie Endteil des ersten Rohres 9 mit der Öffnung 9a. Das heisst, dass im gekoppelten Zustand der beiden Kupplungsteile 1, 2 diese freien Endteile den äusseren 21 sowie den inneren 22 Dichtungsring überdecken.
Jeder Kupplungsteil 1, 2 besitzt eine Platte 23, 23, die von aussen an demselben angeordnet ist. Beim Einführen des zweiten Kupplungsteiles 2 in den ersten Kupplungsteil 1 liegen die beiden Platten 23, 24 einander diametral gegenüber.
Im gekoppelten Zustand der beiden Kupplungsteile 1, 2, beim Verdrehen des zweiten Teiles 2 um annähernd 90#, liegen diese Platten 23, 24 senkrecht zueinander. Beim weiteren Verdrehen des zweiten Teiles 2 im Uhrzeigersinn um annähernd weitere 900 liegen sie dann in einer einzigen Ebene.
Diese Anordnung dient insbesondere den Behinderten (blinden Patienten). Die gegenseitige Position der beiden Platten 23, 24 ist durch Betasten einfach feststellbar. Beim Einführen des zweiten Kupplungsteiles 2 in den ersten Kupplungsteil 1 müssen die beiden Platten einander diametral gegenüberliegen. Beim Verdrehen des zweiten Kupplungsteiles 2 im ersten Kupplungsteil 1 im Uhrzeigersinne um annähernd 900 stehen die Platten 23, 24 senkrecht zueinander; in dieser für den Patienten eindeutig erkennbaren Stellung kann die Flüssigkeit die beiden Durchflusskanäle 3,4 durchfliessen. Beim weiteren Verdrehen des zweiten Kupplungsteiles 2 um annähernd weitere 900 wird der Durchfluss der Flüssigkeit durch die Durchflusskanäle 3,4 unterbunden; in dieser Stellung kann die Kupplung mit ihren Platten 23,24 am Bauch des Patienten anliegen.
In der Fig. 16 ist eine Anordnung zum Ausführen einer peritonealen Heimdialyse bei Patienten mit geschädigten Nieren dargestellt, in welchem die zweiteilige Kupplung eingesetzt ist. In dieser Figur ist mit 27 ein in der Bauchhöhle des Patienten eingelegter Katheter, mit 5 ein mit dem Katheter 27 verbundener Anschlussschlauch, mit 1, 2 die beiden Kupplungsteile, mit 5 der andere Anschlussschlauch, mit 28 ein Gerät zum Klemmen und Trennen des Anschlussschlauches 5, mit 29 ein Dreiwegverteiler, mit 30 der Zuführungsschlauch des frischen Dialysats aus einem Beutel 31 und mit 32 der Abführungsschlauch des verbrauchten Dialysats in den Beutel 33 bezeichnet.
Die von den geschädigten Nieren des Patienten ausgeschiedenen Stoffwechselprodukte werden aus der Bauchhöhle des Patienten durch das verbrauchte Dialysat abgeführt, wonach in die Bauchhöhle des Patienten in umgekehrter Richtung frisches Dialysat eingeführt wird.
Die Durchflusskanäle 3, 4 und die Anschlussschläuche 5 haben über ihre gesamten Längen einen gleichbleibenden lichten Durchmesser. Geht man von einem konstanten Durchmesser von 3 mm im Tenckhoff-Katheter aus, so ha ben die Durchflusskanäle 3,4 4 in der Kupplung den gleichen Durchlass von 3 mm. Dadurch bleibt die Auslaufgeschwindigkeit des verbrauchten Dialysats überall konstant, was eine wichtige Voraussetzung dafür ist, dass alle Festkörper, die im Tenckhoff-Katheter transportiert werden in gleicher Weise durch die Kupplung hindurch weitergeführt werden.
Des weiteren wird dadurch Luft, die während des Kupplungsvorganges in die Durchflusskanäle 3, 4 gelangt, mit dem ausfliessenden, verbrauchten Dialysat zuverlässig evakuiert.
Durch die Elastizität des Pfropfens 12 wird gewährleistet, dass er in jedem Funktionszustand gegen die Wände des schlitzförmigen Innenraumes 11 anliegt. Dadurch wird verhindert, dass in den Durchflusskanal 4 gelangende Luft auch in andere Teile der Kupplung dringen kann; dabei erfüllt der elastische Pfropfen 12 zwei Funktionen, nämlich erstens die Funktion des Federelementes und zweitens die Funktion des Abdichtungselementes.
Die erste Hülse 6 des ersten Kupplungsteiles 1 hat dreierlei Funktionen:
1. Sie verhindert durch ihre Länge ein gefährliches Berühren des ersten Rohres 9,
2. sie ist mit dem Nocken 16 versehen, welcher in die Steuernut 13 des zweiten Kupplungsteiles 2 im gekoppelten Zustand der Kupplung eingreift und
3. im gekoppelten Zustand dichtet sie die Kupplung hermetisch gegen die Umwelt.
Für die Bedienung der Kupplung durch blinde Patienten ist ein Anzeigesystem in Form der Platten 23, 24 vorgesehen.
Da die Kupplung in der gesicherten Position auf dem Körper des Patienten getragen werden muss, ist bei den anderen Positionen die Lage der Platten 23, 24 so, dass sie beim Tragen sehr unangenehm auffallen und somit den Patienten warmen.
Ein weiterer Vorteil der Kupplung besteht darin, dass durch die Verwendung derselben zusammen mit dem Gerät 28 zum Klemmen und Trennen des Anschlussschlauches, welcher Gegenstand des US-Patentes Nr. 4 676 476 ist, die Gefahr einer Kontamination beim An- und Abkuppeln erheblich verringert wird.
Ein grosser Vorteil der Kupplung besteht darin, dass bei geöffneter Kupplung der Querschnitt des zweiten Innenraumes 11 auf seiner gesamten Länge dem Querschnitt des bei geöffneter Kupplung zusammengedrückten elastisch nachgiebigen Mittels 12 entspricht, wobei dann die Kupplung im Prinzip nur noch aus dem Kanal 3, 9a, 4 besteht.
DESCRIPTION
The invention is based on a two-part coupling for the exchange of liquid media, with a first and a second coupling part, each with a flow channel, a connection hose being sealingly connected to each flow channel from the outside of the coupling parts, and use thereof.
Such a two-part coupling is used in particular in an arrangement for performing an outpatient dialytic withdrawal of metabolic products excreted by damaged memorization of a patient. In such a case, used dialysate is removed from the patient's abdominal cavity and subsequently introduced into the same fresh dialysate.
Outpatient peritoneal dialysis can be carried out by a kidney-damaged patient 24 hours a day, 7 days a week without interruption. Home dialysis is carried out in cycles of about 6 hours. The fresh dialysate to be introduced into the patient's abdominal cavity from a sachet usually has a volume of two liters. The used dialysate is discharged into an empty bag. Home dialysis removes metabolic substances excreted from the damaged kidneys of the patient from the patient's body, whereby he can devote himself to his usual daily activities during the change cycles.
In the arrangement for performing peritoneal home dialysis, the catheter (usually Tenckhoff catheter) and the connected connecting tube transport the used dialysate in one direction and the fresh dialysate in the opposite direction. The clutch is interposed in this arrangement. The functional sequence is always such that when the dialysate is changed, the used dialysate flows out of the patient's abdominal cavity and then fresh dialysate flows in in the opposite direction via the same catheter and tube system.
It is a priori to assume that the air outside the coupling is contaminated, while the media in the catheter and in the coupling are assumed to be sterile.
In order to prevent the problem of the spread of contamination by bacterial germs in the device used for home dialysis, the company Fresenius AG, Oberurstel, FRG, has developed a CAPD Safe-Lock SB. A flow channel is formed in the first coupling part of this two-part coupling, in which the catheter or its extension piece ends, in which a spring-loaded flap is accommodated. The second coupling part, in which both the hose for supplying a fresh dialysate and the hose for feeding the used dialysate are accommodated, can be screwed onto the first coupling part and can be locked by means of a bayonet system.
In the first bayonet position, the connection parts are washed around by a volume of the fresh dialysate, which is then discharged into the hose for the used dialysate. In the second bayonet position, the projecting end part of the hose for supplying the fresh dialysate pushes the spring-loaded flap away from its seat, so that the used dialysate can be discharged from the patient's abdominal cavity into the relevant hose; the opening of the end part of the tube for feeding the fresh dialysate through the flap remains closed. In the third bayonet position, the flap is folded down from the mouth of the end part of the tube for supplying the fresh dialysate and the fresh dialysate can flow into the catheter.
With this arrangement of the two-part coupling, the coupling parts are washed around by a smaller amount of the fresh dialysate prior to the expiry of the used dialysate. The effect of this flushing is insufficient.
There is also a risk that the air trapped in the coupling during coupling, together with the fresh dialysate, will get into the patient's abdominal cavity.
Furthermore, some patients excrete vibrating substances that are removed with the used dialysate. These substances can deposit on the coil spring and the flap and clog the flow channel. As a result, the spring and the flap are hindered in their function, which can result in the flap not sealing the exit from the end part of the hose for guiding the fresh dialysate.
The invention specified in claim 1 is based on the object of providing a two-part coupling in which the consequences of possible contamination of the liquids passing through it are excluded.
Any outside air that gets into the coupling during the coupling process must not get further into the catheter or into the patient's abdominal cavity and must be completely evacuated when the used dialysate leaks out of the coupling. All solid parts that may be excreted by the patient and transported with the used dialysate should pass through the coupling with certainty. The operation of the coupling is to be made easier especially for disabled patients (blind people) and incorrect manipulation is to be excluded.
According to the invention, the object is achieved by the features of the characterizing part of patent claim 1.
According to the invention, the two-part coupling is used in an arrangement for performing peritoneal home dialysis in patients with damaged kidneys.

The two-part coupling shown in the figures for the exchange of liquid media consists of a first 1 and a second 2 coupling part which can be connected to one another. The first coupling part 1 has a first sleeve 6, which surrounds an interior space 8, and a first tube 9, which extends centrally in the interior space 8 and forms the first flow channel 3. At the free end of the first tube 9 there is an opening 9a for the liquid.
The second coupling part 2 comprises a second sleeve 7, which centrally encloses a longitudinal bore 10 and an inner space 11 adjoining it. In this interior, an elastically resilient means 12 is accommodated, which consists of an elongated plastic or rubber plug with a helical spring 17 inserted therein. The second sleeve 7 can consist of a titanium part 25 overmolded by the plastic part 26.
The second flow channel 4 in the second coupling part 2 is formed by a trough 19 with a U-shaped cross section and a second tube 20 opening into the trough 19. The open section of the trough 19 is covered by the plug 12. The plug 12 has a conically tapering part 12a which can be guided in the open section of the trough 19.
The plug 12 is supported on the one hand against a shoulder 18 of the rear wall of the second sleeve 7 and can be compressed in the direction of its longitudinal axis (by its tapering part 12a).
In the circumferential surface of the second sleeve 7 (in the illustrated embodiment in the titanium part 25) an essentially helical control groove 13 with two grid points 14, 14a and backstops 15 is incorporated.
The control groove 13 has varying distances from the center in the radial direction (see FIG. 13). The two grid points 14, 14a are located at the closest distance to the center, the outlet line of the control groove 13 being the furthest from the center. A cam 16 is provided on the inner wall of the first sleeve and is intended for engaging in the control groove 13 and for engaging in the grid points 14, 14a. Several grid points can be provided.
The outside diameter of the second sleeve 7 is smaller than the inside diameter of the first sleeve 6 and the inside diameter of the longitudinal bore 10 is larger than the outside diameter of the first tube 9. This measure enables the second coupling part 2 to be partially inserted into the first coupling part 1. By rotating the inserted coupling part 2 in the coupling part 1 clockwise, the locking of the cam 16 in the grid points 14 is then made possible. In the coupled state of the two coupling parts 1, 2, the second flow channel 4 runs eccentrically to the first flow channel 3.
By rotating the second part by approximately 90, the cam 16 will snap into the first grid point 14.
By rotating the second part 2 by approximately 1800 in a clockwise direction, the cam 16 will snap into the second grid point 14a. Before turning the second coupling part 2 clockwise, the second coupling part 2 must be pressed axially inward in order to enable the second coupling part 2 to be rotated further at this point by overcoming the inevitable deformation of the first sleeve 6. The backstops 15 ensure a backstop. The shape of the control groove 13 ensures that the cam 16 cannot be inserted into the groove in the wrong direction.
3 to 5, the two coupling parts 1, 2 are shown in the uncoupled state. 6 to 8, the two coupling parts 1, 2 are shown in the coupled state, the cam 16 engaging in the first grid point 14. In this position, the plug 12 is pushed back in the direction of the rear wall of the second sleeve 7, so that the liquid can pass through the first flow channel 3 and the opening 9a in the first tube 9 into the second flow channel 4 or vice versa. The pushed-back plug 12 fills the entire interior 11 of the second coupling part 2. In doing so, its length decreases and at the same time the cross section of the plug 12 pressed together increases.
The cross section of the interior 11, in which the plug 12 is located, corresponds over its entire length to the respective cross section of the plug 12 compressed at this point. As a result, the interior 11 is hermetically sealed by the plug 12. As a result, the medium (liquid or air) previously located in this space is also evacuated into the second flow channel 4.
FIGS. 9 to 11 show the phase when the cam 16 engages in the second raster point 14. In this position, the plug 12 is returned to its original length and the connection of the first flow channel 3 to the second flow channel 4 is interrupted.
At least one outer sealing ring 21 is inserted in the peripheral surface of the second sleeve 7 in the vicinity of the control groove 13. At least one inner sealing ring 22 is inserted in the inner wall of the longitudinal bore 10. These rings 21, 22 form a further safety factor against contamination of the coupling by bacterial germs. A further safety factor is a disinfectant coating on the inner wall of the first sleeve 6 and the outer wall of the first tube 9 and on the outer wall of the second sleeve, which acts as a bacterial barrier.
The free end parts of the first sleeve 6 protrude beyond the free end part of the first tube 9 with the opening 9a. This means that in the coupled state of the two coupling parts 1, 2 these free end parts cover the outer 21 and the inner 22 sealing ring.
Each coupling part 1, 2 has a plate 23, 23 which is arranged on the outside thereof. When the second coupling part 2 is inserted into the first coupling part 1, the two plates 23, 24 lie diametrically opposite one another.
In the coupled state of the two coupling parts 1, 2, when the second part 2 is rotated by approximately 90 #, these plates 23, 24 are perpendicular to one another. When the second part 2 is further rotated clockwise by approximately another 900, they then lie in a single plane.
This arrangement is particularly useful for the disabled (blind patients). The mutual position of the two plates 23, 24 can be easily determined by touching. When the second coupling part 2 is inserted into the first coupling part 1, the two plates must lie diametrically opposite one another. When the second coupling part 2 is rotated clockwise by approximately 900 in the first coupling part 1, the plates 23, 24 are perpendicular to one another; In this position, which is clearly recognizable for the patient, the liquid can flow through the two flow channels 3, 4. When the second coupling part 2 is further rotated by approximately another 900, the flow of the liquid through the flow channels 3, 4 is prevented; In this position, the coupling with its plates 23, 24 can rest on the patient's abdomen.
16 shows an arrangement for carrying out peritoneal home dialysis in patients with damaged kidneys, in which the two-part coupling is inserted. In this figure, 27 is a catheter inserted in the patient's abdominal cavity, 5 is a connecting tube connected to the catheter 27, 1, 2 the two coupling parts, 5 the other connecting tube, 28 is a device for clamping and separating the connecting tube 5 , with 29 a three-way distributor, with 30 the supply hose of the fresh dialysate from a bag 31 and with 32 the discharge hose of the used dialysate in the bag 33.
The metabolic products excreted by the damaged kidneys of the patient are removed from the patient's abdominal cavity through the used dialysate, after which fresh dialysate is introduced in the reverse direction into the patient's abdominal cavity.
The flow channels 3, 4 and the connecting hoses 5 have a constant clear diameter over their entire lengths. Assuming a constant diameter of 3 mm in the Tenckhoff catheter, the flow channels 3,4 4 in the coupling have the same passage of 3 mm. As a result, the outflow speed of the used dialysate remains constant everywhere, which is an important prerequisite for ensuring that all solids that are transported in the Tenckhoff catheter are passed through the coupling in the same way.
Furthermore, air that enters the flow channels 3, 4 during the coupling process is reliably evacuated with the outflowing, used dialysate.
The elasticity of the plug 12 ensures that it bears against the walls of the slot-shaped interior 11 in every functional state. This prevents air entering the flow channel 4 from penetrating into other parts of the coupling; the elastic plug 12 fulfills two functions, namely firstly the function of the spring element and secondly the function of the sealing element.
The first sleeve 6 of the first coupling part 1 has three functions:
1. Due to its length, it prevents dangerous contact with the first tube 9,
2. It is provided with the cam 16, which engages in the control groove 13 of the second coupling part 2 in the coupled state of the coupling and
3. When coupled, it hermetically seals the coupling against the environment.
A display system in the form of plates 23, 24 is provided for the operation of the coupling by blind patients.
Since the coupling must be worn on the patient's body in the secured position, the position of the plates 23, 24 in the other positions is such that they are very uncomfortable when worn and thus warm the patient.
Another advantage of the coupling is that by using it together with the device 28 for clamping and disconnecting the connection hose, which is the subject of US Patent No. 4,676,476, the risk of contamination during coupling and uncoupling is considerably reduced .
A major advantage of the coupling is that when the coupling is open, the cross section of the second interior 11 over its entire length corresponds to the cross section of the elastically flexible means 12 compressed when the coupling is open, the coupling then in principle only coming from the channel 3, 9a, 4 exists.
