BESCHREIBUNG
Die Erfindung betrifft pharmazeutische Zusammensetzungen mit Polypeptid-Wirkstoffen und deren Bestimmung für gastrointestinale Resorption.
Die medizinische Behandlung mit Polypeptiden hat viele Probleme hervorgerufen, da diese Wirkstoffe nach oraler Applikation in der Körperflüssigkeit leicht abgebaut und schlecht resorbiert werden. Aus diesem Grund war die parenterale Verabreichung bis jetzt der einzige übliche Weg für effektive Behandlung. Allgemein üblich ist die Verabreichung als Injektion. Solche Verabreichungsformen sind immer beschwerlich und wenn die Verabreichung in regelmässigen Zeitintervallen geschehen soll, kann sie sehr schmerzhaft für den Patienten sein. Daher ist das Auffinden alternativer akzeptabler Verabreichungsformen, welche dem Patienten weniger Unannehmlichkeiten bereiten und vorzugsweise eine leichte Selbstapplikation ermöglichen, wobei zur gleichen Zeit eine für die effektive klinische Behandlung ausreichende Bioverfügbarkeit erreicht wird, während vieler Jahre ein wichtiges Ziel geblieben.
Es wurde nun gefunden, dass Polypeptide gastrointestinal unter bestimmten erfindungsgemässen Bedingungen besser resorbiert werden können als bisher der Fall war.
Gegenstand der Erfindung sind pharmazeutische Zusammensetzungen mit Polypeptidwirkstoffen, welche, wenn am Hund verabreicht, bis 12 Stunden nach oraler Darreichung eine relative Bioverfügbarkeit von mehr als 120% im Blutplasma aufweisen.
Die relative Bioverfügbarkeit ist vorzugsweise grösser als 300%, speziell grösser als 350%, wie über 1000%, wie aus den beigefügten Beispielen hervorgeht. Dabei ist die relative Bioverfügbarkeit bezogen auf die Bioverfügbarkeit einer gleichen Menge Wirkstoff, ohne die bestimmten erfindungsgemässen Bedingungen, oral eingenommen.
Da die Bioverfügbarkeit durch die anderen Freisetzungsbedingungen des Wirkstoffes nach rektaler Applikation anders ist als nach oraler Applikation, sind ebenfalls Gegenstand der Erfindung, pharmazeutische Zusammensetzungen mit Polypeptid-Wirkstoffen, welche, wenn am Hund verabreicht, bis 12 Stunden nach rektaler Darreichung eine relative Bioverfügbarkeit von mehr als 160% im Blutplasma aufweisen.
Die relative Bioverfügbarkeit ist bei rektaler Anwendung vorzugsweise grösser als 1000%, was aus den beigefügten Beispielen hervorgeht. Die erfindungsgemässen Bedingungen, welche für die Erhöhung der Bioverfügbarkeit bei gastrointestinaler Resorption erforderlich sind, können durch die Zugabe von speziellen Komponenten realisiert werden.
Die pharmazeutischen Zusammensetzungen sind besonders für intestinale Resorption im Darmkanal bestimmt und werden daher vorzugsweise in orale oder rektale Applikationsform gebracht. Die Zusammensetzungen sind daher besonders in Form von Tabletten, Kapseln, drinkbaren Flüssigkeiten, Rektalkapseln, Suppositorien und Mikroklistieren, speziell Tabletten, Kapseln oder Suppositorien.
Die Polypeptid-Wirkstoffe sind vorzugsweise Peptidhormone, wie Somatostatin oder Somatostatinanaloga, oder Calcitonine, sowie weitere Peptidwirkstoffe, wie ACE-Inhibitoren und Renin-Inhibitoren.

Unter Somatostatin-Analoge werden irgendwelche bekannte Oligopeptide mit somatostatin-ähnlicher Wirkung verstanden, deren Strukturen von jener des natürlichen Tetradecapeptides Somatostatin abgewandelt ist, z.B. solche, worin ein oder mehrere Aminosäurereste(n) oder eine oder mehrerer Aminosäuresequenz(en), welche im Naturprodukt vorkommen, weggelassen, ersetzt, umgekehrt oder auf andere Weise deriviert wurde(n) oder worin die N- oder Cendständige Gruppe abgeändert wurde.
Diese Somatostatin-Analoga können bedeutend weniger Aminosäurereste aufweisen als Somatostatin selbst und eine oder mehrere Peptid-Teilsequenzen des Somatostatin-Moleküls in intakter oder derivierter Form enthalten.
Somatostatinanaloga zur Verwendung gemäss der vorliegenden Erfindung sind z. B. jene, die in dem Europäischen Patent Nr. 29 579 (= US Patent 4395 403) beschrieben und beansprucht sind, insbesondere jene, die in den Beispielen dieses Patents spezifisch beschrieben sind.

Eine zweite Gruppe von zur Verwendung gemäss der vorliegenden Erfindung geeigneten Somatostatinanaloga besteht aus jenen, die in dem veröffentlichten Belgischen Patent Nr. 892 315 (= US Patent 4435 385) beschrieben und beansprucht sind, insbesondere jenen, die in den Beispielen 1 bis 20 dieses Patents spezifisch beschrieben sind. Somatostatin und Analoga werden zur Behandlung von Erscheinungen verwendet, welche mit einer übermässigen Ausscheidung von Wachstumshormon zusammenhängen, z. B. zur Behandlung von Diabetes mellitus, von damit zusammenhängender Netzhautentzündung, von Gefässkrankheiten und von Acromegalie.
Somatostatin und Analoga werden ebenfalls gegen gastrointestinale Störungen eingesetzt, z. B. gegen Magengeschwüre, Magen- und Darmblutungen, gegen Entzündungen der Bauchspeicheldrüse und gegen gastrointestinale Hormone abscheidende Tumore. Auch werden die Somatostatinen bei der Behandlung von Hautkrankheiten verwendet, besonders bei Wucherungen und/oder Keratinisierung von Epidermzellen, besonders bei Psoriasis.
Zusätzlich werden Somatostatin und Analoga bei der Behandlung der degenerativen senilen Demenz, bekannt als die senile Demenz vom Alzheimer Typus (SDAT), und bei Migräne eingesetzt.
Die wirksamen täglichen Mengen sind in diesem Fall 2 Mikrogramm bis 10 mg und werden meistens über 2 bis 4 Dosierungen von 0,5 Mikrogramm bis 5 mg parenteral verteilt.
Somatostatin und Analoga werden vorzugsweise zur Hemmung der Wachstumshormonabscheidung verwendet.
Vorzugsweise wird das SMS eingesetzt.
Weitere mögliche Polypeptid-Wirkstoffe sind Calcitonine, die eine bekannte Klasse von pharmazeutisch aktiven langkettigen Polypeptiden mit verschiedenartiger gut dokumentierter pharmakologischer Aktivität darstellen. Mehrere Calcitonine, z. B. Salm-, Human- und Aalcalcitonin (= Elcatonin) sind kommerziell erhältlich und werden allgemein bei der Behandlung der Paget-Krankheit, der Hypercalcaemie und der Osteoporosis verwendet.
Der Begriff Calcitonin wird in breitem Sinne verwendet und soll nicht nur die natürlichen Calcitonine, sondern auch deren pharmakologisch aktive Derivate und Analoga, z.B.
solche, worin ein oder mehrere der in den natürlichen Verbindungen vorkommenden Aminosäurereste ersetzt sind oder das N- oder C-terminale Ende geändert wurde, umfassen.
Als Polypeptid-Wirkstoffe in den Zusammensetzungen der Erfindung sind auch ACE- und Renin-Inhibitoren möglich.
ACE-Inhibitoren und Renin-Inhibitoren sind bekannt, z.B. wurden ACE-Inhibitoren in Am Med. J. 77, 690 (1984), in Cardiovascular Pharmacology , M. Autonaccio, ed.
1984, Seite 119, in den europäischen Patentpublikationen 158927 A, 156455 Aund in Ann. Rep. Med. Chem. 1985, Vol. 20 Kap. 7 beschrieben. Renin-Inhibitoren wurden in Ann. Rep. Med. Chem. 1985 Vol. 20, Kap. 26 und in den europäischen Patentpublikationen 156 321, 156 322, 165 151, 161 588 und 163 237 beschrieben. ACE-Inhibitoren und Renin-Inhibitoren werden hauptsächlich gegen Hypertonie eingesetzt.
Die Polypeptid-Wirkstoffe können in der freien Baseform, in Komplexform oder in pharmazeutisch annehmbare Säureadditionssalzform vorliegen. Die Komplexe und Salze sind bekannt und haben die etwa gleiche Aktivität und Tolerabilität als die freien Baseformen.
Geeignete Säureadditionssalze sind z. B. die Hydrochloride und Acetate.
Die erfindungsgemäss grosse Bioverfügbarkeit der neuen Polypeptid-Wirkstoffe enthaltenden Zusammensetzungen kann durch den Zusatz von bestimmten Komponenten erreicht werden. Gegenstand der Erfindung sind pharmazeutische Zusammensetzungen für gastrointestinale Resorption bestimmt, welche Polypeptide als Wirkstoffe mit a) Monosaccharin oder Zuckeralkoholen und/oder b) Polyoxyalkylenäthern vermischt enthalten.
Durch diese Kombination sind zum ersten Mal - und das ist ebenfalls Gegenstand der Erfindung -, pharmazeuti- sche Zusammensetzungen, welche Somatostatin, besonders ein Analogon, oder ein ACE- oder Renininhibitor enthalten und für orale oder rektale Applikation bestimmt sind, möglich geworden.
Da für orale und rektale Applikation feste Darreichungsformen möglich sind, sind nur feste Darreichungsformen von Somatostatin, besonders ein Analogon, und von ACEund Renininhibitoren auch Gegenstand der Erfindung.
Falls die erfindungsgemässen Zusammensetzungen Somatostatin. besonders Somatostatinanaloga, wie SMS. als Wirkstoffe enthalten, ist deren Menge besonders 100 Mikrogramm bis 35 mg bei oraler und rektaler Anwendung pro Dosierungseinheit.
Als mögliche Komponenten, welche die Bioverfügbarkeit der Polypeptid-Wirkstoffe vergrössern, werden von den Monosacchariden vorzugsweise Glucose, Fructose oder Xylose gewählt, besonders Glucose oder Xylose. Von den Zuckeralkoholen werden vorzugsweise Mannit oder Sorbit, besonders Mannit gewählt.
Die Mengen der Monosaccharide oder deren Zuckeralkohole sind im Falle von festen Darreichungsformen vorzugsweise zwischen 10 und 500 bei oraler Darreichung besonders 400 und bei rektaler Anwendung besonders 50 bis 100 mg pro Dosierungseinheit, im Falle von flüssigen Darreichungsformen, wie Sirupen, 10 mg bis 10 g pro 20 ml Flüssigkeit.
Als mögliche Polyoxyalkylenäther, welche die Bioverfügbarkeit des Polypeptid-Wirkstoffes vergrössern, werden vorzugsweise Polyoxyalkylenäther eines höheren Alkohols, besonders eines höheren Alkanols wie z. B. Lauryl- oder Cetylalkohol oder eines Alkylphenols, oder eines Sterols, besonders Lanosterol, Dihydrocholesterol oder Cholesterol, sowie Mischungen von zwei oder mehreren solchen Äthern bevorzugt. Bevorzugte Polyoxyalkylenäther sind Polyoxyäthylenund Polyoxypropylenäther, besonders Lauryl-, Cetyl- und Cholesterolpolyoxyäthylen- und polyoxypropylenäther, sowie Mischungen von zwei oder mehreren solchen Äthern.
Die Hydroxygruppe am endständigen Alkylenrest dieser o. e. Polyäther kann teilweise oder vollständig acyliert sein, z. B. mit Acylresten aliphatischer Carbonsäuren, wie z. B. Essigsäure.
Bevorzugte Polyäther, verwendbar bei dieser Erfindung haben ein hydrophil-lipophil Verhältnis (HLB-Gruppennummer) von ca. 10 bis ca. 20, speziell von ca. 12 bis ca. 16.
Besonders geeignete Polyäther zur Verwendung bei der Erfindung sind solche, worin der Mittelwert der sich wiederholenden Einheiten im Polyoxyalkylenteil zwischen 4 und 75, besonders zwischen 8 und 30 und ganz besonders zwischen 16 und 26 liegt. Die Polyäther können gemäss bekannten Methoden erhalten werden. Eine grosse Auswahl solcher Produkte stehen kommerziell zur Verfügung und werden z. B. von der Firma Amerchol unter dem Markennamen So lulan, von den Firmen KAO Soap, ICI und Atlas unter den Markennamen Emalex, Brijs und Laureths und von der Firma Croda unter dem Markennamen Cetomacrogols verkauft.
Beispiele für Polyoxyalkylenäther geeignet für Verwendung bei der Erfindung sind z. B. (POE = Polyoxyäthylenäther; POP = Polyoxypropylenäther; x = Mittelwert der sich wiederholenden Einheiten im POE/POP-Teil)! 1. Cholesteryläther:
1.1 Solulan C-24 - POE, x = 24 2. Äther von Lanolinalkoholen: 2.1 Solulan 16 - POE, x = = 16.
2.2 Solulan 25 - POE, x = 25.
2.3 Solulan 75 - POE, x = 75.
2.4 Solulan D'PB-10 - POP, x = 10.
2.5 Solulan E98 - POE, x = 10 - teilweise acetyliert.
2.6 Solulan 97 - POE, x = 9 - vollständig acetyliert.
3. Lauryläther
3.1 Emalex 709/Laureth 9 - POE, x = 9.
3.2 Laureth 4/Brij 630 - POE, x = 4.
3.3 Laureth 23/Brij 335 - POE, x = 23.
4. Cetyläther:
4.1 Cetomacrogol - POE, x = 20 - 24.
Lanolinalkohole sind auch bekannt als Wollfettalkohole und sind ein Gemisch von Cholesterol, Dihydrocholesterol und Lanosterol.
Bevorzugte Polyäther zur Verwendung bei der Erfindung sind Cholesterylpolyoxyäthylenäther, besonders solche Poly äther, worin die Anzahl der sich wiederholenden Einheiten im Polyoxyäthylenteil 16 bis 26, besonders ungefähr 24 beträgt.
Vorzugsweise sind solche Polyäther frei von Verunreinigungen, besonders von anderen Polyoxyalkylenäthern. Bevorzugt enthalten sie mindestens 75%, besonders bevorzugt mindestens 85% und ganz besonders bevorzugt mindestens 90% (Gewicht) des reinen Cholesterylpolyoxyäthyläthers.
Die Mengen der Polyäther sind im Fall von festen Darreichungsformen vorzugsweise zwischen 5 und 200, bei ora ler Darreichung besonders etwa 20 und bei rektaler Anwendung etwa 40 bis 60 mg pro Dosierungseinheit, im Falle von flüssigen Darreichungsformen 5 bis 500 mg Polyäther pro 20 ml Flüssigkeit.
Kombinieren der Monosaccharide oder Zuckeralkoholen mit den Polyäthern hat einen synergistischen Effekt auf die intestinale Resorption der Polypeptide, wie aus den beigefügten Beispielen, besonders Beispiel 2.5 hervorgeht.
Beim Kombinieren dieser Komponenten werden vorzugsweise 10 bis 500, besonders etwa 400 mg Monosaccharid oder Zuckeralkohol mit 5 mg bis 200, besonders etwa 100 mg Polyäther pro fester Dosierungsform oder 10 mg bis 10 g Monosaccharid oder Zuckeralkohol und 5 bis 500 mg Polyäther pro 20 ml orale Flüssigkeit verwendet. Im Falle von Somatostatinen wird das SMS auch bei den erfindungsgemässen galenischen Formen als bevorzugte Verbindung eingesetzt und zwar in bevorzugten Mengen von 100 Mikro gramm bis 35 mg pro orale oder rektale Dosierungseinheit.
Von diesen galenischen Formen werden pro Tag Mengen eingenommen, welche bis etwa 35 mg SMS entsprechen.
Die Polypeptide werden in den erfindungsgemässen galenischen Formen für die gleichen Zwecke verwendet wie oben für die bisher bekannten galenischen Formen beschrieben.
Beispiele
1. Bestimmung der Bioverfügbarkeit von SMS aus Suppositorien
1.1 Es wurden Suppositorien (1,5 g) aus Triglycerid (Wi tepsol) mit einem Wirkstoffgehalt von 5,8 mg SMS rektal (als Essigsäuresalz) an Kaninchen appliziert (Neuseeländer Albino). Jeweils vor der Applikation und zu den Zeitpunkten 15 Min., 30 Min., 1, 2, 3, 4, 5, 6 und 7 Std. nach der Applikation wurden Blutproben aus der Ohrvene entnommen.
Die Wirkstoffkonzentration im Plasma wurde mit einer RIA-Methode bestimmt. Als Vergleich diente eine i.v. Injektion (0,58 mg SMS-acetat). Dabei wurden die folgenden Resultate erzielt:

Bei reaktaler Anwendung wird die Bioverfügbarkeit relativ gesehen auf über 160% erhöht.
1.2 Es wurden Suppositorien mit der gleichen Dosierung wie im obigen Beispiel rektal an Hunden (Beagle) appliziert.
Die Blutentnahme und Plasmaspiegelbestimmung erfolgte wie oben.

2. Bestimmung der Bioverfügbarkeit von SMS aus oral applizierten Formen
2,3 mg SMS (als Acetat) wurden in einer Hartgelatinkapsel oder in 20 ml wässriger Lösung oral an Hunde (Beagle) von 10 kg Körpergewicht appliziert, nachdem sie 20 Stunden gefastet hatten.
Im Falle von Kapseln wurde direkt nach der Applikation 20 ml isotonische wässrige Lösung (NaC1 0,9%) dargereicht.
Jeweils vor der Applikation und zu den Zeitpunkten 20 Min., 40 Min., 1, 1,5, 3, 5 und 7 Stunden nach der Applikation wurden Blutproben aus der Vena saphena entnommen. Das Blut wurde zentrifugiert und die Wirkstoffkonzentration des unveränderten Wirkstoffs im Plasma wurde mit einer RIA-(Radio immuno assay)-Methode bestimmt.
Die relative Bioverfügbarkeit wurde bestimmt durch Vergleich der Oberflächen unter den Plasmakurven (AUC 0-7 Std.)

Die Plasmakurven von Versuchen 2.2.1, 2.2.5, 2.4.1 und 2.5.1 sind zum Vergleich in Fig. 3 aufgenommen.
Die synergistische Wirkung der Kombination von Solulan C 24 mit Glucose ist deutlich erkennbar.
3. Bestimmung der Bioverfügbarkeit von SMS im Jejunum lokal appliziert
Wistarratten mit 300 g Körpergewicht, wurden, nachdem sie 12 Stunden gefastet hatten, mit Urethan narkotisiert, (2 x 0,7 g/kg i. p.). Nach Offnen der Bauchhöhle wurde das Jejunum hervorgehoben, wonach in diesen Körperteil 50 Mikrogramm SMS-Lösung injiziert wurde.
1 ml Blutprobe wurden 20 Min. 1, 2 und 2 Std. nach der Injektion der Vena cava entnommen. Das Blut wurde sofort zentrifugiert und die Wirkstoffkonzentration im Plasma wurde mit einer RIA-Methode bestimmt.
Die relative Bioverfügbarkeit wurde durch Vergleich der Oberflächen unter den Plasmakurven bestimmt (AUC 0-3 Std.).

DESCRIPTION
The invention relates to pharmaceutical compositions with polypeptide active ingredients and their determination for gastrointestinal absorption.
Medical treatment with polypeptides has caused many problems, since these agents are easily broken down in the body fluid after oral administration and poorly absorbed. For this reason, parenteral administration has so far been the only common way for effective treatment. Administration as an injection is common. Such forms of administration are always troublesome and if the administration is to take place at regular time intervals, it can be very painful for the patient. Therefore, finding alternative acceptable forms of administration which are less inconvenient to the patient and preferably enable easy self-administration while at the same time achieving bioavailability sufficient for effective clinical treatment has remained an important goal for many years.
It has now been found that polypeptides can be better absorbed gastrointestinally under certain conditions according to the invention than was previously the case.
The invention relates to pharmaceutical compositions with polypeptide active ingredients which, when administered to the dog, have a relative bioavailability of more than 120% in blood plasma up to 12 hours after oral administration.
The relative bioavailability is preferably greater than 300%, especially greater than 350%, such as over 1000%, as can be seen from the attached examples. The relative bioavailability, based on the bioavailability of an equal amount of active ingredient, without the specific conditions according to the invention, is taken orally.
Since the bioavailability due to the other release conditions of the active ingredient after rectal administration is different from after oral administration, the invention also relates to pharmaceutical compositions with polypeptide active ingredients which, when administered to the dog, have a relative bioavailability of more than 12 hours after rectal administration than 160% in blood plasma.
The relative bioavailability in rectal use is preferably greater than 1000%, as can be seen from the attached examples. The conditions according to the invention, which are necessary for increasing the bioavailability during gastrointestinal absorption, can be achieved by adding special components.
The pharmaceutical compositions are particularly intended for intestinal absorption in the intestinal tract and are therefore preferably brought into oral or rectal administration form. The compositions are therefore particularly in the form of tablets, capsules, drinkable liquids, rectal capsules, suppositories and micro-enemas, especially tablets, capsules or suppositories.
The polypeptide active ingredients are preferably peptide hormones, such as somatostatin or somatostatin analogs, or calcitonins, and further peptide active ingredients, such as ACE inhibitors and renin inhibitors.

Somatostatin analogs are understood to mean any known oligopeptides with somatostatin-like activity, the structures of which have been modified from that of the natural tetradecapeptide somatostatin, e.g. those in which one or more amino acid residues or one or more amino acid sequence (s) which occur in the natural product have been omitted, replaced, reversed or otherwise derived (s) or in which the N- or C-terminal group has been changed.
These somatostatin analogs can have significantly fewer amino acid residues than somatostatin itself and contain one or more peptide partial sequences of the somatostatin molecule in intact or derivative form.
Somatostatin analogs for use in accordance with the present invention are e.g. B. those described and claimed in European Patent No. 29,579 (= US Patent 4,395,403), especially those specifically described in the examples of this patent.

Somatostatin and analogues are used to treat symptoms associated with excessive growth hormone excretion, e.g. B. for the treatment of diabetes mellitus, associated retinal inflammation, vascular diseases and acromegaly.
Somatostatin and analogues are also used against gastrointestinal disorders, e.g. B. against gastric ulcers, gastric and intestinal bleeding, against inflammation of the pancreas and against gastrointestinal hormone-secreting tumors. Somatostatins are also used in the treatment of skin diseases, particularly in the case of growths and / or keratinization of epidermal cells, especially in psoriasis.
In addition, somatostatin and analogs are used in the treatment of degenerative senile dementia, known as senile dementia of the Alzheimer type (SDAT), and in migraines.
The effective daily amounts in this case are 2 micrograms to 10 mg and are usually parenterally divided into 2 to 4 doses of 0.5 microgram to 5 mg.
Somatostatin and analogs are preferably used to inhibit growth hormone secretion.
The SMS is preferably used.
Other possible polypeptide active ingredients are calcitonins, which are a known class of pharmaceutically active long-chain polypeptides with various well-documented pharmacological activity. Several calcitonins, e.g. B. Salm-, Human- and Eelcalcitonin (= Elcatonin) are commercially available and are generally used in the treatment of Paget's disease, hypercalcaemia and osteoporosis.
The term calcitonin is used in a broad sense and is intended not only to refer to natural calcitonins, but also their pharmacologically active derivatives and analogs, e.g.
those in which one or more of the amino acid residues present in the natural compounds are replaced or the N- or C-terminal end has been changed.
ACE and renin inhibitors are also possible as polypeptide active ingredients in the compositions of the invention.
ACE inhibitors and renin inhibitors are known, e.g. ACE inhibitors were described in Am Med. J. 77, 690 (1984), in Cardiovascular Pharmacology, M. Autonaccio, ed.
1984, page 119, in European patent publications 158927 A, 156455 A and in Ann. Rep. Med. Chem. 1985, vol. 20 chap. 7 described. Renin inhibitors have been described in Ann. Rep. Med. Chem. 1985 Vol. 20, chap. 26 and described in European patent publications 156 321, 156 322, 165 151, 161 588 and 163 237. ACE inhibitors and renin inhibitors are mainly used against hypertension.
The polypeptide drugs can be in the free base form, in complex form or in pharmaceutically acceptable acid addition salt form. The complexes and salts are known and have approximately the same activity and tolerance as the free base forms.
Suitable acid addition salts are e.g. B. the hydrochlorides and acetates.
The high bioavailability according to the invention of the compositions comprising new polypeptide active ingredients can be achieved by adding certain components. The invention relates to pharmaceutical compositions for gastrointestinal absorption, which contain polypeptides as active ingredients mixed with a) monosaccharin or sugar alcohols and / or b) polyoxyalkylene ethers.
This combination has made it possible for the first time - and this is also the subject of the invention - pharmaceutical compositions which contain somatostatin, in particular an analogue, or an ACE or renin inhibitor and are intended for oral or rectal administration.
Since solid dosage forms are possible for oral and rectal administration, only solid dosage forms of somatostatin, in particular an analogue, and of ACE and renin inhibitors are also the subject of the invention.
If the compositions according to the invention are somatostatin. especially somatostatin analogues, such as SMS. Contained as active ingredients, the amount is particularly 100 micrograms to 35 mg for oral and rectal use per dosage unit.
The possible components which increase the bioavailability of the polypeptide active substances are preferably glucose, fructose or xylose, in particular glucose or xylose, of the monosaccharides. Of the sugar alcohols, mannitol or sorbitol, especially mannitol, is preferably chosen.
The amounts of the monosaccharides or their sugar alcohols are preferably between 10 and 500 in the case of solid dosage forms, particularly 400 in the case of oral administration and particularly 50 to 100 mg per dosage unit in the case of rectal administration, and 10 mg to 10 g per in the case of liquid dosage forms, such as syrups 20 ml of liquid.
As possible polyoxyalkylene ethers which increase the bioavailability of the polypeptide active ingredient, polyoxyalkylene ethers of a higher alcohol, in particular a higher alkanol such as, for example, B. lauryl or cetyl alcohol or an alkylphenol, or a sterol, especially lanosterol, dihydrocholesterol or cholesterol, and mixtures of two or more such ethers are preferred. Preferred polyoxyalkylene ethers are polyoxyethylene and polyoxypropylene ethers, especially lauryl, cetyl and cholesterol polyoxyethylene and polyoxypropylene ethers, and mixtures of two or more such ethers.
The hydroxyl group on the terminal alkylene radical of this oe. Polyether can be partially or fully acylated, e.g. B. with acyl residues of aliphatic carboxylic acids, such as. B. acetic acid.
Preferred polyethers useful in this invention have a hydrophilic-lipophilic ratio (HLB group number) from about 10 to about 20, especially from about 12 to about 16.
Particularly suitable polyethers for use in the invention are those in which the average value of the repeating units in the polyoxyalkylene part is between 4 and 75, particularly between 8 and 30 and very particularly between 16 and 26. The polyethers can be obtained according to known methods. A large selection of such products are commercially available and z. B. sold by the Amerchol company under the brand name So lulan, by the companies KAO Soap, ICI and Atlas under the brand names Emalex, Brijs and Laureths and by the company Croda under the brand name Cetomacrogols.
Examples of polyoxyalkylene ethers suitable for use in the invention are e.g. B. (POE = polyoxyethylene ether; POP = polyoxypropylene ether; x = mean value of the repeating units in the POE / POP part)! 1. Cholesteryl ether:

Lanolin alcohols are also known as wool fatty alcohols and are a mixture of cholesterol, dihydrocholesterol and lanosterol.
Preferred polyethers for use in the invention are cholesteryl polyoxyethylene ethers, especially those poly ethers in which the number of repeating units in the polyoxyethylene part is 16 to 26, particularly approximately 24.
Such polyethers are preferably free of impurities, especially other polyoxyalkylene ethers. They preferably contain at least 75%, particularly preferably at least 85% and very particularly preferably at least 90% (weight) of the pure cholesteryl polyoxyethyl ether.
The amounts of the polyethers are preferably between 5 and 200 in the case of solid dosage forms, particularly about 20 in the case of oral administration and about 40 to 60 mg per dosage unit in the case of rectal use, and 5 to 500 mg of polyether per 20 ml of liquid in the case of liquid dosage forms.
Combining the monosaccharides or sugar alcohols with the polyethers has a synergistic effect on the intestinal absorption of the polypeptides, as can be seen from the attached examples, particularly example 2.5.
When these components are combined, 10 to 500, especially about 400 mg of monosaccharide or sugar alcohol with 5 mg to 200, particularly about 100 mg of polyether per solid dosage form or 10 mg to 10 g of monosaccharide or sugar alcohol and 5 to 500 mg of polyether per 20 ml are preferably oral Liquid used. In the case of somatostatins, the SMS is also used as the preferred compound in the galenic forms according to the invention, in preferred amounts from 100 micrograms to 35 mg per oral or rectal dosage unit.
Of these galenic forms, amounts are taken per day, which correspond to about 35 mg SMS.
The polypeptides are used in the pharmaceutical forms according to the invention for the same purposes as described above for the pharmaceutical forms known to date.
Examples
1. Determination of the bioavailability of SMS from suppositories
1.1 Suppositories (1.5 g) of triglyceride (Wi tepsol) with an active ingredient content of 5.8 mg SMS were applied rectally (as acetic acid salt) to rabbits (New Zealand albino). Blood samples were taken from the ear vein in each case before the application and at the times 15 minutes, 30 minutes, 1, 2, 3, 4, 5, 6 and 7 hours after the application.
The drug concentration in the plasma was determined using a RIA method. An i.v. Injection (0.58 mg SMS-acetate). The following results were achieved:

2. Determination of the bioavailability of SMS from orally administered forms
2.3 mg of SMS (as acetate) were administered orally to dogs (beagles) of 10 kg body weight in a hard gelatin capsule or in 20 ml of aqueous solution after they had fasted for 20 hours.
In the case of capsules, 20 ml of isotonic aqueous solution (NaC1 0.9%) was administered immediately after application.
Blood samples were taken from the saphenous vein before each application and at 20, 40, 1, 1.5, 3, 5 and 7 hours after application. The blood was centrifuged and the drug concentration of the unchanged drug in the plasma was determined using a RIA (radio immunoassay) method.
The relative bioavailability was determined by comparing the surfaces under the plasma curves (AUC 0-7 hours)

The synergistic effect of the combination of Solulan C 24 with glucose is clearly visible.
3. Determination of the bioavailability of SMS applied locally in the jejunum
300 g body weight Wistar rats were anesthetized with urethane after fasting for 12 hours (2 x 0.7 g / kg i.p.). After opening the abdominal cavity, the jejunum was highlighted, after which 50 micrograms of SMS solution were injected into this part of the body.
1 ml of blood sample was taken 20 minutes 1, 2 and 2 hours after the injection of the vena cava. The blood was centrifuged immediately and the drug concentration in the plasma was determined using a RIA method.
The relative bioavailability was determined by comparing the surfaces under the plasma curves (AUC 0-3 hours).
