REVENDICATIONS
1. Médicament pour le traitement des affections telles que les ulcères gastro-duodénaux, les gastrites et les oesophagites et les troubles cliniques que ces affections engendrent, caractérisé par l'association optimale d'antiacides gastriques, avec un anesthésique de la muqueuse cEso-gastro-duodénale augmentant la rapidité d'action et la durée de l'efficacité.
2. Médicament selon la revendication 1, caractérisé en ce qu'il contient comme antiacides gastriques à effet prolongé: I'aluminium hydroxyde-magnésium carbonate, I'hydroxyde de magnésium et le trisilicate de magnésium.
3. Médicament selon l'une des revendications 1 et 2, caractérisé en ce qu'il contient comme anesthésique de la muqueuse oeso-gastro- duodénale le chlorhydrate de lignocaine.
4. Médicament selon les revendications 2 et 3, caractérisé en ce qu'il est constitué de:
50 parties en poids d'aluminium hydroxyde-magnésium car
bonate,
- 80 parties en poids d'hydroxyde de magnésium,
- 50 parties en poids de trisilicate de magnésium,
- 5 parties en poids de chlorhydrate de lignocaine.
5. Médicament selon les revendications 2 et 3, caractérisé en ce que, pour 400 ml de gel, il contient:
10 10 g d'aluminium hydroxyde-carbonate de magnésie,
- 16 g d'hydroxyde de magnésie,
- 10 g de trisilicate de magnésie,
- 1 g de chlorhydrate de lignocaine.
6. Médicament selon l'une des revendications 1 à 4, caractérisé en ce qu'il se présente sous toute forme pharmaceutique acceptable et, en particulier, sous une forme pour l'administration par voie orale, en comprimé ou en gel.
La présente invention a pour objet un médicament dont le but est de traiter les malades atteints d'oesophagite, de gastrite ou d'ulcère gastro-duodénal.
Il est connu que la durée d'action d'un antiacide gastrique est d'autant plus longue que le pourcentage d'activité libérée au bout de trente minutes est plus faible.
Il est également connu que les brûlures et les douleurs des malades atteints d'oesophagîte et de gastrite ainsi que les douleurs des malades atteints d'ulcère gastrique ou duodénal peuvent être calmées par un anesthésique de contact muqueux.
Il a paru par suite avantageux de réunir dans un médicament des produits possédant cette propriété en même temps que des antiacides gastriques à effet prolongé.
Conformément à l'invention le médicament se compose avantageusement de trois antiacides gastriques à effet prolongé: I'aluminium hydroxyde-magnésium carbonate (co-dried gel), I'hydroxyde de magnésium et le trisilicate de magnésium, associés à un anesthésique de la muqueuse oeso-gastro-duodénale: le chlorhydrate de lignocaine augmentant la rapidité d'action et la durée de l'efficacité thérapeutique.
Suivant l'invention, le médicament contient de piéférence environ 50 parties en poids d'aluminium hydroxyde-magnésium carbonate (co-dried gel), environ 80 parties en poids d'hydroxyde de magnésium, environ 50 parties en poids de trisilicate de magnésium, environ 5 parties en poids de chlorhydrate de lignocaine.
Egalement suivant l'invention, le médicament contient avantageusement environ 10 g d'aluminium hydroxyde-carbonate de magnésie, environ 16 g d'hydroxyde de magnésie, environ 10 g de trisilicate de magnésie, environ 1 g de chlorhydrate de lignocaine pour 400 ml de gel.
Suivant l'invention encore, le médicament peut se présenter sous forme de comprimé ou sous forme de gel. Dans la suite de cette description, on utilisera pour désigner le médicament suivant l'invention la dénomination gel P .
Ci-après sont décrites les études qui ont été faites en ce qui concerne le gel P.
1' . Expérimentation animale.
A. Test de protection des ulcères de contrainte.
On a pratiqué dans deux séries de huit lots faits de 15 rats femelles (whistar) dont le poids oscillait entre 150 g et 200 g des ulcères par la méthode de contrainte. Les animaux étaient immobilisés horizontalement sur une tige métallique, les pattes antérieures et postérieures liées. Dans la première série la contrainte a été de 12 heures, dans la seconde série la contrainte a été de 24 heures.
Les animaux témoins n'ont pas été protégés (NP), les animaux protégés (P) ont reçu différentes protections administrées avant la contrainte. La gravité des lésions a été définie par un score estimé en fonction des lésions. Les résultats sont repris dans le tableau 1. Ils montrent chez les animaux qui ont subi une contrainte de 12 heures et chez les animaux qui ont subi une contrainte de 24 heures une diminution significative de la gravité des lésions, chez les animaux qui ont été protégés par l'association P selon l'invention par rapport aux animaux qui ont été traités par les principes actifs administrés isolément.

B. Test de cicatrisation des ulcères de contrainte.
On a pratiqué dans cinq groupes de rats mâles (whistar), dont le poids oscillait entre 150 g et 200 g, des ulcères par la méthode de contrainte de 24 heures précédemment décrite. Tous les animaux après la contrainte ont été remis en liberté et ont pu s'alimenter normalement. Les animaux du groupe 1 (témoins) n'ont pas été traités.
Les animaux des groupes 2, 3 et 4 ont été traités dès la fin de la contrainte: le groupe 2 par deux fois 2 g/kg de produit connu sous le nom de Phosphalugel par jour, le groupe 3 par deux fois 2 gikg de produit connu sous le nom de Muthésa par jour, le groupe 4 par deux fois 2 g/kg de produit connu sous le nom de Maalox, le groupe 5 par deux fois 2 g/kg de gel P selon l'invention.
Le gel P contient pour 400 ml:
Aluminium hydroxyde-carbonate de magnésie 10 g
Hydroxyde de magnésie 16 g
Trisilicate de magnésie 10 g
Chlorhydrate de lignocaïne 1 g
Tous les animaux ont été sacrifiés le troisième jour après la cessation de la contrainte. La gravité des lésions ulcéreuses estimée en fonction des lésions a été définie par un score. Les résultats sont repris dans le tableau Il. Ils montrent la supériorité de l'effet cicatrisant du gel P par rapport au Phosphalugel. Et l'effet comparable du gel P par rapport au Muthésa et par rapport au Maalox.

3 Populations et traitements
Le gel P a été administré à des malades qui accusaient des brûlures épigastriques accompagnées parfois d'un syndrome de posture.
Les malades qui ont été traités étaient atteints d'une des trois affections suivantes:
oesophagite de reflux
gastrite atrophique
- gastrite accompagnée d'un reflux biliaire
Ont été exclus de cette étude les malades alcooliques et les malades connus pour abuser de médicament.
Pendant la période de l'étude clinique, les malades n'ont pas absorbé d'autres neutralisants de l'acidité gastrique. Ils n'ont été soumis à aucun régime alimentaire particulier.
Les malades, pendant la période de l'étude clinique, n'ont pas été traités par des sédatifs, des hypnotiques, des anxiolytiques ou des antidépresseurs.
Ils n'ont pas reçu d'anti-inflammatoires ni d'analgésiques.
31 Groupe de malades atteints d'oesophagite
Les malades de ce groupe accusaient des plaintes depuis plus de trois mois (brûlures épigastriques et syndrome de posture). L'oesophagite le plus souvent secondaire à une hernie hiatale par glissement a été confirmée par un examen endoscopique. Aucun de ces malades ne présentait d'ulcération oesophagienne.
Le traitement de ces malades a été le suivant: - pendant la première semaine:
1 cuiller à café de gel P après les 3 repas
pendant la deuxième semaine:
I cuiller à café de gel P après les 3 repas et au besoin pour calmer les brûlures: I cuiller à café supplémentaire; jusqu'au maximum de 3 cuillers à café par jour. Le nombre de cuillers à café supplémentaires absorbées a été comptabilisé pendant cette semaine jusqu'à un maximum de 21 pendant la troisième semaine:
I cuiller à café de gel P après les 3 repas
Chez ces malades l'efficacité thérapeutique globale a été estimée par des scores: 0, 1,2 et 3.
Une comparaison de l'efficacité subjective entre le gel P et les autres neutralisants par lesquels les malades avaient déjà été traités a été effectuée (scores - 1, 0, + 1)
32 Groupe des malades atteints de gastrite atrophique
Les malades de ce groupe accusaient des plaintes depuis plus d'un an. Le diagnostic de gastrite atrophique a été basé sur l'examen endoscopique et confirmé par un examen histologique.
L'examen endoscopique du fundus montrait chez ces malades l'image dite en fond d'oeil réalisée par le liseré vasculaire sousmuqueux. L'examen histologique a confirmé l'atrophie avant que les malades entrent dans ce groupe.
La plupart de ces malades avaient été explorés dans les deux années qui précédaient l'entrée dans le groupe par une épreuve sécrétoire à la pentagastrine IV (6 glkgih) dans le but d'estimer la sécrétion acide maximale. Cette épreuve avait mis en évidence une diminution de la sécrétion acide.
Les malades qui ont présenté un ulcère gastrique dans leurs antécédents ont été exclus de ce groupe.
Le traitement de ces malades a été le suivant: - pendant la première semaine:
1 cuiller à café de gel P avant les 3 repas pendant la deuxième semaine
1 cuiller à café de gel P avant les 3 repas et au besoin pour calmer les brûlures:1 cuiller à café supplémentaire; jusqu'au maximum de 3 cuillers à café par jour. Le nombre de cuillers supplémentaires absorbées a été comptabilisé pendant cette semaine jusqu'à un maximum de 21 pendant la troisième semaine
1 cuiller à café de gel P avant les 3 repas
Chez ces malades l'efficacité thérapeutique globale a été estimée par des scores: 0, 1, 2 et 3.
Une comparaison de l'efficacité subjective entre le gel P et les autres neutralisants par lesquels les malades avaient déjà été traités a été effectuée (score -1,0, fol).
33 Groupe des malades atteints de gastrite accompagnée de reflux biliaire
Les malades de ce groupe accusaient des plaintes depuis plus de deux ans. Le diagnostic de gastrite a été basé sur l'examen endoscopique et confirmé par un examen histologique. L'examen histologique chez les malades de ce groupe montrait un aspect de gastrite su perficielle ou un aspect de gastrite atrophique.
La plupart de ces malades étaient suivis depuis plusieurs années.
Ils sont entrés dans ce groupe si les examens endoscopiques avaient montré un reflux biliaire important aux cours de trois examens endoscopiques.
Les malades qui ont présenté un ulcère gastrique dans leurs antécédents ont été exclus du groupe.
Le traitement de ces malades a été le suivant:

2 cuillers à café de gel P avant les 3 repas
pendant la deuxième semaine
2 cuillers à café de gel P avant les 3 repas et au besoin pour calmer les brûlures: 2 cuillers à café supplémentaires; jusqu'au maximum de 6 cuillers à café par jour. Le nombre de cuillers à café supplémentaires absorbées a été comptabilisé pendant cette semaine jusqu'à un maximum de 42
Chez ces malades l'efficacité thérapeutique globale a été estimée par des scores: 0, 1, 2 eut 3.
Une comparaison de l'efficacité subjective entre le gel P et les autres neutralisants par lesquels les malades avaient déjà été traités a été effectuée (score -1, 0, fol).
4 Analyse des résultats
41 L'analyse de la consommation des suppléments d'antiacides estimée au cours de la seconde semaine de traitement exprimée en au maximum autorisé: 21 cuillers à café pour les gastrites atrophiques et les oesophagites, 42 pour les gastrites accompagnées de reflux biliaire montre que
l'adjonction de lidocaine à la base antiacide améliore significativement les résultats dans les trois étiologies (gastrite atrophique, oesophagite, gastrite avec reflux biliaire), Tableau 1.
L'effet principal lidocaine est respectivement pour ces trois étiologies significatif au seuil p < 0,01.
42 Evaluation globale par le patient(0=0; + = 1; + + = 2; +++=3).
L'adjonction de lidocaine améliore significativement les résultats dans les 3 étiologies. Pour la gastrite atrophique, I'oesophagite de reflux et la gastrite de reflux biliaire, les seuils de signification sont
P < 0,01.
43 Effets par rapport aux antiacides utilisés avant le gel P
(inférieur =-l:égal =0; supérieur = + 1).
L'adjonction de lidocaine améliore significativement les résultats dans 3 étiologies. Les trois seuils de signification sont P < 0,01.
L'adjonction aux antiacides d'un anesthésique local (lidocaine) améliore l'efficacité des antiacides dans trois indications: l'oesopha- gite, la gastrite atrophique et la gastrite accompagnée de reflux biliaire.

CLAIMS
1. Medication for the treatment of affections such as peptic ulcers, gastritis and esophagitis and the clinical disorders which these affections, characterized by the optimal association of gastric antacids, with an anesthetic of the cEso-gastro mucosa - duodenal increasing the speed of action and the duration of the effectiveness.
2. Medicament according to claim 1, characterized in that it contains as long-acting gastric antacids: aluminum hydroxide-magnesium carbonate, magnesium hydroxide and magnesium trisilicate.
3. Medicament according to one of claims 1 and 2, characterized in that it contains as anesthetic of the oeso-gastro-duodenal mucosa lignocaine hydrochloride.
4. Medicament according to claims 2 and 3, characterized in that it consists of:
50 parts by weight of aluminum hydroxide-magnesium because

- 80 parts by weight of magnesium hydroxide,

6. Medicament according to one of claims 1 to 4, characterized in that it is in any acceptable pharmaceutical form and, in particular, in a form for administration by the oral route, in tablet or gel.
The subject of the present invention is a medicament the aim of which is to treat patients suffering from esophagitis, gastritis or peptic ulcer.
It is known that the duration of action of a gastric antacid is the longer the lower the percentage of activity released after thirty minutes.
It is also known that burns and pains in patients with esophagitis and gastritis as well as pain in patients with gastric or duodenal ulcers can be relieved with a mucosal anesthetic.
It therefore appeared advantageous to combine in a medicament products having this property at the same time as gastric antacids with prolonged effect.
In accordance with the invention, the medicament advantageously consists of three long-acting gastric antacids: aluminum hydroxide-magnesium carbonate (co-dried gel), magnesium hydroxide and magnesium trisilicate, combined with an anesthetic for the mucosa. eso-gastro-duodenal: lignocaine hydrochloride increasing the speed of action and the duration of therapeutic effectiveness.
According to the invention, the medicament contains approximately 50 parts by weight of aluminum hydroxide-magnesium carbonate (co-dried gel), approximately 80 parts by weight of magnesium hydroxide, approximately 50 parts by weight of magnesium trisilicate, about 5 parts by weight of lignocaine hydrochloride.

We practiced in two series of eight lots made of 15 female rats (whistar) whose weight oscillated between 150 g and 200 g of ulcers by the method of constraint. The animals were immobilized horizontally on a metal rod, the forelegs and hind legs bound. In the first series the constraint was 12 hours, in the second series the constraint was 24 hours.
The control animals were not protected (NP), the protected animals (P) received various protections administered before the constraint. The severity of the lesions was defined by a score estimated according to the lesions. The results are shown in Table 1. They show in animals which have been subjected to a constraint of 12 hours and in animals which have been subjected to constraint for 24 hours a significant reduction in the severity of lesions, in animals which have been protected by the association P according to the invention with respect to the animals which have been treated with the active principles administered alone.

B. Healing ulcer healing test.
Five groups of male rats (whistar) weighing between 150 g and 200 g were treated with ulcers by the 24-hour restraint method described above. All animals after the restraint were released and were able to eat normally. Group 1 animals (controls) were not treated.
The animals of groups 2, 3 and 4 were treated at the end of the constraint: group 2 with twice 2 g / kg of product known as Phosphalugel per day, group 3 with twice 2 gikg of product known as Muthesa per day, group 4 twice 2 g / kg of product known as Maalox, group 5 twice 2 g / kg of gel P according to the invention.
Gel P contains for 400 ml:
Aluminum hydroxide-carbonate of magnesia 10 g
Magnesium hydroxide 16 g
Trisilicate of magnesia 10 g
Lignocaine hydrochloride 1 g
All animals were sacrificed on the third day after the cessation of restraint. The severity of ulcerative lesions estimated according to the lesions was defined by a score. The results are shown in Table II. They show the superiority of the healing effect of gel P compared to Phosphalugel. And the comparable effect of gel P compared to Muthesa and compared to Maalox.

1 Aim of the study
To assess to what extent the addition of antacids to a local anesthetic (lidocaine) improves the effectiveness of antacids.

3 Populations and treatments
Gel P was administered to patients who suffered from epigastric burns sometimes accompanied by posture syndrome.
Patients who were treated had one of the following three conditions:
reflux esophagitis
atrophic gastritis
- gastritis accompanied by bile reflux
Excluded from this study were alcoholic patients and those known to abuse medication.
During the period of the clinical study, the patients did not absorb other neutralizers of gastric acidity. They were not subjected to any particular diet.
The patients, during the period of the clinical study, were not treated with sedatives, hypnotics, anxiolytics or antidepressants.
They did not receive anti-inflammatory drugs or pain relievers.
31 Group of patients with esophagitis
Patients in this group had been complaining of complaints for more than three months (epigastric burns and posture syndrome). The esophagitis most often secondary to a sliding hiatal hernia was confirmed by an endoscopic examination. None of these patients had esophageal ulceration.
The treatment of these patients was as follows: - during the first week:
1 teaspoon of gel P after 3 meals
during the second week:
I teaspoon of gel P after the 3 meals and if necessary to calm burns: I additional teaspoon; up to a maximum of 3 teaspoons per day. The number of additional teaspoons absorbed was counted during this week up to a maximum of 21 during the third week:
I teaspoon of gel P after 3 meals
In these patients, the overall therapeutic efficacy was estimated by scores: 0, 1.2 and 3.
A comparison of the subjective efficacy between gel P and the other neutralizers with which the patients had already been treated was carried out (scores - 1, 0, + 1)
32 Group of patients with atrophic gastritis
Patients in this group had been complaining for over a year. The diagnosis of atrophic gastritis was based on endoscopic examination and confirmed by histological examination.
Endoscopic examination of the fundus showed the so-called fundus image produced by the submucosal vascular border in these patients. Histological examination confirmed atrophy before the patients entered this group.
Most of these patients had been explored in the two years preceding entry into the group by a secretory test with pentagastrin IV (6 glkgih) in order to estimate the maximum acid secretion. This test had demonstrated a decrease in acid secretion.
Patients who had a gastric ulcer in their history were excluded from this group.
The treatment of these patients was as follows: - during the first week:
1 teaspoon of gel P before the 3 meals during the second week
1 teaspoon of gel P before the 3 meals and if necessary to calm burns: 1 additional teaspoon; up to a maximum of 3 teaspoons per day. The number of additional spoons absorbed was counted during this week up to a maximum of 21 during the third week
1 teaspoon of gel P before the 3 meals
In these patients, the overall therapeutic efficacy was estimated by scores: 0, 1, 2 and 3.
A comparison of the subjective efficacy between gel P and the other neutralizers with which the patients had already been treated was carried out (score -1.0, fol).
33 Group of patients with gastritis accompanied by bile reflux
Patients in this group had been complaining for more than two years. The diagnosis of gastritis was based on endoscopic examination and confirmed by histological examination. Histological examination in patients in this group showed an appearance of perfected gastritis or an appearance of atrophic gastritis.
Most of these patients had been followed for several years.
They entered this group if the endoscopic examinations had shown significant bile reflux during three endoscopic examinations.
Patients who had a gastric ulcer in their history were excluded from the group.
The treatment of these patients was as follows:
during the first week
2 teaspoons of gel P before the 3 meals
during the second week
2 teaspoons of gel P before the 3 meals and if necessary to calm burns: 2 additional teaspoons; up to a maximum of 6 teaspoons per day. The number of additional teaspoons absorbed has been counted during this week up to a maximum of 42
In these patients, the overall therapeutic efficacy was estimated by scores: 0, 1, 2 or 3.

4 Analysis of the results
41 The analysis of the consumption of antacid supplements estimated during the second week of treatment expressed in maximum allowed: 21 teaspoons for atrophic gastritis and esophagitis, 42 for gastritis accompanied by bile reflux
the addition of lidocaine to the antacid base significantly improves the results in the three etiologies (atrophic gastritis, esophagitis, gastritis with bile reflux), Table 1.
The main lidocaine effect is respectively significant for these three etiologies at the threshold p <0.01.
42 Overall assessment by the patient (0 = 0; + = 1; + + = 2; +++ = 3).
The addition of lidocaine significantly improves the results in the 3 etiologies. For atrophic gastritis, reflux esophagitis and bile reflux gastritis, the significance levels are
P <0.01.
43 Effects compared to antacids used before gel P
(lower = -l: equal = 0; upper = + 1).
The addition of lidocaine significantly improves the results in 3 etiologies. The three significance thresholds are P <0.01.
Adding a local anesthetic (lidocaine) to antacids improves the effectiveness of antacids in three indications: esophagitis, atrophic gastritis and gastritis with bile reflux.
