PATENTANSPRÜCHE
1. Pharmazeutische Zusammensetzung, enthaltend eine Mischung von i) einem oder mehreren pharmakologischen Wirkstoffen aus der Gruppe der Aryloxyalkanolamine und ii) einem oder mehreren physiologisch verträglichen anioni schen Tensiden aus der Gruppe der Monoschwefel säureester höherer Fettalkohole.
2. Pharmazeutische Zusammensetzung gemäss Anspruch
1, dadurch gekennzeichnet, dass diese in einer Form vorliegt, die eine kontrollierte Freigabe des Wirkstoffes im Intestinaltrakt gestattet.
3. Pharmazeutische Zusammensetzung gemäss Anspruch
1, dadurch gekennzeichnet, dass sie als Wirkstoffe 4-(2-Hydroxy-3-isopropylaminopropoxy)-indol und/oder 4-(2-Hydroxy-3-isopropylaminopropoxy)-2-methylindol enthält.
4. Pharmazeutische Zusammensetzung gemäss Anspruch 1, dadurch gekennzeichnet, dass sie als Tensid das Natriumlaurylmonosulfat enthält.
5. Pharmazeutische Zusammensetzung gemäss den Ansprüchen 1 bis 4, dadurch gekennzeichnet, dass sie in den Säften des Intestinaltraktes unlösliche wie auch in den Säften des Intestinaltraktes lösliche Füll- und Trägerstoffe enthält, wobei das Gewichtsverhältnis der unlöslichen zu den löslichen Stoffen 1:5 bis 1:0,3 beträgt.

Pharmazeutische Zusammensetzung gemäss Ansprüchen 1 bis 5, dadurch gekennzeichnet, dass sie aus einem Kern mit den Komponenten i) und ii) besteht, der mit einem Film überzogen ist, der gegen Magensäfte widerstandsfähig ist, durch die Säfte des Intestinaltraktes, bei pH-Wert von 5 und mehr jedoch gelöst wird.
7. Pharmazeutische Zusammensetzung gemäss Anspruch 6, dadurch gekennzeichnet, dass der Film aus Hydroxypropylmethylcellulosephthalat besteht.
8. Pharmazeutische Zusammensetzung gemäss den Ansprüchen 6 und 7, dadurch gekennzeichnet, dass der Film mit einer Wirkstoffschicht überzogen ist.
9. Pharmazeutische Zusammensetzung gemäss Anspruch 8, dadurch gekennzeichnet, dass die Wirkstoffschicht als Wirkstoffe die Aryloxyalkanolamine enthält, die sich bereits im filmüberzogenen Tablettenkern befinden.
Die Erfindung betrifft eine pharmazeutische Zusammensetzung, enthaltend eine Mischung von i) einem oder mehreren pharmakologischen Wirkstoffen aus der Gruppe der Aryloxyalkanolamine und ii) einem oder mehreren physiologisch verträglichen anioni schen Tensiden aus der Gruppe der Monoschwefel säureester höherer Fettalkohole.
Erfindungsgemäss wurde gefunden, dass die unter ii) genannten Komponente die enterale Resorption der unter i) genannten Komponente vorteilhafterweise erhöht. Die pharmazeutische Zusammensetzung kann in eine Form gebracht werden, die eine kontrollierte Abgabe des Wirkstoffes in den Intestinaltrakt gestattet.
Die pharmazeutische Zusammensetzung gemäss der vorliegenden Erfindung ist insbesondere geeignet zur Verabreichung als herz- und kreislaufwirksame Mittel, insbesondere als ,8-Blocker und Antiarrhythmika. Solche Mittel sind aus der Literatur bekannt.
Der Aryl-Rest der Aryloxyalkanolamine kann beispielsweise bis 10 Kohlenstoffatome enthalten und insbesondere für Phenyl oder Naphthyl stehen; er kann jedoch ebenfalls 1 oder 2 Heteroatome, insbesondere Stickstoffatome enthalten und beispielsweise für Indolyl oder Stickstoff- und SchwefeIatome enthalten und beispielsweise für ThiazolyI stehen. Der Aryl-Rest kann ebenfalls einen oder 2 Ringsubstituenten, beispielsweise Acetyl, Allyl, Allyloxy, Aminocarbonylmethyl, Cyano, Methyl, Chlor, Methoxy, Methoxyäthyl, Amido, Hydroxy, Nitro, Propionyloxy, Alkanoylamino -wobei die Alkylgruppe 14 Kohlenstoffatome enthält, Methylsulfamoyl, Morpholino, Methylthio und Tetrahydrofurylmethyloxy-Substituenten enthalten.
Der Aryl-Rest kann ebenfalls eine Alkylkette zwischen zwei nebeneinander befindlichen Kohlenstoffatomen enthalten, wobei ein gesättigter Ring gebildet wird. Beispielsweise kann der Aryl-Rest auch für Tetralon oder Tetralin stehen.
Die Aminogruppe enthält vorzugsweise als Substituenten einen verzweigten Alkylrest mit 3-8 Kohlenstoffatomen, insbesondere einen Isopropyl- oder tert.-Butylrest.
Die Alkanolaminogruppe ist zweckmässigerweise eine 2 Hydroxy-3-alkylaminopropyl-Gruppe.

Besonders interessante Verbindungen sind: das 4-(2-Hydroxy-3-isopropylaminopropoxy)indol (Pindolol) und das 4 (2-Hydroxy-3-isopropylaminopropoxy)-2-methylindol und andere Verbindungen, die eine schwache Löslichkeit in den neutralen oder alkalischen Säften des Intestinaltraktes besitzen.
Die Wirkstoffe können auch in Form ihrer pharmazeutisch annehmbaren Säureadditionssalze verwendet werden.
Vorzugsweise jedoch verwendet man die Wirkstoffe in Form der freien Basen.
Die erfindungsgemäss verwendbaren Tenside haben einen hydrophilen lipophilen Gleichgewichtswert (HLB-Wert) zwischen vorzugsweise 35 und 45 [die HLB-Werte sind ein Mass für den relativen Anteil an hydrophilen Gruppen eines Emulgatormoleküls und sind beschrieben beispielsweise in Pharm. Acta Helv. 44, 9 (1969) und im Lexikon der Hilfsstoffe für Pharmazie, Kosmetik und angrenzende Gebiete von H.P. Fiedler auf Seite 263, Editio Cantor KG., 1971].
Als günstig erweisen sich solche Tenside, deren kritische Mycelkonzentration (CMC) vorzugsweise zwischen 10 - 2 und 10 - 3 liegt.
Bevorzugte Tenside haben eine hohe Dissoziationskonstante und können mit dem Wirkstoff in einem stark sauren Medium stabile Komplexe bilden. Diese Komplexe sollen vorzugsweise in der Lage sein, sich beispielsweise in Magensäften bei pH-Werten unter 3 sehr langsam, in Darmsäften bei pH-Werten zwischen 3 und 7,5, insbesondere oberhalb von 5, hingegen sehr rasch aufzulösen.
Die Auswahl von Tensiden wird selbstverständlich von
deren Verträglichkeit mit den verwendeten Wirkstoffen u. a.
unter Berücksichtigung von beispielsweise deren Molekülgrösse und Molekülgestalt sowie deren Basizität, aber auch von anderen pharmazeutischen Verdünnungsmitteln und Trägerstoffen, die ebenfalls anwesend sein können, abhängig sein. Beispielsweise sollen Tenside vermieden werden, die stabile unlösliche Niederschläge mit den Wirkstoffen bei pH Werten von 5 und mehr bilden.
Die höheren Fettalkohole können geradkettig oder verzweigt sein und alicyclische und/oder aromatische Gruppen enthalten. Bevorzugt ist ein primärer Alkohol mit 8-18 Kohlenstoffatomen, insbesondere mit 12-14 Kohlenstoffatomen.
Falls erwünscht, können diese Fettalkohole äthoxylierte Fettalkohole sein, die pro Mol Fettalkohol 1-5, vorzugsweise 1-3 Äthylenoxidgruppen enthalten. Bevorzugt sind Fettalkohole, die 2,5 Mol Athylenoxidgruppen pro Mol des Tensids enthalten.
Vorzugsweise werden die Tenside in Form ihrer Salze verwendet. Bevorzugte Salze sind diejenigen, die im Wasser löslich oder fein dispergierbar sind, insbesondere Erdalkalimetallsalze, beispielsweise Magnesiumsalze, Salze mit organischen Aminen, beispielsweise Triäthanolaminsalze oder Ammoniumsalze. Bevorzugt werden jedoch Alkalimetallsalze, wie z.B. die Natriumsalze.
Ein bevorzugtes Tensid, zumindest für die Verwendung zusammen mit den obengenannten Indolderivaten, ist das Natriumlaurylmonosulfat. Selbstverständlich können, falls erwünscht, mehr als ein Tensid verwendet werden.
Das Gewichtsverhältnis der Tenside zu den Wirkstoffen und die Gesamtmenge der verwendeten Tenside kann u. a.
abhängig sein von den chemischen und physikalischen Eigenschaften der Tenside, der Art und Menge der Wirkstoffe und der Art und Menge von allfälligen pharmazeutischen Träger und Füllstoffen und der gewünschten Freisetzungsdauer der Wirkstoffe im Intestinaltrakt.
Die obere Grenze des Gewichtsverhältnisses von Tensid zu Wirkstoff und der einzusetzenden Tensidmenge richtet sich selbstverständlich nach den physiologischen Verträglichkeiten der jeweiligen Tenside, gegebenenfalls unter Berücksichtigung der geplanten Behandlungsdauer und Verabreichungshäufigkeit. Das Gewichtsverhältnis der Tenside zu den Wirkstoffen soll im allgemeinen 0,2:1 bis 2:1, vorzugs weise 0,3:1 bis 1:1, betragen.
Falls das Tensid ein Salz eines Monoschwefelsäureesters eines Alkohols ist, beispielsweise Natriumlaurylmonosulfat, und der Wirkstoff ein oben erwähntes Indolylderivat, so soll das bevorzugte Gewichtsverhältnis von Tensid zum Wirk stoff von 0,25:1 bis 1:1, insbesondere von 0,4:1 bis 0,7:1, betragen.
Andere pharmazeutische Träger- und Verdünnungsmittel können ebenfalls anwesend sein. Die pharmazeutischen Verdünnungsmittel sollen dabei bevorzugt so gewählt sein, dass der Wirkstoff gleichmässig über eine lange Zeitdauer freigesetzt wird. Bevorzugte Verdünnungsmittel tragen zur Ausbildung einer Matrix im Verdauungstrakt bei und sind widerstandsfähig gegen bzw. schwer angreifbar durch Säfte des Intestinaltraktes.
Beispiele von solchen Füll- und Trägerstoffen, die in den Säften des Intestinaltraktes unlöslich sind, sind beispielsweise physiologisch annehmbare Calciumsalze, wie Calciumsulfat oder Calciumhydrogenphosphatdihydrat; Cellulosederivate, wie Äthylcellulose; synthetische Polymere, wie Polyvinylacetat oder Copolymerisate von Vinylpyrrolidon und Vinylacetat; oder natürliche synthetische oder semisynthetische Fette, wie beispielsweise Mono-, Dioder Triglyzeride der Palmitin- und Stearinsäure; hydriertes Castoröl und Wachs und höhere Fettalkohole, wie Cetylstearylalkohol DAB 7. Einige dieser unlöslichen Stoffe sind jedoch zur Matrixbildung befähigt.
Falls die Füllstoffe anderseits in den Säften des Intestinaltraktes löslich oder leicht dispergierbar sein sollen, bevorzugt man beispielsweise physiologisch annehmbare Verbindungen, wie Lactose, Mannitol und andere Zucker; Polyvinylpyrrolidon und Polyäthylenglykole. Weitere pharmazeutische inaktive Füllstoffe, wie z. B. Fliessmittel, beispielsweise Talk und Magnesiumstearat, können überdies noch anwesend sein.
Zweckmässigerweise umfassen die pharmazeutisch annehmbaren Zusätze ein Gemisch von in den Säften des Intestinaltraktes unlöslichen und den Säften des Intestinaltraktes löslichen Füllstoffen. Das Gewichtsverhältnis von unlöslichen zu löslichen Füllstoffen hängt natürlich u. a. von der Menge und den physikalischen und chemischen Eigenschaften des Wirkstoffes und des Tensids sowie von der gewünschten Verzögerung der Freisetzung des Wirkstoffes ab.
Falls die erfindungsgemässe Zusammensetzung in einer Form verabreicht wird, die rasch mit den Säften des Intestinaltraktes .n Berührung kommt, so soll darin das Verhältnis von unlöslichen Füllstoffen (beispielsweise unlösliche Füll- und Trägerstoffe, matrixbildendes Material sowie Fliessmittel zu den löslichen Füllstoffen (beispielsweise lösliche Füll- und Trägerstoffen und Tensiden, die ebenfalls die unter ii) angeführten Komponenten umfassen) von 1:5 bis
1:0,3 betragen, damit eine 40- bis 80%ige, insbesondere 50%ige Wirkstofffreisetzung während 1-7 Stunden im Intestinaltrakt erfolgt.
Bei Gemischen, welche als Wirkstoffe die oben angeführten Inolylderivate und als Tensid Natriumlaurylmonosulfat enthalten, soll das Verhältnis von unlöslichem Füllstoff zu löslichem Füllstoff im allgemeinen von 1:3 bis 1:1,5 betragen, das geeignet ist, eine 40%ige Freisetzung des Wirkstoffes im Intestinaltrakt während 1-4 Stunden zu erreichen.
Ein bevorzugter Gegenstand der vorliegenden Erfindung, bestehend aus einer Kombination eines Wirkstoffes i) und eines Tensids ii), ist ein Tablettenkern, der mit einem physiologisch annehmbaren Film bedeckt ist, wobei dieser Film widerstandsfähig ist gegen Magensäfte, von den Säften des Intestinaltraktes jedoch aufgelöst wird.
Die Wahl des filmbildenden Materials und seine Dicke wird u. a. vom Wirkstoffe, dem Tensid und der Löslichkeit eines möglichen Komplexes des Wirkstoffes mit dem Tensid abhängig sein. Im allgemeinen besteht der Film aus einem Polymeren.
Das filmbildende Polymere soll so, insbesondere ausgewählt sein, dass der Film bei pH-Werten von 5 und mehr, vorzugsweise 5-8, besonders 97, im Intestinaltrakt innerhalb von 1/4 bis 2 Stunden, vorzugsweise von 1/2 bis 1 Stunde aufgelöst wird. Vorzugsweise soll sich der Film während mindestens 3 Stunden in künstlichen Magensäften bei pH Werten von weniger als 5 nicht lösen. Solche Filme können unter bekannten makromolekularen Polymeren, die für die Herstellung von Einheitsdosen, die gegen Magensäfte widerstandsfähig sind, jedoch im Dünndarm gelöst werden, bekannt sind, ausgewählt werden. Geeignete Polymere sind beispielsweise in Hagers Handbuch der pharmazeutischen Praxis, 4. Auflage, Bd. 7a, Seiten 739-742 und 776-778, sowie in Remington's Pharmaceutical Sciences, 13.
Ausgabe, Seiten 1689-1691 aufgeführt, und umfassen insbesondere Celluloseesterderivate, Acrylharze, wie Methacrylat-Copolymere, Copolymere der Maleinsäure und Derivate der Phthalsäure.
Bevorzugte Filme sind solche aus Celluloseacetatphthalat, Copolymeren von Methacrylsäure und deren Estern, die mindestens 40% Methacrylsäure enthalten, und insbesondere Hydroxypropylmethylcellulosephthalaten.
Die Dicke des Filmes hängt vom Permeabilitätsgrad des Filmes für Wasser und Säure ab. Vorzugsweise soll die Dikke der Filmschicht von 5-100 llm und insbesondere von 20-8011m betragen. Im allgemeinen soll jedoch das Gewicht des Filmes 15% des überzogenen Tablettenkerns nicht überschreiten und vorzugsweise von 3-10% des überzogenen Tablettenkerns betragen.
Der Film kann eine als Kern Mehrzahl von voneinander getrennten Granulatteilchen oder Kügelchen überziehen, die jeweils einen oder mehrere Wirkstoffe i) und einen oder mehrere Tenside ii) beinhalten und in zumindest einem pharmazeutischen Träger- oder Füllstoff eingebettet sind.
Vorzugsweise wird eine Einheitsdosis hergestellt, die den Wirkstoff i) auch aus einer äusseren Wirkstoffschicht in den Magensäften freisetzt, wobei diese äussere Wirkstoffschicht aus dem Wirkstoff und in den Magensäften löslichen oder dispergierbaren Träger- und/oder Füllstoffen besteht. Der Wirkstoff wird dabei rasch durch die Magenwände resorbiert, wobei initial eine hohe Konzentration des Wirkstoffes in der Blutbahn erreicht wird. Die Höhe des Wirkstoffblut- spiegels wird durch die verzögerte Freigabe des Wirkstoffes aus dem Tablettenkern in den neutralen oder alkalischen Säften des Intestinaltraktes aufrechterhalten. Falls erwünscht, kann deshalb der Filmüberzug mit einer Wirkstoffschicht bedeckt sein.
Das Gewichtsverhältnis der Aryloxyalkanolamine, die sich in dem vom Film überzogenen Tablettenkern befinden zu denen, die sich in der auf dem Film aufgetragenen Wirkstoffschicht befinden, kann von 0,75: 1 bis 1,25: 1, insbesondere 1:1, betragen.
Falls erwünscht, können auch andere pharmakologische Wirkstoffe, insbesondere in der äusseren Wirkstoffschicht, anwesend sein. Solche Wirkstoffe sind insbesondere diejenigen, die die Herz- und Kreislauffunktionen beeinflussen, wie beispielsweise Antihypertensiva, Diuretika, a-Blocker usw., insbesondere jedoch Iangwirksame Diuretika. So kann beispielsweise ein Aryloxyalkanolamin, wie 4-(2-Hydroxy-3isopropylaminopropoxy)indol, kombiniert werden mit einem lang wirksamen Diuretikum, wie beispielsweise 2-Methyl-3-o-tolyl-6-sulfamoyl-7-chlor-1,2,3-tetrahydro-4(3H)chinazolinon (Metolazon) oder seinen pharmazeutisch annehmbaren Salzen, wobei beispielsweise beide sich in der äusseren Wirkstoffschicht befinden und der erstere Wirkstoffauch im Tablettenkern.
Die äussere Schicht kann aus einer Mehrzahl von voneinander getrennten Granulat-Teilchen oder -Kügelchen bestehen, die jeweils einen oder mehrere Wirkstoffe, die in zumindest einem pharmazeutischen Träger- und/oder Füllstoff, der in den Säften des Magentraktes löslich ist, eingebettet sind. Die äussere Schicht kann eine Mehrzahl von mit Film überzogenen Medikamenten-Kernen bedecken. Vorzugsweise bedeckt die äussere Schicht nur einen mit Film überzogenen Medikamenten-Kern.
Zweckmässigerweise soll das Gewicht des Tensides von ca. 1 bis 30 Gew.-%, vorzugsweise von 1 bis 10 Gew.-% der pharmazeutischen Mischung oder des Tablettenkerns, sofern einer anwesend ist, betragen. Falls sich die pharmazeutische Mischung in Form einer Einheitsdosis befindet, soll die Gesamtmenge des Tensids im allgemeinen weniger als 50 mg betragen.
Die erfindungsgemässe pharmazeutische Zusammensetzung kann auf an sich bekannte Weise hergestellt werden. So können beispielsweise die Komponente i) mit der Komponente ii) miteinander vermischt und das erhaltene Gemisch beispielsweise nach Granulierung der Mischung unter Verwendung bekannter Trocken- oder Nassgranulierungsverfahren zu einem Tabletten kern verpresst oder das erhaltene Gemisch auch ohne Granulierung zu Tablettenkernen verpresst und danach in eine geeignete galenische Form übergeführt werden.
Wie bereits ausgeführt, ist der bevorzugte Gegenstand der Erfindung eine pharmazeutische Mischung, die mit einem Film überzogen ist, der gegen Magensäfte widerstandsfähig ist, jedoch in den Säften des Intestinaltraktes gelöst wird. Solch ein Film kann auf die Mischung der Komponenten i) und ii) auf an sich bekannte Weise, beispielsweise aus einer Lösung bei Temperaturen von 10O bis 60 "C aufgetragen werden.
Die nachfolgenden Beispiele beschreiben die Erfindung.
Die in den Beispielen verwendeten Materialien sind die üblichen Materialien, die in der Galenik verwendet werden.
Die in den Beispielen verwendeten Zusätze sind übliche in der Galenik verwendete Materialien. So wird beispielsweise Hydroxymethylcellulosephthalat unter der Marke HPMCPHP 50 (Reg.) von Shinetsu Chem. Co, Tokyo, Äthylcellulose unter der Marke Ethocel N (Reg.) 7 cps von Dow Chemical Co., Midland, Michigan USA, Polyvinylpyrrolidon/polyvinylacetat, ein Makrocopolymer von Vinylpyrrolidon und Vinylacetat (Molverhältnis 6:4), bekannt als Luviskol VA 64 von der BASF Ludwigshafen BRD, mikrokristalline Cellulose unter der Marke Avicel (Reg.) von FMC Corp. Markus Hook, Palo Alto USA, Polyvinylpyrrolidon unter der Marke Kollidon 90 (Reg.) von der BASF Ludwigshafen BRD, das Palmat/Stearinsäuretriglyzeridgemisch unter der Marke Precirol (Reg.) von den Etablissements Gattefosse H. Priest, Frankreich, das hydrierte Castoröl unter der Marke Cutina HR (Reg.) von Henkel, Düsseldorf, BRD, vertrieben.

Herstellung
Herstellung der filmüberzogenen Kerne:
Die Wirk- und Hilfsstoffe werden auf an sich bekannte Weise gemischt, mit Granulierflüssigkeit angefeuchtet, granuliert und nach Zusatz der Gleitmittel wird das Granulat unter Verwendung gewölbter Tabletten stempel zu Tablettenkernen mit 5 mm Durchmesser verpresst.
Die Tablettenkerne werden anschliessend durch Besprühen mit einer 1 Ogewichtsprozentigen Lösung von Hydroxypropylmethylcellulosephthalat in Äthanol/Aceton 1:1 auf an sich bekannte Weise mit einem Filmüberzug versehen.
Herstellung der Manteltablette:
Der Wirkstoff wird mit den übrigen Bestandteilen auf an sich bekannte Weise gemischt, feucht granuliert und nach Zusatz von Gleitmittel wird das Granulat mit den filmüberzogenen Kernen zusammen zu Manteltabletten verpresst.
Analog Beispiel 1 können auch Tabletten mit den in der folgenden Tabelle angegebenen Kernzusammensetzungen erhalten werden. In der Tabelle sind die Mengen der einzelnen Bestandteile in Gew.-% bezogen auf Kerngewicht angegeben.
