PATENTANSPRÜCHE
1. Mittel zur operativen Vereinigung von Knochenbrüchen, dadurch gekennzeichnet, dass die Mittel aus einem elektrisch isolierten Material bestehen.
2. Mittel nach Anspruch 1, dadurch gekennzeichnet, dass die Mittel aus Metall mit einem Überzug aus biologisch verträglichem thermoplastischem Kunststoff aus der Gruppe der Polyamide bestehen.
3. Mittel nach Anspruch 1, dadurch gekennzeichnet, dass die Mittel aus Metall mit einem Überzug aus biologisch verträglicher Glaskeramik bestehen.
4. Mittel nach Anspruch 1, dadurch gekennzeichnet, dass die Mittel aus einem biologisch verträglichen thermoplasti- schen Polymer aus der Gruppe der Polycarbonate bestehen.
Die Erfindung bezieht sich auf Mittel zur operativen Vereinigung von Knochenbrüchen.
Es sind verschiedene aus Metall bestehende Mittel für die Osteosynthese bekannt, die mittels Schrauben oder ähnlichen Mitteln in oder am Knochen befestigt werden. Die Mittel bestehen im allgemeinen aus einem elektrisch leitenden Metall oder einer Legierung.
Bei einem bekannten Verfahren wird die Vereinigung durch Immobilisierung mit gebranntem Gips durchgeführt.
In Knochen treten drei elektrische Erscheinungen auf und zwar ein bioelektrisches Potential, das vom beanspruchten Knochen, Tendos und Cartilagines herrührt (piezoelektrisches Potential), das durch Muskelkontraktionen und Nervenstimulenzien während einiger Millisekunden erzeugt wird und das sich aus der aus lebendem Gewebe bestehenden Potentialdifferenz ergibt. Letzteres ist konstant und hängt nicht von den Muskelkontraktionen und Beanspruchungen ab, sondern nur von der Vitalität des Gewebes.
Es ist bekannt, das konstante Potential in langen Röhrenknochen durch eine Spannungsdifferenz über die Länge des Knochens anzugeben, die in Epiphyse immer elektrisch negativ und in der Diaphyse hauptsächlich elektrisch positiv ist. Die maximalen negativen Werte liegen unterhalb der epiphysikalischen Linie der Metaphysen, wo ein erhöhter Metabolismus beobachtet wird. Das konstante bioelektrische Potential spielt eine wichtige Rolle.
Es ist bekannt, dass sich das normale Biopotentialbild im Falle eines Bruches abrupt verändert. An der Bruchstelle tritt ein zweites negatives Extrem auf, das bedeutend höher als das an Epiphysen ist. Das zweite negative Extrem bleibt während der Vereinigung der Knochen bestehen und nimmt im Laufe der Zeit konstant ab.
Ist die Osteosynthese abgeschlossen, so nimmt das bioelektrische Potential seine ursprünglichen Werte an. Das Vorhandensein eines negativen, bioelektrischen Potentials ist zwingend und ist direkt mit gesteigertem Metabolismus verbunden, der während des Knochenwachstums und der Knochenumwandlung sowie bei der Regenerierung des Knochengewebes auftritt. Wird die bekannte Metallplatte bei der Osteosynthese angewendet, stellt das elektrisch leitende Metall über seine Länge eine direkte Verbindung her und gleicht die negativen bioelektrischen Potentiale im Bereich des Bruches aus. Daraus folgt, dass, wenn eine Metallplatte zur Vereinigung verwendet wird, die zur normalen Regeneration des Knochens erforderlichen natürlichen -Konditionen gestört werden, das zu einer relativen Verzögerung beim Zus am- menwachsen führt oder sogar das Zusammenwachsen verhindert.
Dies bestätigt die allgemein vertretene Meinung, dass mit der konservativen Behandlung des Bruches durch Immobilisierung mittels Gips, durch welche die natürlichen Biopotentiale im Knochen aufrechterhalten werden können, der Prozentsatz der nicht erfolgreichen Heilung im Vergleich zur chirurgischen Behandlung kleiner ist.
Ein Nachteil der konservativen Behandlung besteht in der langen Behandlungszeit, der eine lange Rekonvaleszenzperiode folgt.
Ziel der Erfindung ist es, Mittel zur operativen Vereinigung von Knochenbrüchen zu schaffen, bei dem die erwähnten nachteiligen Erscheinungen nicht auftreten.
Dieses Ziel wird erfindungsgemäss dadurch erreicht, dass die Mittel aus einem elektrisch isolierten Material bestehen.
Diese Mittel haben den Vorteil, dass das all der Bruchstelle erforderliche negative bioelektrische Extrem nicht verringert wird, dass diese Mittel, wie die Platte, die inneren und äusseren Fixierungselemente und die Befestigungselemente einerseits miteinander elektrisch isoliert sind und anderseits vom Knochen elektrisch isoliert sind oder aus Kunststoff be- stehen können. Diese Mittel stören die natürliche und erforderliche Längsaufteilung des bioelektrischen Potentials des verletzten Knochens nicht und stehen einer normalen Regeneration des Knochengewebes nicht entgegen. Gleichzeitig ergibt sich daraus die erforderliche Stabilität und an beiden Seiten werden hohe negative bio elektrische Potentiale aufrechterhalten.
Diese schaffen normalere Bedingungen für einen beschleunigten Metabolismus im Knochen und für den Transport positivere Mineralionen. Aufgrund dieser elektrisch isolierten Mittel ergeben sich vorteilhaftere Bedingungen für die Ausheilung der Wunde im weichen Gewebe. Ein weiterer Vorteil ist, dass sie bessere Möglichkeiten für die elektrische Stimulation der Knochenverbindung bieten.
Im folgenden sind Ausführungsbeispiele des Erfindungsgegenstandes anhand der beiliegenden Zeichnungen näher erläutert. Es zeigen:

Fig. 2a und 2b ein Diagramm der Verteilung der bioelektrischen Potentiale im Schienbeinknochen bei der Verwendung einer Metall- bzw. Kunststoffplatte bei der Osteosynthese und
Fig. 3 eine Röntgenaufnahme eines zusammengefügten Knochens eines Testkaninchens einen Monat nach der Osteotomie mit einer Metallplatte, linke Figur, und einer Kunststoffplatte, rechte Figur.
Das chirurgische Verfahren bei der Osteosynthese eines gebrochenen Röhrenknochens oder der Osteotomie eines Knochens ist das gleiche wie bei der bekannten Osteosynthese. Der einzige Unterschied liegt darin, dass im vorliegenden Fall Kunststoff verwendet wird
In dem Fall, wo Kunststoffplatten verwendet werden, wird das folgende Verfahren angewendet. Die Ausführung und die Funktion der Kunststoffplatten sind die gleichen, wie bei den bekannten Metallplatten.
Der Unterschied liegt in ihrer zusätzlichen Isolierungseigenschaft, die durch a3 Überziehen der Metallplatte mit einem verträglichen thermoplastischen Kunststoff aus der Klasse der Polyamide, das in Pulverform als Rostschutz der Metallteile hergestellt wird und unter der Formel H(NH-(CEI2)l0-CO)nOH bekannt ist, b) Überziehen der Metallplatte mit einem verträglichen Glas- oder Porzellan, das mittels Elektrophorese auf die Glaszwischenlage aufgetragen wird, und c) Herstellen der vollkommen aus einem verträglichen thermoplastischen Kunststoff bestehenden Platte, wobei der Kunststoff aus der Gruppe der Polycarbonate stammt. Die experimentelle Anwendung des Verfahrens und die Mittel zu
seiner Realisierung wurden durch vergleichende Experimente mit Testkaninchen erhalten. Bei einem mit einer Kunststoffplatte verbundenen Kaninchenknochen tritt eine gegen über einem mit einer Metallplatte verbundenen Kaninchen knochen beschleunigte Regeneration des Knochengewebes ein. Bereits fünf Wochen nach der Operation wurde bei der
Platte eine dickere und vollständige Ausfüllung der Bruchstelle mit Callus ohne periostale Bildung, sowohl mikroskopisch wie röntgenologisch beobachtet. Die bei anderen mit Metallplatten verbundenen Knochen zeigten einen osteotalen Callus ohne ausgeprägte endostale Callusbildung und Ausfüllung der Bruchstelle.
Diese Resultate wurden in Fällen starker Pseudoarthrose klinisch wieder bestätigt.

The invention relates to means for the operative association of broken bones.
Various metal osteosynthesis means are known which are fastened in or on the bone by means of screws or similar means. The agents generally consist of an electrically conductive metal or an alloy.
In a known method, the union is carried out by immobilization with fired gypsum.
There are three electrical phenomena in bones: a bioelectric potential, which results from the stressed bones, tendos and cartilagines (piezoelectric potential), which is generated by muscle contractions and nerve stimulants for a few milliseconds and which results from the potential difference consisting of living tissue. The latter is constant and does not depend on the muscle contractions and stresses, but only on the vitality of the tissue.
It is known to indicate the constant potential in long long bones by a voltage difference over the length of the bone, which is always electrically negative in the epiphysis and mainly electrically positive in the diaphysis. The maximum negative values are below the epiphysical line of the metaphyses, where an increased metabolism is observed. The constant bioelectric potential plays an important role.
It is known that the normal biopotential picture changes abruptly in the event of a break. A second negative extreme occurs at the break, which is significantly higher than that of the epiphyses. The second negative extreme persists during the union of the bones and decreases steadily over time.
When the osteosynthesis is complete, the bioelectric potential returns to its original values. The presence of a negative, bioelectrical potential is imperative and is directly connected to the increased metabolism that occurs during bone growth and transformation as well as during the regeneration of the bone tissue. If the known metal plate is used in osteosynthesis, the electrically conductive metal creates a direct connection along its length and compensates for the negative bioelectric potentials in the area of the fracture. It follows that if a metal plate is used for union, the natural conditions required for normal regeneration of the bone are disturbed, which leads to a relative delay in growing together or even prevents the growing together.
This confirms the generally held opinion that the conservative treatment of the fracture by means of plaster immobilization, which can maintain the natural biopotentials in the bone, means that the percentage of unsuccessful healing is smaller compared to the surgical treatment.
A disadvantage of conservative treatment is the long treatment time followed by a long period of convalescence.
The aim of the invention is to provide means for the operative association of broken bones in which the above-mentioned disadvantageous phenomena do not occur.
According to the invention, this aim is achieved in that the means consist of an electrically insulated material.
These means have the advantage that the negative bioelectrical extreme required for all the breaking point is not reduced, that these means, such as the plate, the inner and outer fixing elements and the fastening elements, are on the one hand electrically insulated from one another and on the other hand are electrically insulated from the bone or from plastic can exist. These agents do not interfere with the natural and required longitudinal division of the bioelectrical potential of the injured bone and do not prevent normal regeneration of the bone tissue. At the same time, this results in the required stability and high negative bioelectrical potentials are maintained on both sides.
These create more normal conditions for accelerated metabolism in the bone and for the transport of more positive mineral ions. These electrically isolated means result in more advantageous conditions for the healing of the wound in the soft tissue. Another advantage is that they offer better opportunities for electrical stimulation of the bone connection.

The surgical procedure for the osteosynthesis of a broken tubular bone or the osteotomy of a bone is the same as for the known osteosynthesis. The only difference is that plastic is used in the present case
In the case where plastic sheets are used, the following procedure is used. The design and function of the plastic plates are the same as in the known metal plates.
The difference lies in their additional insulation properties, which are achieved by a3 coating the metal plate with a compatible thermoplastic from the class of polyamides, which is produced in powder form as rust protection for the metal parts and using the formula H (NH- (CEI2) l0-CO) nOH is known, b) covering the metal plate with a compatible glass or porcelain, which is applied to the glass intermediate layer by means of electrophoresis, and c) producing the plate completely consisting of a compatible thermoplastic, the plastic originating from the group of the polycarbonates. The experimental application of the method and the means too
its realization was obtained through comparative experiments with test rabbits. In the case of a rabbit bone connected to a plastic plate, regeneration of the bone tissue accelerated compared to a rabbit bone connected to a metal plate occurs. Already five weeks after the operation the
Plate a thicker and complete filling of the fracture with callus without periosteal formation, observed both microscopically and radiographically. The bones associated with other metal plates showed an osteotal callus without pronounced endosteal callus formation and filling of the fracture site.
These results were clinically confirmed in cases of severe pseudoarthrosis.
