Verfahren zur Herstellung des Hydrochlorids des 10-(p -Morpholylpropionyl) -Phenthiazin-2-Karbaminsäureäthylesters
Die vorliegende Erfindung bezieht sich auf ein Verfahren zur Herstellung des Hydrochlorids des 10-(ss-Mor- pholylpropionyl) -phenthiazin-2-karbaminsäureäthylesters, das als Arzneimittel in der medizinischen Praxis Verwendung findet.
Das erfindungsgemässe Verfahren zur Herstellung des Hydrochlorids des 10-(,B-Morpholylpropionyl)-phen- thiazin-2-karbaminsäureäthylesters der folgenden Formel

besteht darin, dass Phenthiazin-2-karbaminsäureäthylester mit 3-Chlorpropionsäurechlorid in einem indifferenten organischen Lösungsmittel beim Siedepunkt des Lösungsmittels umgesetzt wird. Der erhaltene 10-(B- -Chlorpropionyl)-phenthiazin -2- karbaminsäureäthylester wird mit Morpholin im Medium eines indifferenten organischen Lösungsmittels behandelt, dann der erhaltene 1 0-(-Morpholylpropionyl)-phenthiazin-2-karbaminsäure- äthylester der Einwirkung von Chlorwasserstoff unterzogen, worauf Endprodukt abgetrennt wird.
Man verwendet zweckmässig als indifferentes organisches Lösungsmittel Benzol, Toluol, Chloroform oder Dichloräthan.
Das erfindungsgemässe Verfahren zur Herstellung des Hydrochlorids des 1 O-Op-Morpholylpropionyl)-phen- thiazin-2-karbaminsäureäthylesters wird zweckmässig wie folgt durchgeführt:
Den Phenthiazin-2-karbaminsäureäthylester löst man in einem indifferenten organischen Lösungsmittel, wobei man als solches zweckmässig Benzol, Toluol, Chloroform oder Dichloräthan verwendet. Dann fügt man das ,-Chlorpropionsäurechlorid hinzu und erhitzt das Gemisch auf den Siedepunkt des organischen Lösungsmittels während 3-6 Stunden.
Aus dem Reaktionsgemisch fällt nach dessen Klärung mit Kohle und Abkühlung auf Zimmertemperatur der 1 0-(p-Chlorpropionyl)-phenthiazin-2-karbaminsäureäthyl- ester aus. Die Ausbeute an Endprodukt beträgt 65-70% der Theorie. Schmp. 169-170 C.
Den erhaltenen 1 0-(l-Chlorpropionyl)-phenthiazin-2- -karbaminsäureäthylester löst man in einem indifferenten Lösungsmittel und fügt Morpholin hinzu.
Zur Beschleunigung des Prozesses erwärmt man die Reaktionsmasse auf eine Temperatur von 60-120 C, trennt dann das als Niederschlag ausgefallene Morpholinhydrochlorid ab und gibt dem Gemisch Salzsäure bis zur sauren Reaktion hinzu. Die erhaltene Lösung klärt man mit Kohlen und stellt alkalisch. Die ausgeschiedene Base, den 1 O-(p -Morpholylpropionyl) -phenthiazin-2-karb- aminsäureäthylester extrahiert man mit einem organischen Lösungsmittel und behandelt ihn mit Chiorwasser- stoff. Man erhält das Hydrochlorid des 10-(-Morpholyl- propionyl)-phenthiazin- 2- karbaminsäureäthylesters. Die Ausbeute an Endprodukt beträgt 70-75% der Theorie.
Der Prozess zur Herstellung des Hydrochlorids des 1 0-(0-Morpholylpropionyl) -phenthiazin-2-karbaminsäure- äthylesters kann ohne Abtrennung des 1O-(1p-Chlorpro- pionyl)-phenthiazin-2- karbaminsäureäthylesters durchgeführt werden. Dabei löst man nach dem Abdestillieren des organischen Lösungsmittels und des überschüssigen Säurechlorids den Rückstand in einem indifferenten organischen Lösungsmittel, gibt Morpholin zu und führt den Prozess wie oben beschrieben durch.
Wir fanden, dass das Hydrochlorid des 10-(1l3-Morpho- lylpropionyl)-phenthiazin-2-karbaminsäureäthylesters eine pharmakologische Aktivität besitzt und der Wirkstoff des Arzneimittels antiarrhytmischer Wirkung ist, das von uns vorläufig als Äthmosin bezeichnet wurde.
Das Arzneimittel Äthmosin besitzt eine ausgeprägte antiarrhythmische Wirkung. Besonders zweckmässig ist die Anwendung des Arzneimittels bei Arrhythmien, die mit der Störung der Funktion der Erregbarkeit der Herzmuskel verbunden sind, d.h. bei verschiedener Art Extrasystolen, paroxysmalen Tachykardie und im Falle der Paroxysmen des Vorhofflimmerns.
Das Arzneimittel Äthmosin besitzt Vorteile gegenüber den bekannten antiarrhythmischen Mitteln, dem Chinidin und Novocainamid, da es eine grössere therapeutische Breite besitzt und keine troxischen Effekte hervorruft. Ausserdem unterdrückt das Chimidin und das Novocainamid alle Funktionen des Herzens (Automatismus, Erregbarkeit, Leitfähigkeit und Zusammenziehbarkeit).
Das Äthmosin unterdrückt, wie in der Klinik festgestellt wurde, selbst bei intravenöser Einführung nur unbedeutend die Leitfähigkeit der Herzmuskel und beeinflusst die Funktion der Zusammenziehbarkeit nicht.
Das Arzneimittel enthält den Wirkstoff in Kombination mit einem pharmazeutischen Füllstoff für Tabletten oder mit einem pharmazeutischen Verdünnungsmittel für Injektionslösungen.
Die therapeutische Wirkung des Äthmosins wurde in Kliniken an 298 Kranken mit verschiedenen Formen der Störung des Herzrhythmus verschiedener Äthiologie untersucht. Die Störungen des Rhythmus kamen bei diesen Kranken im wesentlichen in Form von Sinustachykardie, in verschiedenen Formen von Extrasystolen, Anfällen von paroxysmalen Tachykardie, Anfällen von Vorhofflimmern, in ständiger Flimmerarrhythmie und in gemischten Formen von Arrhythmie zum Ausdruck.
Die klinische Untersuchung des Äthmosins ergab, dass ein besonders guter Effekt bei der Behandlung solcher Formen der Störung der Herzarrythmie beobachtet wurde wie Extrasystole, paroxysmale Tachikardie und Anfälle von Vorhofflimmern, deren Genesis Erkrankungen zugrundeliegen wie atherosklerotische Koronarokardiosklerose, Myokarbinfarkt, hypertonische Krankheit, Thyreotoxikose und vergetative Neurose. Etwas weniger leicht lassen sich die oben aufgezählten Formen der Arrhythmie rheumatischen Ursprungs behandeln.
Das Äthmosin verwendet man peroral und intramuskulär in Form von Pulver oder Tabletten mit je 0,025 g und intravenös in Form von Injektionslösungen mit einem Gehalt an Wirkstoff von 2,5 Gew.-%.
Die einmalige Dosis von Äthmosin bei der peroralen Verabreichung beträgt 25 mg. Man beginnt die Einnahme des Arzneimittels mit Tabletten von 25 mg dreimal pro Tag; bei guter Verträglichkeit (Fehlen von dispeptischen Erscheinungen) steigert man die Dosis auf 4-6 Tabletten pro Tag. Wenn sich der Effekt beim Fehlen von Nebenerscheinungen nicht entwickelt, erhöht man die Dosis auf 8-9 Tabletten pro Tag (je 2 Tabletten 4mal pro Tag). Die mittlere therapeutische Äthmosindosis beträgt 4-6 Tabletten pro Tag. Nach dem Erreichen des Effektes bei maximalen Dosen geht man zu den schwächeren Dosen, die etwa 2/3 der maximalen ausmachen.
Die Behandlungsdauer beträgt mindestens 7-15 Tage.
Wenn es notwendig ist, kann das Arzneimittel länger (bis 4-6 Wochen) verwendet werden. Die Kurdosis von Äthmosin ist individuell und hängt von der Form der Arrhythmie ab.
Für intramuskuläre Injektionen verwendet man die 2,5%ige Ampullenlösung, die vor der Injektion in 0,250,5%iger Novokainlösung oder in physiologischer Lösung verdünnt wird.
Die Verwendung des Arzneimittels beginnt man mit der Dosis von 2 ml 2,5%iger Lösung, die auf 10 ml mit physiologischer Lösung oder mit 5%iger Glukoselösung verdünnt wird.
Das Arzneimittel wird langsam während 3-4 Minuten eingeführt. Bei guter Verträglichkeit kann die einmalige Dosis des Arzneimittels auf 4 ml erhöht werden. Der Ampulleninhalt wird auch mit physiologischer Lösung oder mit 5%iger Glukoselösung auf 10 ml verdünnt und langsam während 4-5 Minuten in die Vene eingeführt.
Die verwendeten Dosen des Arzneimittels und die Wege seiner Einführung hängen von der Form der Arrhythmie ab.
Das Äthmosin wird von den Kranken gut vertragen.
Das Arzneimittel besitzt unbedeutende Nebenerscheinungen.
In einzelnen Fällen beobachtet man schwache Schmerzen im epigastralen Gebiet und leichter Kopfschwindel, die nach der Verminderung der Dosis oder nach der Einstellung der Verwendung des Arzneimittels verschwinden.
Kontraindikationen für die Verwendung des Arzneimittels sind Störungen der Funktion der parenchymatosen Organe und der Leitfähigkeit des Herzens, bei vor ausgegangener Behandlung mit den Inhibitoren der Monoaminooxydase und gemeinsamer Verwendung mit diesen.
Das Arzneimittel ist in einem trockenen dunklen und kühlen Raum unter den Stoffen der Liste aB)y aufzubewahren.
Zum besseren Verstehen des Verfahrens zur Herstellung des Hydrochlorids des 10-(lp-Morpholylpropionyl)- -phenthiazin-2-karbaminsäureäthylesters werden nachstehend folgende Beispiele angeführt.
Beispiel I
Zu einer Lösung von 10 g (0,035 Mol) Phenthiazin -2-karbaminsäureäthylester in 30 ml wasserfreiem Toluol tropft man 5,3 g (0,042 Mol) 0-Chlorpropionsäurechlorid hinzu und kocht das Gemisch bei einer Temperatur von 1 10-1200C während 4 Stunden. Dann klärt man das Reaktionsgemisch mit Kohle und kühlt auf Zimmertemperatur ab.
Der 10- (f3t-Chlorpropionyi) - phenthiazin -2- karbaminsäureäthylester fällt als Niederschlag aus; diesen filtriert man ab und erhält 10,2 g (77,5% der Theorie) Schmp.

10,2 g erhaltenes Produkt löst man in 50 ml Toluol auf, fügt 4,72 g Morpholin hinzu und kocht das Gemisch bei einer Temperatur von 110-1200C während 3 Stunden. Das ausgeschiedene Morpholinhydrochlorid filtriert man ab. Das Toluolfiltrat wäscht man zum Entfernen des überschüssigen Morpholins mit Wasser und stellt dann mit verdünnter Salzsäure auf pH=3. Die wässerigsaure Lösung trennt man ab, klärt mit Aktivkohle und stellt alkalisch auf pH=8,9. Man erhält die Base, den 10-q3- Morpholylpropionyl)-pbenthlazin-2-karbaminsäureätayl ester. Schmp. 156-1570C.

Die erhaltene Base wird mit 100 ml Toluol extrahiert.
Den Toluolextrakt trocknet man mit Magnesiumsulfat und gibt der getrockneten Toluollösung trockene ätherische Wasserstoffchloridlösung bis zur vollständigen Ausscheidung des Endproduktes zu. Man erhält 9,53 g (76,2%) Hydrochlorid des 10-(p-Morpholylpropionyl)- -phenthiazin-2-karbaminsäureäthylesters. Nach der Umkristallisation aus dem Dichloräthan Schmp. 1950C (Zer- setzung).

Beispiel 2
10 g Phenthiazin-2-karbaminsäureäthylester löst man in 30 ml Dichloräthan auf, tropft 5,3 g 0-Chlorpropion- säurechlorid hinzu und kocht das Gemisch bei einer Temperatur von 850C innerhalb 5 Stunden. Weiter wird der Prozess ähnlich dem Beispiel 1 unter Verwendung von Dichloräthan als Lösungsmittel durchgeführt. Man erhält 9 g (72%) Endprodukt.
Beispiel 3
10 g Phenthiazin-2-karbaminsäureäthylester löst man in 30 ml Chloroform auf, tropft 5,3 p-Chlorpropionsäure- chlorid hinzu und kocht das Gemisch bei einer Temperatur von 650C während 5 Stunden. Im übrigen führt man den Prozess ähnlich dem Beispiel 1 unter Verwendung von Chloroform als Lösungsmittel durch. Man erhält 8,3 g (70%) Endprodukt.

The 10- (B- -Chlorpropionyl) -phenthiazine -2-carbamic acid ethyl ester obtained is treated with morpholine in the medium of an inert organic solvent, then the 10- (- morpholylpropionyl) -phenthiazine-2-carbamic acid ethyl ester obtained is subjected to the action of hydrogen chloride , whereupon the end product is separated.
It is expedient to use benzene, toluene, chloroform or dichloroethane as the inert organic solvent.

The ethyl phenthiazine-2-carbamate is dissolved in an inert organic solvent, benzene, toluene, chloroform or dichloroethane being used as such. The, -chloropropionic acid chloride is then added and the mixture is heated to the boiling point of the organic solvent for 3-6 hours.
After clarifying with charcoal and cooling to room temperature, the reaction mixture precipitates from the 10- (p-chloropropionyl) -phenthiazine-2-carbamic acid ethyl ester. The yield of the end product is 65-70% of theory. 169-170 C.
The resulting 1 0- (1-chloropropionyl) -phenthiazine-2-carbamic acid ethyl ester is dissolved in an inert solvent and morpholine is added.

We found that the hydrochloride of 10- (1l3-morpholylpropionyl) -phenthiazine-2-carbamic acid ethyl ester has a pharmacological activity and is the active ingredient of the drug with an anti-arrhythmic effect, which we have provisionally called ethmosine.
The medicinal product ethmosine has a pronounced antiarrhythmic effect. The use of the drug is particularly useful in arrhythmias associated with the disturbance of the excitability function of the heart muscles, i.e. in various kinds of extrasystoles, paroxysmal tachycardia, and in the case of paroxysms, atrial fibrillation.
The medicinal product ethmosine has advantages over the known antiarrhythmic agents, quinidine and novocainamide, as it has a greater therapeutic range and does not cause any troxic effects. In addition, the chimidine and the novocainamide suppress all functions of the heart (automatism, excitability, conductivity and contractibility).
As has been established in the clinic, ethmosine only insignificantly suppresses the conductivity of the heart muscle even when administered intravenously and does not affect the function of contractibility.
The drug contains the active ingredient in combination with a pharmaceutical filler for tablets or with a pharmaceutical diluent for injection solutions.
The therapeutic effect of ethmosine was studied in clinics on 298 patients with various forms of disturbance of the heart rhythm of different etiology. The disturbances of the rhythm in these patients were mainly expressed in the form of sinus tachycardia, in various forms of extrasystoles, attacks of paroxysmal tachycardia, attacks of atrial fibrillation, in constant ciliated arrhythmia and in mixed forms of arrhythmia.
The clinical examination of ethmosine showed that a particularly good effect was observed in the treatment of such forms of cardiac arrhythmia disorder such as extrasystole, paroxysmal tachicardia, and attacks of atrial fibrillation, the genesis of which are underlying diseases such as atherosclerotic coronary cardiosclerosis, myocarbic infarction, hypertonic disease, thyrotoxicosis and thyrotoxicosis . The forms of arrhythmia of rheumatic origin listed above are somewhat less easy to treat.
The ethmosine is used perorally and intramuscularly in the form of powder or tablets with 0.025 g each and intravenously in the form of injection solutions with an active ingredient content of 2.5% by weight.
The single dose of ethmosine for oral administration is 25 mg. You start taking the drug with tablets of 25 mg three times a day; if tolerated (lack of dispeptic symptoms) the dose is increased to 4-6 tablets per day. If the effect does not develop in the absence of side effects, the dose is increased to 8-9 tablets per day (2 tablets 4 times per day). The mean therapeutic dose of ethmosine is 4-6 tablets per day. After achieving the effect at maximum doses, one goes to the weaker doses, which make up about 2/3 of the maximum.
The duration of treatment is at least 7-15 days.
If necessary, the medicine can be used longer (up to 4-6 weeks). The treatment dose of ethmosine is individual and depends on the form of the arrhythmia.
For intramuscular injections, the 2.5% ampoule solution is used, which is diluted in 0.250.5% novocaine solution or in physiological solution before the injection.
The use of the drug is started with the dose of 2 ml of 2.5% solution, which is diluted to 10 ml with physiological solution or with 5% glucose solution.
The drug is introduced slowly over 3-4 minutes. If well tolerated, the single dose of the drug can be increased to 4 ml. The contents of the ampoule are also diluted to 10 ml with physiological solution or with 5% glucose solution and slowly introduced into the vein over 4-5 minutes.
The doses of the drug used and the ways of its introduction depend on the form of the arrhythmia.
The ethmosine is well tolerated by the sick.
The drug has minor side effects.
In isolated cases, mild pain in the epigastric area and mild head dizziness are observed, which disappear after reducing the dose or after stopping use of the drug.
Contraindications to the use of the drug are disorders of the function of the parenchymatous organs and the conductivity of the heart, in the case of prior treatment with the inhibitors of monoamine oxidase and joint use with them.

Example I.
To a solution of 10 g (0.035 mol) of ethyl phenthiazine-2-carbamate in 30 ml of anhydrous toluene is added dropwise 5.3 g (0.042 mol) of 0-chloropropionic acid chloride and the mixture is boiled at a temperature of 110-1200C for 4 hours. The reaction mixture is then clarified with charcoal and cooled to room temperature.
The 10- (f3t-Chlorpropionyi) -phenthiazine -2-carbamic acid ethyl ester separates out as a precipitate; this is filtered off and 10.2 g (77.5% of theory) mp.
