Deuxième <I>opération.</I>
A method and device for rapid localized and centered radiography. The present invention relates to a method and a device for rapid localized and centered radiography, applicable mainly but not exclusively to stereo-radiography.
The method consists in using, to determine the localization, centering, exposure and transparency of the final image (s), a first rough-cut X-ray image, immediately developed and examined without the patient having left the X-ray machine. .
For the practice of this method, a device is preferably used ensuring the guiding of the locator tube of the X-ray emitting bulb, so that the beam always passes whatever the position and orientation of the beam. bulb, by the central point of a movable member carrying the sensitive film or, in other words, at the point where the diagonals of the film meet, the Edit organ being able to take, with respect to the operating table, trans- lation in two directions perpendicular to each other, movements which are determined by marking on an appropriate table receiving the roughing cliché.
The point of centering of the X-ray beam on the movable member which carries the sensitive film is preferably located on the sensitive layer of the film, as is done in certain known devices, the translational movements s 'performing in the direction of the longitudinal and transverse axes of the operating table. It is advantageous to arrange the locator tube so that the field of the X-ray beam can be varied at will.
This operating mode has the advantage of ensuring the automatic and certain centering on the object to be examined; it makes it possible to use small format films (especially 9 X 12) for the final radiograms, which constitutes a significant saving in film; it improves the sharpness of the radiograms obtained, which facilitates their reading and makes it possible to obtain particularly readable stereoradiographs.
In particular, the first image makes it possible to recognize the location of the large diffusion blacks, which are harmful. much, as is known, to the clarity of the radiograms, and to eliminate them by carrying out both centering and appropriate localization of the beam.
The roughing X-ray avoids the risk of errors resulting from too narrow a localization.
We understand, on the other hand, that the roughing cliché has two purposes: first, it allows. localization for more precise definitive radiographs, better than a simple anatomical table would allow, since it gives the image of the patient himself, and secondly, it allows to appreciate the correct exposure time in view of these same final x-rays. Thus, the time required by the preparatory operations for the. taking definitive x-rays does not take longer than in usual practice, which also requires taking a preparatory image to allow the correct exposure time to be assessed.
This process ensuring rigorous and automatic centering of the X-ray beam can be used without preparation by any operator, even inexperienced, with great success. It can be implemented in particular by means of the particular device which will be described and represented below by way of example, and which can be easily adapted in a mobile and interchangeable manner on all existing devices without requiring the implementation of a special, complicated and expensive tools.
The device described enables the desired centering to be carried out automatically. Its emitting hen is guided during its <B> displacements- </B> by a three-sided frame, which is articulated, on the. examination table by means of a ball joint in the form of a spherical cap, the pivoting center of which is on the sensitive face of the plate or film. The spherical ball joint is preferably mounted with the interposition of balls on a spherical bearing surface or on a raceway formed in the operating table or in a part integral with the latter.
With this device, the pivoting center, which is none other than the center of the sphere of which the cap-ball joint is an element, is placed on the face of the sensitive film whatever the inclination given to the emitting tube. .
This arrangement allows radiographs to be made very quickly and with certainty from different angles which makes it possible, by restitution, to obtain stereoradiographs making it possible to easily locate the part examined.
The guide device according to the invention is easily connected to the <B> do </B> bulb locator tube: it can slide on the latter during the movements of the bulb while ensuring the latter an orientation directed towards the center of the cap-ball joint. This guiding device can therefore be adapted to any existing device without causing mechanical complications.
We can. also have diaphragms on the locator tube making it possible to: adjust the field of the X-ray beam suitably and the locating device then comprises circular traces corresponding to the different diaphragms of the locator tube.
The appended drawing represents, by way of example, an embodiment of the device according to the invention, and variants.
The fi-. 1 is a perspective view of the operating table arranged for the use of the method.
Fig. 2 is a sectional elevation view of the locator tube with its field adjuster, or diaphragm.
Fig. 3 shows the entire radiographic bulb guiding device.
Fig. 4 is a schematic view showing an auxiliary centering means of the device.
The operating table 1 (fig. 1), which can be formed from one or more tilting parts, has an opening in the operating area l 'receiving a movable member 2 arranged normally below the median position of the ampoule with its locali sator tube 3. The movable member 2 carries the cassette (not shown) containing the sensitive plate or film.
It can take a translational movement in the transverse direction of the table in the direction of arrow F under the action of the screw 4 controlled by the button 5 (or by any other suitable means). It can also take a translational movement along the longitudinal direction of the table along arrow G by means of the stick (or other operating member) 7 wedging the screw 6.
The opening 1i comprises a rebate capable of receiving a large-format plate-holder frame, not integral with the mobile assembly and serving to take the original roughing radiogram.
On the movable member 2 is fixed an index 8, the end 9 of which can move in front of the marking device 10. This marking device is preferably constituted by a glass plate mounted in a recess of the table 1 and bearing , engraved on its lower face, graduations in the direction of the transverse and longitudinal axes of the table; these graduations can also constitute a grid of the glass plate.
In the example shown, the index 8 has also been provided with a series of circles 8 'corresponding respectively to the beams of rays delimited by the diaphragms of the locating tube. The plate is illuminated transparently by means of any suitable device, for example a lamp 11 with reflector 12 and frosted glass plate 13.
One could also make integral with the in dex a transparent plate carrying the outline of the circles corresponding to the different diaphragms, and sliding flat under the grid glass plate.
The locator tube 3 (fig. 2) comprises, at its upper end, a longitudinal slot 14 which can give passage to a disc 15 pierced with a calibrated hole 16 and provided with a rod 17. To adjust the field of the locator tube , we introduce through the slot 11 a. disc comprising the suitable opening 16; then, when this disc has entered the tube, it is subjected to a rotation of <B> 90 '</B> until it comes to rest on supports 18.
The lower end of the tube can also receive removable diaphragms of different calibers such as 19, which cooperate with the diaphragm 15 to limit the cone or the cylinder forming the X-ray beam. These lower diaphragms are interchangeable by means of a suitable mounting, for example bayonet.
The locator tube is connected to the movable member 2 of the operating table 1 by a positive 3rd guide device 20. This can, in particular, be constituted, as shown, by a frame with three arms, the end 21 of which can be rotate in a cap-ball joint integral with the member 2. Moreover, a particular embodiment of this device will be described later in detail.
Accessorily can be provided in the center of the inner ball-cap, an optical light system (bulb, reflector, lenses for example) emitting a light beam along the axis of the tube 3. This light ray, during the preparation of the shot, hit the body. of. patient exactly at the X-ray exit point, making adjustment easier. This ray materializes, in some way, the fourth side of the bulb holder frame.
According to the method forming the subject of the invention, the operation is carried out as follows: <I> First operation </I> (Automatic centering).
The patient being placed on the operating table, the tube locator of the hen is brought to the suitable incidence by means of landmarks or anatomical planes, possibly using the light adjustment device indicated above. In this operation, the locator is provided with a fairly large field. A first roughing X-ray is taken on a large scale, keeping the end 9 of the index 8 in the center of the locating device 10. The X-ray thus taken is developed.
Second <I> operation. </I>
The radiograph having been developed, the patient remaining motionless on the operating table during this development, which lasts about ten minutes, the wet film is then deposited on the graduated glass plate 10, so that the center of the image comes to eider corner with the center of the marking device, an operation which is facilitated by the lines provided on the glass plate.
We then bring. end 9 of index 8 on the part which is to be specially examined, using screws 4 and 6, which means that this index marks the geometric lines. of the organ to be examined. A certain anatomogeometric centering of the X-ray beam is thus achieved.
With this registration, we can then orient the bulb in any direction, the locator tube always being guided with respect to the center of the movable member 2, and the point of exit of the X-ray beam will always pass through the center of that. -this. In this way, without the patient having been moved, an X-ray image of the object to be examined, or even several exactly centered radiographs specific to stereoradiography, will automatically be obtained on the skin carried by organ 2.
The centering operation which has just been described requires a few minutes.
As we mentioned, there will be. advantage for the final X-ray (s) thus taken, in reducing the field of the localizer, by modifying the diaphragms of the latter; the circles 81 carried by the index 8 will directly give the optimum diaphragm. We can also. assess, for the development of the final radiography, the necessary corrections of the exposure time, emerging from the examination of the roughing test.
Perfectly clear radiographs are thus obtained, automatically embracing the part to be examined and capable of being taken in small format. By carrying out successive radiographs under various angles of view, the stereoradiography of a specific organ can be carried out with certainty.
The bulb guide and oscillation device is established as shown in FIG. 3.
The Y-ray emitting bulb is shown at 23. It comprises a cylindrical locator tube 24. The bulb 23 is supported by an arm 25 which can be swiveled in all directions and secured to an operating device (not shown). . On the locator tube is mounted, which can slide and turn on it, a bracket 26 connected to a frame with three arms 20, 27 and 28 connecting it to the examination table shown schematically at 1 and in which there is no not shown, for clarity, the mobile cassette carrying the small format photograph.
One can, of course, provide any other means of connection of the frame relative to the bulb holder cup. The connection of this frame with the examination table 1 is obtained by a cap-ball joint 29, the pivoting center of which 29 'is, by construction, located on the center of the sensitive plate or film 30 (enclosed in a cassette) . The cap-ball joint 29 pivot- with the interposition of a ball bearing 31, in a spherical surface integral with the table 1, having at the lower part an orifice 32. In this orifice passes freely the rod 33 integral with the cap 29, on the other hand frozen to the movable frame 20, 27, 28.
The frame consists of the part 20 fitted with a stabilizing counterweight 34 and on which the sleeve 35 can be mounted, immobilized by a screw 36 in the suitable position and connected to the arm 28. On this arm 28 is mounted, by the sleeve 37, the arm 27 carrying the caliper 26.
The various components being suitably adjusted during assembly, it will be understood that it will be possible to make the whole of the support take all orientations appropriate to the various cases, by pivoting around the head 29, with, as the center, the point 29 < B> '</B> defined above.
The operator will therefore be certain that the X-ray beam will always be centered at 29 ′, whatever the orientation he gives to the bulb 28. This dis p os-1 ition allows us to use it, without fear of "failures", plates or films of very small format and facilitate stereoradioscopy or very localized stereoradiography,
given that for the second shot, the setting remains automatically.
The markings will be facilitated by: the fact that the carriage and the organs which support the bulb, not shown, independently (their rotational movement, will be movable in two directions perpendicular and parallel to the axes of the divisions of table 10.
The freedom of movement of the plate holder, achieved as has just been explained, allows in particular the mode of use of the apparatus shown in FIG. 8, in which the bulb has been designated at A at the location corresponding to a first taking of a stereoradiographic image. At this position, the axis of the beam 1 indeed passes through the object 0 and through the center of the image.
If the bulb is then brought to the second position A 'for taking the second image, the axis of the beam always passes through the center: of the image, but no longer through the object. If the beam is strongly diaphragmatic, said object may be outside the beam. It is then that by acting on button 5, we will move the image to bring it to the position shown in dotted lines, at which the bulb, the object and the center of the image will again be in a straight line.
The value of the horizontal displacement M can for example be determined by experience or calculation, based on the obliquity of the x-ray beam and the estimated height of the body above the plane of the film.
It can be seen that the device: described,: with a horizontal operating table, can also be established with a vertical panel by equipping it with means. compensation and balancing with counterweights and chains, cables. or levers @defence.
