Vorrichtung zur Vornahme von Darm- und -Seheidenspülungen ausserhalb einer Badewanne. Es sind bisher Apparate für Darmspü lungen sowohl in der Wanne, sogenannte Subaqualbäder, wie auch solche für Spülun gen ausserhalb der Wanne bekannt. Die Subaqualbäder weisen nicht nur dadurch sehr viele Mängel auf, dass sie umständlicher Vor bereitungen bedürfen, wie auch hoher In vestierungskosten, sondern es hat sich er geben, dass diese Bäder bei den Badenden Übelkeit, Angstzustände, Schmerzempfindun gen, ja selbst schädigende Nebenwirkungen hervorgerufen haben.
Die sogenannten Trok- kenapparate hingegen entsprechen infolge ihrer komplizierten, umständlichen Hand habung keinesfalls den allgemein notwen digen und praktischen Anforderungen. Au sserdem entsprechen sie infolge des ausströ menden Geruches keinesfalls den hygieni schen Bedürfnissen.
Durch die vorliegende Erfindung werden nicht nur die vielen Mängel und unangeneh men Begleiterscheinungen behoben, wie sie die bisher bekannten Apparate aufweisen, sondern es wird ein Apparat geschaffen, der allen medizinischen, praktischen und selbst hygienischen Anforderungen bestens ent spricht und die Bäder infolge ihrer nahezu vollständig druckfreien Applikation zu einer angenehm empfundenen Behandlungsart wer den lässt.
In der beigegebenen Zeichnung ist der Apparat gemäss der Erfindung in beispiels weiser Form dargestellt. Fig. 1 zeigt -den Apparat im Schaubilde, Fig. 2 einen grösse ren Schnitt durch die Mittelpartie, Ableit- haube und Trag- und Verstellvorrichtung für Spülrohr und Brause: Fig. 3 zeigt die Ab- leithaube im Schaubilde;
Fig. 4 zeigt ein Fil ter, Fig. 5 ein in die Spülleitung eingesetztes Tropfklysma mit doppelter Wandung, Fig. <B>6</B> ein dreiarmiges Rückschlagventil; Fig. 7 zeigt ein Spülrohr; Fig. 8 stellt im Schau bilde den gotauffangbehülter dar;
Fig. 9 zeigt im Schaubilde die Schaltvorrichtung; Fig. 10 zeigt gleichfalls im Schaubilde die Trag- und Versstellvorrichtung für Spülrohr und Brause und Fig. 11 und 12 zeigen bei spielsweise Ausführungsformen der Ableit- haube.
Beim Gebrauch der Vorrichtung liegt der Patient in angenehmer Lage mit vollständig entspannter Bauchdecke auf einem hierzu ge eigneten Lager, das beispielsweise aus mit einander verbindbaren Teilen besteht. Der eine, der Kopfteil 1, ist mit einer verstell baren Rückenlehne 3, ähnlich der einer übli chen Liegevorrichtung, ausgestattet, die an der Stelle, wo sie unter Kreuz und Hüfte zu legen kommt, zur Entlastung bezw. Ent spannung aller Gesäss- und Rückenmuskeln entsprechend kurvenförmig gebogen ist und in eine mit Ausbuchtung versehene Wulst 6 (Widerlager) übergeht,
die zur Abstützung des Gesässes dient und an dieser Stelle mit- telst Scharnieren 2 mit der Rückenlehne 3 verschwenkbar verbunden ist. Beide Teile 1 und 7 sind gepolstert und ausserdem sorgt ein Kopfpolster 4 für grössere Bequemlichkeit. Der verschwenkbare Rückenteil 3 ist an Zahnbögen 5 in der Höhe verstellbar. Der Fussteil 7, der an dem Kopfteil 1 durch Haken 8 fest angeschlossen werden kann,. trägt einen mit dem Kissen 4" versehenen Schlitten 9, der mittelst einer Klinke 14 an einer Zahnstange 15 gegen Rückverschiebung gesichert werden kann.
Es ist somit möglich, dem Patienten eine seiner Gestalt oder dem Bedürfnis entsprechende bequeme Lage zu geben. An der Wulst 6 ist in deren Längs mitte ein Ausschnitt 12 vorgesehen, der eines teils zur Aufnahme der Trag- und Verstell vorrichtung für das Spülrohr und die Brause und andernteils zur Aufnahme der Ableit- haube dient.
Diese Ableithaube 19 (Fig. 2 und 3) ist ein gekrümmter Hohlkörper ähnlich einer Schaukelwanne, der mittelst Bolzen 16 in Lagerschlitze 17, die beiderseits des Aus schnittes 12 vorgesehen sind, eingesetzt wird. Die Lagerschlitze 17 laufen in ihrer Fort setzung in Führungsschienen 18 aus, in wel chen die Ableithaube 19 in herabhängendem Zustand unter die Liegefläche 3 verschwenkt und verschoben werden kann. An der Ableit- haube 19 sind zwei Rohransätze vorgesehen, deren einer, 23, zum Anschluss einer Darm druckentlastungsleitung dient.
Die Ableit- haube ist so konstruiert, dass sie die Trag- und Verstellvorrichtung für Spülrohr und Brause überdeckt und vom Steissbein zwi- sehen den beiden Oberschenkeln an deren Innenseite abschliessend bis zur Symphyse bezw. auf den Bauch reicht, allen Körper formen sich anpasst und so sämtliche um den After liegende Muskelpartien von allen be klemmenden oder anpressenden Flächen voll ständig frei hält.
Es wird daher kein Druck <U>auf</U> die um den After liegenden Muskelpar tien erzeugt, so dass zu einer Defäkation nahezu kein Gegendruck notwendig wird und der Defäkationsakt leicht vor sich gehen kann.
Am untern Ende der Ableithaube 19 ist eine dichtende Walze 24, beispielsweise aus Gummi, vorgesehen, die in ihrer Mitte für die Führung der Trag- und Verstellvorrich- tung eine Aussparung 24' aufweist. An den Rändern der beiden Seitenwände der Ableit- haube 19, wie auch an ihrem obern Rande weist sie gleichfalls dichtende Streifen 25, beispielsweise aus Gummi, auf, die sich zu sämmen zu einer Decke 26 erweitern. An dieser Decke 26 sind Spannbänder 27 be festigt, die gleichfalls aus Gummi oder sonst geeignetem Material hergestellt sind.
An der obern Seite der Decke 26 befinden sich ausser dem noch zwei Löcher 26', um den Durch gang der Leitungen 54" für die Spülflüssig keit und die Leitung 55' für die an der Trag und Verstellvorrichtung angebrachte Brause (Fig. 2 und 10) zu ermöglichen. Dadurch kann die Ableithaube dichtend und mit sanf tem Drucke an den Unterleib angeschoben werden, ohne unangenehme Spannungen zu verursachen und das leichte Atmen zu be hindern. Am untern Ende der Ableithaube ist noch ein Rohrstutzen 28 angeschlossen. der zur Ableitung der ausgespülten Fäkalien durch ein Rohr 29 dient, welches in den Kot auffangbehälter 30 führt.
Durch das gut dichtende Anliegen der Walze 24, der Gummistreifen 25 und der Decke 26 am Körper soll verhindert werden, dass sieh der Geruch der Fäkalien ausbreitet. Um der Ableithaube 19 einen weiteren Halt zu bie ten, kann diese ausserdem noch mittelst der an der Rückenlehne 3 angebrachten Spann gurten 27 an den Haken 19' der Ableithaube 19 befestigt werden.
In der Abflussleitung 29 ist an beliebiger Stelle ein Kotauffangbehälter angebracht, der beispielsweise die Form eines Kastens 30 besitzt, in welchen die abgehenden Fäkalien und das Spülwasser durch dessen obern Stut zen 32 einlaufen und durch den untern Stut zen 33 wieder auslaufen. Zum Zwecke der Entnahme von Kotproben lässt sich in diesen Behälter ein zum Ableiten des Spülwassers mit Löchern versehener, herausziehbarer Auffangsiebkörper 34 (Fix. 8) einschieben, wodurch jederzeit, ohne die Spülung zu un terbrechen, Kotproben entnommen werden können. Zur genaueren Kontrolle befindet sich an der Oberfläche des Kastens 30 ein Schauglas 35.
In dem untern Rohrstutzen 33 befindet sich ein Siebnetz 31, welches nur zerkleinerten Fäkalien den Durchgang ge stattet, wodurch eine Verstopfung der Ab laufrohre vermieden wird.
Um Schmerzempfindungen oder Verlet zungen durch metallische Darmspülrohre zu vermeiden, wird bei dem vorliegenden Appa rate nur ein biegsames Darmspülrohr verwen det. Dieses Darmspülrohr wird durch ein entsprechendes Zwischenstück mit der eigent lichen Spülleitung verbunden, welches Zwi schenstück in einer hierzu bestimmten Trag- und Verstellvorrichtung drehbar, seitlich ver schiebbar und mittelst Schrauben beliebig fixierbar ist.
Dadurch kann das Darmspül- rohr leicht und ohne jede Gefahr den jewei ligen Verhältnissen entsprechend weit in den After eingeführt werden. ohne unliebsame Begleiterscheinungen hervorzurufen, wie es auch allen Bewegungen des Afters willig folgt.
Die Trag- und Verstellvorrichtung für Darmspülrohr und Brause (Fix. 2 und 10) besteht aus einem in einer Scheide 37 ver schiebbaren Schlitten 36, 40, der mittels einer Schraube 39 in seiner jeweiligen Lage fest gestellt werden kann, ferner aus einem in den rechtwinklig aufgebogenen Rohren 41 des Schlittens 40 vertikal verschiebbaren Bügel 42, der ein dreh- und verschiebbares, fest stellbares Lagerstück 44 trägt, wobei alle erforderlichen Stellungen mittelst Klemm schrauben 39, 40', 43 und 45 fest eingestellt werden können. In diesem Lagerstück 44 ist ein Knierohr 46 verschieb-, verdreh- und fest stellbar, welches beispielsweise mittelst einer.
Holländermutter 47 an ein Rohrstück 48 an schraubbar ist, auf welches das Darmspül- rohr 49 aufgeschoben wird. An das freie zweite Ende des Knierohres 46 wird die Spülleitung 54" angeschlossen. Um die Ein stellbarkeit des Darmspülrohres noch zu ver feinern, können die Rohre 41 des Schlittens 40 noch ausserdem -um eine Welle 40' oder Gelenke verdrehbar sein. Es ist ersichtlich.
dass einerseits durch die Verschiebbarkeit des Bügels 42 und .durch die mehrfach vorgese hene Verstellbarkeit des Spülrohrträgers eine genaue, dem Bedürfnis und der Lage des Patienten, sowie der des Afters entsprechende Stellung des Darmspülrohres erreicht werden kann, und dass anderseits ein Herauspressen des Darmspülrohres durch den Organismus unmöglich ist. Es ist selbstverständlich, dass an das Anschlussrohr 48 bezw. Knierohr 46 oder Lagerstück 44 ein beliebig geeignetes Scheidenspülrohr angeschlossen werden kann.
Ausserdem befindet sich an den aufrech ten Rohren 41 des Schlittens 36, 40 eine Vor richtung, die als Brause dient. Sie besteht aus den beiden Lagerstücken 20, die durch ein Brauserohr 21, welches an der Vorderseite mit kleinen Bohrungen versehen ist, durch welches das Brausewasser fliesst, miteinander verbunden sind, welches an der Hinterseite einen Stutzen 22 trägt, der mittelst einer Holländermutter 22' oder dergleichen mit dem Rohrstück 22" verbunden ist. An dieses Rohrstück 22" wird die Schlauchleitung 55' angeschlossen.
Diese Brausevorrichtung kann den jeweiligen Anforderungen entsprechend auf den aufrechten Rohren 41 verschoben werden und mittelst.der an-den-Lagerstücken 20- befindlichen Schraube 20" in der jewei- ligen Lage festgehalten weiden.
Statt der vorbeschriebenen Anordnung der Ableithaube 19 kann auch ein fest oder ab nehmbar angeordneter gasten 38 (Fig. 11) vorgesehen sein, der das Spülwasser aufnimmt und ableitet. Der gasten 38 ist in die Aus- nehmung 12 eingebaut und umschliesst die Trag- und Verstellvorrichtung für Darmspül- rohr und Brause.
Nach Fig. 12 kann der gasten 38 zugleich als Träger der Trag- und Verstellvorrichtung ausgebildet sein, wobei diese Tragvorrichtung an der Stirnseite des Kastens eintreten kann. Auch die übrigen Rohrarmaturen werden an der Stirnseite an gebracht. Der Boden 38' des Kastens 38 wird zweckmässig etwas schräg verlaufend vorgesehen, wobei der Ablaufrohrstutzen 28 an der tiefsten Stelle angeordnet wird.
Die ser Kasten 38 kann auch unter die Liege fläche verschiebbar oder an einem .Scharnier 50 unter diese verschwenkbar vorgesehen sein, so dass er unter die Liegefläche zurück verschwenkt werden kann. Bei den beiden Ausführungsformen (Fig. 11 und 12) sind an den -beiden Seitenrändern der Ableithaube wie an der Oberseite derselben Dichtungs- Organe angebracht, die ebenfalls in den den Bauch überdeckenden Teil 26 der Ableit- haube mit den angeschlossenen Befestigungs bändern 27 übergehen.
Haben Darmspülapparate mit aufsteigen den Ablaufrohren durch die vorhandene Was- sersäule auf den After und auf die umliegen den Muskelpartien einen Druck ausgeübt, der die Defäkation behinderte, so haben Apparate. bei denen das Gesäss auf eine Sitzfläche zu liegen kommt, den vorhandenen Druck noch mehr oder weniger erhöht und die Defäka tion noch schwieriger gestaltet oder sogar unmöglich gemacht, was oft zu unangeneh men Begleiterscheinungen, wie selbst zu schädigenden Nebenwirkungen führte.
Diese Umstände werden bei dem Gegenstande vor liegender Erfindung schon durch die Bauart der Trag- und Verstellvorrichtung für Spül rohr und Brause der Ableithaube, wie selbst der--Liegevorrichtung, vermieden, die keinen Druck auf den After und - umliegende Mus kelpartien ausüben.
Um dennoch einen -unvorhergesehenen Darmdruck zu vermeiden, ist eine Darm druckentlastungsleitung vorgesehen, die durch die angeordnete Schaltvorrichtung in Tätig keit gesetzt wird und ein gegendruekloses Rückströmen der Spülflüssigkeit ermöglicht. Solche fast gegendrucklose rückläufige Bewe gungen des Spülwassers können auch künst lich hervorgerufen werden und haben sich bei gewissen Krankheiten sogar als notwen dig erwiesen, weil sie dadurch eine stärkere Belebung der Darmmuskulatur herbeiführen, wie sie immer für den Darm von besten Fol gen begleitet sind.
Diese rückläufigen Be wegungen lassen sich dadurch erreichen, dass der Badende durch öfteres, in kürzeren Zeit intervallen getätigtes Ein- und Ausschalten der angeordneten Schaltvorrichtung und der dadurch gänzlich Darmdruckentlastung her beiführt, so dass das eingelaufene Spülwasser durch die dadurch einsetzenden peristalti- schen Bewegungen des Darmes stossweise fast wieder gegendrucklos ausgestossen wird.
Während der Spülung sind Badende von äussern Muskelkrämpfen befallen worden, die eine Unterbrechung oder vorzeitige Beendi gung der Spülung notwendig erscheinen lie ssen. Um derart plötzlich auftretende Fälle zu beheben, wurde an der Trag- und Verstell- vorrichtung für das Spülrohr (Fig. 10) eine Brausevorrichtung angebracht, welche vom Wasserbehälter 52 gespeist wird und eben falls durch die angeordnete Schaltvorrichtung in Tätigkeit gesetzt werden kann. Sie be- spült den After und die umliegenden Muskel partien von aussen, wodurch die Krampf erscheinungen behoben werden.
Auf einem Gestell 51 (Fig. 1) befindet sich ein Wasserbehälter 52, der mit den entspre chenden Wasserstandsgläsern, sowie Thermo metern versehen ist und durch die Zwischen wand 53 in zwei Teile 52' und 52" geteilt ist. -Der Teil 52' dient zur Aufnahme des Spülwassers, während im Teil 52" sich das Wasser für die Brausevorrichtung, die in- nerhalb der Ableithaube 19 oder des Ableit- kastens 38 angeordnet ist, befindet.
Um infolge des ziemlich hoch am Gestell 51 angebrachten Wasserbehälters 52 ein um ständliches Beimengen von Heilmitteln in den selben zu vermeiden, ist am Plateau des Ge stelles 51 oder am Boden des Wasserbehälters 52 noch ein Medizin- bezw. Heilmittelbehäl ter 56 für diese Zwecke vorgesehen, der zum Ablesen der Verbrauchsmenge graduiert ist, ein Rühr- bezw. Zerkleinerungswerkzeug 56' für erst aufzuIösende Medikamente besitzt und hat in seinem Ablaufhahn 58 ein Filter 59 eingebaut, welches das Mitreissen von un gelösten Bestandteilen etc. verhindert.
Der Zufluss von diesem Behälter zur Spülleitung wird ebenfalls durch die angeordnete Schalt vorrichtung geregelt.
Dem Zwecke der ganzen Konstruktion entsprechend, münden die Leitungen 54 für die Spülflüssigkeit, 57 für das Heilmittel, 55 für die Brause und 68 eine Leitung, deren Zweck später erklärt wird, in eine gemein same Schaltvorrichtung (Fig. 9), welche als Hahn 60 mit HahnkegeT 60' ausgebildet ist und die Bohrungen 61, 62, 63 und 64 be sitzt.
Von diesem Hahn (Fig. 9) führt die Leitung 54" zu dem Knierohr 46 an der Trag- und Verstellvorrichtung, das mit dem Darmspülrohr 49 in Verbindung gebracht wird, die Leitung 55' zur Brause (21) an der Tragvorrichtung (Fig. 10), während die Leitung 71, die die Fortsetzung der Leitung 57 bildet, mit der Leitung 54 gekoppelt wird, so dass sich an diser Stelle das in Lö sung befindliche Heilmittel mit der Spül flüssigkeit verbindet.
Die Leitung 69 ent spricht der verlängerten Leitung 68, welche von der Leitung 54" abzweigt und zum Rohr stutzen 23 an der Ableithaube 19 führt und der Darmdruckentlastung dient. An der Stelle, wo die Leitung 68 von der Leitung 54" abzweigt, ist ein Rückschlagventil- gehäuse 72. 76, 77 und 7<B>7</B> angebracht, wel ches dreiarmig ausgebildet ist. In dem Arm 77, der an die Leitung 54' angeschlossen ist, sitzt ein Rückschlagventil 73, der Arm 76 bildet die Fortsetzung der Leitung 54', wäh- rend -der Arm 7<B>7</B> zum Zwecke der Darm druckentlastung mit der Leitung 68 verbun den wird.
Die vorbeschriebene Schaltvorrich tung (Fig. 9) ist auf einer Trägerplatte 76' montiert, an welcher die der gewünschten Funktion entsprechende Stellung mittelst Be zeichnungen, wie "Offen", "Brause", "Ge- schlossen" eta. ablesbar ist Der Badende oder Patient legt sich auf den zur grösseren Bequemlichkeit mit entspre chenden Kissen 4, 4' und 4" versehenen Liegestuhl, wobei einerseits durch die er höhte Fusslage, anderseits durch die vorhan dene Ausbuchtung und .Kurve an Sitz und Rückenlehne 3 eine vollständige Entspan nung aller Muskelpartien hervorgerufen wird,
die zu einer leichten Defäkation un bedingt notwendig erscheint. Hat der Ba dende diese Lage eingenommen, so wird das entsprechende Spülrohr, in diesem Falle das Darmspülrohr 49, in den After eingeführt und an der Trag- und Verstellvorrichtung be festigt und mit der Schaltvorrichtung in Ver bindung gebracht.
Die herabgelassene Ableit- haube 19, die der Tragvorrichtung für Spül rohr und Brause ziemliche Bewegungsfreiheit überlässt, wird über diese hinauf gezogen und mittelst der vorhandenen Spannbänder 27 an der Rückenlehne- 3 seitlich befestigt, so dass die Ableithaube einen Halt erhält, ohne am Körper des Badenden anzupressen und da durch einen Druck auszuüben, wodurch eine vollständig druckfreie Applikation erreicht wird.
Die Ableithaube 19 steht einesteils mit den Zuleitungen der Schaltvorrichtung, an dernteils mit dem Kotauffangbehälter in Verbindung.
Soll nun die Spülung vorgenommen wer den, so erfolgt diese auf folgende Weise: Die Schaltvorrichtung bezw. der Hahnkegel 60' des Hahnes 60 wird in "Offenstellung" ge bracht (Fig. 9 in vollen Linien), es fliesst dann nicht nur die Spülflüssigkeit vom Be hälterteil 52' dem offenen Ventil 78 und der Leitung 54" dem Darmspülröhrchen 49 zu. sondern es fliesst gleichzeitig das gelöste Heilmittel vom Heilmittelbehälter 56 durch die Leitung 57, die Bohrung 64 und die Lei- tung 71 der Spülflüssigkeit zu.
Treten nun mehr Umstände ein, die eine Unterbrechung der Darmspülung, eine Darmdruckentlastung oder gleichzeitig eine Funktion der Brause bedingen, so wird die Schaltvorrichtung bezw. der Hahnkegel 60' des Hahnes 60 auf "Brause" gestellt bezw. verdreht, wodurch die Spülleitung 54 wie die Zuleitung 57 für das gelöste Heilmittel gesperrt wird, so dass sich das Ventil 73 selbsttätig schliesst, gleich zeitig wird nicht nur der Wasserzulauf für die Brause 21 vom Teil 52" des Wasserbehäl ters 52 durch die Leitung 55, die Bohrung 61 und der Leitung 55' ermöglicht,
sondern auch der gegendrucklose Rücklauf des bereits in den Darm eingelaufenen Spülwassers durch das Darmspülrohr 49 die Leitung 54", die Leitung 68, die Bohrung 63 und die Leitung 69 in die Ableithaube 19 gestattet.
Zur vollständigen Sperrung aller Organe muss der Hahn bezw. die Schaltvorrichtung weiter verdreht und auf die Bezeichnung "Geschlossen" der Trägerplatte 76' gestellt werden.
In die Spülleitung 54 ist ein Filter 74 mit einem Auffangsieb 75 eingeschaltet, um einer eventuellen Verlegung der Spülleitung vorzubeugen. Sodann ist in die Leitung ein Tropfklysma 78 eingeschaltet, um dem Ba denden die Tätigkeit des Apparates augen fällig zu machen.
