Elektrothermischer Apparat für therapeutische Zwecke. Vorliegende Erfindung betrifft einen elektrothermischen Apparat für therapeu tische Zwecke, insbesondere zur raschen Rei- lun.g akuter, sowie chronischer Gonorrhoe und ihrer Folgeerscheinungen.
Es ist bereif;s bekannt, die Gonorrhoe, deren Erreger, die Gonokokl,zen, bekanntlich schon durch eine Wärmeeinwirkung von zirka 40 C vernichtet werden und deren Fortpflanzung unterbun den wird, durch Thermopenetration zur ra schen Ausheilung zu bringen, indem mittelst geeib ieter Vorrichtungen mit Hilfe der Elek- trizität Wärme von entsprechender Höhe an den betreffenden Körperstellen, also insbeson dere in der Harnröhre, erzeugt wird,
wobei histologisch nachgewiesen ist, dass die Schleimhaut :der Harnröhre Temperaturen bis 4\3 ohne Schädigungen erträgt. Das hierfür gebräuchliche sogenannte Diathermieverfah- ren, welches auf der Applikation von H:och- frequenzströ.men, die in der Tiefe der durch strömten Gewebe die .gewünschte Wärmewir kung erzeugen, basiert, wird vielfach mit mehr .oder weniger Erfolg angewandt. Die sem Verfahren, sowie der zu seiner Ausfüh rung erforderlichen Apparatur. haften nun noch mannigfache Mängel .an.
Zunächst ist diese Tiefenwirkung der Hochfre:quenzströme äusserst problematisch und je nach dem In dividuum .verschieden, wie auch :eine gleich mässige Wärmeverteilung über die gesamte Ausdehnung der Peniselektrode nicht .ge währleistet werden kann.
Je nach den Um ständen, Lage der Gegenelektroden etc., kön nen sogar eine Verbrennung der Schleimhaut herbeiführende lokale Überhitzungen auf treten, wenn nicht während der ganzen Be handlung, deren kritische Grenzwerte inner halb einiger Centigrade liegen, eine äusserst gewissenhafte und' dauernde Temperaturmes sung durchgeführt wird. Aus Vorstehendem ergeben sich auch ohne weiteres alle übrigen Nachteile;
es sind Aussenelektroden als Ge genelektroden erforderlich, welche die Pars pendula umgeben und den untern -Teil des Abdomens, das Perineum und die Ober schenkel bedecken. Trotzdem nun keine elek trolytischen Wirkungen auftreten, löst der Übergang des Stromes von der Innenelektrode auf die erkrankte Schleimhaut der Harnröhre beim Patienten, besonders bei empfindlichen Individuen, sensible Reizungen aus, was die Schmerzhaftibkeit der Behandlung erhöht.
Weiterhin bedingt die notwendige peinliche Temperaturmessung und Überwachung des Vorba.nges, welches übrigens ein Grund dazu ist, cl.ass nur ganz besonders mit dem Verfah ren vertraute Spezialisten dasselbe anwenden können, besondere Einrichtung hierfür, und zwar ein verschiebbares Therm-oelement in der Peniselektrode nebst zugehörigem i41 essinstru- inent und Regulierwiderständen.
Dies kom pliziert noch die ,an sich schon verwielielte Hochfrequenzapparatur und deren Hand- liabun;. Alle diese Faktoren traben dazu bei, dass bis heute die Thermotherapie der Go- norrhoe nicht im entferntesten .die Verbrei tung befunden hat, die ihr gegenüber den älteren Heilmethoden gebührt.
Diese Mängel sollen nu.n behoben werden durch den Gegenstand vorliegender Erfin dung.
Der Apparat gemäss der Erfindung besitzt ein in die zu behandelnde Körperhöhlung einzuführendes, bei Stromdurchgang infolge seines Eigenwiderstandes an seiner Ober fläche geheiztes Heizelement, in welchem iso liert von demselben ein den Strom rückleiten- der Stab gelagert ist, so dass praktiseli kein Stromiib",r@@anb auf das Gewebe der betreffen den Körperhöhlung stattfindet,
wobei eine selbsttätig wirkende Reguliervorrichtung im Stromkreis angeordnet ist, welche, sobald die Ausrlehnunw dieses Heizelementes infolge der Einwirkung der in ihm erzeugten @,#Ttirme einen bestimmten Betrag erreicht hat, den Stromkreis unterbricht, zum Zwecke,
bei Überschreiten einer bewi.sen Maximaltempe ratur die Wärmeerzeu-ung selbsttälig sofort bis zum Sinken der Temperatur zu unter brechen.
Dank dieser Einrichtung ist es möglich, iii der Harnröhre die erforde.rliclie Tempera tur zu erzeugen und ohne jegliche Tempera turmessung genau konstant zu halten, ohne dass die Gefahr einer Temperaturüberschrei- tung besteht, ohne dass irgend ein sensibles Reiz-efühl durch Stromübergang auf die bockempfindliche Schleimhaut oder elektro- lvtische Wirkung auftritt.
und ohne dass der Patient der Unannehmlichkeit des Anlebens vieler Apparatteile-, wie beim eingangs er- wälinten Verfahren, ausgesetzt wird.
Auch kann man, da eintromüberwanb in den Kör per nicht erzielt werdc,n muss, mit sehr nie derer Spannung und infolgedessen hoher Stromstärke arbeiten, so dass hierbei der Joul.esehe Effekt der ver,.verndeten Energie voll ausgenutzt.

Der Apparat kann nach dem bleichen Prinzip nicht: nur für Heilung der CTonorrhor-" sondern aueli für andere therapeutische Zwecke bebaut werden, z. B. für lar@-nbolo- wische Behandlungen, für Ohrenerkrankun gen, für V .agi nalbehandlung ete.

förm,iwes Gehäuse 1 auf, dessen unterer senkrechter Teil als Handgriff dient. An der vordern Seite des Gehäuses ist :das katheter- föianiwe Heizelement 3 eingesetzt, welches vermittelst des metallischen Zapfens 2 mit dem Gehäuse 1 leitend verbunden und in die sem wehalten ist.
Dieses Heizelement besteht aus einem aussen v ersilberten Metallrohr: Silber ist, gewählt mit Rüelrsieht auf dessen linearen Wärineausdehnungskoeffizienten und den spezifischen elektrzsehen LeitungswIder- stand. Konzentrisch im Rolir 3 und durch eine Isolationsschicht -1 von diesem getrennt ist ein sich über dessen ganze Länge er streckender Stab 5
aus Invar-Nickelstahl ge- lagert, clesscn Wärmeausdehnungskoeffizient praktisch gleich Null ist. Beide sind dadurch miteinander leitend verbunden, dass sie am vordern Ende miteinander verlötet sind;
letz teres ist abgerundet, um das Einführen des Heizelementes in die Harnröhre zu .erleich- tern. Die Länge des Heizelementes 3 ist die übliche eines Katheters, und dieses kann ent weder gerade, gestaltet, wie gezeichnet, oder mit Dittelkrümmung versehen sein.
Der Stab ragt etwas ins Gehäuseinnere vor und trägt auf seinem Ende aufgeschraubt eine Klemme 6, an der ein elastisohes Leitungsband 7 aas geklemmt ist, das zu einer Klemme 8 am ent gegengesetzten Gehäuseende führt, die dort isoliert eingesetzt und mittelst Schraube 9 festgehalten wird und welcher .der Strom von aussen zugeführt wird, während vermittelst einer weiteren Klemme 10 der andiere Pol di rekt ans Gehäuse 1 gelegt ist.
Am Ende des Stabes 5 ist ein Spanndraht 11 befestigt, wel- eher anderends an einer an der Gehäusewau- dung angebrachten Feder 12 fest ist, und von dieser Feder aus geht ein Spanndraht 13 zur entgegengesetzten Wand des Gehäuses. Der Spanndraht 13 besitzt in der Mitte ein Ge lenk 14, an welchem ein weiterer Spanndraht 15 angreift, der ebenfalls in der Mitte ein Ge lenk 16 hat und anderends an der im Ge häuse 1 festen Feder 17 befestigt ist.
Ein weiterer Draht 18 verbindet die Feder 17 ,mit dem im Gehäuse festen Tragarm 19 einer Kontaktvorrichtung. An diesem Tragarm ist isoliert eine Kontaktfeder 20 angebracht, weleher ein Hilfsstrom über die Klemm schraube 21 zugeführt wird.
Ein im Ge häusegriff isoliert angebrachter Tragarm 22, dler seitlich und dann wieder aufwärts abge bogen ist, trägt an seinem Ende eine der Kon taktfeder 20 gegenüberliegende Kontakt schraube 23 .mit Stellmutter und ein am Spanndrahtgelenk 16 aasgelenkter Verbin dungsdraht 24 bewirkt im Normalzustande eine Schliessung .dieses Kontaktes durch An drücken der Feder 20 an die Sehraube 23. Der andere Pol des erwähnten Hilfsstromes ist an .die Klemme 25 des Tragarmes 22 ,an geschlossen.
Seitlich am Gehäuse sitzt eine isolierende Ausführungstülle 26 und ;durch einen Erdungsdraht 27, der an einer der die Kontaktfeder 20 am Tragarm 19 haltenden Schraube fest ist, kann das ganze Gehäuse geerdet werden.
Der Stromanschluss an die Klemmen 8 und 10 gesehieht vermittelst flexiblen Kabels, ebenso werden die Zuleitungen für den Hilfs- strom: und .die Erdungsleitung in flexiblem Kabel durch die Tülle 26 ausgeführt. Das Sühaltungssehema der Einrichtung zeigt Fig. 4.
A ist,der Apparat, der mittelst flexiblen Kabels .an die Niederspannungs klemmen eines oberspannungsseitig an ein Wechselstromliehtnetz angeschlossenen Klein transformators T angeschlossen ist, und zwar geht, wie bereits erwähnt, der eine Pol an das Gehäuse 1, -der andere an den Rü.ck- leitungsstab 5.
Zwischen beiden verläuft der Strom durch das Heizrohr 3 und bringt dies hierbei zur Erwärmung. Die beiden Klem men 21 und 23 der Hilfskontaktvorrichtung H sind an eine Gleichstromquelle, im vor liegenden Falle an eine Batterie B angeschlos- sen, und, durch diesen Hilfsstrom wird ein Relais R betätigt,
welches bei Öffnen des Hilfskontaktes den Hauptstromkreis ober- spannungsseitig unterbricht. Zur Schonung der Kontakte des Relaisschalters und insbe sondere zur Verhinderung des Verschweissens und Kleb.ens von dessen Kontakten durch Funkenbildung, ist parallel zum Transforma tor T unmittelbar an dessen Oberspannungs- klem,men ein Kondensator C und in Serie mit diesem ein Widerstand W geschaltet,
deren elektrisolhe Konstanten derart gewählt sind, d-ass wesentliche Übersp-apnungen beim Abschaltern und zu grosse Ströme im Augen blick des Einschaltens nicht entstellen kön nen. Die Betriebsspannung des Apparates wird in ,der Praxis zu höchstens 1. Volt an genommen, und die Stromstärke kann hierbei bis auf 40 Amp. gehen.
Der Apparat ist der art einreguliert, .dass nie eine über 42 C lie gende Temperatur, welche jeder Patient er tragen und die keinerlei Schädigungen der Schleimlhaut verursachen kann, erzeugt wird.
Zum Schutz sowohl des Patienten, als auch ,des Operateurs, ist das Gehäuse 1 über die Klemme 27 geerdet; nebstdem kann durch Anlegen dieser Leitung an :den Körper des Patienten dieser auf gleiches Potential mit dem Apparat gebracht und somit jeglicher Stromübergang vom Heizelement ü auf die Schleimhaut der Harnröhre vermieden wer den.
Die Wirkungsweise des Apparates ist. folgende: Sobald das Heizelement in der Harnröhre e rwi -irmt wird, wird infolge der Ausdehnung en des Rohres der am Ende des Rückleitungs- stabes feste Draht 11 etwas gespannt, so dass die Feder 12 den Spanndraht 13 etwas nach- lä.sst, welcher hierdurch ebenfalls den Spann draht 15 entsprechend naehlässt,
wobei jedoch die im -NL-ormalzustandb gespannte Feder 20 immer noch den Hilfskontakt geschlossen hält. Sobald nun das Mass der Erwärmung und[ damit der Ausdehnung des Heizelementes das eingestellte Mass überschreitet, werden die Spanndrähte derart gelockert, .dass das un tere Ende der Kontaktfeder 20 die Kontakt schraube 23 v erlässt, wodurch der Hilfsstrom kreis geöffnet wird,
und der durch diesen festgehaltene Schaltarm des Relais R den Hauptstrom oberspaunungsseitig selbsttätig unterbricht. Diese Unterbrechung dauert so lange, bis die Temperatur -wieder auf das normale Mass zurückgegangen und somit die Ausdehnung geringer geworden ist, wobei die Federn 12 und 17 die Spanndrähte wieder spannen.
Bei der beschriebenen Anordnung findet .also durch die beiden Spanndiähte eine zweimalige Vergrösserung des Ausdehnungs hubes statt, so da.ss auch kleine Abweichun gen noch zu voller Auswirkung gelangen. Dank der getroffenen Anordnung und Scha-1- tung wird der Hauptstromkreis auch dann unterbrochen, wenn irgend ein Defekt im be schriebenen Reguliermechanismus, z.
B. ein Bruch der Spanndrähte oder dergleichen, ein tritt. Ebenso ist der Hauptstromkreis unter- brochen, wenn die Stromquelle des Hilfs stromkreises aussetzt, also zum Beispiel im vorliegenden Falle die Batterie erschöpft ist. Es kann also unter keinen Umständen eine Überschreitung der kritischen Temperatur er folgen. Den Einflüssen veränderlicher Raum temperatur auf das Gehäuse und die einzelnen Glieder der Reguliervorrichtung .ist dadurch Rechnung getragen, dass die freien Enden der Federn 12 und 17 durch die als Kompensa tionsglieder dienenden Drähte 11 und 18 fest gelegt sind..
Anstatt der dargestellten Form eines Katheters könnte das Heizelement auch jede beliebige andere Form für die eingangs erwähnten Zwecke erhalten.
