Abstract

The repositioning and immobilisation appts. is for acromio-clavicular luxation. A shell of plastics nitrate is placed round the elbow. A horizontal belt holds the elbow against the body, while a second belt has a collarbone cushion. A third belt is at right angles to the second with a larger cushion for the arm pit of the other arm, this cushion being adjustable. A wrist sleeve is also included.

  Het bestaat uit een schelp van plastazote die rond de elleboog wordt aangebracht, voorzien van :   - een horizontale   riem om de elleboog tegen het lichaam te houden. 



   -twee onderling loodrecht verbonden riemen. De ene riem draagt een   sleutelbeenkussentje   en de andere een groter, verstelbaar kussen voor de oksel van de andere arm. 



   - een manchet voor de pols. 



  Het   Aanbrengen   - De schelp wordt rond de elleboog aangebracht, daarna brengt men de vertikale steunriem voor het sleutelbeen aan : de twee elementen zijn voor en achter met elkaar verbonden met een gesp en met een klittenband. 



    - De   horizontale riem, die aan de schelp bevestigd is, wordt rond de thorax aangebracht. Met deze riem kan de arm in adductie   gehouden   worden. 



  - De riem met kussen wordt in de oksel van de andere arm gesteund en wordt rond de thorax bevestigd en aangespannen. Op die manier kan de steun voor het sleutelbeen juist gepositioneerd worden. 



    - De   manchet wordt rond de pols bevestigd. Deze manchet is verbonden met de horizontale riem van de schelp d. v. m. een daarvoor voorziene riem. Deze is verstelbaar zodat de elleboog in   gedeeltelijke   extensie kan   geniaalst   worden.

  Opmerkingen - De spanning op de riem die de elleboog verbindt met de claviculaire steun wordt bepaald wanneer de elleboog gebogen is en op tafel steunt. 



   De luxatie wordt gerepositioneerd door een lichte druk te geven op het   slel ! telbeenkussen. Deze spanning   moet in de loop van de tweede week vermeerderd worden. 



  - Een lichte extensie van de   elleboog maakt   het   makkelijker   om de repositie van de luxatie te behouden. 



  - Wanneer de patiënt zit, steunt hij beter op de schelp om de druk op het sleutelbeen te verlichten. 



   Wanneer hij op die manier op de schelp steunt, kan men het   sleutelbeenkussen   1 à 2 maal per dag enkele minuten vrij maken om de damengedrukte plaatsen even +e masseren. Een aanduiding op de riem geeft de oorspronkelijke positie aan. 



  - Na het aanbrengen van de band wordt een röntgenfoto gemaakt om de doeltreffendheid te controleren,
Een wekelijkse radio-klinische controle is wenselijk gedurende vier tot vijf weken, naargelang de ernst van de luxatie.

  Resultaten - Voor de stadia   l   en li treedt definitieve stabilisering op na vijf weken. 



  - Voor stadium   H ! kan   de luxatie weer optreden,   maar   de   ecrnmio-claviculaire   diastasis is aanzienlijk gereduceerd.
