De préférence, l'immunoglobuline est utilisée pour le traitement des troubles respiratoires allergiques, par exemple l'asthme allergique et la rhinite allergique (rhume des foins) en particulier l'allergie respiratoire secondaire à une maladie infectieuse, par exemple la bronchite allergique due à une infection bronchique. Le traitement peut être soit de nature thérapeutique, soit prophylactique, c'est-à- dire qu'il peut apporter une amélioration dans le cas d'une attaque allergique afgue, ou il peut empêcher le déclenchement d'attaques argues chez les patients allergiques de façon chronique.

Pour son utilisation en thérapeutique, l'Ig sera administrée par perfusion intraveineuse sous forme d'une solution à 2-6%, de préférence sous forme d'une

solution à 3% dans du sérum physiologique Le débit approprié pour la perfusion d'une solution à 3% est, par exemple, de 10 à 20 gouttes/minute pendant les 15 premières minutes, de 20 à 30 gouttes/minute pendant les 15 minutes-suivantes et de 40 à 50 gouttes/minute par la suite.
Pour un effet optimal, l'Ig est administrée par perfusion intraveineuse, de préférence au rythme de 2à 5 perfusions par semaine, préférablement plusieurs jours de suite. La dose totale donnée par traitement est de préférence comprise entre 0,05 et 2 g/kg, plus préférablement entre 0,2 et 0,5 g/kg. La quantité d'Ig administrée à chaque perfusion ne doit pas de préférence dépasser environ 10 g, plus préférablement environ 6g.

intervalles de 3 semaines ou plus si nécessaire. Par exemple, un patient sensible au rhume des foins provoque par le pollen peut être soumis à un traitement prophylactique annuel, au débutde la saison du pollen. EXEMPLE 1 Traitement de la rhinite allergique (rhume des foins)
Une femme de 36 ans souffrant de rhinite allergique chronique a reçu 2 perfusions intraveineuses de 6 9 d'Ig intacte (Sandoglobuline R) : injectable par voie intraveineuse, équivalent à un total de 0,2 g/kg.
On a constaté chez cette patiente une forte diminution des symptômes pendant 2 à 4 semaines.
EXEMPLE 2 Traitement de l'allergie multiple
Un homme de 60 ans souffrant du syndrome de l'allergie multiple a reçu 5 perfusions de 6 9 de SandogtobuHn, soit au total 0,43 g/kg. Le contrôle du taux d'IgE dans le sang a révélé qu'après le traitement la concentration d'IgE était tombée à la moitié de son-niveau d'origine ; l'état du patient s'était nettement amélioré.
EXEMPLE 3 Traitement de l'asthme bronchique
On avait diagnostiqué une bronchite asthmatiforme et une broncho-pneumonie chez une petite fille de 3 ans qui souffrait de toux depuis 2 mois. Lors de son admission à l'hôpital, l'enfant présentait les signes physiques d'une complication pulmonaire et les rayons X ont montré la présence d'infiltrats éparpillés. Les examens de laboratoires ont indiqué que le taux d'IgE était légèrement élevé et le test R. A. S. T. (spécifique pour les IgE) avec des cellules épidermiques d'animaux et de l'albumine de poulet était positif.
