Médicament à base de dérivé de cystéine.
De nombreuses publications permettent d'affirmer que la cystéine, acide aminé sulfhydrylé, joue un rdle important dans les phénomènes de kératinisation, tant pour l'épithélium malpighien que pour les phanères.
Malheureusement, si l'administration de cystéine par voie générale à des doses thérapeutiques semble par- faitement tolérée et peut exercer une influence favorable sur certains troubles de ces organes, il est pratique- ment impossible d'employer localement cet acide aminé puisque son instabilité en phase aqueuse est bien connue et sa conservation, même dans des excipients non aqueux, très limitée dans le temps.
Il semble d'ailleurs que, dans les troubles dos phanères et plus spécifiquement dans'les cas de fragilité unguéale, l'administration de cystéine par voie générale

donne des résultats très inconstants.
Le présent médicament comprend, comme substance active, un composé chimique connu, la S-carboxy-méthyl

cystéine répondit à lu. formule HOOC-?H-CH2-S-(,H2-COOH, NH2 plus spécialement en association avec un excipient pour topique.
La demanderesse a trouvé en effet qu'en raison de sa stabilité en solution aqueuse et de son activité la S-carboxy-méthyl cystéine est capabl'e d'exercer une action trophique sur l'ongle lui-même lorsqu'elle est appliquée localement sur la racine de l'ongle, c'est-à-dire une zone qui détermine la croissance et la poussée cellu- laire.
La S-carboxy-méthyl-cystéine se présente sous la forme d'une poudre blanche, soluble dans l'eau bouil- lante, insoluble dans l'eau froide et insoluble dans les solvants organiques usuels. Elle accuse une réaction positive à la ninhydrine et donne un sel cuprique bleu foncé insoluble dans l'eau, un sel mercurique blanc inso- luble dans l'eau et une réaction négative au nitroprus- siate.
Son point de fusion est de 249-250 (avec décomposition) au bloc instantané.
Les résultats de l'analyse sont les suivants :

On peut préparer la S-carboxy-méthyl-cystéine par alcoylation de la cystéine au moyen d'un monochloracé- tate alcalin en présence d'un accepteur de proton, par exemple de la façon suivante :
Dans un ballon à quatre cols muni d'un agitateur, d'un thermomètre, d'une ampoule à brome et d'un tube adducteur d'azote, on introduit une molécule gramme de chlorhydrate de cystéine (anhydre ou hydraté) que l'on dissout par addition de 515 ml d'eau.
On refroidit la solution entre 0 et + 5 . On y fait barboter de l'azote et l'on y introduit 2 molécules grammes de soude caustique en solution 5N en maintenant la température entre 0 et + 5 . La durée d'introduction varie avec les moyens de refroidissement employés. Elle est d'environ 30 minutes lorsque le ballou est plongé dans un mélange de glace et de chlorure de sodium.
Lorsque toute la soude a été introduite, la température étant maintenue entre 0 et + 5 on ajouta, avec agitation et toujours en atmosphère d'azote, 1,025 molécule gramme de monochloracétate de- sodi-un en solution dans 400 ml d'eau.
L'introduction est réalisée en 1 à 2 heures.

Lorsqu'elle est terminée, on porte le mélange ractionnel à 50 et on le maintient à cette température jusqu'à ce que la réaction de la fonction SH au nitro- prussiate de sodium devienne négative (généralement après
15 à 30 minutes à 50 ). Durant cette phase de la réaction, on doit vérifier périodiquement que le pH reste aux envi- rons de 8 et, au besoin, l'y maintenir par addition de soude 5N.
Lorsque la réaction au nitroprussiate est devenue négative, on arrête l'introduction d'azote et on ramène le pH aux environs de 6 par addition d'acide chlorhydrique concentré.
On agite le mélange réactionnel 5 minutes avec
5 g de charbon actif, on filtre pour éliminer le charbon et on refroidit la solution incoloro à la température ambiante.
Pour précipiter la S-carboxy-méthyl cystéine on ajoute de l'acide chlorhydrique concentré jusqu'à pH 2,8 (environ 1 molécule gramme).
Cn essore le précipité cristallin puis on le lave sur filtre jusqu'à l'absence d'ions chlore dans le filtrat.
On peut le purifier de la façon suivante :
On met la B-carboxy-methyl cystéine impure en suspension dans de l'acide chlorhydrique 2N (excès de 25 % de c1H), on porte à l'ébullition pendant 5 minutes, on refroidit la solution à 30 et on l'additionne lentement de soude caustique 2N jusqu'à pH 2,8, avec agitation.
On lave le produit à l'eau jusqu'à l'absence d'ions C1.

Le titrage acidimétrique des produits ainsi purifiés indique me pureté supérieure à 99,5 %.
Le proluit: est ensuite séché à l'air.
Le rendement est de 90 à 95 %.
Les propriétés pharmacologiques principales'de la S-carboxy-méthyl cystéine sont les suivantes :
La toxicité du composé est très faible ; la toxicité aiguë chez la souris (voie intra-veineuse ; solution aqueuse à 10 % et à pH 7) conduit à une DL 50 de 3,1 g/kg ; la très faible toxicité n'a pas permis de aéterminer la DL 50 pour les voies intrapéritonéale, sous- cutanée et buccale chez la souris, ni pour la voie intra- péritonéale chez le rat, le cobaye et le lapin.
L'étude de l'intoxication subaiguë chez le rat (dose quotidienne 0,200 g/kg pendant 21 jours ; contrôles hématologiques hebdomadaires ; examens macroscopiques et histologiques des viscères au bout de trois semaines) n'a révélé aucun signe défavorable, et, en particulier : - Aucune anomalie dans le comportement et la courbe de croissance des rats et des souris ayant reçu chaque jour 0,075 g/kg de S-carboxy-méthyl cystéine en solution aqueuse à 10 % et à pH 7 (voie intra-péritonéale chez le rat, intragastrique chez la souris) pendant 7 semaines et aucune altération à l'examen histologique des viscères n'ont été relevées par comparaison avec les témoins ;
- Aucune anomalie à l'examen anatomo-pathologique d'un fragment de peau d'animaux ayant reçu une application journalière (pendant 15 jours sur une surface de peau d'un cm 2 correspondant à la zone du prélèvement) d'une crème identique à celle qui sera décrite ci-après dans l'exemple 1.

Les actions anti-histaminique, anti-inflam toire et anti-cholérétiique sont nulles.
L'action sur la fragilité des ongles a été@tablie par des expériences sur l'être humain, comme il sera expose ci-après.
Pour obtenir des compositions pharmaceutiques satisfaisantes contenant de la S-carboxy-méthyl cystéine, on a trouvé qu'il était préférable d'utiliser celle-ci sous la forme d'un dérivé nettement plus soluble dans l'eau que la substance mère qui y est presque insoluble. A cet effet, on peut combiner la S-carboxy-méthyl cystéine qui possède deux groupes carboxyliques avec un composé basique, de pré- férence en quantité telle qu'un seul des groupes carboxyli- ques soit neutralisé afin d'obtenir un sel. Des composés basiques convenables sont les alcalis tels que l'hydroxyde do sodium, l'hydroxyde de potassium et l'ammoniaque et les bases organiques telles que la colamine et la triéthanolamine

ou encore des amino-acides basiques tels que l'arginine, la lysine et la glycine.
A titre d'exemple, pour solubiliser 25 g do S- carboxy-méthyl cystéine en suspension dans 500 ml dteau, on mélange avec de l'eau 10 ml de soude caustique (NaOH) ayant une densité de 1,33 (36 Bé) pour former une solution aqueuse normale d'hydroxyde de sodium et l'on ajoute suffisamment de cette dernière solution pour élever jusqu'à 6 le pH de la suspension aqueuse de S-carboxy-méthyl cystéine ; ainsi se forme le sel soluble dans l'eau, en l'espèce le sel mono- sodique de la S-carboxy-méthyl cystéine. Lorsque au lieu d'une base minérale on ajoute une base organique mono basique ou un amino-acide basique, il en faut mettre une molécule par molé- cule de S-carboxy-méthyl cystéine.
Il est préférahle ensuite de disperser ou émulsifier dans un milieu lipidique ou dans quelque autre base semblable à une pommade la solution aqueuse,ainsi produite, de sel de S-carboxy-méthyl cystéine, afin de former un onguent suscep- tible d'être appliqué comme topique. Néanmoins, si on le désire, on peut incorporer un dérivé de ;ystéine à tout véhicule convenable, aqueux ou non-aqueux, en vue d'une appli- cation directe sur les ongles.
Il est préférable d'utiliser au moins une substance tensio-active pour disperser finement la solution aqueuse dans le milieu lipidique. Des substances tensio-actives appor- priéec sont celles qui possèdnt à la fois des groupes hydro- philos et des groupes lipophiles, le fween 80 étant typiquo.
On peut cependant se servir d'autres agents, tels que le Tween 60, les lauryl-sulfonates de métaux alcalins et le stéarate de triéthanolamine.

Le milieu lipidique peut comprendre des constituants employés en cosmétologie, en particulier des excipients dits "pénétrants", c'est-à-dire des excipients susceptibles de favoriser la pénétration d'une substance thérapeutique active (la S-carboxy-méthyl cystéine dans le cas présent) dans la racine de l'ongle. Des exemples typiques sont le mono- stéarate de glycérol, le monopalmitate de glycérol, la lanoline, la lanoline hydrogénée et le perhydrosqualène.
D'autres exemples sont le stéarate de propylène-glycol, les huiles hydrogénées et les huiles estérifiées. Des polyéthy- lène-glycols tels qu'un Carbowax peuvent également figurer dans le milinu lipidique.
Les huiles hydrogénées mentionnées ci-dessus sont des huiles dans lesquelles les liaisons éthyléniques présentes dans les huiles naturelles ont été saturées par de l'hydro- gène. Industriellement, on effectue l'hydrogénation à une pression supérieure à la pression atmosphérique et à une température élevée, en présence de catalyseurs. Par exemple, l'acide oléique, qui est un acide gras non saturé à 18 atomes de carbone, est transformé ainsi on acide stéarique, qui est l'acide gras saturé correspondant.
Par "huiles estérifiées" on ontend des esters mixtes que l'on obtient par alcoolyse d'huiles végétales naturelles en présence de ployoxyéthylène-glycol ayant un poids molé- culaire de 200 à 400, donc des esters mixtes dérivant du glycérol et du polyoxyéthylène-glycol.
La forme pharmaceutique adoptée de préférence est une crème contenant 0,5 à 10 % de S-carboxy-méthyl cystéine sous forme de sel monosodique dans un excipient aqueux gras . pénétrant.

Le colorant généralement employé est un colo- rant rouge. D'ordinaire, on utilise en association deux sortes de colorants : un'premier pour colorer la phase aqueuse (par exemple le Ponceau Drillant 4N ou la Coccine nouvelle), un deuxième pour la coloration de la phase huileuse (par exemple l'érythrosine).

Fabrication : a) phase aqueuse : dans 54 ml d'eau à 50 , on met la S-carboxy-méthyl cystéine et, avec agitation, on ajoute de la soude en pastille jusqu'à pH 6. On ajoute ensuite l'acide sorbique, le sorbitol, le Ponceau bril- lant 4N et le Tween 80 tout en maintenant la température à 50 , b) phse huileuse : on fait fondre à 55-60 le monostéarate de glycérine et la lanoline hydrogénée puis on ajoute le cholestérol, le palmitate de cholestérol et l'érythrosine, c) on ajoute la phase aqueuse à la phase huileuse en maintenant l'agitation jusqu'à prise de la crème.
La demanderesse a expérimenté cliniquement les préparations décrites précédemment dans le traitement de la fragilité unguéale et en particulier celles qui correspondent aux exemples 3-3a et 5. Pour ces deux préparations, la posologie et le mode d'emploi ont été absolument identiques : dépôt sur la base de l'ongle, tous les soirs, d'une quantité de crème correspondant à environ 0,25 g et massage de la matrice unguéale.
L'oncle est formé d'une variété compacte de kératine ou kératine dure. On sait que la maturation de cette kératinp dure, qui ne contient pas de graisses, se fait sans l'intermédiaire de couche granuleuse ni de ' kératohyaline.

La kératinisation résulte de la transformation des protéines globuleuses des cellules malpighiennes en protéines fibreuses. Cette transformation réside surtout dans l'oxydation de deux molécules de cystéine (à grou- pement SH) qui aboutit par déshydrogénation à une molé- cule de cystine (radicaux réunis par un pont disulfure
S-S-). Ainsi, le passage de SH à S-S- est un processus déterminant de la kératinisation. Le rôle de la S-carboxy- méthyl cystéine est important à cet égard dans le traite- ment des altérations unguéales par le médicament décrit dans le présent mémoire.
La fragilité unguéale est d'une extrême fréquence chez la femme. On admet qu'une femme sur trois est atteinte.
Les ongles peuvent être clivables (re dédou- blant ou s'écaillant), cassants (présentant des éclats ou des fissures) ou mous.
Le dommage esthétique qui résulte de la fragilité de la lame unguéale se double d'une gène dans les moindres opérations manuelles quotidiennes.
Les causes les plus fréquentes de ces altéra- tions sont : l'effet desséchant de certains dissolvants du vernis, l'usage des détwrsfs et produits chimiques ménagers, les lésions de la cuticule abusivement repous- sée, ...
Les causes générales sont habituellement dis- crètes. Aussi la demanderesse estime-t-elle que le traitement susceptible de remédier à de tels désordres- doit 8tre local.

Sous l'effet de ce traitement, on peut constater, dès le huitième jour du début des applications, que la surface de l'ongle, souvent granulée ou ponctuée, prend un aspect plus lisse et plus brillant. A partir du quin- zième jour, les ongles qui se dédoublaient ou s'écail- laient, prennent une structure plus ferme et on note en particulier un durcissement net des ongles mous. Les éclats ou fissures qui avaient tendance à se produire spontanément au moindre contact disparaissent.
L'originalité de ce traitement tient dans le fait que la réorganisation de la structure unguéale n'est pas obtenue par les moyens classiques de fixation des protéines, et dénaturation (comme avec les préparations à base de formaldéhyde), mais bien par un véritable enri- chissement en acide aminé soufré de la zone mitotique de l'ongle.
Enfin, le médicament décrit dans le présent mémoire ne détériore pas le vernis et l'emploi de celui-ci peut être continué durant le traitement. Il convient de signaler en outre qu'il ne provoque aucun phénomène d'irritation ou de sensibilisation cutanée, méme après traitement prolongé.
