Die Erfindung bezieht sich auf ein Verfahren zur Herstellung von menschlichem Antistaphylokokkusimmunoglobulin.
Das Antistaphylokokkusimmunoglobulin findet Verwendung in der Therapie, Chirurgie, Pädiatrie, Geburtshilfe und Gynäkologie, Otolaryngologie, Ophthalmologie und Dermatologie zur Behandlung der an Staphylokokkusinfektionen leidenden Kranken (Staphylokokkussepsis, Septikopyämie, Peritonitis, Pneumonie, Endokarditis, Meningitis, Staphylokokkuslungendestruktion, Empyem, Osteomyelitis, Staphylodermie, Staphylokokkusläsionen der HNO-Organe, sowie lokalisierte akute eitrige Staphylokokkuserkrankungen).
Es ist ein Verfahren zur Herstellung des Präparats Y-Globulin bekannt, das zur Prophylaxe einiger Infektionserkrankungen verwendet wird.
Das Verfahren zur Herstellung von'y-Globulin besteht in der Fraktionierung des Spenderblutplasmas mit Äthylalkohol. Hiezu kühlt man es auf eine Temperatur von OOC ab und gibt Azetatpuffer in 53, 5% iger wässeriger Äthylalkohollösung bis PH von 7, 2 hinzu. Im Laufe der Zugabe des Äthylalkohols muss die Temperatur des Gemisches auf-2, 5 bis 3 C herabgesetzt werden. Die Endkonzentration des Äthylalkohols im System beträgt 8%. Der dabei ausgefallene Fibrinogenniederschlag wird durch Zentrifugieren entfernt. Zur Flüssigkeit über dem Niederschlag wird der den Azetatpuffer enthaltende 53, 5% igue Äthylalkohol bis zu einem PH von 6, 9 und einer Alkoholkonzentration im Gemisch von 25% hinzugegeben.
Die Temperatur des Gemisches ist gegen-5 . Die Fällung wird zweimal durchgeführt. Die isolierten Niederschläge der zweiten und dritten Fällung werden mit Äthylalkohol bis zu einer Konzentration desselben im Gemisch von 17% und einem PH von 5, 2 bei einer Temperatur von-6 C behandelt. behandelt. Der ausgefallene Niederschlag besteht hauptsächlich aus'y-Globulin. Der Niederschlag wird durch
Zentrifugieren bei einer Temperatur von-6 C abgetrennt. Man bringt den PH-Wert der Flüssigkeit über dem
Niederschlag auf 7, 4 : t 0, 2 durch Zugabe von Natriumhydrogenkarbonat und die Äthylalkoholkonzentration auf
20 bis 25% durch Zugabe von 95%igem Äthylalkohol. Es wird eine Temperatur von ungefähr -50C eingehalten.
Der ausgefallene Niederschlag besteht hauptsächlich aus dem löslicheren'Y-Globulinteil. Der Niederschlag wird durch Zentrifugieren bei einer Temperatur von -50C abgetrennt. Aus diesem Niederschlag wird eine 16, 5 g 'y-Globulin in 100 ml enthaltende Lösung zubereitet und der Sterilisation unterworfen.
Das nach dem bekannten Verfahren hergestellte'y-Globulin besitzt keine aktive spezifische Wirkung gegen verschiedene Infektionserkrankungen und besonders gegen Staphylokokkusinfektionen. Das erwähnte'Y-Globulin enthält niedrigere a-antitoxische Antistaphylokokkusantikörpertiter bis 4 bis 10 lE/ml, und es besitzt daher keine aktive Schutzwirkung bei der Staphylokokkusinfektion, besonders bei der Staphylokokkussepsis.
Das Ziel der Erfindung ist es, die genannten Nachteile zu vermeiden und durch Erhöhung des Staphylokokkus-a-antitoxingehalts in der y-Globulinfraktion menschliches Antistaphylokokkusimmunoglobulin mit einer aktiven spezifischen Wirkung gegen Staphylokokkusinfektionen zu erhalten.
Das erfindungsgemässe Verfahren zur Herstellung von menschlichem Antistaphylokokkusimmunoglobulin durch Faktionierung von Blutplasma mit Äthylalkohol bei einer unterhalb 0 C liegenden Temperatur und nachfolgende Abtrennung der Gammaglobulinfraktion, deren Reinigung, Trocknung und Auflösung in physiologischer oder isotonischer Lösung ist dadurch gekennzeichnet, dass Plasma aus dem Blut von Spendern verwendet wird, die mit Staphylokokkusanatoxin in Dosen von 1, 0 ; 1, 0 und 2, 0 ml in einem 7-tägigen Abstand zwischen den Injektionen immunisiert worden sind, und aus diesem Plasma das Antistaphylokokkusimmunoglobulin mit dem Staphylokokkus-a-antitoxingehalt im Titer von 50 bis 640 IE/ml und Antistaphylokokkusagglutiningehalt im Titer bis 1 :
20480 ausgeschieden wird.
Das erfindungsgemäss erhaltene menschliche Antistaphylokokkusimmunoglobulin wird experimentell an Tieren und klinisch an Menschen geprüft.
Durch experimentelle Tieruntersuchungen wurde festgestellt, dass der Schutzeffekt des Antistaphylokokkusimmunoglobulins bei der Staphylokokkusinfektion dreimal höher als der des gewöhnlich unspezifischen Gammaglobulins ist.
Das vorgeschlagene Antistaphylokokkusimmunoglobulin hat einen a-Antitoxintiter von 50 bis 640 lE/ml und Antistaphylokokkusagglutinin in Titer bis 1 : 20480, was den a-Antitoxintiter im gewöhnlichen'y-Globulin bedeutend übertrifft, in dem der a-Antitoxintiter 4 bis 10 IE/ml beträgt.
Dementsprechend ist der Schutzeffekt des Antistaphylokokkusimmunoglobulins bei der Staphylokokkusinfektion bedeutend höher als der des unspezifischen Gammaglobulins.
Dies wurde durch klinische Beobachtungen bei der Behandlung von an Staphylokokkussepsis Erkrankten bestätigt. Eine einmalige Dosis des Antistaphylokokkusimmunoglobulins mit dem a-Antitoxintiter von mindestens 100IE/ml betrug bei Kindern 3 mu 10% niger Lösung und bei Erwachsenen 6 ml 10% ige Lösung. Das Präparat wurde intramuskulär einmal täglich verabreicht und die Behandlung wurde jeden zweiten bis dritten Tag wiederholt. Für eine Behandlungskur brauchte man drei bis fünf Injektionen der therapeutischen Dosen. In schweren Fällen allgemeiner Staphylokokkusinfektion wurden bis 10 therapeutische Dosen des Antistaphylokokkusimmunoglobulins verwendet.
Die Beobachtungen haben ergeben, dass unter dem Einfluss des Antistaphylokokkusimmunoglobulins die

Temperatur der Kranken schnell abnahm, die Toxämieerscheinungen nachliessen, der akute Verlauf der
Staphylokokkussepsis kupiert wurde, der Allgemeinzustand der Kranken bedeutend besser wurde, die
Blutanalysen allmählich normale Werte ergaben und bei der wiederholten Aussaat nach der durchgeführten spezifischen Behandlungskur keine pathogenen Staphylokokken im Blut der Kranken nachgewiesen wurden.
Unter Beobachtung befanden sich 50 an Staphylokokkussepsis Erkrankte, darunter 35 Kinder und 15
Erwachsene. Von 35 kranken Kindern erhielten 26 Frühgeborene und 9 Kinder von verschiedenem Alter eine spezifische Antistaphylokokkusimmunoglobulintherapie. Die Staphylokkussepsis verlief bei allen Patienten sehr schwer.
Als Ergebnis der durchgeführten spezifischen Antistaphylokokkusimmunoglobulintherapie wurden von 50 an Staphylokokkussepsis Erkrankten 40 geheilt, 10 starben (zwei davon an Hauterkrankungen).
Die Gesamtmortalität an Staphylokokkussepsis beträgt 16%, während nach Literaturangaben die Mortalität an Staphylokokkussepsis bei der Durchführung einer komplexen Therapie, einschliesslich Antibiotika, 50 bis 70% beträgt.
Das erfindungsgemässe Verfahren zur Herstellung des genannten Immunoglobulins wird folgendermassen durchgeführt.
Es wird ein notwendiges Spenderkontingent (Volontäre) im Alter von 20 bis 40 Jahren ausgewählt. Die Spender werden zunächst einer medizinischen, serologischen und Laborkontrolle gemäss den Vorschriften für die Blutabgabe und Plasmaphorese unterzogen.
Bei normalen hämatologischen und biochemischen Befunden werden die Spender durch subkutane Injektion von adsorbiertem Staphylokokkusanatoxin in die Gegend unter dem Schulterblatt in Dosen von 1, 0 ; 1, 0 und 2, 0 ml in einem 7-tägigen Abstand zwischen den Anatoxininjektionen immunisiert. 7 Tage nach der Beendigung der Immunisierungsbehandlung wird das Vorhandensein des Staphylokokkus-a-antitoxins im Spenderblutserum kontrolliert. Die Spnder, die einen Staphylokokkus-a-antitoxintiter von mindestens 5 IE in 1 ml Blutserum aufweisen, werden zur Durchführung der Plasmaphorese zwecks Gewinnung von hyperimmunem Antistaphylokokkusplasma zugelassen.
Der Staphylokokkus-a-antitoxingehalt im Blutserum der mit Staphylokokkusanatoxin immunisierten Spender wird in der Reaktion der Lyse-Neutralisation von Kaninchenerythrozyten bestimmt. In dieser Reaktion wird die Methode der Neutralisation von hämolytischen Eigenschaften des Staphylokokkus-a-toxins genutzt, dessen Lh genau festgestellt ist.
Das zur Standardisierung anwendbare Staphylokokkustoxin wird mit physiologischer Lösung bis Lh/5 verdünnt.
Die Plasmaphorese wird in an sich bekannter Weise durchgeführt.
Nach dieser Methode wird das Antistaphylokokkusplasma in einem sterilen geschlossenen System unter der Verwendung von verdoppelten weichen PVC-Säcken abgenommen.
Die Plasmaphorese wird bei den Spendern 2 bis 4 mal monatlich in einem 7 bis 14-tägigen Abstand durchgeführt, wobei jedesmal bis 250 ml immunes Antistaphylokokkusplasma erhalten werden. Das Immunplasma wird vor der Fraktionierung im gefrorenen Zustand in weichen PVC-Säcken bei einer Temperatur von-20 bis-30 C aufbewahrt.
Zur Extraktion der Gammaglobulinfraktion aus dem Antistaphylokokkusplasma wird eine Plasmafraktionierung mit Äthylalkohol bei niedrigen Temperaturen unter 00 durchgeführt.
In der ersten Stufe der Plasmafraktionierung wird Fibrinogen isoliert.
In der zweiten Fraktionierungsstufe geht die Fällung von Globulinen vor sich. In dieser

Niederschlag wird auf einer Ultrazentrifuge bei einer Temperatur von -40 abgetrennt und zur Herstellung von
Fibrinolysin verwendet. Das Zentrifugat wird bei einer Temperatur von-3 bis-5 C durch Papierbrei filtriert, bis ein durchsichtiges Filtrat erhalten wird.
Im durchsichtigen Filtrat wird der pH-Wert mit 1 m NaHCO-Lösung auf 6, 9 bis 7, 0 erhöht. Dann wird der auf eine Temperatur von-15 C abgekühlte 95% igue Alkohol in einer Menge von 0, 161 je 11 Lösung zugegeben, was im Gemisch eine Alkoholkonzentration von 25% ergibt. Die Temperatur dieses Gemisches muss - 9 C betragen. Unter diesen Bedingungen fällt Immunoglobulin aus, das in der Ultrazentrifuge abgetrennt wird.
Der rohe Niederschlag des gereinigten Immunoglobulins wird gewogen, in steriler physiologischer Lösung im
Verhältnis von 1 : 1, 5 gelöst, in Flaschen von 500 ml Inhalt zu 200 ml abgefüllt, bei einer Temperatur von-35 bis-40 C eingefroren und lyophil getrocknet. Aus dem getrockneten Immunoglobulin wird eine 10% igue Lösung bereitet, indem das trockene Pulver in destilliertem Wasser gelöst wird, dessen Menge der Menge der zum rohen
Niederschlag vor der Trocknung hinzugegebenen physiologischen Lösung gleich sein muss.
Zu 100 g trockenem Pulver werden 0, 9 g Natriumchlorid zugegeben.
Der Proteingehalt in der erhaltenen Lösung wird mit dem Refraktometer bestimmt und gegebenenfalls mit physiologischer Lösung auf 10 1 Gew.-% eingestellt.
Die erhaltene Lösung wird durch Papierbrei und anschliessend durch sterilisierende Keramikkerzen unter
Vakuum filtriert. Die sterile Lösung wird in Ampullen zu 3 ml abgefüllt, die Ampullen werden zugeschmolzen.
Die Ausbeute beträgt 8 bis 10 Dosen von Antistaphylokokkusimmunoglobulin pro 11 Plasma.
Im fertigen Präparat werden der Proteinprozentsatz, die Sterilität, Pyrogenität, Toxizität und
Fraktionsreinheit nach der Papierelektrophorese bestimmt.
Das erhaltene Antistaphylokokkusimmunoglobulin stellt eine durchsichtige oder etwas opaleszierende, farblose oder schwach gelbe Lösung dar.
Das Antistaphylokokkusimmunoglobulin muss spezifisch, steril, unschädlich und apyrogen sein. Es enthält in 1 ml mindestens 50 IE an anti-a-toxischen Antistaphylokokkusantikörpern nach der Neutralisationsreaktion der hämolytischen Wirkung des Staphylokokkus-a-toxins auf Kaninchenerythrozyten. Der Proteingehalt in der Antistaphylokokkusimmunoglobulinlösung muss 10 ! : 1% entsprechen. Der Gehalt an der Gammaglobulinfraktion, die mit Hilfe der Elektrophorese bestimmt wird, muss mindestens 97% vom Gesamtprotein betragen.
Beispiel : Zur Herstellung von Antistaphylokokkusimmunoglobulin wird ein notwendiges Spenderkontingent (Volontäre) im Alter von 20 bis 40 Jahren ausgewählt. Die Spender werden zunächst einer medizinischen Kontrolle gemäss den Vorschriften für Blutgabe und Plasmaphorese unterzogen. Dann werden die Spender mit dem adsorbierten Staphylokokkusanatoxin nach dem Schema : 1, 0 ; 1, 0 und 2, 0 ml in einem 7-tägigen Abstand zwischen den Anatoxininjektionen immunisiert.
Wenn im Spenderblut das Staphylokokkus- < x-antitoxin in einer Menge von mindestens 5 IE/ml in der Neutralisationsreaktion der hämolytischen Eigenschaften des Staphylokokkus-a-toxins vorhanden ist, werden die Spender zur Durchführung der Plasmaphorese zwecks Gewinnung und Anreicherung der notwendigen Menge des hyperimmunen Antistaphylokokkusplasmas zugelassen. Das erhaltene Plasma wird vor der Fraktionierung im gefrorenen Zustand in weichen PVC-Säcken bei einer Temperatur von -20 bis -300C aufbewahrt.
Zur Extraktion der Y-Globulinfraktion werden 10 l des genannten hyperimmunen Antistaphylokokkusplasmas mit Äthylalkohol bis zur Alkoholkonzentration im Plasma von 8% bei-3 C und einem PH-Wert von 7, 2 behandelt. Dabei fällt Fibrinogen aus, das entfernt wird.
Die Flüssigkeit über dem Niederschlag wird mit Äthylalkohol bis zu dessen Gehalt im Gemisch von 25% bei-50 und einem pH-Wert von 6, 9 behandelt. Es fällt der Niederschlag aus, welcher die Hauptmenge der ss-und Y-Globuline enthält. Der Niederschlag wird abgetrennt und die Flüssigkeit über dem Niederschlag wird mit Äthylalkohol wiederholt behandelt. Der Niederschlag der zweiten und dritten Fällung wird in einer Menge von 500 g in 500 ml einer auf OOC abgekühlten 2% i n Natriumchloridlösung suspendiert. Zur erhaltenen Suspension werden 750 ml auf OOC abgekühltes destilliertes Wasser zugegeben.
Man misst das Volumen des erhaltenen Gemisches und gibt unter Rühren den auf 0 C abgekühlten Azetatpuffer (11, 5 ml 2-m Lösung von CH3COOH, 11, 5 ml 2-m Lösung von CH3COONa) in einer Menge von 1, 51 pro 11 Suspension hinzu. In dieser Suspension wird der pH-Wert auf 5 bis 5, 1 eingestellt.
Das Gemisch wird bei einer Temperatur von OOC während 12 Stunden stehengelassen. Unter diesen Bedingungen fällt der Prothrombinniederschlag aus, der durch Zentrifugieren bei 20000 Umdr/min und einer Temperatur von OOC abgetrennt wird.

Nachher wird der auf-15 C abgekühlte 95%ige Alkohol in einer Menge von 0, 161 pro 11 Lösung zugegeben, was im Gemisch eine Alkoholkonzentration von 25% ergibt. Die Temperatur dieses Gemisches muss-9 C betragen.
Unter diesen Bedingungen fällt Immunoglobulin aus, das abgetrennt wird.
Der rohe Niederschlag des gereinigten Immunoglobulins in einer Menge von 150 g wird in steriler physiologischer Lösung im Verhältnis von 1 : 1, 5 gelöst, in Flaschen von 500 ml Inhalt zu 200 ml abgefüllt, bei einer Temperatur von -35 bis -400C eingefroren und in Trocknungsapparaten lyophil getrocknet. Aus dem getrockneten Antistaphylokokkusimmunoglobulin wird die 10% igue oder die 16% igue Lösung in apyrogenem destilliertem Wasser zubereitet.
Zu 100 g trockenem Pulver werden 0, 9 g Natriumchlorid zugegeben. Der Proteingehalt in der erhaltenen Lösung wird mit dem Refraktometer bestimmt und erforderlichenfalls mit physiologischer Lösung auf 10 1 Gew.-% eingestellt. Die erhaltene Lösung wird durch Papierbrei und dann durch sterilisierende Keramikkerzen unter Vakuum filtriert. Die sterile Antistaphylokokkusimmunoglobulinlösung wird in Ampullen zu 3 ml abgefüllt, die Ampullen werden zugeschmolzen. Die Ausbeute beträgt 80 bis 100 Dosen von Antistaphylokokkusimmunoglobulin pro 10 1 immunen Plasmas.
In den erhaltenen Präparaten sind in 1 ml Antistaphylokokkusimmunoglobulin 50 bis 640 antitoxische Einheiten (IE) und Antistaphylokokkusagglutinine in Titer bis 1 : 20480 enthalten. Die hergestellten Präparate des Antistaphylokokkusimmunoglobulins sind steril, apyrogen, untoxisch und spezifisch wirksam.
